The increasing burden of chronic diseases requires nursing care that is continuous, effective, and grounded in scientific evidence. This study aims to explore the implementation of evidence-based nursing care in the management of patients with chronic diseases within modern healthcare services. A qualitative case study design was employed, involving in-depth interviews, observations, focus group discussions, and document analysis with nurses working in chronic disease management settings. Data were analyzed using content and interpretative analysis through systematic stages of data reduction, data display, and data verification. The results indicate that evidence-based nursing care has been partially integrated into routine nursing practice, particularly in patient assessment, health education, and long-term monitoring. Nurses reported that evidence-based practices improved patient adherence, self-management abilities, and the consistency of care delivery. However, challenges such as workload constraints, limited access to scientific literature, and varying levels of research literacy were identified as barriers to optimal implementation. The findings highlight the importance of organizational support, continuous professional development, and access to evidence-based resources to strengthen nursing practice. This study contributes to nursing science by providing empirical insights into real-world implementation of evidence-based nursing care and offers practical implications for improving chronic disease management in modern healthcare systems.

Hypertension is a chronic non-communicable disease that requires long-term management through pharmacological treatment and healthy lifestyle modification. The success of hypertension control is strongly influenced by patients’ ability to perform self-management in daily life. This study aimed to analyze the determinants of self-management among hypertensive patients at Nusa Bakti Community Health Center, East Ogan Komering Ulu Regency, South Sumatra Province. This research used an observational analytic method with a cross-sectional design. The sample consisted of 245 hypertensive patients selected using the Lemeshow formula, and the valid data analyzed involved 240 respondents. Data were collected using questionnaires covering demographic characteristics, self-efficacy, and hypertension self-management. Data analysis was performed using univariate analysis, chi-square test, and logistic regression. The results showed that marital status, family history of hypertension, comorbidities, and self-efficacy were significantly associated with self-management. Multivariate analysis revealed that self-efficacy, marital status, and family history of hypertension were the dominant factors influencing hypertension self-management. These findings indicate that hypertension management should not only focus on medication but also strengthen patients’ confidence, family support, and awareness of hereditary risk. The study implies the need for family-based counseling and self-efficacy strengthening programs in primary health care services.

Tuberculosis remains a major infectious disease that threatens child health, particularly in communities where parental knowledge of transmission and prevention is still limited. This study aimed to determine the effect of health education on parental knowledge regarding the prevention of tuberculosis in children at Kenali Besar Community Health Center, Jambi City. This research used a quantitative approach with a pre-experimental one-group pre-test and post-test design. The population consisted of 753 parents visiting the immunization room, and 34 respondents were selected using purposive sampling. Data were collected through a knowledge questionnaire before and after the health education intervention and analyzed using univariate and bivariate analysis with a T-test. The results showed that the mean parental knowledge score increased from 6.38 before the intervention to 8.15 after the intervention. The statistical test obtained a p-value of 0.001, indicating a significant effect of health education on parental knowledge. These findings suggest that structured health education can improve parents’ understanding of tuberculosis transmission, cough etiquette, environmental hygiene, and early prevention. The study implies that community health centers should integrate routine tuberculosis education into child health services, Posyandu activities, and other community-based health promotion programs.

ST-Elevation Myocardial Infarction is a serious cardiovascular emergency commonly characterized by acute chest pain caused by impaired coronary blood flow and myocardial ischemia. This study aimed to analyze the effectiveness of deep breathing relaxation therapy in reducing acute pain among patients with ST-Elevation Myocardial Infarction in the Intensive Cardiac Care Unit of Sidoarjo Hospital. This research used a descriptive analytical design with a case study approach. Data were collected through interviews, observation, physical examination, and nursing documentation. The subject of this study was one patient diagnosed with ST-Elevation Myocardial Infarction who experienced acute left chest pain with an initial pain scale of 8. The nursing intervention provided was deep breathing relaxation therapy, which was performed when the patient experienced chest pain. The results showed that the patient’s pain scale decreased from 8 before the intervention to 3 after the intervention, indicating a substantial reduction in pain intensity. These findings suggest that deep breathing relaxation therapy can support pharmacological treatment by promoting relaxation, reducing muscle tension, and improving patient comfort. This study implies that deep breathing relaxation therapy may be used as a simple, safe, and low-cost non-pharmacological nursing intervention for acute pain management in patients with ST-Elevation Myocardial Infarction.

Type 2 Diabetes Mellitus is a chronic metabolic disease that can lead to unstable blood glucose levels and complications when not properly managed. This study aimed to describe the use of diabetic foot exercise as a nursing intervention to support blood glucose stability in a patient with Type 2 Diabetes Mellitus. A descriptive case study design was used with one female patient treated in the Tulip 2 Room at RSUD Sidoarjo. Data were collected through nursing assessment, observation, physical examination, laboratory records, and blood glucose measurements before and after the intervention. Diabetic foot exercise was given in three sessions, with each session lasting 15–30 minutes. The patient was guided to perform simple movements involving the feet and lower legs. The results showed a gradual reduction in blood glucose levels during the intervention period. Blood glucose decreased from 576 mg/dL before the first session to 475 mg/dL after the third session. Diabetic foot exercise may help support glucose control by increasing lower limb muscle activity, improving peripheral circulation, and encouraging self-care. This intervention is simple, low cost, and easy to apply in nursing care. However, it should be used together with medication, dietary management, and regular blood glucose monitoring

This study aims to describe the application of Qur’anic murottal therapy as a complementary nursing intervention to reduce anxiety in a pregnant woman with premature rupture of membranes in the VK Room of RSUD Sidoarjo. This research used a qualitative case study design with an experimental nursing intervention approach. Data were collected through nursing assessment, observation, interview, physical examination, anxiety measurement, and documentation of patient progress during three days of care. The patient was a 36-year-old pregnant woman with a gestational age of 39–40 weeks who experienced continuous amniotic fluid discharge, abdominal tightening, worry, confusion, restlessness, tension, increased blood pressure, and sleep disturbance. The intervention was conducted by asking the patient to listen to Qur’anic murottal recitation for approximately 15 minutes in a calm and comfortable environment. The results showed gradual improvement after the intervention. The patient’s confusion, worry, restlessness, and tension decreased, while sleep pattern improved. The patient also became calmer and more cooperative during the nursing care process. These findings indicate that Qur’anic murottal therapy can be used as a simple, safe, low-cost, and culturally appropriate spiritual nursing intervention to support psychological comfort in pregnant women with premature rupture of membranes.
