Abstract

Background: Tuberculous (TB) Spondylitis results from the hematogenous spread of Mycobacterium Tuberculosis bacteria to the spinal blood vessels. The incidence of extrapulmonary tuberculosis in the world is 3%, of which 10% of cases are bone. This paper will describe the case of a patient with Inferior Paraparesis Upper Motor Neuron Type due to Destruction of Thoracic 6-7 Vertebrae due to Tuberculous Spondylitis at Dr. Moewardi General Hospital, Surakarta, Indonesia. In addition, this case can provide information to health services and be a step forward in discussing tuberculous spondylitis further. Purpose: This study aims to report the diagnosis and management of tuberculous spondylitis. Case  Presentation: A Female, 31 years old, presented with complaints of lower limb weakness felt four months before admission and associated with back pain. The patient’s consciousness was compos mentis. There were tenderness in the back midline and neurological deficits. There was a decrease in muscle strength in the lower extremities, both left and right and also a decrease in physiological reflexes in all four extremities. There was positive Babinski on the right side and positive clonus on both sides. Thoracolumbar MRI results showed destruction of the corpus of Thoracic 6&7 Vertebrae with gibbus deformity and kyphosis with a picture of soft tissue abscess. Conclusion: The patient was diagnosed with paraparesis inferior upper motor neuron type due to destruction of thoracic 6-7 vertebrae due to tuberculous spondylitis. Then she was treated and planned for laminectomy, PSF, debridement, and corpectomy.

Abstract

  Background: Malnutrition is a common issue in hospital settings, particularly in the ICU and among elderly patients. Objective: This study aimed to analyze the nutritional characteristics of ICU patients and their association with clinical outcomes and length of stay. Methods: This prospective observational study was conducted in the ICU of Dr. H. Abdul Moeloek Hospital, Lampung Province, from December 2023 to February 2024. Patients hospitalized during this period were the study population, with total sampling used to collect data. The mNUTRIC score was employed to assess nutritional status. Bivariate analysis using chi-square was conducted to analyze the association between mNUTRIC scores and patient outcomes, as well as length of stay. Results: The majority of patients were elderly aged 50-75 years (45%) and female (95%). A significant portion (68.3%) had an abnormal body mass index (underweight, overweight, or obese). Additionally, 91.7% had comorbidities, and most stayed in the ICU for more than three days, with 51.7% having extended stays. Furthermore, 48.3% passed away during hospitalization. The study found that 51.7% of patients were at high nutritional risk, while 48.3% were at low risk. Bivariate analysis revealed a significant correlation between mNUTRIC score and clinical outcomes, including length of stay (p<0.05). Conclusion: This study concludes that ICU patients often have an abnormal nutritional risk, either high or low, based on mNUTRIC scores. Furthermore, mNUTRIC scores are correlated with clinical outcomes and length of stay.

Abstract

Background: Intermittent Fasting (IF) is a type of fasting where a person alternates between fasting and eating within specific time intervals. Medical students tend to have poorer sleep quality compared to students from other faculties. This can negatively affect their academic performance, as well as their physical and mental health. While intermittent fasting is known to offer various health benefits, its effect on sleep quality in medical students has not been widely studied. Therefore, further research is needed to explore the impact of intermittent fasting on sleep quality among medical students. Objective: To determine the effect of intermittent fasting on the sleep quality of medical students. Methods: This study employed a quasi-experimental design with a one-group pretest-posttest design. The sample consisted of 24 medical students from the Faculty of Medicine, Diponegoro University, Semarang. The sample was selected using purposive sampling based on predetermined criteria. In this study, the intervention involved intermittent fasting using the 12:12 Time Restricted Feeding method. Fasting started from 6 PM to 6 AM on Mondays, Wednesdays, and Fridays for three consecutive weeks. Data analysis on sleep quality before and after the intervention was conducted using the Paired Sample T-Test. The correlation between confounding variables, such as stress levels and physical activity levels, and sleep quality was analyzed using Spearman's correlation test. Results: There was a significant decrease in the global PSQI score after the participants underwent intermittent fasting compared to before, with a p-value < 0.05 (p = 0.05 (p = 0.493). Conclusion: Intermittent fasting can improve the sleep quality of medical students.

Abstract

Introduction: Attention plays a role in supporting productivity and safety in industrial environments, particularly among shift workers. Shift Work Sleep Disorder (SWSD), experienced by those on rotating or night shifts, is characterized by insomnia and/or excessive daytime sleepiness (EDS), both of which can decrease attentional function. These prevalence rates reaches up to 76.4% and 60.7% respectively, higher than in the general population. Attention impairment in shift workers has been linked to an increased risk of occupational accidents. Objective: To analyze the effect of SWSD symptoms on attention function among factory workers. Methods: A cross-sectional study was conducted in November 2024 involving 51 factory workers in Jepara. The Insomnia Severity Index, Epworth Sleepiness Scale, and Digit Span Test were employed to assess insomnia, EDS, and attention, respectively. Chi-square tests and logistic regression were used to analyze associations. Results: SWSD was found in 29.4% of participants and was significantly associated with attention impairment (RR = 2.4; 95% CI: 1.417–4.064; p = 0.002). Both insomnia (RR = 2.15; p = 0.016) and EDS (RR = 2.00; p = 0.019) increased the risk of impaired attention. Participants with both symptoms had the highest risk (RR = 2.17; p = 0.008). Multivariate analysis revealed that insomnia remained independently associated with impaired attention (AOR = 4.129; p = 0.018). Conclusion: EDS combined with insomnia increases the risk of attention impairment. Insomnia was found to be an independent predictor. These findings highlight the importance of sleep disorder screening to reduce the risk of attentional impairment and prevent occupational accidents in shift work.

Abstract

Background: Dyspepsia syndrome can be defined as a collection of complaints described as pain or discomfort in the pit of the stomach, bloating, nausea, vomiting, belching, feeling full quickly, stomach feeling full or full. Based on the 2007 Indonesian Health Profile Information, dyspepsia was ranked 10th in the category of disease inpatients hospitalized in 2006 with 34,029 sufferers or approximately 1.59%. One of the various aspects of the risk of developing dyspepsia is consuming caffeine. Objective: This study aims to determine the influence of coffee consumption patterns on the incidence of dyspepsia in medical students of the Faculty of Medicine. Methods: This study uses an observational analytic method. The research was conducted in October 2021 – November 2021 at Medical Faculty, Ahmad Dahlan University. Research population was 110 active students of the 2018-2020 according to the inclusion and exclusion criteria. Analysis bivariate using chi-square. Results: It was found that the pattern of coffee consumption in students with the majority being in the moderate category was 87 people (79.1%) and the majority experienced the incidence of dyspepsia as many as 73 people (66.4%) while students with moderate consumption patterns had the greatest risk opportunity 1.753 times for experienced dyspepsia events with a relative risk ranging from 0.810 to 3.791. This shows the influence of coffee consumption patterns on the incidence of dyspepsia in students with a value of p = 0.002 (p <0.005). Conclusion: Coffee consumption patterns influence the incidence of dyspepsia in medical students, Medical Faculty Students, Ahmad Dahlan University.

Abstract

Background: Type 2 diabetes mellitus (T2DM) is a metabolic disease that can cause low-grade chronic inflammation related to insulin resistance. Recently, neutrophil to lymphocyte has become a potential marker of chronic inflammation. An abnormal lipid profile occurs due to lipotoxicity from insulin resistance. Objective: To prove the correlation between NLR and lipid profiles in T2DM patients. Methods: An analytic observational study used a cross-sectional approach to 45 medical records of T2DM patients who received treatment at Nasional Diponegoro Hospital Semarang. This study was conducted from October to November 2022. Data was collected by using consecutive sampling. Analysis of the correlation between NLR and lipid profiles used spearman’s correlation test. Results: The correlation between NLR and HDL cholesterol levels was p=0.035, r=-0.315. The correlation between NLR and cholesterol, LDL cholesterol, and triglyceride levels was p = 0.061, r = -0.281; p = 0.170, r = -0.208; p = 0.563, r = -0.089, respectively. Conclusion: There was a weak negative correlation between NLR and HDL cholesterol levels in T2DM patients, but there was no correlation between NLR and other lipid profiles.

Abstract

Background : Cigarette smoke contains dangerous chemical components that can produce free radicals and cause oxidative stress, which can reduce sperm quality, including sperm concentration. Rat studies demonstrate that cigarette smoke exposure lowers sperm count by 13–17% and severely affects morphology and motility. Red dragon fruit (Hylocereus polyrhizus) contains vitamin C, vitamin E, beta carotene, lycopene, and flavonoids that function as antioxidants and can counteract the free radicals that enter the body. Purpose : To determine the effect of red dragon juice on the concentration of Wistar rat spermatozoa exposed to cigarette smoke. Method: This study used a post-test only control group design. The total sample was 35 male Wistar rats, divided into 5 groups. Healthy control group is the group without intervention. Negative control group was only exposed to cigarette smoke. Group P1 was exposed to cigarette smoke and given 2 grams/2.5 ml of red dragon fruit juice. Group P2 was exposed to cigarette smoke and given 4 grams/2.5 ml of red dragon fruit juice. Group P3 was exposed to cigarette smoke and given 8 grams/2,5 ml of red dragon fruit juice. Intervention is given for 28 days and on day 29 all rats are terminated and their spermatozoa concentration is checked. Result : The mean concentrations of spermatozoa were: Group K(-) = 42.83; Group K(+) = 31.17; Group P1 = 29.17; Group P2 = 63.33; Group P3 = 72.00. The One Way ANOVA test found a significant difference between the five treatment groups. The Post-Hoc test found significant differences in the K(-) group against K(+), P1, P2, P3. In the K(+) group toward P2 and P3 there was a significant difference. In the P1 to P2, P3 groups, there were significant differences. In the P2 to P3 groups, there was a significant difference. Conclusion : Giving red dragon fruit juice can affect the concentration of spermatozoa in Wistar rats exposed to cigarette smoke.

Abstract

Background: Intracellular and serum potassium levels have important effects on homeostasis cardiovascular system. Patients undergoing cardiac surgery may develop perioperative hypokalemia. Electrocardiographic (ECG) manifestations of hypokalemia include flattening of the T wave followed by T wave inversion, ST wave depression, prominent U wave, prolongation of the QTc interval, torsades de pointes, to asystole. Case Presentation: In this case report, a patient with coronary heart disease who underwent coronary artery bypass surgery, with moderate to severe hypokalemia, before surgery to postoperative care in the ICU. Management of hypokalemia in this patient using potassium chloride 10-50meq/hour (total 250 meq). Conclusion: Establishing a diagnosis, looking for underlying disease related to hypokalemia, and a history of preoperative medication should also be considered. Optimal conditions preoperatively can reduce morbidity and mortality. Appropriate management of severe hypokalemia should be known to all medical personnel. Periodic monitoring of ECG and potassium levels is necessary.

Abstract

Background: High purine consumption can increase uric acid levels. Elevated uric acid levels can lead to complications in the kidneys, including damage and the formation of kidney stones. Lemon peel contains vitamin C, flavonoids, and citrate that can act as antioxidants and inhibitors of kidney stone formation. Objective: Investigating the effects of graded doses of lemon peel extract (Citrus limon) on the macroscopic and microscopic characteristics of kidneys in male wistar rats with hyperuricemia and comparing among control, positive control, and treatment groups. Methods: Thirty samples of experimental rats were divided into 5 groups: a control group, a positive control group, and 3 treatment groups, with each group consisting of 6 rats. The samples were administered Maggie cube broth produced by PT. Nestle Indonesia at a dose of 140 mg/200gBW for Wistar rats and potassium oxonate at a dose of 50 mg/200gBW for inducing hyperuricemia. Intervention involved oral administration of lemon fruit peel extract at doses of 17.5, 35, and 70 mg/kgBW for 14 days. Rats were terminated for kidney extraction, followed by macroscopic and microscopic readings. Differences between groups were analyzed using statistical tests. Results: No significant differences were found in the macroscopic appearance of the kidneys among the groups, characterized by smooth and even surfaces. Microscopic observations showed that kidney preparations P1, P2, and P3 had lower rates of tubular cell degeneration compared to K1, although no significant differences were found in the inflammation observed between K1 and P1, P2, and P3. Conclusion: This suggests that lemon fruit peel extract affects the rate of tubular cell degeneration in Wistar rat kidneys. However, there was no effect on the inflammation observed.

Abstract

Background: Elderly people are susceptible to degenerative diseases such as hypertension. Hypertension with comorbidities is one of the leading causes of death worldwide. One of the comorbidities of hypertension is diabetes mellitus. Hypertension with comorbidities can cause serious symptoms. Elderly also prone to decreased physical function, which can decrease their independence in performing daily activities, thus can affect their quality of life. Objective: To understand the characteristics, daily activities independence, polypharmacy prescribing, quality of life, and the relationships between characteristics, independence in daily activities, number of medications consumed, and quality of life in geriatric patients with hypertension and comorbid diabetes mellitus at RSD K.R.M.T Wongsonegoro Semarang. Methods: This was a descriptive study with a cross-sectional approach was employed. The instruments used the WHOQOL-BREF and Katz Index questionnaire. The questionnaire was directly given to the patients. Results: From 75 respondents, the majority were female (61.3%), had a normal BMI (53.3%), had low-moderate education (70.7%), were married (54.7%), had high independence (93.3%), and received polypharmacy prescriptions (89.3%). Conclusion: There is a relationship between gender, BMI, marital status, and daily activity independence with the quality of life of patients. However, there is no relationship between education level and the number of medications consumed with the quality of life of patients. The factor that most influences quality of life is independence in daily activities.
