Abstract

Background: Tuberculosis is an infectious disease with a high prevalence rate in Denpasar City. Medication adherence affects increased tuberculosis cases in Denpasar City. Medication adherence in tuberculosis patients is associated with a low attitude toward treatment, so it is necessary to make efforts to improve treatment attitudes and medication adherence of tuberculosis patients by providing health education. Online-based education with Google Meet can be a solution in delivering health education with virtual face-to-face features, so education delivery becomes more effective. Objective: This study aims to determine the effect of online-based education on treatment attitudes and medication adherence of tuberculosis patients at the Denpasar City Health Center. Methods: The pre-experiment study used one group pretest-posttest design that was conducted from February until April 2022 at several public health centers in Denpasar City. The total samples of 30 tuberculosis patients accorded the inclusion criteria were taken by purposive sampling. A questionnaire was used to measure treatment attitudes, where online-based education is given a week after the pre-test and the post-test is given 2 weeks after online-based education. Pill-count method was used to measure medication adherence. Wilcoxon analysis was used to analyze research data. Results: Wilcoxon's analysis showed a significant increase in treatment attitudes and medication adherence of tuberculosis patients before and after online-based education with google meet (p<0.05). Conclusion: Online-based education with google meet provides an increase in treatment attitudes and medication adherence of tuberculosis patients at the Denpasar City Health Center. Abstract

Background: One of the skin problems that are often experienced by teenagers today is acne. Acne vulgaris or acne is a chronic inflammatory disease of pilosebaceous follicles which is characterized by the formation of papules, pustules, or nodules. Objective: This study aims to provide an accurate description of the use of topical antibiotics in acne vulgaris patients and to find out which drugs are often used as self-medication for acne vulgaris. Methods: This research is included in a descriptive research design. Data collection was done by direct observation and recorded on the checklist sheet for Acne Vulgaris patients who did selfmedication at the pharmacy. Results: Judging from the evaluation of the accuracy of the use of topical antibiotics in AV patients at the Denpasar Regional Pharmacy, the results obtained were 53 patients, or 92.98% of patients included in the right indication group, 57 patients, or 100%, included in the right dose group, 54 patients or 94.74% included in the right group for the duration of administration, 57 patients or 100% were included in the appropriate time interval group for administration and 100% of patients were included in the side effect alert group. The accuracy of giving antibiotics, which is included in the category of inappropriate indications by 5.71% of respondents, and inappropriate duration of administration by 5.08% of respondents. The drugs used for AV therapy are Topical Retinoids, Mediklin Gel, Mediklin TR, and Erymed. The drug most often given by pharmacists is topical antibiotics, namely Mediklin TR from the Semisynthetic Lincosamide group, as much as 38.57%. Conclusion: There is still suitability for prescribing topical antibiotics in acne vulgaris seen from its severity. Keywords: Acne; Evaluation; Topical antibiotics

Abstract

Background: Telepharmacy appears to be a technology that represents a unique and innovative way to deliver quality pharmaceutical services to rural and regional areas in particular. Providing appropriate pharmaceutical services to patients and conducting consultations with other health workers have an important role in reducing the use of inappropriate and less effective drugs. The development of telepharmaceutical services can be influenced by the knowledge and practice of the community. Objective: to determine the relationship between knowledge and community practice on the use of telepharmaceutical services in Tabanan City. Methods: This research is an observational study using a cross-sectional design. The sample in this study amounted to 100 people who met the inclusion and exclusion criteria. The sample was taken from the population using a purposive random sampling technique. The research instrument used in this study was a questionnaire (questionnaire), collecting data formally for the subject to answer questions in writing. Data were analyzed using computer software, namely the Chi-square test. Results: The respondents who have sufficient knowledge, as many as 55 people have sufficient behavior, and as many as 42 people, while those who have good behavior, 13 people and respondents who have good knowledge, 45 people have sufficient behavior, as many as 23 people and behavior well as many as 22 people (p-value <0.05). There is a relationship between knowledge and practice regarding the use of telepharmaceuticals in the community of Tabanan City. Abstract

Background: Xylocarpus granatum has been used traditionally by coastal communities to treat various diseases. It is known that this plant contains secondary metabolites with various pharmacological activities, including as an antibacterial. Objective: This review article aims to provide information regarding the potential antibacterial activity of banangbanang plants and to summarize the content of compounds that have antibacterial properties and their mechanism of action. Methods: The preparation of this article is through literature studies from various international journals and national journals obtained online by taking into account predetermined inclusion and exclusion criteria. Then it was selected and studied further to obtain data related to the antibacterial activity of banangbanang plants and the content of secondary metabolites that have potential as antibacterials. Result: The banang-banang plant, especially the leaves, roots, fruit, seeds, fruit flesh, fruit peels, stems and bark with its secondary metabolites can inhibit several bacterial species such as Staphylococcus epidermidis, Pseudomonas aeruginosa, Escherichia coli, Vibrio alginolyticus, Staphylococcus aureus, Shigella boydii , Proteus spp., Streptococcus pyogenes, Ralstonia solanacaerum, Propionibacterium acnes, Agrobacterium tumefaciens, Aspergillus paraciticus, Bacillus subtilis, Candida albicans, Pseudomonas fluorescence, Micrococcus luteus, Saccharomyces ceresiviae, Salmonella typhi, Vibrio alginoliticus and Aeromonas hydrophilla. The secondary metabolites of this plant that have potential as antibacterial are tannins, saponins, steroids, phenols, triterpenoids, flavonoids, alkaloids, terpenoids and glycosides which have their respective mechanisms of action as antibacterial agents. Conclusion: Secondary metabolites contained in each part of the Xylocarpus granatum plant are thought to have a role in its antibacterial activity. Abstract

Background: One of the most important hospital management in the provision of health services as a whole is drug management. Remember the important position of drugs for hospitals; their management must be carried out effectively and efficiently because if they do not, they can have a negative impact on hospitals, both medically, socially and economically. Drug management is crucial in the drug management cycle to determine drug stocks that suit the needs of health services with guaranteed quality and can be obtained when needed. One of drug management is by controlling expired drugs. Medicines that have passed their expiry date can be dangerous because they reduce the stability of the drug and can cause toxic effects. Objective: This study aims to describe drug management in terms of expired drug profiles and determine the factors that influence the number of expired drugs in "X" Hospital. Methods: Research in the form of observational research with a descriptive approach. The samples used are all expired drugs in the warehouse and have not been destroyed. Factors affecting the number of expired drugs were observed by interview. Results: The results of the study show that the percentage of expired drugs in the period 2019 to 2022 is obtained from the loss value and inventory management value of 0.11%, respectively; 0.18%; 0.14%; and 0.93%. Expired drugs were also classified based on the ABC analysis, with the highest number in group C with 137 items; in the VEN analysis, the essential medicines group with the highest number of 122 items; and in the ABC-VEN combination analysis, the EC group occupied the highest rank. The existence of expired drugs is caused by several factors, including the influence of information systems, human resources and methods of recording drugs that are still manual. Conclusion: The management of expired drugs in the pharmaceutical installation of "X" Hospital is quite good because the percentage is still under control because it is less than 2%. Abstract

The high prevalence of cervical cancer patients and the variety of necessary expenses needed causing financial hardship to both the patient and hospital. Furthermore, the disparity between the INA-CBGs rates and the costs of treatment incurred by hospitals and the limited information regarding the costs of cervical cancer undergoing cisplatin paclitaxel chemotherapy as viewed from a societal perspective were the reasons to conduct this research. The aim of this study was to determine the cost of squamous cell cervical cancer patients at stage IIB-IIB who received cisplatin paclitaxel chemotherapy from a social perspective. So in this study the difference in costs is used to calculate costs from a hospital perspective.This research was an observational study with a prospective incidence approach and consecutive sampling as the sampling method. This research was conducted from January to June 2017 at Sanglah General Hospital, Denpasar. Cost data from a societal perspective for six chemotherapy series were obtained from the Claim Guarantee Installation, Fund Mobility Section, interviews and filling in the logbook by patients and then calculated using the micro-costing method. The results of this study obtained five patients who met the inclusion criteria. From the perspective of BPJS Kesehatan, the average cost of cervical cancer with paclitaxel cisplatin chemotherapy was IDR 24.703.470,00. From a hospital perspective, the average cost of cervical cancer with paclitaxel cisplatin chemotherapy was IDR 6.619.204,00., and from the perspective of patients and their families, the average burden of costs for cervical cancer with paclitaxel cisplatin chemotherapy was IDR 9.149.300,00. Cost analysis on five squamous cell cervical cancer patients with stage IIB-IIIB with six series of paclitaxel cisplatin chemotherapy viewed from the perspective of BPJS Kesehatan, hospital was IDR 24,703,470.00, IDR 6,619,204.00, and IDR 9,149,300.00, respectively. Abstract

Background: Endophytic fungi are known as a producer of a myriad of bioactive natural products, including those originally produced by the host plants. Huperzine A (Hup A), a lycopodium alkaloid, which was reported as an active acetylcholinesterase inhibitor (AChEI) agent used for the treatment of Alzheimer’s disease, was originally isolated from medicinal Chinese herbs Huperzia serrata. Moreover, various species of endophytic fungi associated with H. serrata and other species within Huperziaceae plants were found capable of producing Hup A, suggesting these microorganisms as promising sources of Hup A. Objective: This review aimed to summarize the evidence of Hup A produced by various endophytic fungi as part of efforts to assess alternative producer of Hup A. Methods: Scientific articles on endophytic fungi producing Hup A published from 2000 until 2022 were screened through scientific databases. Results: Thirty-two endophytic fungal strains belonging to fifteen fungal genera were documented as capable of producing Hup A. These fungal endophytes were isolated from Huperziaceae plants, H. serrata, Phlegmariurus phlegmaria and Phlegmariurus taxifolius. Conclusion: We summarize herein the capability of endophytic fungal strains associated with Huperziaceae plants to produce Hup A, an active AChEI agent, which could be considered as an alternative producer of Hup A on a larger scale. Abstract

Backgrounds: Cyanidin and peonidin, anthocyanin compounds, have many in silico, in vitro, and in vivo activities, including antioxidant, anticancer, antihyperlipidemic, and antidiabetic. Toxicity testing is carried out to determine the potential hazard that may be produced by the test compound. Objective: This study was aimed to determine the in silico toxicity of anthocyanins (cyanidin and peonidin) using Toxtree v3.1.0 software. Methods: In silico toxicity testing was carried out using 2D structures of cyanidin and peonidin with Cramer rules, Verhaar scheme, Benigni/Bossa rulebase, Kroes TTC decision tree, Eye Irritation/Corrosion and Skin Irritation/Corrosion parameters. Data analysis on the results of the tested toxicity parameters was carried out descriptively. Results: The results showed that the two compounds have the same category for the toxicity parameters of Cramer Rules (class III), Kroes TTC Decision Tree (substance would not be expected to be safety concern), Benigni/Bossa Rulebase (Negatif for genotoxic and nongenotoxic carcinogenicity), Eye Irritation/Corrosion and Skin Irritation/Corrosion (not irritating or corrosive). Different results are shown in the parameters of the Verhaar Scheme, where cyanidin is included in class 5 (cannot be classified based on this parameter), while peonidin is included in class 1 (narcosis or basic toxicity). Conclusion: Based on in silico toxicity, cyanidin and peonidin have a chemical structure that has the potential for toxicity, but these compounds are neither potentially genotoxic nor non-genotoxic carcinogenicity, and are not potentially toxic to the skin and eyes. The toxicity mechanism of cyanidin cannot be classified based on the test parameters while peonidin is narcosis or basic toxicity. Keywords: Anthocyanin; Cyanidin; Peonidin; Toxicity; In Silico

Abstract

Background: Purple sweet potato (Ipomoea batatas L.) is a crop that contains rich amounts of anthocyanins and flavonoids. For maintaining its quality in preparations, fingerprint analysis using Thin Layer Chromatography (TLC) is commonly used. Objective: In this study, image analysis was implemented on the chromatogram to develop fingerprint profiles of the leaves of various types of purple sweet potatoes. Methods: Fingerprint analysis and acquisition of data were carried out by combining modern TLC equipment with software for image analysis. The in-situ fingerprint analysis using WinCats was compared with the image analysis results obtained in this study. The chemometric technique, cluster analysis, was performed to measure their difference in terms of sample classification. Cluster analysis was carried out to confirm that the clusters formed from image analysis were the same as those from WinCats. Results: The results obtained were two dendrograms, both showing the formation of two clusters, but with a different order of samples. This study concluded that the data obtained from image analysis using ImageJ software resulted in clustering with a similarity of 38.15%, while the data obtained from WinCats software resulted in classification with a similarity of 28.31%. Both methods had issues in determining the fingerprint profiles, as evidenced by the low level of similarity. Conclusion: These results indicate that image analysis has the potential to be implemented for fingerprint profile development. However, further analysis is needed to align the Rf values and confirm the compounds in the peaks. Abstract

Background: Tuberculosis is an infectious disease caused by Mycobacterium tuberculosis. The treatment behavior of tuberculosis patients plays an important role in preventing transmission and recovery of tuberculosis patients, if the treatment behavior of tuberculosis patients is not good it can cause drug resistance, recurrence of tuberculosis disease, even death. Objective: to describe the treatment behavior of tuberculosis patients in the Tabanan Community Health Center area. Methods: This research is a type of quantitative descriptive research conducted in April 2023 at Tabanan Health Center. Measurements were carried out using TPB (Theory of Planned Behavior) questionnaires. Results: This study presents that most respondents had attitudes with enough categories , while the fewer categories amounted and the good category amounted. Conclusion: We found that 57% of respondents suffering from tuberculosis at the Tabanan Health Center had adequate behavior. There is a need for education for tuberculosis patients to prevent and overcome tuberculosis transmission and increase knowledge about tuberculosis infectious diseases either with booklets, leaflets, or animated videos.
