Abstract

Acute myocardial infarction is one of the leading causes of death in cardiovascular disease cases. In addition to pharmacological therapy, non-pharmacological nursing interventions play an important role in helping to maintain the patient's hemodynamic stability. This case study aims to assess the effectiveness of applying a 30-degree head-up position in improving hemodynamic conditions in myocardial infarction patients treated in the emergency room at Ibnu Sina YW UMI Hospital in Makassar. The research subject was a 60-year-old man with a medical diagnosis of myocardial infarction accompanied by nursing problems in the form of decreased cardiac output. The interventions provided included monitoring vital signs, oxygen therapy, education on healthy lifestyles, and setting the head up 30 degrees. After 10 minutes of intervention, there was an improvement in hemodynamic parameters, marked by an increase in oxygen saturation from 93% to 98%, blood pressure from 101/46 mmHg to 110/50 mmHg, and an improvement in respiratory rate from 12 breaths/minute to 18 breaths/minute. However, peripheral perfusion did not show optimal improvement, as evidenced by a CRT of more than 3 seconds and decreased level of consciousness. The 30-degree head-up position can be a simple, effective, and safe non-pharmacological measure to improve oxygenation while helping to maintain hemodynamic stability in myocardial infarction patients. Even so, this intervention still needs to be combined with other medical therapies in order to achieve maximum results.

Abstract

Work stress is a common problem in the modern work environment and can have an impact on employees' mental, physical, and productivity health. Various efforts are made to manage stress, one of which is through non-pharmacological approaches such as aromatherapy. This study aims to analyze the use of lavender aromatherapy in reducing work stress in employees of the North Sumatra Indonesian Broadcasting Commission Office. The method used is qualitative research with a case study approach to explore in depth the experiences and perceptions of informants. The data collection technique was carried out through in-depth interviews, observations, and documentation studies, with the selection of informants using purposive sampling. Data analysis uses the Miles and Huberman model through the stages of data reduction, data presentation, and conclusion drawn. The results showed that most of the informants understood the benefits of lavender aromatherapy as a relaxation and stress-reducing effect. The use of aromatherapy has a positive impact on work comfort, such as increasing a sense of calm, freshness, and focus, and creating a more conducive work environment. However, the response to scent is subjective. Overall, lavender aromatherapy can be an effective alternative in managing work stress in a nonpharmacological way.

Abstract

The implementation of health information systems (HIS) in primary care facilities faces significant challenges relating to the digital readiness and competence of healthcare workers, with multidimensional barriers in both social and technical aspects. This article aims to synthesise empirical evidence regarding the digital readiness and competence of healthcare workers in the implementation of HIS in primary care facilities, focusing on socio-technical challenges and capacity-building strategies. The method employed was a literature review. The findings indicate that the identified socio-technical challenges include digital literacy gaps between professions, resistance to change, limitations in internet infrastructure (9.45 per cent of community health centres (Puskesmas) lack access) and electricity supply, system fragmentation (an average of 30 platforms per facility), interface designs that do not align with clinical workflows, and the absence of ongoing technical support. Capacity-building strategies encompass 12 integrated interventions at the individual level (competency frameworks, tiered training, mentoring), the organisational level (transformational leadership, workflow redesign, a culture of learning), and the system level (infrastructure investment, interoperability standards, curriculum integration, technical helpdesks, and sustainable policies). Consequently, the digital readiness and competence of healthcare staff are critical determinants of the successful implementation of health information systems in primary care facilities. Digital transformation requires a holistic approach that balances technological investment with the sustainable capacity-building of human resources.

Abstract

Indonesian hospitals face a dual challenge in the era of the National Health Insurance (JKN) and the Standard Inpatient Care Class (KRIS): financial pressure resulting from the INA-CBGs prospective payment scheme, which often fails to cover the actual costs of services, and the need for significant infrastructure investment to comply with the 12 KRIS criteria that must be implemented by 30 June 2025 at the latest. This article aims to synthesise empirical and conceptual evidence regarding the integration of the Balanced Scorecard (BSC), Lean Hospital and digital transformation as a comprehensive strategic management approach for hospitals in addressing these dual challenges. The method used was a literature review. The findings indicate that the BSC offers a balanced strategic framework through four interrelated performance perspectives (financial, customer, internal business processes, and learning and growth) linked by a logical causal chain, with internal process efficiency serving as the primary driver of financial performance and service sustainability. The implementation of Lean Hospital in priority areas such as inpatient workflows, bed management, JKN claims processing, and pharmaceutical logistics has been empirically proven to significantly reduce waste (by up to 70% in some studies), speed up service delivery times, and improve operational efficiency without compromising clinical quality. Digital transformation, driven by the mandate for Electronic Health Records (EHRs), the integration of the SATUSEHAT platform, and the electronic claims policy commencing in August 2026, serves as a critical enabler that strengthens the BSC–Lean synergy through the provision of real-time data, process automation, and predictive analytics for evidence-based decision-making. The integrated BSC–Lean–Digital model enables hospitals to translate abstract strategies into measurable operational actions, with concrete examples such as reducing the backlog of claims through standardised coding and electronic claims automation, or improving KRIS compliance through the redesign of the admission–transfer–discharge workflow supported by a ward capacity alert system. Hospitals that successfully adopt this integrated approach will gain a competitive advantage in the form of improved cost efficiency, higher quality of care, increased patient satisfaction and greater financial resilience in the face of policy fluctuations. Conversely, hospitals that fail to adapt risk chronic claims deficits, regulatory non-compliance, a decline in service quality, and a loss of trust from the public and funding partners.

Abstract

Optimising hospital management is a strategic imperative in meeting the demands for improved quality of healthcare services, operational efficiency and financial sustainability in the era of digital transformation. This literature review aims to synthesise empirical evidence regarding the role of governance innovation, human resource development (HRD) and clinical process transformation in enhancing organisational performance and the quality of healthcare services. The results of the synthesis indicate that the application of the principles of Good Hospital Governance (GHG) forms the cornerstone for creating a management system that is responsive to the dynamics of change in the external environment. The digitalisation of asset management, ERP-based performance appraisal systems, and the integration of AI into regulatory documentation significantly enhance operational efficiency and accountability. HRD through effective education, training and work motivation, supported by a high level of digital readiness, is a determining factor in the successful implementation of innovations. Clinical process innovations integrated with digital technology—including evidence-based clinical guidelines within electronic health records, digital patient safety interventions, and AI-assisted clinical decision-making—consistently demonstrate a reduction in medication errors, improved protocol adherence, and faster service delivery times. The efficiency of internal business processes based on clinical standards supports the sustainability of services within prospective payment systems such as the National Health Insurance (JKN). The conclusion of this review is that the optimisation of hospital management requires synergy between strong governance, competent and digitally-ready human resources, and standardised, technology-supported clinical processes within an organisational culture oriented towards continuous learning.
