Abstract

Background: Self-medication is a common approach to managing uncomplicated health issues, including gastrointestinal disorders. Community pharmacists are essential in facilitating successful self-medication by thoroughly evaluating patient information, offering rational suggestions, and delivering clear instructions regarding medication administration. Objective: This study aims to characterize pharmacists' abilities in gathering information on patient symptoms and providing appropriate treatment recommendations for self-medication cases of diarrhea and constipation. Methods: This study employed a cross-sectional methodology accompanied by descriptive analysis. Data was collected using two validated vignette case studies: one for measuring information-gathering competencies and another for assessing the correctness of therapy recommendations. Results: 35 pharmacists participated, most of whom had finished professional pharmacist education (88.6%) and had been practicing in pharmacies for 1 to 7 years (40%). The results showed that 66.0% of pharmacists demonstrated moderate information-gathering skills for diarrhea cases and a lower proportion for constipation cases (45.7%). Only 51.4% of pharmacists could provide correct recommendations for diarrhea and constipation cases. Conclusion: These findings highlight the importance of interventions to enhance pharmacists' roles in providing patients with appropriate, safe self-medication services. Abstract

Background: Nowadays, many small-volume injection products are circulating on the market. Small-volume parenteral (SVP) products are usually designed for treatment purposes that provide systemic effects. If it is not produced strictly, the product will be hazardous and even lifethreatening. Quality assurance is one of the primary tools used to ensure the acceptable performance of SVP products. Quality by design (QbD) represents a systematic strategy for product development that starts with setting a target product quality and emphasizes product process control based on a scientific approach and quality risk assessment. The purpose is to ensure SVP product quality from design to production process. Objective: This review will determine generally Critical Quality Attributes (CQA), Critical Material Attributes (CMA), and Critical Process Parameters (CPP) in terms of explaining the quality assurance of SVP products with a quality-by-design approach. Methods: All articles were obtained by electronic search using ICH guidelines, Science Direct, and Google Scholar. Results: From the literature review, it was found that using a quality-by-design approach through integrated CQA, CMA, and CPP can produce SVP products that comply with QTPP (Quality Target Product Profile) and regulatory requirements. Conclusion: QbD has been established as a valuable scientific approach for ensuring quality assurance within the pharmaceutical industry. Pharmaceutical companies prioritize obtaining regulatory approval before introducing any product to the market. Abstract

Background: Consumer satisfaction with pharmaceutical services in pharmacies has not met expectations. This is due to several factors, such as a lack of communication skills and low levels of empathy among pharmacists. Friendly, polite, and communicative pharmaceutical services are crucial determinants of the quality of pharmacy services. Emotional intelligence enhances pharmacists' ability to empathize, control emotions, and listen actively. Objective: This study aims to identify pharmacists' characteristics and assess their emotional intelligence level in Banjarmasin, South Kalimantan. Methods: This study employed a descriptive quantitative design with a cross-sectional approach. A purposive sampling technique was used, and 30 pharmacists who met the predetermined criteria were selected. The instrument utilized was a questionnaire. Results: The study found that the average age of pharmacists was 28.17 years. The average length of practice as a pharmacist was 4.02 years, and 70% of pharmacists reported being present in the pharmacy daily at specific hours. The emotional intelligence levels among pharmacists showed that 13.3% had low emotional intelligence, 76.7% had moderate emotional intelligence, and 10% had high emotional intelligence. Conclusion: These findings indicate the need to enhance pharmacists' emotional intelligence through training and improving working conditions to support optimal pharmaceutical service quality in pharmacies. Abstract

Background: The body needs antioxidants to reduce the number of free radicals in the body. The anthocyanin compounds in red dragon fruit can be a source of antioxidants to counteract free radicals. Many types of solvents can be used to extract anthocyanins, such as water, methanol, or ethanol. The extracts produced from this process usually contain solvents that can reduce the anthocyanin content in the extract. Objective: Researchers believe that a literature review is needed to find out the factors that affect the isolation of anthocyanin compounds from dragon fruit skin, such as extraction methods, types of solvents, temperatures, and pH. Methods: The methods used in this study were obtained from Google, Google Cendekia, and Pubmed using keywords such as anthocyanin isolation and red dragon fruit. The article used was selected in the range of 10 years, from 2014 to 2024. The inclusion criteria in the search for literature are the articles that explain the anthocyanin isolation method of red dragon fruit and articles are available in Indonesian or English, while the exclusion criteria in this study are articles that are an article review. Results: The methods of maceration, Ultrasonic Assisted Extraction, Microwave Assisted Extraction, and Microwave Assisted Hydro Distillation can be used to isolate anthocyanin compounds using solvents such as citric acid, ethanol, and acetic acid. Conclusion: The ultrasonic assistance method is known to be able to attract the most anthocyanin compounds from red dragon fruit compared to other methods, which is 29,640 mg/5 g. The solvent with the acid environment is more suitable for extracting anthocyanin compounds. Prolonged extraction time can cause a decrease in anthocyanin content, which is associated with anthocyanin degradation by citric acid. The optimal time is 45 minutes with the UAE method. Abstract

Background: Good pharmaceutical service quality can be seen through the level of patient satisfaction. Patient satisfaction is a feeling that arises from the performance of healthcare services obtained after patients compare them with their expectations and their needs are met. Pharmaceutical services still have shortcomings and require continuous improvement to enhance trust and minimize patient disappointment. Objective: The purpose of this systematic review was to identify and gather evidence related to the factors that determine patient satisfaction. Methods: This systematic review follows the guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA). Information collection was conducted through searches on PubMed, Scopus, and Google Scholar from 2018 to 2023, considering eligibility and inclusion criteria. Results: Eight studies have been identified that provide various determining factors of satisfaction, including 10 factors based on patient characteristics: age, gender, education, job, salary, race, marital status, religion, disease status, and visit. Additionally, there were eight determining factors based on pharmaceutical service providers: location, facilities, medication, pharmacy services, working time, waiting time, assurance, and additional non-paid services. Conclusion: The study identified and collected evidence related to the determining factors of patient satisfaction from 2018 to 2023, along with the reasons from each article. The determinants of satisfaction based on these factors can be considered by pharmacists when determining errors that need to be corrected and services that need improvement. Abstract

Background: Eugenia uniflora (family Myrtaceae), a medicinal plant traditionally used for the treatment of infectious and inflammatory diseases, has attracted increasing interest due to its potential antibacterial properties. Objectives: This systematic review aimed to comprehensively evaluate and synthesize published evidence on the antibacterial activity of E. uniflora. Methods: A systematic review was conducted following the PRISMA guidelines by searching Scopus, ScienceDirect, PubMed, and Google Scholar for English-language original studies published between 2015 and 2025 that evaluated the antibacterial activity of Eugenia uniflora. Of the 296 records identified, nine studies met the eligibility criteria and were included in the final analysis. Results: Nine included studies consistently demonstrated antibacterial activity of E. uniflora against both Gram-positive and Gram-negative bacteria. Gram-positive bacteria were generally more susceptible than Gram-negative bacteria, while antibacterial activity varied according to the plant part, extraction method, solvent, and extract concentration. Several studies also reported low minimum inhibitory concentration (MIC) values and broad-spectrum antibacterial activity, supporting the therapeutic potential of E. uniflora. Conclusions: The findings of this systematic review support the antibacterial potential of E. uniflora; however, further standardized and well-designed studies are necessary to confirm its efficacy and support its use and development as a natural antibacterial agent.  Keywords:  Eugenia uniflora; Antibacterial activity; Medicinal plants; Systematic review

Abstract

Background: Parents of children with special needs often experience psychological distress, including anxiety and parenting stress, due to the continuous demands of caregiving and concerns regarding their children's development and future. Long-term psychological burden may adversely affect parental well-being and caregiving quality. Agarwood (Aquilaria malaccensis Lam) is a medicinal plant containing various bioactive compounds, including flavonoids, alkaloids, terpenoids, and essential oils, which have been reported to exhibit sedative, anxiolytic, and relaxation effects. Objective: This study aimed to determine the effect of agarwood aromatherapy on anxiety levels and parenting stress among mothers of children with special needs. Methods: A pre-experimental study with a one-group pretest-posttest design was conducted among mothers of children with special needs selected through purposive sampling. Anxiety levels were assessed using the State-Trait Anxiety Inventory (STAI), while parenting stress was measured using the Parenting Stress Index (PSI). Agarwood aromatherapy was administered through inhalation during the intervention period. Data were analyzed using descriptive and inferential statistical methods with a significance level of p < 0.05. Results: The results showed that agarwood aromatherapy did not significantly reduce anxiety levels measured by STAI (p > 0.05). Most participants remained in the moderate anxiety category throughout the intervention period. However, a significant reduction in parenting stress scores was observed after the intervention (p < 0.05). Conclusion: Agarwood aromatherapy was effective in reducing parenting stress but did not significantly affect anxiety levels among mothers of children with special needs. Therefore, agarwood aromatherapy may serve as a complementary intervention to improve parental psychological well-being. Abstract

Background: Small traditional medicine enterprises must implement Good Manufacturing Practice for Traditional Medicines (GMP-TM) to ensure that the resulting products meet established safety, quality, and efficacy requirements. The successful implementation of GMP-TM must be supported by the competence of the human resources involved. Adequate knowledge is expected to foster more good attitudes toward work standards or policies. However, research specifically examining this matter remains lacking. Objective: To determine the relationship between employees' knowledge and attitudes regarding GMP-TM at a small-scale traditional medicine enterprise. Methods: An analytical observational study using a cross-sectional design involving 30 participants (total sampling). The instrument used was a questionnaire, and data analysis was performed using Spearman's test. Results: All respondents had a good level of knowledge regarding the topic under study, namely 30 respondents (100%). A total of 28 respondents also demonstrated a very good attitude (93.33%), while 2 respondents (6.67%) had a good attitude. The Spearman correlation test yielded a p-value of 0.006 (p<0.05) and an r-value of 0.492. Conclusion: There is a significant relationship between the respondents' level of knowledge and attitudes, with a moderate strength of relationship and a positive direction.

Abstract

Background: Herbal products require a proper quality assurance system to ensure consistent manufacturing and compliance with safety and quality standards. The quality assurance system is crucial for overseeing the entire production process, starting from the acquisition of raw materials to the distribution of finished products. Objective: This research aims to assess the implementation of a quality assurance system in the manufacturing of herbal products at PT. X based on CPOTB. Methods: This study employed a qualitative descriptive approach using a case study methodology. Data were collected through observations, interviews, and document reviews, which included standard operating procedures, batch production records, testing documents, equipment logbooks, and quality control records. The assessment focused on the quality management system, raw material oversight, staff, infrastructure and machinery, manufacturing methods, warehousing, and record-keeping. Results: The findings indicated that PT. X has established a cohesively integrated quality assurance system by fostering collaboration between the QA and QC divisions, despite facing challenges related to dual functional roles in the absence of a separate R&D department. Cross-contamination risks are addressed from both facility and production perspectives through the implementation of a structural three-zone workflow (Black, Green, and Gray Areas), supported by integrated HEPA filtration and Reverse Osmosis (RO) water systems. Additionally, quality control in the industry is rigorously maintained through strict adherence to instrument logbook regulations, effective batch-tracking methods for addressing complaints and product recalls, and a thorough and organized documentation system. Conclusion: PT. X requires strategic enhancements in structural aspects concerning personnel dual roles and the modernization of equipment. However, its comprehensive documentation, standardized internal audits, and strong environmental controls indicate that the facility's quality management aligns effectively. Abstract

Background: Hypertension is a prevalent chronic disease and a major cause of cardiovascular morbidity and mortality. Flavonoid-containing medicinal plants have attracted attention as potential natural antihypertensive agents. Objective: This review summarizes the antihypertensive potential of flavonoid-containing medicinal plants and their underlying mechanisms. Methods: A narrative literature review was conducted using PubMed and Google Scholar. Original articles published between 2015 and 2024 were selected based on predefined inclusion and exclusion criteria. Eligible studies investigated flavonoid-containing medicinal plants using in silico, in vitro, ex vivo, or animal models. Ten studies were included for qualitative analysis. Results: The reviewed plants contained flavonoids, including quercetin, kaempferol, luteolin, and rutin, which exhibited antihypertensive potential through ACE inhibition, enhanced nitric oxide bioavailability, and antioxidant activity. Evidence varied across molecular docking, laboratory, and animal studies. Conclusion: Flavonoid-containing medicinal plants are promising preclinical antihypertensive candidates; however, further pharmacological and clinical studies are required to confirm their efficacy and safety.
