Abstract

Background: Recent studies have highlighted the significance of inflammatory markers in diagnosing appendicitis; however, the specific roles of stab and segmented neutrophils in assessing disease severity remain poorly understood. Objective: This study aims to analyze the relationship between the types of appendicitis (complicated and uncomplicated) and neutrophil counts (stab and segmented), along with demographic variables such as age and gender, to evaluate their diagnostic significance. Methods: A retrospective, descriptive study was conducted on 48 appendicitis patients treated at Gatot Soebroto Army Central Hospital during 2020–2021. Data were analyzed using the Mann-Whitney U test, with a significance threshold set at p<0.05. Results: Neutrophil segment counts demonstrated a statistically significant difference between complicated and uncomplicated appendicitis cases (p=0.000), with higher levels observed in complicated cases. In contrast, neutrophil stab counts, age, and gender did not show statistically significant differences (p>0.05). Conclusion: Neutrophil segment counts serve as a reliable marker for assessing appendicitis severity, while neutrophil stab counts and demographic factors such as age and gender lack predictive value. Further research is warranted to investigate the temporal dynamics of neutrophil levels and their integration with other inflammatory markers.

Abstract

Background: The rapid development of communication technology makes access to social media easier. The interesting features which are available on social media bring not only positive impacts, but also the negative ones. The negative impacts can affect the life of students. As students spend more time on social media, it makes them prone to academic procrastination. Objective: This study is to examine the correlation between the intensity of social media usage and academic procrastination among medical students in Universitas Diponegoro Methods: : The study was conducted with a cross sectional method. The number of respondents are 76 who were selected by random simple sampling. Social Networking Time Use Scale (SONTUS) are used to measure the intensity of social media usage and Procrastination Assessment Scale for Students (PASS) are used to measure the level of academic procrastination among respondents. Penelitian ini menggunakan metode belah lintang. Results: The study showed that among 76 respondents, 96,1% considered very high media social users, 2,6% considered high, and 1,3% considered mild users, there’s none in the low category. Additionally, the prevalence of academic procrastination 47,4% is in the high category, 52,6% is in the mild category and there’s none in the low category. The were positive significant correlation between the intensity of social media usage and academic procrastination with p-value 0,028 (p<0.05) Conclusion: There is a significant correlation between the intensity of social media usage and academic procrastination among medical students of Universitas Diponegoro.

Abstract

Background:Multiple decubitus ulcers are a condition of injury that appears on several parts of the body’s skin due to pressure for a long time without any change in position. This condition usually occurs in someone with limited movement, for example, due to experiencing certain disease conditions that require the person to rest in bed for a long time, especially patients in the Intensive Care Unit (ICU) or after ICU care with chronic diseases and limited movement. Case Presentation: This case report assesses the success of multiple cases of decubitus ulcers in patients with type II diabetes mellitus with Autologous Plasma Rich Platelet therapy. Case report: Autologous PRP therapy is given by intralesional injection; debridement is done every three days and assessed every one week. Patients are also subjected to blood sugar control, blood pressure control, blood correction, and albumin to speed up the wound healing process in multiple decubitus ulcers. Conclusion:the conclusion of this case report with additional management in the form of intralesional injection of autologous PRP performed on multiple decubitus ulcer patients with Diabetes Mellitus, which is rich in growth hormone, will greatly help the wound healing process.

Abstract

  Background: The COVID-19 pandemic-related social restriction policy has a spillover effect on sleep quality. Sleep quality worsened after the social restriction policy was implemented, followed by an improved sleep quality longitudinal trajectory. Previous studies have focused on the pre-pandemic and post-implementation impacts of social restrictions on sleep quality, but not after social restrictions were lifted. Objective: Evaluate the sleep quality after the ease of the COVID-19 pandemic-related social restriction policy. Methods: We recruited voluntary academic staff who lived in Indonesia, had no leave during the work from home (WFH) period, and were non-shift workers to participate in the study. We administered the Pittsburgh Sleep Quality Index (PSQI), Sleep Hygiene Index, Connor-Davidson Resilience Scale, Reduced Morning-Eveningness, and micro-Munich Chronotype Questionnaires. We additionally attempted to get the subject to recall their sleep quality one year after the pandemic. The repeated measure of sleep quality was analyzed using a linear mixed-effect model, and the determinant factors of sleep quality in the WFO period were analyzed using a linear model. Results: A total of 52 academic staff participated in this study. We found non-significant overall PSQI score increments between the two periods (β = 0.20, p-value > 0.05). In addition, we found heterogeneity in sleep quality trajectories among subjects. On the other hand, sleep hygiene, individual resilience, and marital status significantly impact sleep quality among academic staff in the WFO period. Conclusion: There was no difference in sleep quality between the two time periods. The study highlighted the significant influence of sleep hygiene, resilience, and marital status on sleep quality among academic staff during the WFO period.

Abstract

Background: Cerumen impaction is a common condition all over the world. It is observed in around 5% of adults and has a higher prevalence in children, the elderly, and people with intellectual impairment. The management of cerumen impaction must be treated by general practitioners regarding insurance policies in Indonesia. Besides cerumen removal, in difficult cases for cerumen extraction, we can use cerumenolytic agents. Objective: This scoping review is to identify the current data on cerumenolytic drugs, emphasizing their effectiveness, safety profiles, and optimal treatment practices. Methods: Electronic databases were searched for medicinal treatment of cerumen impaction. The search technique entailed querying three esteemed databases—PubMed, SCOPUS, and ScienceDirect—utilizing the following search terms: (cerumen impaction) OR (cerumen prop) OR (cerumen obturans) OR (earwax) AND (cerumenolytic agent). Results: Ex vivo and in vitro studies demonstrated that carbamide peroxide had significantly more rapid cerumen degradation than phenol glycerol, distilled water was superior to commercial agents, and sodium bicarbonate surpassed docusate sodium, and potassium hydroxide had the quickest disintegration among the agents studied. In vivo investigations validated these patterns, with glycerin-hydrogen peroxide attaining the highest rates of tympanic membrane visualization and sodium bicarbonate demonstrating comparable efficacy to docusate sodium. Conclusion: Sodium bicarbonate was the most evaluated cerumenolytic agent, followed by docusate sodium and distilled water. There was no significant difference in weight growth or disintegration between sodium bicarbonate and docusate sodium, while wet cerumen exhibited a more rapid response than dry cerumen.

Abstract

Background: An unwitnessed death in a man with epilepsy, found in a bathroom with a head injury and immersion, raises key clinical and forensic concerns. This case highlights the complex intersection between epilepsy-related death, head trauma, and possible drowning. Case Presentation: A 39-year-old man with a history of epilepsy was found unconscious in the bathtub, face down with his head submerged and a bleeding forehead wound, approximately 10 minutes after showering. The man had a history of monthly generalized tonic-clonic seizures with recurrent falls and head injuries, and was on regular antiepileptic medicine. On admission, he was unresponsive with a GCS of E1V1M1, absent vital signs, dilated pupils, decreased skin temperature, and a laceration wound on the forehead. An ECG showed asystole. Conclusion: This case reviews the high risk of sudden death in patients with epilepsy, likely involving seizure-induced trauma and possible drowning. It emphasizes the need for comprehensive epilepsy management and thorough medicolegal evaluation to determine the cause of death.

Abstract

Introduction: Tuberculosis (TB) is an infectious and chronic disease with the highest mortality rate after ischemic heart disease and cerebrovascular disease. Indonesia has become the second-ranked worldwide for the highest estimated new TB cases. The TB treatment success rate (Treatment Success Rate/TSR) in Central Java Province stands at 87%, which is below the target of 90%. This study aims to provide an overview of the characteristics of TB patients at R.A. Kartini Hospital and the factors that influence the results of pulmonary TB therapy.Methods: This study is descriptive-analytical with a cross-sectional approach, using secondary data from the Tuberculosis Information System of R.A. Kartini Hospital from January 2023 to December 2023. Data were processed and analyzed using SPSS.Results: The average age of subjects was 39.26 ± 19.36, of which the majority were male (58.5%). Data showed that 58.1% of subjects had jobs, and the majority of subjects had no history of diabetes mellitus (DM) or human immunodeficiency virus (HIV) (86.6% and 96.7%, respectively). All subjects received tuberculosis (TB) treatment; most recovered (76.4%). Based on body mass index (BMI), 50.8% had a normal nutritional status. Chi-square test analysis showed a significant relationship between age and employment status with the success rate of TB treatment. However, the bivariate logistic regression analysis demonstrated that age was significantly related to TB treatment outcomes with an odds ratio (OR) of 1.033 (p = 0.000). Employment status revealed significant results; working subjects were 2.261 times more likely to experience suboptimal treatment outcomes (p = 0.013). Moreover, HIV history showed a significant relationship with TB treatment outcomes after adjusting for age with multivariate logistic regression analysis.Conclusion: Several factors were significantly related to TB treatment outcomes, such as age, employment status, and TB-HIV co-infection.

Abstract

Background: Cancer is a deadly disease that gets less attention in childhood. One of the manifestations of cancer in children is anemia. As many as 50% of cancer patients will experience anemia. Radiotherapy is one of the last treatments for various types of cancer. Radiotherapy can affect hematopoiesis, thereby reducing hemoglobin production. Aim: To describe pre and post-therapy Hb levels in pediatric cancer patients undergoing radiotherapy. Methods: This research is a descriptive study with Cross-sectional design using electronic medical records of as many as seventy eight people and then collecting data in the form of age, gender, patient diagnosis, and hemoglobin levels before and after radiotherapy. Results: Pre-radiotherapy and post-radiotherapy hemoglobin levels decreased significantly between the two groups (p<0.05). Conclusion: There is a significant difference between pre-and post-radiotherapy reduced hemoglobin levels in pediatric cancer patients undergoing radiotherapy

Abstract

Background: Ossifying fibroma (OF) is a benign bone neoplasm that mostly appears in the jaw. OF generally occurs between the second and fourth decades of life. OF can be challenging for clinicians, especially if the patient is in a vulnerable state. This case is particularly significant as it presents OF in a pediatric patient with developmental delay, which can complicate the diagnostic process and affect the accuracy of the patient's complaints. Purpose: This article presents a pediatric patient with developmental delay, which shows evidence of a jaw tumour. Case: A 12-year-old male patient with a chief complaint of swelling that gradually increased in size on the left lower jaw three months ago reported to the Department of Oral and Maxillofacial Surgery Mardi Rahayu Hospital Kudus and later on was diagnosed with OF. This case presents the management of OF in a patient with a particular condition of developmental delay, specifically in socioemotional delay. Case Management: Management of OF may need comprehensive treatment, including pre-operative preparation, supporting examination through radiographic and microscopic imaging, consideration of excision technique, and post-surgery follow-up. Conclusion: The unique challenges presented by the patient's developmental delay highlight the need for careful and clear communication of the treatment plan to ensure a good prognosis.

Abstract

Background: Bradyarrhythmias are infrequently encountered in women of reproductive age and are even less common during pregnancy, with an estimated prevalence of approximately 1 in 15,000. Total atrioventricular block (TAVB) identified during pregnancy is most often congenital in origin. In cases involving high-risk pregnancies, cardiac pacing can be safely administered under careful clinical supervision. Vaginal delivery is generally considered safe and does not confer additional risk, whereas cesarean section is reserved for established obstetric indications. Case Description: A 38-year-old pregnant woman at 38 weeks of gestation presented with bradycardia, and electrocardiographic evaluation revealed a total atrioventricular block (TAVB). Despite a heart rate of 41 beats per minute, she remained asymptomatic. Given that she had no prior history of pacemaker implantation, an urgent cardiology consultation was arranged. Subsequently, a transvenous temporary pacemaker was inserted prior to delivery. The patient underwent a cesarean section under spinal anesthesia and experienced an uneventful postoperative recovery. Conclusion: Management of total atrioventricular block during pregnancy requires a multidisciplinary approach involving both obstetricians and cardiologists. The implantation of a pacemaker is recommended and can be safely performed in high-risk pregnancies with appropriate monitoring. While cesarean section may be indicated based on obstetric factors, vaginal delivery remains the preferred mode of delivery when clinically feasible.
