Abstract

A hemophiliac boy was hospitalized due to head injury in the previous sixteen days. On admission the' consciousness decreased and visus of both eyes was 1/on. CT scan showed blood clot in the area of dorsomedial of medial cerebral arteria and sign of hydrocephalus. A surgical procedure could not be taken due to lack of facilities (antihemophilic factor supply and assay of factor VIII activity). The patient was given 6 cryoprecipitate everyday and supportive treatment as indicated. Fortunately the patient recovered, no neurologic complication occurred except the visus of the right eye that was still 1/oo and the hydrocephalus decreased.Key words : hemophilia - laboratory diagnostics - head injury - hydrocephalus and decreased visus - cryoprecipitate

Abstract

Frozen-thawed human spermatozoa are routinely used for many assisted reproduction program.However, cryopreserved spermatozoa was reported to yield lower pregnancy rates compared tofresh semen in both intra uterine inseminations and in vitro fertilization/intracytoplasmic sperminjection (IVF/ICSI) due to the reduction of sperm motility and viability induced by cryopreservationprocedures. This study was aimed to evaluate the influence of cryoprotective agent (CPA) andcryopreservation methods on human sperm motility. This was a quasi experimental study. Thirtyseven normozoospermic semen samples collected in Permata Hati Infertility Clinics of Dr. SardjitoGeneral Hospital, Yogyakarta were recruited. Four different cryopreservation methods were appliedusing and without CPA (TEST-yolk buffer). In simple two steps freezing, cryostraw were graduallyfrozen from 8 to -4°C. In simple graduated freezing, cryostraus were directly frozen at -4°C. Invapor phase freezing method, the samples in cryostraw were placed 1 cm above liquid nitrogen.In the last method, the samples were directly submerged into liquid nitrogen. Thawing wasconducted by incubation at 37°C for 5 minutes. The sperm motility recovery after cryopreservationin the 4 different cryopreservation methods was evaluated and analyzed by analysis of variance(ANOVA). The fresh sperm motility before cryopreservation was 52.9 ± 4.50%. The recovery ofmotile sperms was 17.00 ± 7.83%, 20.96 ± 5.81%, 15.06 ± 8.55% and 15.68 ± 8.3%, whenusing CPA and 5.63 ± 4.63%, 5.47 ± 3.95%, 4.45 ± 4.46% and 6.08 ± 5.06% when withoutCPA following direct plunge to liquid nitrogen freezing, vapor liquid nitrogen freezing, simplegraduated freezing and simple 2-steps freezing, respectively. Among methods using CPA, thevapor phase method resulted in highest sperm motility recovery. In methods without CPA, nosignificant difference of sperm motility recovery was observed among the 4 differentcryopreservation methods. In conclusion, the use of CPA for cryopreservation improves spermmotility recovery.

Abstract

Patients with stroke, particularly during the hours following the acute episode, are in evolution, undergo various stages of pathophysiological readjustment. The brain lesion plays an important role in this process. In addition to affecting the central nervous system function, it keeps a close pathophysiological relationship with the respiratory system, the circulatory system, and all systems that participate in keeping the internal milieu in balance. These systems are often altered secondary to the brain lesion, but in other cases they are directly or indirectly responsible for the brain lesion or are worsening.The pathophysiological interrelation in all seriously ill or potentially seriously ill patients is of great importance, and of course, the same is true in stroke patient. Hence management is required that the treatment should be integrated and not divided by systems or according to the various medical specialties. The importance of comprehensive rehabilitation for the stroke patients with complex physical, psychic and social problems should be underlined. Medical, social, psychological and vocational measures must start early. Abstract

Heart disease is a major cause of death in both men and women in industrialized countries. In fact, many individuals,who develop heart disease, have normal cholesterol and blood pressure levels. It suggests that other risk factorsmay also play a role. Iron (Fe), zinc (Zn) and copper (Cu) may promote CHD by increasing lipid peroxidation andcausing oxidant-induced damage in various organs. These minerals are essential as cofactors for functional enzymesin the body. The hypothesis that Fe and Cu depletion protects against ischemic heart disease has generated significantdebate, but this hypothesis cannot be rejected until stronger evidence that high Fe stores increase the incidence ofCHD or death from myocardial infarction is proven. This research was aimed to find out the levels of Fe, Cu, and Znplasma as risk factors for heart disease patients, compared with those in hyperlipidemic patients and controls. Inthis study, we examined the Fe, Zn and Cu levels in plasma of CHD patients, hyperlipidemic patients, and healthycontrols by atomic absorption spectrophotometer. Thirty five CHD patients, 31 hyperlipidemic patients, and 26healthy controls. Age, body weight, body height, BMI, systolic and diastolic blood pressure of each group were notsignificantly different (p>0.05). There were higher cholesterol and lower HDL-C levels in CHD patients comparedwith those in hyperlipidemic patients and controls. Therewere significantly higher triglyceridemia level in hyperlipidemicpatients compared to those in CHD patients and controls. Iron level in CHD patients were not significantly differentthan those in hyperlipidemic patients and controls. Zinc level in CHD patientswere not significantly different comparedwith those in controls, but in hyperlipidemic patients, they were significantly lower than those in CHD patients andcontrols. Copper level in CHD and hyperlipidemic patients were significantly lower than those in controls. In thisresearch, it could be concluded that the decrease in Cu level might be one of the risk factors of CHD.Keywords : iron-zinc-copper-heart disease-risk factor

Abstract

Liver cytochrome P450 microsomal protein metabolizes certain xenobiotic compounds into reactive intermediates which covalently bind to DNA, RNA or protein of nucleus. The action of cytochrome P450 can be Influenced by nutrition status and environmental pollution. Aflatoxin Bi (AFB) is the environmental exposure problem in the countries with tropical climate. Aflatoxin Ell can also be oxidized by cytochrome P450 to DNA and protein - binding metabolites and can give rise to cell injury, mutation and cancer. The objective of this study is to Investigate the effect of dietary vitamin A (deficient, adequate, fourfold-adequate) against the toxic effect of AFBi in the liver cytochrome P450 microsomal protein of rats. Variance analysis of the result showed that the interaction of the vitamin A in the diet and treatment with AFB, had no significant effect. However the rats with dietary deficient and adequate in vitamin A, treated with AFBi showed significantly increased the cytochrome P450 (p< 0.00'1 I. The dietary fourfold-adequate in vitamin A was able to maintain the concentration of cytochrome P450 in liver microsomal protein of rats treated with AFB1 within normal range. It Is concluded that the effect of dietary vitamin A against the toxic effect of AFB, in liver cytochrome P450 microsomal protein of rats depends on the vitamin A concentrations in the diet consumed.Keywords: dietary vitamin A - aflatoxin Bt - liver microsomal protein - cytochrome P450 - Rattus norvegicus

Abstract

Presbyopia is a problem of old people. Usually asthenopic symptoms begin when the patients approach 40 years old. Refraction anomalies of hypermetropia and myopia influence directly the measure of reading spectacles, so that both these anomalies influence the possibility of asthenopic symptoms. Data from Out•Patient Department of Ophthalmology, Dr. Sardjito Hospital, during 6 months were 92 presbyopic•emmetropic, 15 presbyopic-myopic, and 75 presbyopic-hypermetropic. The asthenopic symptoms which excite patients to go for medical consultation were not in conformity with the theory concerning asthenopic symptoms. Abstract

Commemorating the centennial of the birth of the reknown Indonesian feminist, R. A. Kartini, the author touched upon the discrimination against women and the achievement of the emancipation movement in the medical profession. The author then reviewed the natural vigour of the female sex, starting with the thesis of Greulich and sex dimorphism. Subsequently, sex ratio and sex differences in mortality, life expectancy and prevalence of various diseases are discussed. Abstract

Background: About 30% patients with chronic idiopathic urticaria (CIU)have circulating histamine-releasing autoantibodies against the a-subunit of high affinity IgEreceptor (FceRla),or IgE.This subgroup of patients has a disorder commonly referred as autoimmune urticaria. Objectives: This study was conducted to reveal the autoimmune urticariacases in Indonesian patients Methods: The autologous serum skin test (ASST)and histamine release assay (HRA)were conducted on 79 patients with CIU (53 females and 26 males). Patients with predominant physical urticaria and urticarial vasculitis were excluded from the study. Results: Seventeen patients had both positive autologous serum skin test and histamine release assay confirmative of autoimmune urticaria. Conclusion:CombinedpositiveASSTand HRAwere seen in 21.5% of CIUpatients, indicatingautoimmune urticaria. Abstract

Vitamin A deficiency canoccur not only from inadequate intake but also because of poor in testinal absorption or inadequate conversion of provitamin A, as occurs in diseases of the liver or malnutrition. It has recently been reported that Zinc, Vitamin E and Vitamin C are necessary to-maintain normal concentrations of vitamin A in the plasma. Hypervitaminosis A may occur as a consequence of the administration of large doses of vitamin A concentrates. Vitamin A toxicity causes increases in levels of lipids, glycogen and citrate in the liver. As important therapeutic for vitamin A deficiency, supplementations of protein, lipid, Zinc, Vitamin E and vitamin C increase the efficacy of vitamin A therapy.
