Abstract

Diabetes mellitus is a multisystemic disease characterized by hyperglycemia due to defects ininsulin secretion and action, or both. Hyperglycemia is the most important factor in the onsetand progress of diabetic complications. Hyperglycemia increases the expression of receptor foradvanced glycosylation end products (RAGE) which leads to pancreatic -cells damage. Yellowsoybean (Glycine max) is reported to contain isoflavones which have various biological propertiesincluding antidiabetes. Dietary soybean can prevent the progression of diabetic complications.This study was conducted to investigate the effects of yellow soybean powder suspension onexpression of RAGE and pancreatic -cells damage of diabetic rats. Thirty streptozotocin (STZ)-induced diabetic Sprague Dawley male rats aged 11-12 weeks with body weight 200-250 g wereused in this study. The rats were divided into 5 groups with 6 rats in each group. Group 1 wasnon diabetic rats. Group 2 was diabetic rats without treatment. Group 3-5 were given yellowsoybean powder suspension of 400, 800 and 1600 mg/kg BW for four weeks, respectively. Atthe end of the experiment, pancreases tissues were removed for examination of RAGE expressionand pancreatic -cells. The results showed that yellow soybean powder suspension ingestionsignifantly decreased blood glucose level of diabetic rats toward normality (p<0.05). Moreover,the percentage of RAGE expression on Group 3 (50.01±2.75%) and Group 2 (53.03±4.02%)were not significantly different (p>0.05). Meanwhile, the percentage of RAGE expression onGroup 4 (42.43±4.08%) and Group 5 (40.62±3.42%) were significantly lower than Group 2(p<0.05). The percentage of pancreatic -cells on Group 2 (10.04±1.56%) was not significantlydifferent compared to Group 3 (8.61±0.81%) (p>0.05), whereas the percentage of pancreatic-cells in Group 4 (16.78±7.79%) and in Group 5 (22.03±11.51%) were significantly higherthan Group 2 (p<0.05). In conclusion, yellow soybean powder suspension can decrease RAGEexpression and prevent pancreatic -cells damage on STZ-induced diabetic rats.Keywords: pancreatic cells - Langerhans islets - streptozotocine - diabetic rats - soybeans

Abstract

Post-operative pain treatment in pediatric is frequently inadequate that could lead to psychological,physiological and behavioral changes. Therefore, adequate pediatric pain management is needed.Some analgesics such as ketorolac and regional anesthesia techniques such as caudal blockhave been applied to relieve pain. Ketorolac and caudal block have its own advantages anddisadvantages. The aim of study was to compare the cost-effectiveness of ketorolac and caudalblok as a post-operative analgesic in pediatric. This was double blind randomized controlledclinical trial with parallel design conducted in Dr. Sardjito General Hospital. The subjects werechildren who underwent surgery below umbilicus segment. Seventy patients were randomlydivided into two groups with 35 patients in each group. Group I (Caudal block Group) was givencaudal block with bupivacaine 0.12% 1 mL/kg body weight (BW) whereas Group II (KetorolacGroup) was given ketorolac 0.5 mg/kg BW intravenously (IV). The patient’s pain was scored at 0,15, 30, 45 minute and 1, 2, 3 hours after the conscious patients using modified Children’sHospital of Eastern Ontario Pain Scale (mCHEOPS). Furthermore, the cost-effectiveness theboth interventions was also compared. The caudal block was more effective in reducing painthan the ketorolac at minutes 0 (27/8 vs 10/25) and 15 (34/1 vs 18/17) (p<0.05). However, atthird hour the ketorolac revealed more effective than the caudal block (29/6 vs 32/3) (p<0.05).The cost of the caudal block was higher than the ketorolac (IDR 95.860 ± 5.745 vs IDR 7.200± 14.886) (p <0.05). However, the length of stay after the caudal block was shorter than theketorolac (40.43 ± 13.899 vs 48.57 ± 14.068) (p <0.05). Morphine was more needed forrescue analgesic in the ketorolac (p < 0.05) in first hour of operation, whereas after three houroperation paracetamol was more needed in caudal block (p < 0.05). In conclusion, the caudalblock is not more cost-effective than ketorolac in reducing post-operative pain in pediatricpatients underwent surgery below umbilicus segment.

Abstract

Sejak tahun 1970-an insektisida organofosfat (OP), temefos dan malathion, digunakan dalam program nasional pengendalian wabah demam berdarah Dengue di Indonesia, yang vektor utamanya adalah nyamuk Aedes aegypti. Dalam kurun waktu yang sama kedua senyawa OP itu diketahui menimbulkan resistensi pada Ae. aegypti stadium dewasa dan larva di Malaysia. Timbul dugaan bahwa Ae. aegypti stadium larva dan dewasa di Kodya Yogyakarta juga telah resisten terhadap kedua senyawa OP tersebut. Penelitian ini bertujuan untuk menetapkan status kerentanan Ae. aegypti stadium larva di Kodya Yogyakarta terhadap temefos dan malathion, dan mengetahui potensinya untuk menjadi resisten terhadap insektisida OP dibandingkan dengan populasi Ae. aegypti hasil kolonisasi di insektarium. Cara penelitian meliputi penggunaan uji mikroplat untuk peningkatan aktivitas enzim esterase non-spesifik pada Ae. aegypti stadium larva, diperkuat dengan uji hayati terhadap malathion dan temefos. Dari analisis data hasil penelitian tersebut disimpulkan, bahwa Ae. aegypti di Kodya Yogyakarta, stadium larvanya berkesan mulai resisten terhadap malathion dan temefos. Hal ini berbeda nyata dari Ae. aegypti stadium larva hasil kolonisasi insektarium yang masih sangat rentan terhadap kedua insektisida OP tersebut. Abstract

Malnutrition and malnourishment are still big problems among children in Indonesia. Age between12 to 59 months is the most important and critical time periods for the physical and intelligencedevelopment of children. Therefore, the children should obtain a good health care and nutritionaccording to their needs. The purpose of this study was to evaluate the influence of foodsupplementation on the nutritional status of children in Sleman District, Yogyakarta SpecialRegion. This was a quasi experimental study involving 60 severe malnourishment children betweenthe age of 12-59 months from Sayegan Sub Districts. Body weight and height of the childrenwere measured before and after food supplementation. Food supplements were given for 100days. The outcome of this study was the change of nutritional status based on Z-score accordingto the Ministry of Health classification adopted from WHO-NCHS (World Health Organization-US National Center for Health Statistics) recommendations. According to Z score for weight-forage(WAZ), the food supplementation did not improve the nutritional status of children. Althoughthe nutritional status of 25.87% children increased and the mean of WAZ significantly increased1.04 to be -3.29 after supplementation (p<0.05), the mean of nutritional status of the childrenwas still the worst (WAZ <-3SD). According to Z score height-for-age (HAZ), foodsupplementation did not improve nutritional status of the children either. The mean of HAZbefore food supplementation (-2.53) was not signifantly different compared to after foodsupplementation (-2.45). Moreover, mean of nutritional status of the children after foodsupplementation was still short (HAZ <-2SD). According to Z score weight-for-height (WHZ),food supplementation improved the nutritional status in 86.21% children. The mean of WHZsignificantly increased 1.13 from -3.35 to be -2.22 after food supplementation (p<0.05) indicatingthe change of nutritional status from wasted (WHZ <-3SD) to thin (WHZ <-2SD to -3SD). Inconclusion, food supplementation on children aged 12-59 months succeeds to improve theirnutritional status according to WHZ.Keywords: poor family - nutritional status - children - severe malnourishment - Z-score

Abstract

The reticuloendothelial system is the body's defence units, which are situated in every tissue's layer and tissue's folds of the body, and function as one functional unit.Its cellular components have the capacity to move to approach foreign particles by the help of the circulation function or by active amoeboid movements within the tissue, to phagocytose those foreign particles. Some of these cellular components have the capacity to make humoral antibody. Abstract

Combination of gypsum with carbonated hydroxyapatite (CHA) has shown to improve bone healing process. Softtissue biocompatibility test is required prior to clinical application of bone substitutes since the materials contactwith the soft tissues upon application and can cause severe inflammatory response. The aim of this study was toevaluate histocompatibility of gypsum compared to combination of gypsum-CHA in subcutaneous tissue. Disks ofgypsum and gypsum-CHA were implanted on paravertebral subcutaneous tissue of 25 male Wistar rats. Histologicalsection were stained with Hematoxylin Eosin then evaluated and scored with a histological grading scale for softtissueimplants. A two-way ANOVA was used to analyze the histomorphometrical results with 95% confidenceinterval (p<0.05). The results showed that acute inflammatory cells were found in both groups at 6 hour and on day5 and 7 after implantation at similar level. Chronic inflammatory cells and capsule were observed on day 7, 14, and21. Moreover, there was no significant difference (p>0.05) of histomorphometrics score between both implants ineach implantation periods. It could be concluded that histocompatibility level of gypsum and combination of gypsum-CHA to soft tissue was the same until day 21 iof mplantation.Key words: gypsum - carbonated hydroxyapatite – biocompatibility – histocompatibility - subcutaneous implant

Abstract

The Vogt-Koyanagl-Harada Syndrome is characterized by bilateral panuveitis and exudative retinal detachments, in association with cutaneous and neurosensory manifestations. This syndrome is seen most commonly in darker pigmented races, such as Orientals, Hispanics, American Indians, and blacks. It affects both sexes, although it appears to be a distinct female predominance. Most patients are between 20 to 50 years of age, but children and older adults also may be affected. A patient with some clinical symptoms of Vogt-Koyanagi-Harada (VKH) syndrome was reported. This patient had chronic bilateral iridocyclitis, posterior uveitis, exudative retinal detachment, cataracts related to cutaneous manifestations. This patient well responded on ophthalmologic manifestations by corticosteroid treatment but the other signs of VKH syndrome were still developing after 4 months period. The therapy for VKH syndrome was the high doses of systemic corticosterolds, and, with the severity of the anterior uveitis, topical corticosteroid was administrated frequently. A short-acting corticosteroid such as prednisone, in the range of 100-120 mg/day was given, based on the severity of the Inflammation. Some patients might require the addition of cytotoxic agents or cyclosporine.Key words : Vogt-Kayanagi-Harada syndrome - bilateral iridocyclitis - exudative retinal detachment - cutaneous manifestations.

Abstract

Two hundred and sixteen (9.1% of all deliveries) newborn infants with caesarean section in Dr. Sardjito Hospital during the period of November 1, 1982 to October 31, 1985 have been studied. It was shown that 54.4% of the cases with mothers age ranging between 25 —34 years, 40.5% of the mothers were primigravida and 67.6% with term gestational age. Caesarean section indications were placenta previa (47.7%), prolonged labour (23.0%) and cephalopelvic disproportion (16.4%) There were 76.4% of infants with a birth weight 2500 gram, 54.6% suffered from severe asphyxia and early neonatal deaths occurred in seven infants (3.2%). There were no significant differences between the type of caesarean section indications and the grade of asphyxia (p>0.05). A statistically significant difference was found between cases in 1 and 5 minute Apgar score resulted from resuscitation applied.
