Abstract

Low back pain is one of the common health problems that 50–80% of adults experience at some point in their life. The overall burden of LBP arising from ergonomic exposures at work was estimated at 21.8 million disability adjusted life years (DALYs). To apply the principles of family doctor medicine according to the problems found in patients in various aspects and carry out a holistic and comprehensive management that includes patient-centered, family approached, and community-oriented based on evidence-based medicine. This study is a case report. Primary data were obtained through autoanamnesis, physical examination and home visits, filling in family folders, completing family data, psychosocial and environmental data. Patient Mr. S, 57 years old, working as a mosque cleaner, came to Karang Anyar Primary Health Care, has a complaint of lower back pain since 2 months ago. The pain felt was like being stabbed and radiated to the patient's left leg. Initially, the patient admitted that the pain didn’t interfere with his work activities, but since the last 2 weeks before came to the Primary health care, the patient complained that the pain was getting worses and interfered with his activities. The patient was diagnosed as Low Back Pain (LBP). There are several factors that influence the patient's condition, namely internal and external risk factors. Holistic management with a family medicine approach is needed for patients with to low back pain to identify problems in various aspects in order to support the success of therapy.

Abstract

One of the main public health problems for the elderly, both men and women, is osteoporosis. Many factors accompany problems in the elderly such as aging, menopause, endocrine disorders, weakening of physical activity, as well as side effects of certain drugs, which contribute to complaints of osteoporosis. The presence of osteoporosis results in increased bone fragility while almost all bones are susceptible to fracture. The incidence of fracture events is closely related to the increase in osteoporosis which is indicated by increasing age. Measures to diagnose and prevent osteoporosis and its complications are a major issue and concern at this time as an effort in public health because the increased incidence of fractures is also associated with increasing health care costs and accompanied by physical disability and impaired quality of life, even the effect has an impact on increasing mortality rate. Currently, BMD (bone mass density) measurement is a key factor in identifying patients at risk of fracture. The measurement results of BMD are used as a reference for making treatment therapy decisions, as well as for monitoring the progress of the therapy being undertaken. Several risk factors for osteoporosis have also been identified for their treatment potential, which can be modified and cannot be modified. Treatment of potentially modifiable risk factors along with sports activities accompanied by calcium (Ca) and vitamin D supplements is important information for the management or pharmacological management of osteoporosis.

Abstract

Glomerulonephritis is a medical term used to describe a condition that involves some kidney damages. Glomerulonephritis is an abnormality of the kidney that may lead to end-stage renal disease (ESRD). The current glomerulonephritis therapy is considered not fully effective due to the possibility of additional morbidity and complications that may occur. The use of Astragalus as adjuvant therapy can inhibit the progression of glomerulonephritis. The journals, that was reviewed, obtained from the Google Scholar, Sciencedirect, PubMed, and Springer with a publication year range of 2009-2023. The journals were selected to obtain valid and reliable journals, then do a literature review and article writing. Astragalus membranaceus has a bioactive content that can act as an immunomodulator, antiinflamation, and renoprotector. Astragalus is able to improve kidney function through IL-6 secretion and improve kidney interstitial fibrosis. Administration of Astragalus in combination with conventional therapy can reduce serum creatinine and proteinuria. Astragalus has potential as adjuvant therapy in glomerulonephritis patients.

Abstract

COVID-19, which spread throughout the world, causing the disease to be declared a pandemic, has affected all aspects of people's lives without exception in the world of education, which was previously conducted face-to-face, now the learning method has changed to online learning. This causes many things to be done by using communication tools as a socializing chain with the surrounding environment. Online education is a new method of learning through networks that has been in place since early 2020, which is one of the steps to reduce the spread of COVID-19. Distance Education can have an impact on students' psychology. This causes many things to be done by using communication tools as a socializing chain with the surrounding environment. Mental health is becoming a problem in the midst of the COVID-19 pandemic, especially for elementary school students. Mental health is constantly changing, creating a variety of major problems that must be resolved. This article aims to determine the mental health of students while undergoing online learning during the COVID-19 pandemic era. The type of review used in this journal is in the form of a literature review of 40 articles found through the NCBI database, PubMed, Google Scholar, only 22 articles were selected from 2020 to 2022. The result of this article is to explain the latest updates regarding the impact of online learning in the era of the COVID-19 pandemic. 19 on mental health.

Abstract

HIV encephalitis, also referred to as HIV-associated neurological disorder (HAND), includes a range of neurocognitive defects of varying severity following HIV infection. The clinical presentation of this condition varies from asymptomatic or minor neurocognitive impairment to severe dementia. HIV encephalopathy, also known as AIDS-dementia complex, lies at the most severe end of this spectrum. The diagnosis is based on a combination of clinical neuropsychiatric evaluation and radiological studies. Prompt and effective administration of antiretroviral therapy (ART) is the most effective therapy in managing HIV encephalitis. It has been reported that a 32-year-old male suffers from HIV AIDS with encephalitis. Patient Treated with hemiparese dextra, History of high fever, seizures, and loss of consciousness. The patient also has a history of pulmonary TB and has been taking OAT for the past 9 months. The patient also admitted that he had previously been infected with HIV but had never taken ARVs. From the physical examination found awareness of compos mentis, oral candidiasis in the mouth, and black spots all over the patient's body. There is no neck stiffness, there is parese N.VII and N.XII dextra and positive Babinsky on both legs. Organizational function and memory are impaired. On investigations, a reactive HIV Rapid test was obtained, anemia, leukocytopenia, lymphocytopenia, hyponatremia, hypokalemia, and the results of a CT scan of the head without contrast found hydrocephalus and a chest X-ray within normal limits. Patients were treated with dexamethasone, ranitidine, vitamin B12, and folic acid and were consulted by specialists in internal medicine and neurosurgery for management according to the field.

Abstract

Diabetes mellitus (DM) is a group of metabolic diseases associated with increased blood glucose. Chronic hyperglycemia in diabetes mellitus contributes to the emergence of various complications in the form of damage, dysfunction and failure of various organs, one of which is the kidney. Kidney is a vital organ that plays a very important role in maintaining the stability of the environment in the body. The kidneys regulate the balance of body fluids, electrolytes and acid base by filtering blood through the kidneys, selective reabsorption of water, and excreting excess as urine and removing metabolic waste (urea, creatinine and uric acid) and foreign chemicals. Diabetic nephropathy is a degenerative kidney vascular disorder, associated with disturbances of carbohydrate metabolism or sugar intolerance (diabetes mellitus). This study aims to see the relationship between diabetes mellitus and the incidence of chronic kidney failure. Several journals have found a positive relationship between diabetes mellitus and the incidence of chronic kidney failure.

Abstract

Indonesia is ranked 19th in the world regarding deaths due to asthma. Asthma is a heterogeneous disease characterized by chronic airway inflammation. Asthma is a reversible obstructive disorders which is requires control to improve the patient's quality of life. Holistic management through a family medicine approach is expected to be able to assist patients in identifying, controlling precipitating factors and preventing individuals with predisposition to asthma from developing asthma and preventing asthma patients from experiencing asthma attacks. In this study, primary data were obtained from autoanamnesis, physical examination and home visits. Secondary data was obtained from the patient's medical record. Patient Ms. C, 21 years old, has a main complaint of shortness of breath accompanied by a feeling of heaviness in the chest, coughing, and "wheezing" breath sounds that are felt to come and go and get worse, especially at night, early in the morning, cold weather, dust exposure and when the patient is under stress. The patient was diagnosed as mild persistent asthma. Internal risk factors include family history of asthma and atopy, lack of patient knowledge about the disease, and curative treatment behavior. External risk factors include lack of knowledge and attention of the family towards the patient's illness and dusty house conditions and lack of air circulation through windows. Medical and non-medical interventions are carried out for patients and families according to theory and literature with a patient-centered, family-oriented, and community-oriented approach.

Abstract

Gout arthritis is a condition where there is an increase in serum uric acid levels above normal. Risk factors that influence high uric acid levels are age, genetics, excessive purine intake, obesity, and post menopause. This disease is important to be managed comprehensively because treatment goals can be achieved so that management needs to be carried out through a family approach. Identify internal and external risk factors and clinical problems present in the patient. Apply a holistic and comprehensive approach to family doctors according to the problems found in patients and carry out evidence-based medicine-based management that is family-approach, patient-centered and community-oriented. The study conducted is a Case Report. Primary data were obtained through history taking, physical examination through home visits and filling in the family folder, secondary data were obtained through patient medical record. Assessment is based on a holistic diagnosis of initial, process, and end visits. Patient Mr. I, 85 years old, complained of pain in the joints of the fingers of his right hand since 7 days ago. The patient has had these complaints since ± 20 years ago. Clinically the patient was diagnosed with Gout Arthritis (ICD-X M10.0). Furthermore, holistic management is carried out, namely intervention using poster media. In the evaluation, the results were obtained in the form of a better understanding of the disease and changes in behavior that have an impact on the success of therapy. The diagnosis and management of these patients has been carried out holistically, patient centered, family approach and community oriented based. In the process of behavior change, the patient has reached the trial stage.

Abstract

Excessive consumption of monosodium glutamate (MSG) has a neurotoxic effect on the cerebellum, which will lead to a state of oxidative stress characterized by decreased activity of catalase. This study aims to determine the effect of Lampung Robusta Coffee extract (Coffea canephora) againsts to activity of the cerebellar catalase enzyme in male rats (Rattus norvegicus) strain sprague-dawley induced by monosodium glutamate. This research is a type of experimental study with a Posttest Only Control Group Design approach. The subjects were 25 rats which were divided into 5 groups which are K- (aquadest 3,5 ml/day), K+ (MSG 4 g/kgBW/day), P1, P2, P3 (MSG 4 g/kgBW/day and Lampung Robusta Coffee extract 1,5 ml/200gBW/day with a concentration of 0,03 g/ml; 0,06 g/ml; 0,12 g/ml respectively) for 14 consecutive days. The activity of the cerebellar catalase enzyme is calculated with spectrophotometer 210 nm. The average specific activity of the cerebellum catalase enzyme in K +, K-, P1, P2 and P3 were 0.00104 U/mg, 0.00154 U/mg, 0.00139 U/mg, 0.00161 U/mg, and 0.00190 U/mg respectively. One-Way ANOVA test obtained p value = 0.001 (p <0.05). Post Hoc LSD test showed a significant difference (p <0.05) between the K + and K- (p = 0.007), K+ and P2(p = 0.003), K + and P3 (p = 0.000), and P1 and P3 (p = 0.007), which means that there is an effect of Lampung Robusta Coffee extract (Coffea canephora) againsts to activity of the enzyme catalase of cerebellar in male rats (Rattus norvegicus) strain Sprague-dawley induced by MSG.
