Abstract

Electronic medical records that occur are a threat to hospitals, because they can cause problems with electronic medical records. The purpose of this study was to identify the risks in the inpatient electronic medical records using the FMEA method at the Islamic Hospital of Banjarmasin. This research method uses quantitative. The risk of incomplete data on inpatient electronic medical records using the FMEA method at Banjarmasin Islamic Hospital can find out failures that occur by calculating the RPN in the NIK section with a score of 270 in the high category, Name with a score of 81 in the very low category, Age with a score of 42 very disturbing, and Address a score of 30 is very annoying. This occurs because there is no SOP regarding data completeness, and there is no system that can control when the patient's identity is not filled in completely in the inpatient electronic medical record. The risk of inaccuracy of data in the inpatient electronic medical record using the FMEA method at Banjarmasin Islamic Hospital can identify failures that occur by calculating the RPN in the NIK section with a score of 900 in the very dangerous category, Name score 90 in the very low category, Age score 35 slightly disturbing, and Address score 25 is a bit annoying. This occurs as a result of the absence of an SOP regarding data accuracy, and the absence of a system that can recognize when the patient's identity is incorrect or the need for quantitative analysis to find out inaccurate data in the inpatient electronic medical record. The risk of data duplication in the inpatient electronic medical record using the FMEA method at Banjarmasin Islamic Hospital can find out failures that occur by calculating the RPN in the NIK section with a score of 270 in the high category, Name score 243 in the medium category, Age score 54 in the low category, and Address score 54 with low category. This happens due to the absence of an SOP regarding duplication of data, and the absence of a support system when inputting data that fails to be stored, so it cannot be re-entered. In order to avoid duplication of data in the inpatient electronic medical record.

Abstract

The medical record form is used in carrying out the process of recording and processing data in the patient's medical record. The information system on the contents of the outpatient registration form implemented by RSUD dr. H. Moch Ansari Saleh Banjarmasin is guided by or still follows the Regulation of the Minister of Health Number 269/Menkes/Per/III/2008 Article 3 concerning the contents of medical records. This study aims to analyze the information system for outpatient registration forms at dr. H. Moch Ansari Saleh Hospital Banjarmasin in 2023. This type of research uses descriptive qualitative research with data collection methods in the form of observation and interviews with 1 registration officer and head of the medical record unit related to analysis registration form information system. The results of the study are based on anatomical aspects, physical aspects and content aspects. The content aspects are guided or still follow the Regulation of the Minister of Health Number 269/Menkes/Per/III/2008 so that the variables and meta data need to add value to the item name of parents, gender , religion and marital status. And on the user interface for the ease and usefulness of using electronic forms. Based on the results of the study, it can be concluded that dr. H. Moch Ansari Saleh Banjarmasin Hospital to adjust the contents of the registration form is guided by the Decree of the Minister of Health of the Republic of Indonesia Number HK.01.07/MENKES/1423/2022 concerning Guidelines for Variables and Meta Data in the Implementation of Electronic Medical Records. aspects.

Abstract

Tennis elbow characterized by the emergence of pain around the elbow that often occurs in tennis players causing the tendons extensor wrist length epicondylus lateral and radiohumeral joint pain from gripping. This study aims to find out the description range of motion elbow joint in suffererstennis elbow in PB. Lalu Lalang Banjarbaru Community. This study uses a type of quantitative research with a descriptive research design conducted to determine the value range of motion (ROM) using a gonometry. The research results obtained were mostly at the age of 31-40 years (52%), male sex at most with a total of 15 people (88%), the frequency of exercise was at most 7 people 2x/week (41%), andrange of motion elbow obtained the average in the extension movement is 3°, in the flexion movement is 150°, in the supination movement is 87°, and in the pronation movement is 89°.

Abstract

Use of information systems is a strategic means for providing registration services that are oriented towards customer satisfaction. Several research results show that there are manual and electronic patient registration systems. The development of Android-based information systems is increasing. Medical records at the Almeta Clinic currently still use a manual system from registration to storage. This research is development research (RnD) with 6 research stages. Carrying out research by creating an outpatient medical resume information system design, researchers created a DFD (Data Flow Diagram), ERD (Entity Relationship Diagram) as well as the appearance and design of a patient registration information system design created with Microsoft Access and Microsoft Visio in accordance with the identification of user needs and requirements. data. The media expert's results showed 80% with the qualification indicators "Very Good" and the results "Decent". validation results from material experts show 82.5% of the qualification indicators are "Very Fulfilling" and the results are "Decent".

Abstract

The administration of medical records in the patient reception (registration) section has now developed. The development that can be seen in health services, especially in hospitals, such as the patient reception or registration section. The purpose of this study is to find out the implementation of the Electronic Registration System for Existing Patients at TPPRJ Bhayangkara Hospital in Banjarmasin. This study uses a descriptive qualitative method with data collection techniques using interviews with 5 informants and observation. The results of the study for the man aspect found that the quantity of human resources was sufficient and the quality of human resources was also in accordance with the specified standards. For the machines aspect, the facilities and infrastructure (hardware) used at TPPRJ are very good and the amount of hardware provided is very sufficient for the current implementation of electronic registration. The software used is SIMRS Isena. In the methods aspect, there are Standard Operating Procedures (SOP) available for electronic patient registration. Based on the results of the study, it can be concluded that the implementation of the electronic registration system for the man aspect, in terms of quantity and quality, has been fulfilled and is appropriate. For the machines aspect, related hardware and software are available, and in the methods aspect, there are already SOPs.

Abstract

Ostheoarthritis is a joint disorder characterized by progressive degeneration of joint cartilage resulting in narrowing of joint gaps including cartilage, subchondral, synovial fluid, and ligaments around the joint. Ultrasound is one of the non-pharmacological treatment methods that results in increased tissue temperature, increased blood flow as well as pain threshold and reduced spasm in muscles. Isometrric Exercise exercise therapy that serves to increase capacity respondents at Idaman Banjarbaru Hospital, using ultrasound modalities and isometric exercise. This research method is quantitative with a case study descriptive research design. The results of this study showed an increase in the interpretation of medium-weight to medium-light with an initial average of 56.7 which was included in the weight category to 34.4 With the mild final category, which concludes with this study, there has been an increase in functional capacity with the use of ultrasound modalities and isometric exercise in respondents who have bilateral genetic osteoarthritis.

Abstract

Human Resources (HR) play a crucial role in the operation of any company or organization. The medical records unit is a key supporting organization within healthcare facilities, requiring high performance and meticulous attention from its staff. A preliminary study conducted on February 2, 2024, in five Community Health Centers (Puskesmas) in the Banjarmasin Tengah District—namely Puskesmas S. Parman, Puskesmas Sungai Mesa, Puskesmas Gedang Hanyar, Puskesmas Teluk D, and Puskesmas Cempaka Banjarmasin—revealed through interviews with the heads of the medical records departments that while these centers have implemented Electronic Medical Records (EMR), several challenges remain. These challenges include a lack of understanding among staff about EMR, errors in patient medical record data entry, misfiling, inadequate training, and staff lacking educational background This research employs a qualitative method with a descriptive approach, using observation guidelines, interview guidelines, and questionnaires as tools for data collection. The findings indicate that from the perspective of human resource quality, the staff is not yet prepared to support the implementation of electronic medical records because they have not received training on EMR, and many do not have a background in medical records. In terms of quantity, there are 17 medical records staff across the five Community Health Centers in Banjarmasin Tengah District, averaging three medical records staff per center. It is recommended that future hiring for medical records positions in the Community Health Centers within Banjarmasin Tengah District prioritize candidates with educational backgrounds in medical records and health information to enhance the quality.

Abstract

Di Puskesmas Anjir Pasar Lama dijelaskan masih adanya hambatan di pendaftaran. Penyebab lamanya waktu tunggu pendaftaran pasien adalah kurangnya sumber daya manusia yang memahami rekam medis elektronik di bagian pendaftaran dan banyaknya pasien yang datang. Dalam pelaksanaan di bagian pendaftaran, pasien juga sering ketinggalan kartu hal ini juga mempengaruhi waktu pelayanan di bagian pendaftaran. Penelitian ini menggunakan jenis penelitian kualitatif dengan sub variabel sebagai berikut: Man, Methods, Material. Hasil dari aspek man diketahui pelaksanaan pendaftaran pasien dilakukan oleh 2 orang petugas. Petugas di bagian pendaftaran masih belum sepenuhnya berlatar belakang rekam medis, Petugas sudah mendapatkan pelatihan E-puskesmas namun dalam pelaksanaan pelatihan tersebut tidak merata, petugas di bagian pendaftaran sudah bekerja sekitar 14 tahun dan petugas lainnya bekerja kurang dari 1 tahun, 2 orang petugas pendaftaran terpaut perbedaan umur yang cukup jauh yang dimana salah satu petugas berumur 22 tahun dan salah satu petugas berumur 49 tahun. Dari aspek Material sarana seperti komputer, wifi, pengeras suara dan softwear, Prasarana di ruang pendaftaran bersebelahan dengan ruang rujukan BPJS dan belum memiliki ruangan rekam medis tersendiri, ruangan rekam medis masih menyatu dengan tempat loket pendaftaran yang hanya dibatasi dengan sekatan dinding triplek. Dari Aspek Method tentang rekam medis elektronik masih belum dibuat karena masih dalam tahap baru pelasanakan. Dengan adanya kebijakan peraturan PERATURAN Menteri Kesehatan Republik Indonesia Nomor 24 Tahun 2022 perlu dikaji kembali atau perlu di revisi terkait SOP

Abstract

Kepuasan pasien mempengaruhi kesesuaian tingkat kepentingan atau harapan pelanggan sebelum mereka menerima jasa pelayanan dengan sesudah pelayanan yang mereka terima. Kualitas mutu pelayanan, antara lain tangibles ( tampilan ), reliability ( keandalan ), responsiveness ( daya tanggap ), assurance ( jaminan ), emphaty ( empati ). Pelayanan kesehatan yang diberikan petugas kepada pasien berpengaruh besar dalam pandangan pasien terhadap kepuasan pelayanan kesehatan. Penelitian ini bertujuan untuk menggambarkan tingkat kepuasan pasien terhadap mutu pelayanan di unit pendaftaran rawat jalan. Jenis penelitian yang digunakan adalah penelitian kuantitatif dengan pendekatan deskriptif, pedoman observasi, serta kuesioner merupakan alat bantu untuk pengumpulan data. Hasil penelitian ini menunjukkan bahwa mayoritas pasien merasa puas terhadap aspek tangibles sebanyak 82.8 %, aspek reliability sebanyak 82.2 %, aspek responsiveness sebanyak 82.95 %, aspek assurance sebanyak 83.15 %, dan aspek emphaty sebanyak 81.5 % menyatakan puas. Hasil ini menunjukkan bahwa Puskesmas XYZ mampu memberikan pelayanan yang memenuhi harapan pasien, meskipun masih terdapat hal yang memerlukan perbaikan untuk meningkatkan mutu pelayanan secara keseluruhan.

Abstract

HNP is a degenerative process that causes decreased flexibility and thinning of the intervertebral disc nucleus pulposus resulting in tearing of the annulus fibrosus which causes low back pain which can interfere with functional activities in the elderly. The purpose of this study was to determine Management Physiotherapy of Hernia Nucleus Pulposus Lumbar Conditions For Increase Functional Ability Elderly With TENS Modalities And Core Stability. The type of research used is quantitative with a descriptive approach, namely a case study using 3 patient samples. Treatment was carried out 6 times within a period of 3 weeks. The measuring instrument used is the Oswestry Disability Index to determine the functional abilities of the elderly. The results of the study obtained after evaluation were an increase in the functional abilities of the elderly after providing interventions in the form of TENS and Core Stability.
