Abstract

Background:Aphasia is a common stroke complication that significantly impacts patients' quality of life (QoL). While Constraint-Induced Aphasia Therapy (CIAT) has been found to improve language capacity, its impact on QoL remains uncertain. Objectives: The purpose of the present study was to meta-analyze and systematically review the impact of CIAT on post-stroke aphasic patients' quality of life. Methods: A thorough search of five databases (PubMed, CINAHL, ScienceDirect, Scilit, and Cochrane Library) was conducted for 2009-2024 literature. Studies included experimental studies with assessments of CIAT's effect on QoL using valid instruments. Risk of bias was assessed using RoB 2 and JBI tools. Meta-analysis was conducted using Review Manager 5.4 and studies that utilized the SAQOL-39 scale. Results: Six trials were included based on inclusion criteria; three trials were subjected to the meta-analysis. CIAT had significant trends favorable for improved QoL, especially in areas of communication, although the combined effect was not significant statistically (MD = 0.11; 95% CI: –0.14 to 0.35; p = 0.40). There was low heterogeneity (I² = 0%). Conclusion: CIAT may enhance QoL in aphasia patients, particularly in communicative functioning. While lacking high statistical significance, outcomes are in favor of CIAT's role in neurorehabilitation. Additional high-quality trials would be needed to ensure these results.

Abstract

Background: Scabies diagnosis is greatly influenced by the experience of the examiner, since the diagnosis is rely on a complex diagnostic criteria. Antigen detection test is an alternative technique for diagnosing scabies in the future. Tropomyosin is a highly conserved protein which expressed by Sarcoptes scabiei mites.Objective: This research aimed to develop recombinant protein from exon 5 of the S. scabiei tropomyosin gene for further scabies diagnostic development.Methods: An exon from tropomyosin gene was cloned into pLATE-51 plasmid using ligation independent cloning methods. Protein overexpression was done using Eschericia coli strain BL21. Insoluble protein was solubilized using 5 mM dithiothreitol (DTT). Protein purification was done with Ni-NTA affinity chromatography principle. Analysis of the resulting recombinant protein was done using SDS-PAGE, immunodot blotting, and western blotting.Results: A 550-bp exon was successfully cloned into pLATE-51 plasmid. Overexpression result showed protein was mostly expressed in pellet (insoluble form). Confirmation using immunodot blotting showed the presence of recombinant protein from the overexpression. However, further confirmation using western blotting was unsuccessful to detect the intended recombinant protein.Conclusion: The exon of S. scabiei tropomyosin gene could be cloned in pLATE-51 plasmid and produced recombinant protein. However, the protein yield was low and the protein is mostly insoluble, preventing the success of purification of the intended recombinant protein.

Abstract

Background: Acute Decompensated Heart Failure (ADHF) is a leading cause of hospitalization among heart failure (HF) patients, contributing to high rates of morbidity and mortality. In such acute decompensated conditions, the body's innate immune system is activated, leading to oxidative stress and the release of proinflammatory cytokines. The Neutrophil-to-Lymphocyte Ratio (NLR) and Platelet-to-Lymphocyte Ratio (PLR) are emerging as cost-effective, easily measurable markers of inflammation that could act as potential prognostic indicators. Objectives: The goal of this study was to evaluate the association between NLR and PLR with in-hospital mortality in patients with ADHF. Methods: A cross-sectional study was carried out involving 69 ADHF patients who visited the Emergency Department of Muhammadiyah Lamongan Hospital between January 1 and June 30, 2021. NLR and PLR values were recorded at the time of admission. Pearson's correlation test was applied to assess the relationship between NLR, PLR, and in-hospital mortality. The cut-off values were established using the Receiver Operating Characteristic (ROC) curve. Results: This study shows that NLR (12.7±9.1 vs 6.8±3.4; r=0.418; p=0.001) and PLR (336.9±214.8 vs 205.4±78.5; r=0.402; p=0.001) values are higher in patients who died in the hospital compared to those who survived and have a statistically significant correlation with hospital mortality. The cut-off value for NLR was 8.33 (sensitivity 67.9%, specificity 68.3%, AUC 73.1%), while the cut-off for PLR was 216 (sensitivity 60.7%, specificity 58.5%, AUC 67%). Conclusion: Increased NLR and PLR values are strongly linked to in-hospital mortality and could act as straightforward, dependable prognostic indicators in ADHF patients

Abstract

Background: Chronic lymphocytic leukemia, or CLL, is a chronic hematological malignancy marked by the overproduction of mature but nonfunctional lymphocytes. This condition is uncommon among Asian individuals. As a result, variations of this type of CLL are even less frequently observed. A CLL that presents with prolymphocytic progression is now recognized as a new illness in in the fifth edition of the World Health Organization classification. It is identified as a CD5+ non-mantle B-cell tumor featuring a minimum of 15% prolymphocytes in the bloodstream or bone marrow, and it partially supersedes the previous classification of B-cell prolymphocytic leukemia. Case Presentation: A 78-year-old woman experiencing increasing fatigue and difficulty breathing was brought to the emergency room and subsequently found to have Chronic lymphocytic leukemia. In this report, we highlight an uncommon instance of prolymphocytic leukemia advancement in a CLL patient within an exceptionally brief period of one week. Conclusion: Prolymphocytic transformation of CLL is a rare case with poor prognosis.This change was illustrated by the presence of undeveloped cells found in the peripheral blood, resembling prolymphocytes. The CLL patient unfortunately passed away shortly after being admitted to the hospital for a few days, which is atypical for traditional CLL patients who generally experience a longer duration of life. Thus, it is necessary to monitor the number of prolymphocyte in CLL patient as indicator of poorer transformation of chronic leukemia.

Abstract

Background: Carotid cavernous fistula (CCFs) is characterized by an aberrant channel linking the carotid artery directly to the cavernous sinus. Most of the CCFs are classified as traumatic CCFs, which is occurred following trauma. Meanwhile spontaneous CCFs, which is occurred spontaneously, are very rare. Spontaneous CCFs typically occur in elderly women and are usually classified as low-flow, indirect fistulas of Barrow type D. This study aimed to present a rare case of a spontaneous high-flow CCFs classified as Barrow type A, due to its uncommon occurrence. Case presentation: A 56-years-old woman came fully alert presented with swelling and redness on the right eye since 1-month prior admission. Palpebral edema, episcleral injection, conjunctival chemosis, proptosis, and bruit were found from her right eye. CT-angiography found engorgement on the right cavernous sinus with superior ophthalmic vein (SOV) dilatation. The RICA DSA revealed that the contrast filled the cavernous sinus, SOV, facial vein, and internal jugular vein on the early artery phase, which indicates a high flow CCFs at the C4 with Barrow classification type A. The patient was planned for fistula embolization with coil. Conclusion: Spontaneous high flow CCFs is an extremely rare condition. Accurate history taking and thorough physical examination are essential to identify the classic triad of CCFs symptoms: proptosis, chemosis, and ocular bruit. The definitive diagnostic tool is a cerebral angiogram, which can reveal the presence of a fistula by demonstrating contrast filling of the cavernous sinus.

Abstract

Background: According to information provided by the World Health Organization (WHO), around 15% of the world's total population experiences tinnitus. The causes of tinnitus vary, including underlying diseases, one of which is hypertension. Meanwhile, in the Special Region of Yogyakarta it ranks 4th for the province with the highest cases of hypertension in Indonesia. Objective: This study aims to determine the relationship between hypertension and the incidence of tinnitus at PKU Muhammadiyah Gamping Hospital. Methods: This type of research is quantitative with analytical observation using a cross-sectional approach using medical records as the data source. The total sample in this study was 56 patients at the ENT Polyclinic at PKU Muhammadiyah Gamping Hospital who met the exclusion and inclusion criteria. Data analysis used univariate and bivariate Fisher exact tests. Results: The research results showed that 30 of the 56 subjects had hypertension with tinnitus. The results of the bivariate test of the relationship between hypertension and tinnitus produced a p value of 0.008 (p < 0.05) and an r value of 0.377. Conclusion: There is a significant weak relationship between hypertension and tinnitus in patients at the ENT Polyclinic at PKU Muhammadiyah Gamping Hospital. Among the degrees of hypertension that have been studied, grade 2 hypertension has the strongest relationship with tinnitus. Hypertension conditions can cause damage to blood flow in the cochlea due to damage to blood flow autoregulation which then causes ischemia in the cochlea and causes clinical manifestations in the form of tinnitus.

Abstract

Background: While effective in treating the disease, anti-tuberculosis drugs can also potentially harm the liver. Monitoring liver health using the transaminase enzyme and leukocyte counts can help in the early detection and prevention of such damage in tuberculosis patients. Objectives: The aim of the study was to determine the correlation between transaminase enzyme levels and the leukocyte count in patients with tuberculosis who took anti-tuberculosis drugs. Methods: This research was an observational analytical study with a cross-sectional approach. It used medical record data from TB patients who met the inclusion and exclusion criteria for anti-tuberculosis drug therapy at the medical records installation. Results: Forty-four samples were included in the research. The levels of aspartate transaminase (AST), alanine transaminase (ALT), and leukocyte count were 30 IU/L, 19.5 IU/L, and 9.35x103/µL. The Spearman correlation test showed no correlation between AST and ALT levels and leukocyte counts, with p-values of 0.585 and 0.337. Conclusion: There was no correlation between AST and ALT levels and leukocyte counts in tuberculosis patients undergoing anti-tuberculosis drug therapy.

Abstract

Hypovolemic shock due to acute hemorrhage is a life-threatening condition, particularly in cancer patients undergoing chemotherapy. Although recent studies have established protocols for managing hypovolemic shock in trauma and sepsis, the role of chemotherapy-induced thrombocytopenia in exacerbating post-procedural bleeding risks remains underexplored. This case emphasizes the clinical complexity of managing hemorrhagic shock in advanced-stage cervical cancer patients following percutaneous nephrostomy (PCN). Case Presentation: A 52-year-old female with stage IVB cervical cancer and liver metastasis presented with hypovolemic shock following PCN. She exhibited severe hematuria and chemotherapy-induced thrombocytopenia, leading to prolonged bleeding and subsequent shock. Initial management involved aggressive fluid resuscitation, blood transfusions, and correction of thrombocytopenia. Despite the challenges of managing shock in a patient with concurrent chronic kidney disease (CKD) stage IV, prompt intervention led to significant clinical improvement, stabilization of vital signs, and eventual discharge. Conclusion: This case highlights the importance of early intervention in managing hypovolemic shock in oncology patients with multiple comorbidities, including chemotherapy-induced thrombocytopenia and CKD. A multidisciplinary approach and close monitoring are crucial for improving outcomes in such high-risk patients. Further research is needed to establish tailored management protocols for this patient population.

Abstract

Background: In the context of global competition, industrial companies must prioritize strategic factors, particularly the development and well-being of human resources. In Indonesia’s manufacturing sector, especially in Banten Province, employees are often subjected to considerable stress resulting from irregular working hours and demanding production targets. Mental health frequently overlooked by employers and organizational systems has a direct and significant impact on employee performance, influencing productivity, motivation, and work quality. Objective: To identify the relationship between mental health and work performance of manufacturing plant employees at PT. X in Banten Province. Methods: This study uses an analytical observational method with a cross-sectional design to examine 396 employees at PT. X's paper packaging plant in Serang City, selected through purposive sampling based on inclusion and exclusion criteria. Data were collected with a questionnaire using the DASS and IWPQ scales and analyzed using the Chi-square test. Results: This study found that depression, anxiety, and stress were each significantly associated with work performance (all p < 0,001), with stress having the strongest effect (OR = 4,062; 95% CI = 2,362–6,985). Conclusion: Depression, anxiety, and stress are significantly associated with reduced work performance among employees at PT. X. Therefore, employers and policymakers should prioritize implementing comprehensive mental health programs and supportive workplace policies to enhance employee well-being and improve productivity in the manufacturing sector.

Abstract

Background: Infertility affects 15% of couples worldwide, with 40-50% of cases attributed to unhealthy lifestyle factors in men, such as smoking, alcohol consumption, drug use, obesity,poordietaryhabits,coffeeintake,andpsychologicalstress.Smokinghasanegative impactonreproductivehealthandreducesthechancesofhavingchildren.Smokingleadsto oxidative stress, lowers antioxidant levels in the testes, thereby disrupting spermatozoa formation. The antioxidant content in pineapple is expected to prevent and repair damage caused by oxidative stress. Objective: To investigate the effects of pineapple juice administration on sperm viability in Wistar rats exposed to cigarette smoke. Methods: Anexperimentalstudywasconductedwithapost-testonlygroupdesigninvolving 25maleWistarratsdividedinto5groups:K(notreatment),K+(exposuretocigarettesmoke only),T1(exposuretocigarettesmokeand1mlofpineapplejuiceperday),T2(exposureto cigarettesmokeand2.5mlofpineapplejuiceperday),andT3(exposuretocigarettesmoke and 4 ml of pineapple juice per day). All groups received treatment for 28 days. Results: ThemeanpercentageofspermatozoaviabilityinWistarratswasK=90.00;K(+)= 76.80; T1 = 76.80; T2 = 89.20; T3 = 93.40. Significant differences were observed between groupsKandK+andT1,withrespectivep-valuesof0.005.Significantdifferenceswerealso found between groups K+ and T2 (p = 0.008) and T3 (p = 0.001), while no significant difference was observed in group T1 (p = 1.000). Furthermore, there were significant differencesbetweengroupsT1andT2(p=0.008)andT3(p=0.001).Finally,nosignificant difference was found between groups T2 and T3 (p = 0.331). Conclusion and Recommendations: Pineapple juice can prevent and inhibit spermatozoa damage caused by free radicals from cigarette smoke. Further research should measure oxidative stress, provide exposure to each rat, and use varying doses for optimal effectiveness
