Abstract

Background : Smoking is a major global health problem, causing morethan 8 million deaths each year 7 million from direct tobacco use and 1.3million from exposure to secondhand smoke. In Indonesia, the prevalenceof smokers aged 10-18 years increased from 7.2% (2013) to 9.1% (2018),making it the third country with the highest number of smokers. Data fromthe Global Youth Tobacco Survey (2019) shows 40.6% of students aged13-15 years have used tobacco, with 19.2% currently smoking. Body MassIndex (BMI) is used to assess body weight relative to height. Based on the2018 Riskesdas, in Indonesia 9.3% were underweight, 55.3% were normal,13.6% were obese, and 25.8% were obese. Objective: Knowing the impactof the number of cigarettes, the length of smoking and the BMI of studentsof the Faculty of Agriculture, Methodist University of Indonesia Medanbased on the Brikman index in students of the Faculty of Agriculture,University of Indonesia in 2024.Research method: This research is an analytic observational study with aresearch design using a cross-sectional study.Research results: This study found the most students consuming thenumber of cigarettes with < 9 cigarettes / day, as many as 37 respondents(46.3%), then smoking duration found 51 respondents (63.7%) withsmoking duration < 10 years and Body Mass Index found 28 respondents(35%) with body mass index < 18.5. There is a relationship between thenumber of cigarettes with Body Mass Index with a p value = 0.045 (p0.05)and there is a relationship between smoking duration with Body MassIndex with a p value of 0.001 (p0.05). The more influential factor is theamount of cigarette consumption with a p value = 0.013 and OR = 2.4.Conclusion: There is a significant relationship between the number ofcigarettes and body mass index with a p value = 0.045 and there is arelationship between smoking duration and body mass index with a p valueof 0.001.

Abstract

Background: Stunting, or failure to thrive, is a chronic nutritionalcondition resulting from prolonged inadequate nutrient intake. In the shortterm, stunting can increase morbidity; in the long term, it manifests asdelayed bone growth and low body weight. Children with stunting aresignificantly shorter than their peers. Objective: This study aims to identifythe multifaceted risk factors contributing to stunting. Discussion: Stuntingis influenced by a complex interplay of environmental and genetic factors.Key environmental determinants include maternal nutritional status, childfeeding patterns, and environmental sanitation. While hereditary factorsand hormonal imbalances play a role, malnutrition remains a primarypreventable cause of short stature in children. Conclusion: Understandingboth environmental and genetic factors is crucial for the effectiveprevention of stunting and the improvement of children's nutritionalhealth.

Abstract

Background: Low Birth Weight (LBW) can be defined as a baby weighingless than 2500 grams or 5.5 pounds. The causes of LBW are generallymultifactorial, including maternal factors, pregnancy factors, fetal factors,and other factors. Based on research that has been conducted, severalmaternal factors that can influence the occurrence of LBW include age,hemoglobin levels, and maternal parity.Research Objectives: The purpose of this study is to analyze therelationship between maternal age, hemoglobin levels and parity and theincidence of LBW babies in mother giving birth at Dr. pirngadi Hospital in2022-2023.Methods: This study used an observational-analytic method with a cross-sectional research design.Research Result : The total number of research samples is 119 samples.The analysis used chi square test at p < 0,05. The result showed that agehave a correlation with the incidence of LBW with a p-value 0,036(<0,05), maternal hemoglobin levels also have a correlation with theincidence of LBW with p-value 0,022(0,05) and parity has correlation withthe incidence of LBW with p-value 0,031(<0,05).Conclusion :Maternal age, hemoglobin levels and parity are associatedwith the incidence of LBW at Dr. Pirngadi Hospital in 2022-2023.Keywords : LBW, Age, Maternal Hemoglobin levels, Parity

Abstract

Background : Lumbar radiculopathy may arise from the compression oflumbosacral nerve roots, leading to pain and disability. We report a caseof chronic lumbar radiculopathy that was successfully treated withperipheral blood stem cells over 24 weeks.Case Presentation : A 62-year-old woman with sharp pain in the back,radiating to both lower legs. Her pain became intense when she waswalking about 200 meters, and has been happening for 2 years. She felland hit her back 7 years ago, and refused to undergo spine surgery in ahospital. Magnetic resonance imaging revealed facet joint hypertrophyand thickening of the ligamentum flavum at L3/L4, L4/L5, and narrowingof the spinal canal. The pretreatment numerical pain rating (NPR) scorewas 7 (out of 10), and the Oswestry Disability Index (ODI) for low backpain was 19 or 38% (19/50 x100%). She was administered a 5-day courseof granulocyte colony-stimulating factor (G-CSF, 5mg/kg/day)subcutaneously, followed by leukapheresis on the fifth day. Circulatingand intravenous infusion of autologous peripheral blood stem cells wereanalyzed using flow cytometry. She also received transforaminal injectionsof 3 ml of peripheral blood stem cells (PBSCs) at each affected lumbarlevel. Peripheral blood CD34+ cells increased fro-treatment, the percentage reductions in pain and ODI were 100% and79.0%, respectively. There were no serious adverse effects from theprocedure.Conclusion: Autologous peripheral blood stem cells are an effectivetreatment for chronic lumbar radiculopathy.Keywords : lumbar radiculopathy, peripheral blood stem cells, spinalstenosis, granulocyte colony-stimulating factor, leukapheresis

Abstract

Background: Hemorrhoids are a very commonanorectal condition defined as symptoms ofenlargement and displacement of the normal anal padto distal. The causes of hemorrhoids themselves arestill idiopathic or not widely known, but many arecaused by age, gender, lack of drinking and fiberconsumption, constipation, occupation, genetics, lackof exercise, overweight, increased intraabdominalpressure. Objective: This study aimed to determinethe relationship between body mass index andoccupation in hemorrhoid incidence. Methods: Thistype of study uses an observational analyticalmethod, with a crosssectional design, a datacollection method using secondary data obtainedfrom medical records diagnosed with hemorrhoids atMitra Sejati Hospital, Medan city in 2023. Abstract

Background : Uncontrolled diabetes causes chronic complications, bothmicroangiopathies and angiopathies. Diabetic ulcers are one of thechronic complications of diabetes, caused by macroangiopathy resulting invascular insufficiency and neuropathy. Ulcers are a concern which willlead to a level of disability that will prolong the patient's treatment period.Factors that affect the length of care in diabetic ulcer patients aremetabolic control (poor blood sugar, hba1c, albumin, hemoglobin, LDL,LDL), infection control (elevated leukocytes), vascular control (increasedd-dimer coagulation). The length of treatment of diabetic ulcer patientswill burden both the patient and the state. Research: To analyze therelationship of predictor factors (hemoglobin, leukocytes, platelets, LDL,HbA1c, D-dimer, Albumin) to the length of treatment of diabetic ulcerpatients at Dr. Pirngadi Hospital Medan City. Methods: This study is ananalytic observational study with a research design using a cross-sectionalstudy. Results: Based on the results of the proportion test with the Chi-square correlation test there is a relationship between hemoglobin (p-value of 0.008), the relationship between leukocytes (p-value of 0.010), therelationship between platelets (p-value of 0.010), the relationship betweenLDL (p-value of 0.047), the relationship between HbA1C (p-value of0.046), the relationship between D-dimer (p-value of 0.025), therelationship between albumin (p-value of 0.041) with the length oftreatment of diabetic ulcer patients. In multivariate results, the factor thatmost influences the length of treatment is leukocytes. Conclusion:Hemoglobin, leukocyte, platelet, LDL, HbA1c, D-dimer, Albumin levelsare factors associated with the length of treatment of diabetic ulcerpatients at Rsud Dr. Pirngadi Medan City in 2022-2023 and leukocytesare the most influential factor.Keywords: diabetes, diabetic ulcer, length of stay

Abstract

Background: Nosocomial infections, particularly those occurring after 72hours of patient admission, present a serious challenge in hospitals. Thesegram-negative bacteria, including Escherichia coli, Klebsiellapneumoniae, and Shigella, can lead to various infections such as urinarytract infections and nosocomial pneumonia. Nosocomial pneumonia,which encompasses Hospital-Acquired Pneumonia (HAP) and Ventilator-Associated Pneumonia (VAP), is associated with high mortality rates andcan spread through contact transmission. The primary causes of theseinfections include prolonged hospitalization, severity of illness, andexcessive use of antibiotics, which can enhance bacterial resistance. Toprevent the spread of nosocomial infections, stringent hygiene practicesand proper aseptic procedures are essential, along with prudent antibioticmanagement based on laboratory results. These preventive measures arecrucial for improving the quality of healthcare services in hospitals andreducing the risk of nosocomial infections

Abstract

Background: Diuretics are a primary pharmacological class used toregulate kidney function in the excretion of water and electrolytes, leadingto increased urine production and volume. The mechanism involves thesuppression of receptors responsible for the reabsorption of sodium(Na+)—the primary extracellular cation—within the renal tubules. Thisinhibition increases tubular osmolality, thereby suppressing waterreabsorption.Mechanism of Action: This study explores two major classes: Loopdiuretics and Thiazides. Loop diuretics exert their effect by inhibiting theNa+−K+−2Cl− (NKCC2) cotransporter on the apical membrane of thethick ascending limb of the loop of Henle. In contrast, Thiazides inhibitsodium-chloride (Na/Cl) channels in the distal convoluted tubules of thenephron.Synergistic Effect: Clinical evidence suggests that the addition ofThiazides can potentiate the efficacy of Loop diuretics. This synergyoccurs by inhibiting sodium reabsorption in the distal tubule and blockingthe compensatory responses—such as distal nephron hypertrophy—thattypically arise from long-term exposure to Loop diuretics.Conclusion: Understanding the distinct and combined mechanisms ofFurosemide and Thiazides is essential for optimizing fluid managementand overcoming diuretic resistance in clinical practice.Keywords: Diuretics, Furosemide, Thiazide, Renal Tubules, ElectrolyteExcretion.

Abstract

Background : Dyspepsia can be caused by eating and drinking habits suchas spicy food, excessive acidity or consumption of alcoholic drinks,carbonated and caffeinated drinks such as coffee, smoking, stress,Helicobacter Pylori infection, use of drugs and a history of disease (gastritis and gastric ulcers) which have an impact as risk factors fordyspepsia.Objective : to determine the relationship between diet, lifestyle and stresslevels on the recurrence of dyspepsia at the Dalu ResearchMethod : This research is an observational analytical research. Theresearch will be carried out in Jully of 2024. The research was carried outat the DaluConclusion : The number of respondents' eating patterns at the Dalu Thenumber of respondents' lifestyle at the Dalu X Tanjung MorawaCommunity Health Center was in the good category (84.3%). The totalstress level of respondents at the Dalu X Tanjung Morawa Health Centerwas in the medium category (47.1%). The number of recurrences ofdyspepsia at the Dalu X Tanjung Morawa Health Center was in thedyspepsia category (52.9%). There was a relationship between diet andrecurrence of dyspepsia at the Dalu X Tanjung Morawa CommunityHealth Center (p=0.000). There was no relationship between lifestyle anddyspepsia recurrence at the Dalu X Tanjung Morawa Community HealthCenter (p=0.150). There was a relationship between stress levels anddyspepsia recurrence at the Dalu X Tanjung Morawa Community HealthCenter (p=0.000).Keywords : Diet, lifestyle, stress, dyspepsia
