Abstract

Medical decision-making in the Emergency Department (ED) is often coloured by ethical dilemmas and the need for doctor discretion due to the pressure of critical situations, time constraints, and the condition of patients who are often unable to give consent to medical action. Biomedical ethical principles such as beneficence, non-maleficence, autonomy, and justice must be carried out in a balanced manner, while doctors are also required to comply with legal standards and operational procedures. In an emergency, doctors have a moral and legal obligation to provide immediate help to save lives or prevent disability, even without informed consent, as long as the action is professionally accountable and in accordance with medical standards. However, the application of this discretion still has the potential to cause ethicomedicolegal problems, especially if communication and documentation are not carried out optimally. Therefore, strengthening the understanding of ethics, crisis communication training, and the preparation of clear ethical protocols are needed so that doctors can make decisions that are professional, accountable, and orientated towards patient safety in the emergency room.

Abstract

This study discusses the relationship between professional ethics and discretionary practices in the implementation of informed consent in hospitals using a literature review method. Informed consent is a fundamental right of patients rooted in the principles of autonomy, beneficence, and non-maleficence, and must be fulfilled by health workers in every medical action. However, in practice, healthcare professionals often face situations that require discretion, such as emergency conditions or communication limitations, forcing them to balance compliance with ethical standards and situational demands. Professional and measured discretion, while adhering to legal regulations and professional ethical codes, can provide legal protection for healthcare professionals while safeguarding patients' rights. This study emphasises the importance of a deep understanding of professional ethics, clinical communication training, and transparent documentation to ensure that discretion remains within the ethical and legal boundaries applicable in hospitals.

Abstract

Team-based healthcare requires strong ethical collaboration and discretion among healthcare professionals to ensure patient quality and safety. Effective interprofessional collaboration is built through open communication, mutual respect, clarity of roles, and shared responsibility among healthcare professions. However, practices in the field often face challenges such as differences in competence, professional hierarchy, communication barriers, and resource limitations. Discretion in the delegation of authority among healthcare professionals is a critical aspect that must be carried out ethically, taking into account legal boundaries, competencies, and applicable operational standards. This literature review emphasises that the proper application of principles of collaborative ethics and discretion can improve the quality of care, strengthen professional relationships, and create a harmonious and safe work environment for patients. Sustained efforts in interprofessional education, regulatory strengthening, and the development of a collaborative work culture are key to maintaining ethical collaboration in healthcare settings.

Abstract

The implementation of nursing practice standards in Indonesia has been regulated in various laws and regulations, such as Law No. 38 of 2014 concerning Nursing and its implementing regulations. These standards aim to ensure the quality of nursing services, patient safety, and legal protection for nurses and the community. However, implementation in the field still faces various challenges, such as disparities in nurses' competencies, limited facilities, insufficient supervision, and an inadequate documentation and evaluation system. Efforts to improve implementation include strengthening education and training, digitising nursing documentation, harmonising regulations, and fostering cross-sectoral collaboration between the government, professional organisations, and educational institutions. With shared commitment and adaptive regulatory updates, the implementation of nursing practice standards is expected to run optimally and sustainably, thereby enhancing the quality of healthcare services in Indonesia.

Abstract

Standard Operating Procedures (SOPs) are essential guidelines for ensuring the quality and safety of healthcare practices in hospitals. This study aims to examine the role of SOPs in improving service quality and patient safety through a review of literature from various scientific sources. The findings indicate that consistent implementation of SOPs can create a structured, efficient, and accountable work system, thereby minimising the risk of medical errors and enhancing service accountability. Additionally, SPO serves as a control tool in the monitoring, evaluation, and fulfilment of hospital accreditation standards. However, the effectiveness of SPO implementation is significantly influenced by factors such as healthcare staff compliance, management support, training, and the availability of adequate resources. Therefore, optimising SPO implementation requires the commitment of all hospital elements to achieve sustainable improvements in healthcare quality and safety.

Abstract

The police profession is known for its high levels of occupational stress and psychological risk. Exposure to traumatic events and institutional pressure makes police personnel vulnerable to stress and burnout. This community service activity aims to enhance the psychological resilience of members of the Metro Bekasi City Police through logotherapy-based psychoeducation. Logotherapy, an existential approach, emphasizes the search for life’s meaning as a source of motivation and mental endurance. The method used was an interactive lecture supported by a psychoeducational module developed from Viktor Frankl’s theory. The materials covered the basic concepts of logotherapy, the principle of meaning in suffering, and practical strategies for maintaining psychological resilience. Evaluation was conducted through pretests and posttests to assess participants’ understanding. The results indicated an increase in participants' awareness of the importance of values and meaning in coping with occupational stress. This psychoeducational intervention not only improved knowledge but also served as a preventive strategy for police mental health development. Logotherapy proves to be a relevant humanistic approach that empowers officers to view their duties as meaningful life missions.

Abstract

Pressure ulcers are a common complication in patients with immobilisation in the Intensive Care Unit (ICU). One effective prevention strategy is the provision of turning positions or periodic changes in body position. This article aims to systematically review various studies evaluating the effect of turning positions on the incidence of pressure ulcers in ICU patients. The research method used was a systematic literature review of databases such as Google Scholar, Garuda, and PubMed using the keywords ‘pressure ulcers,’ ‘mobilisation,’ and ‘position changes.’ The selected articles were empirical journals published in the last 10 years, written in Indonesian or English, and relevant to the focus on pressure ulcer prevention. The selection process followed the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) framework. From 50 identified articles, 10 were selected for analysis (7 empirical studies, 2 case studies, and 1 literature review). The most commonly used interventions were position changes every 2 hours, repositioning mobilisation, progressive mobilisation, and log roll techniques. All studies demonstrated a significant reduction in the risk and incidence of pressure ulcers, as evidenced by improved Braden or Norton scores and reduced pressure ulcer severity (p < 0.05). Based on these findings, mobilisation and position changes were proven effective as preventive interventions for pressure ulcers. Actions such as repositioning, lateral positioning, early mobilisation, and log roll should become standard nursing practice in ICUs and inpatient wards.

Abstract

Background: Patient Safety in Hospitals (PSH) is a system designed to ensure patient safety during the provision of healthcare services. This system encompasses six safety objectives, one of which remains a serious concern: patient falls, particularly among inpatients. The objective of this study is to analyse nurses' compliance with Standard Operating Procedures (SOPs) for fall risk prevention in relation to fall risk prevention efforts in hospitals. This research used a literature review design, searching for literature in three databases, namely Google Scholar, Pubmed, and Sciencedirect, using the keywords ‘nurse compliance,’ ‘standard operating procedures,’ and ‘fall risk.’ The articles used in this literature review were published in the last five years, from 2020 to 2025. Based on the screening results, 10 articles were found to meet the criteria relevant to the research question. The findings of several studies revealed that nurse compliance is influenced by various internal and external factors that are interrelated. Knowledge level, age, and workload significantly influence nurse compliance in implementing fall risk prevention SOPs. Nurses with good knowledge and controlled workloads tend to be more compliant in performing procedures. The conclusion of this study is that nurses' compliance in implementing SOPs for fall risk prevention is the result of a complex interaction between knowledge, attitude, work experience, caring, motivation, and organisational and facility support. Although SOPs are available, their implementation is often not optimal due to various structural and personal barriers.

Abstract

Background: Phlebitis is one of the most common complications of intravenous catheterisation that can affect patient safety and comfort. One of the main causes of phlebitis is nurses' non-compliance with aseptic technique principles during intravenous catheterisation procedures. The high incidence of phlebitis in various hospitals indicates the need for evaluation of the implementation of aseptic techniques by healthcare workers. Objective: This study aims to identify and synthesise research findings related to the relationship between aseptic techniques in intravenous catheterisation and the incidence of phlebitis in hospitalised patients. Method: This literature review examined ten research articles published between 2017 and 2024, obtained from various institutions and national journals. Inclusion criteria included quantitative studies with observational or experimental designs that examined the relationship between aseptic techniques and phlebitis. Data were extracted and synthesised narratively and presented in tabular form. Results: All ten studies reviewed demonstrated a significant association between aseptic techniques and the incidence of phlebitis, with a p-value < 0.05. Factors such as nurse compliance with six-step hand washing at five moments, use of sterile personal protective equipment (PPE), skin disinfection before puncture, and infusion placement according to SOP were proven effective in reducing the incidence of phlebitis. The study also showed that the location of placement, catheter size, and infusion duration contributed as risk factors. Conclusion: The consistent application of aseptic techniques in accordance with procedures has been proven to reduce the incidence of phlebitis. Continuous education, monitoring, and audits of nurses' compliance with aseptic procedures are highly recommended to improve the quality and safety of nursing care.

Abstract

The quality of the environment in which children live significantly impacts their respiratory health. In densely populated areas, children often face challenges such as poor air circulation at home and constant exposure to cigarette smoke from the environment or family members. This unhealthy environment plays a significant role as a major risk factor for high cases of respiratory diseases, including pneumonia, in children. This study aims to describe the procedure for implementing clapping in children with pneumonia, analyze the challenges faced in its implementation, and evaluate the effectiveness and patient response to this intervention. The research method used a literature review and case report analysis. The evaluation results show that providing clapping systematically and integratedly can reduce respiratory frequency, reduce rhonchi sounds, improve oxygen saturation, and increase patient comfort and activity. Therefore, the success of therapy is greatly influenced by the skills of the physiotherapist, close clinical monitoring, education to the family, and parental commitment in supporting the home program.
