Abstract

Most glaucoma drugs lower the intraocular pressure (IOP) by decreasing the aqueous humor production and increasing the outflow through uveoscleral pathway. None of these drugs work mainly on increasing outflow through the trabecular pathway. Consequently, the experiment to develop glaucoma drugs directly target at the trabecular outflow pathway is highly required. The purpose of this study is to reveal the effect of rho-kinase inhibitor Y-27632 on the thickness of trabecular meshwork in juvenile rats model injected with sodium hyalu-ronate.  This study was an experimental study with posttest only control group design. Twenty-four rats were included in this study. Each eye of the rat would be considered as one sample. Samples were divided into 6 groups, negative control group, positive control I group with intracameral sodium hyaluronate injection, posi-tive control II group with topical Y-27632 10 mM, and three experimental groups with intracameral injection of sodium hyaluronate and Y-27632 10-1 mM, 1 mM, and 10 mM respectively. After the procedures all rats were sacrificed and enucleated. Trabecular meshwork tissue was stained with Hematoxilene-Eosin and evalu-ated under 400× microscopic magnification. Quantitative measurements were taken using computerized image analysis with dot slide program.  There were significant statistic differences among the positive control I group and the experimental groups (p-value < 0.05) as well as the positive control II group and the experi-mental groups (p-value < 0.05). The highest mean of decreasing trabecular meshwork thickness was noted in the group given by sodium hyaluronate and Y-27632 10 mM with value of 118.42 µm. There was decreasing thickness of trabecular meshwork due to the effect of rho- kinase inhibitor Y-27632 in juvenile rats injected with sodium hyaluronate.

Abstract

Retinoblastoma adalah kanker intraokular yang terjadi pada anak usia dini, disebabkan oleh terganggunya supresor tumor gen RB baik secara herediter maupun nonherediter. Tatalaksananya adalah enukleasi, eksentrasi, kemoterapi, laser fotokoagulasi, cryotherapi, dan radioterapi. Apabila tidak diobati hampir seluruh pasien mengalami proses desak ruang dan penyebaran tumor. Kemoterapi bukan merupakan pilihan terapi selektif karena hal ini semakin merusak sel normal pada jaringan yang normal. Sehingga peneliti mencari alternatif terapi seperti ekstrak kulit jeruk keprok yang memiliki fungsi sebagai anti karsinogenik, anti tumor, anti invasif dan anti metastasis. Penelitian ini merupakan analitik eksperimental pada sel kultur retinoblastoma yang dipapar dengan ekstrak kulit jeruk keprok. Sel kultur retinoblastoma dibagi menjadi 4 kelompok, yaitu kelompok kontrol,  kultur+ekstrak kulit jeruk keprok dosis 10mg/mL, kultur+ekstrak kulit jeruk keprok dosis 20mg/mL dan kultur+ekstrak kulit jeruk keprok dosis 40mg/mL. Diinkubasi selama 72 jam lalu dilakukan pengecatan dengan propidium iodide untuk diperiksa menggunakan flowcitometry. Hasil pemeriksaan dengan flowcitometry berupa gambaran histogram dari siklus sel dan apoptosis. Sel kultur retinoblastoma pada fase G1/S, kelompok kontrol (15,03±3,03), kelompok ekstrak kulit jeruk keprok dosis 10mg/mL (12,47±2,51), 20mg/mL (12,30±1,33) dan 40mg/mL (10,80±1,52) (p=0,027) menunjukkan bahwa ekstrak kulit jeruk keprok tidak menginduksi G1 arrest. Pada data apoptosis, kelompok kontrol (35,0±3,96), kelompok ekstrak kulit jeruk keprok dosis 10mg/mL (33,63±0,88), 20mg/mL (38,48±8,96) dan 40mg/mL (64,42±2,09) (p=0,000) menunjukkan bahwa pemberian ekstrak kulit jeruk keprok menginduksi apoptosis pada sel kultur retinoblastoma. Kulit buah jeruk keprok (Citrus reticulata) mengandung berbagai macam senyawa flavonoid, yang bertindak sebagai anti-karsinogenik, anti-tumor, anti-invasif dan anti-metastasis. Pemberian ekstrak kulit jeruk keprok tidak menginduksi cell-cycle G1 arrest tetapi menginduksi proses apoptosis.Kata Kunci: Apoptosis, cell-cycle G1 arrest, ekstrak kulit jeruk keprok (Citrus reticulata), sel kultur retinoblastoma

Abstract

Katarak pediatrik  merupakan salah satu penyebab utama kebutaan dan low  vision  pada anak, terutama di negara-negara berkembang.  Studi  data  sekunder  berbasis  rumah  saki  dilakukan  selama  4  tahun,  sejak  Desember  2006  sampai  Desember 2010.  Pengumpulan  data  meliputi jenis  kelamin,  usia,  usia  onset,  faktor  penyebab,  bilateralitas,  visus  pre  dan  post  operasi, jenis operasi, komplikasi pre dan post operasi, dan follow up dari 58 mata yang berasal dari 43 penderita. Data dianalisa secara  deskriptif  serta  dilakukan  analisis  korelasi  antara  usia  operasi  dan  jenis  operasi  dengan  visus  hasil  operasi. Penderita berjenis kelamin laki-laki lebih banyak didapatkan daripada perempuan (67,4%), dengan penyebab terbanyak katarak pediatrik adalah non traumatik (67,4%) yang didapatkan sejak lahir (51,6%). Pada kelompok traumatik katarak, usia penderita  dan usia operasi  terbanyak  adalah  6-10  tahun  (73,3%),  sedangkan  kelompok  non traumatik  adalah  1-6 (38,7%). Jenis operasi yang terbanyak dilakukan adalah Extra Capsular Cataract Extraction+Intraocular Lens+iridektomi perifer  (31%).  Komplikasi  nystagmus  adalah  yang  terbanyak  didapatkan  pada  katarak  non  traumatik  (34,9%).  Pada evaluasi post operasi, 10 dari 15 penderita (66,7%) pada kelompok traumatik mencapai visus &gt;6/18, sedangkan 30,2% penderita  katarak  non  traumatik  mendapatkan  visus  1/60-3/60.  Pada  penelitian  ini  didapatkan  korelasi  kuat  antara penatalaksanaan operasi yang tertunda dengan buruknya visus pasca operasi (r=0,707; p=0,000) tetapi tidak didapatkan korelasi  antara  tipe  operasi  dengan  angka  kejadian  Posterior  Capsular  Opacification  (r=0,053;  p=0,691).  Dapat disimpulkan katarak non traumatik adalah penyebab utama katarak pediatrik, dengan sebagian besar kasus terjadi sejak lahir .  Visus  post  operasi  pada  mayoritas  kasus  katarak  non  traumatik  masih  tergolong  dalam  low  vision  akibat keterlambatan  penatalaksanaan  operasi.

Abstract

ABSTRACT Retinal neovascularization is the main problem that causes blindness in many types of eye desease. Neovascularization occurs in ischemic retina, and several important mediators that induce neovascularization is secreted by the surrounding cell and endothelial cell vessels. Matrix metalloproteinase-9 is an enzyme secreted by endothelial cell vessels and plays an important role in the pathogenesis of retinal induce neovascularization. Vascular endothelial cells were cultured and transfected with Matrix Metalloproteinase -9 RNA interference in two doses (100nM or 200nM) and the cells were taken at 24,  48  and  72  hours.  The  expression  of  Matrix  Metalloproteinase  -9  protein  levels  were  examined  by immunocytochemistry, and compared with control group. The data were analyzed with factorial anova. Statistical analysis showed that transfection with different doses of MMP-9 RNAi gave a significant different effect on MMP-9 expression (p-value &lt; 0.05), while exposure time did not give significant difference (p-value &lt; 0.05). This in-vitro study indicated that transfection  of  MMP-9  RNAi  with  dose  200nM  reduced the  expression  of  MMP-9  by  50%  compared  to  the  control. Because exposure time was not significant, the lowest expression of MMP-9 can be achieved by transfecting MMP-9 RNAi 200nM for 24 hours. This is the first study that demonstrates the effectiveness of RNA interference mediated targeting  of MMP-9 to reduce the MMP-9 expression in vascular endothelial cell lines Key Words  : Matrix Metalloproteinase -9 - RNA interference - vascular endothelial cell lines.

Abstract

Introduction: Pseudomonas aeruginosa (P. aeruginosa) is the leading cause of corneal ulcers in children 0 to 3 years of age compared to children in general. Case presentation: A two-months-old infant presented with whitish patches on the right eye two days before admission. A central corneal ulcer with a size of 7-mm x 7-mm accompanied by corneal thinning and melting was shown on the right cornea. It is was surrounded by greyish white creamy infiltrates. Corneal scraping showed Pseudomonas aeruginosa specimens. The cornea became perforate and crystalline lens extrusion was found at the day after intravenous ceftriaxone, levofloxacin eye drop, and cefazoline fortified eye drop administering. It might be caused by bacterial elastase and toxin which contributed to corneal damage. The patient was underwent a multilayer Amniotic Membrane Transplantation (AMT) combined with a pericardial patch graft due to corneal perforation. Two months post-AMT and pericardial patch graft the corneal perforation became entirely heal due to multilayer AMT, despite lysis of the pericardial patch graft. Corneal scar formation and reduction of vitreous opacity in ultrasound examination were shown. The patient was planned to undergo keratoplasty. Conclusions: Corneal ulcers due to Pseudomonas aeruginosa are highly destructive. The levels of infection, diagnostic, and therapeutic are still problems in pediatric patients. Lens extrusion and lysis of the pericardial patch graft are examples in this case. Keratoplasty is the definitive treatment for corneal ulcers with perforation; however, multilayer AMT combined with pericardial patch graft can be used as an alternative therapy to accelerate wound healing, reduce inflammation, and maintain the integrity of the eyeball.

Abstract

Introduction: To present the clinical findings of Autoimmune hemolytic anemia (AIHA) Retinopathy and its rapid resolutions following treatment with steroid. Case report: A 14-year-old female patient presented with decreased vision in the left eye. There was history of AIHA. Visual acuity was 1/60 in LE and 6/6 in RE. There was conjunctival pallor, and the other anterior segment were unremarkable. Fundus examination of left eye revealed flame shaped and dot blot hemorrhage, roth’s spots, optic disc swelling, venous turtuosity, and elevated macula. There were afferent pupil defect, red green deficiency, and contrast sensitivity decline. Hematological evaluation revealed anemia. A MRI Head and Orbital examination were unremarkable. Discussion: This patient was assessed with LE Anemic retinopathy due to AIHA. The patient’s visual acuity improved as the retinopathy resolved after 1 month of oral steroid therapy. Conclusion: Anemia may play a role in the occurrence of retinopathy. The diagnosis of retinopathy can be made by linking ophthalmic fndings with positive serological test. Accurate comprehensive examination can establish the systemic diagnosis, and control of systemic parameters will improve retinopathy, reverse vision loss, and avoid permanent blindness

Abstract

Introduction: To determine the effect of mangosteen (Garcinia mangostana) pericarp extracts on retinoblastoma cell culture proliferation. Methods: This study is a true experimental in vitro design with pre and post-test control group design. Research using retinoblastoma cell culture exposed by mangosteen pericarp extract (Garcinia mangostana) at a dose of 10 µg/ml, 20 µg/ml, and 40 µg/ml. Result: This study used retinoblastoma cell line cultures were obtained from the American Type Culture Collection (ATCC) 10801 University Boulevard Manassas, VA 20110 USA. The samples were divided into 4 groups: control group and 3 groups treated with doses of 10 µg/ml, 20 µg/ml and 40 µg/ml and then incubated for 48 hours, and then examined using MTT Cell Proliferation Assay. Group 1 (10 µg/ml) obtained a decrease of 151.8%, group 2 (20 µg/ml) of 134.6% and group 3 (40 µg/ml) of 134.36%. Conclusion: Mangosteen pericarp extract (Garcinia mangostana) can reduce retinoblastoma culture cell proliferation.

Abstract

Tujuan : Melaporkan kasus manifestasi okuli metastase neuroblastoma pada anak dan tatalaksananya. Metode : Sebuah laporan kasus pasien dengan diagnosa proptosis bilateral oleh karena metastase neuroblastoma dengan komplikasi ulkus kornea.Presentasi Kasus : Seorang anak laki-laki berumur 2 tahun dikonsulkan pada Departemen Oftalmologi dengan kedua bola mata yang menonjol, kemerahan dan keputihan. Dilakukan anamnesa, pemeriksaan umum dan oftalmologi dan juga radiologis (CT Scan dan MRI). Pemeriksaan segmen anterior kedua mata didapatkan ekimosis palpebra, proptosis bilateral, kemosis konjungtiva dan defek epitel kornea dengan defek stroma hanya pada mata kanan. Diagnosa proptosis bilateral oleh karena metastase neuroblastoma dengan komplikasi ulkus kornea ditegakkan. Diberikan antibiotik topikal (Moxifloxacin tetes mata dan Kloramfnikol salep mata), transplantasi membran amnion, air mata buatan, karbonik anhidrase inhibitor (Acetazolamide). Protokol kemoterapi sistemik neuroblastoma (Vincristine, Cyclophosphamid, Cisplatin and Etoposide) juga diberikan oleh dokter anak. Didapatkan perbaikan dengan epitelialisasi total kornea, perbaikan inflamasi kornea dan hilangnya proptosis pada kedua mata. Kesimpulan : Manifestasi okuli pada pasien ini adalah proptosis bilateral berat, ekimosis palpebra, kemosis konjungtiva, defek epitel dan stroma kornea serta ulkus kornea. Tatalaksana holistik dari Departemen Oftalmologi and Pediatri dengan pertimbangan terapi yang tepat dapat memberikan perbaikan klinis dan menjaga penglihatan.

Abstract

Introduction: Dislocated lens can be subluxated (partial) or luxated (complete), which can cause visual impairment in children. Conservative visual rehabilitation is an option for luxated lens treatment. Purpose: This study reports the possibility for visual rehabilitation of bilateral of bilateral luxated lens in children. Case Report: A 3-year-old boy came to the outpatient clinic of Dr. Saiful Anwar General Hospital with complaints of blurred vision on both eyes. The child would always bring objects close to his eyes since the past year. There was no history of ocular injury nor development disorder. Uncorrected visual acuities with LEA symbol on both right and left eyes were 6/114. The best-corrected visual acuity (BCVA) of the right eye was 6/45 and left eye was 6/9 with both S+11.00. Iridodonesis and aphakic lenses were found during slit-lamp examination of both eyes. The intraocular pressures were 14.2 mmHg on the right eye and 17.3 mmHg on the left eye. The lenses were seen in the vitreous cavities during funduscopy and ultrasonography examination. After 1 month of using spectacles, the BCVA of the right eye was 6/18 and that of the left eye was 6/18 with the binocular BCVA was 6/15. Lensectomy with pars plana vitrectomy was planned to prevent complication. Conclusion: Conservative visual rehabilitation is important to prevent amblyopia in children with luxated lenses and surgical treatment is needed to prevent complication. In this patient, aphakic spectacles were given for visual rehabilitation pending operative treatment.
