Abstract

Measles is a highly contagious disease. Measles is an important cause of child death worldwide. Groups of children of pre-school age and elementary school age are vulnerable to contracting measles because they have weak immune systems. Measles can cause immune amnesia which will affect the immune system, making sufferers susceptible to other diseases or complications. This indicates that holistic and comprehensive patient care is an important matter through the role of the family doctor. The purpose of writing this article is to identify internal, external risk factors and clinical problems found in patients, apply a holistic and comprehensive family doctor approach according to the problems found in patients, and carry out evidence-based medicine management that is family-approach, patient-centered and community oriented. The study is a case report. Primary data were obtained through history taking, physical examination and home visits. The assessment is based on a holistic diagnosis from the beginning, process and end by means of in-depth interviews, observations and patient filling sheets. Patient An. H, 3 years old, has had a continuous fever since 6 days ago, accompanied by a cold cough. On the fourth day of fever, reddish spots appear, starting behind the ears and then spreading to the face, neck, chest, stomach and arms, accompanied by red and watery conjunctiva of the eyes. Neighbor's children experienced the same complaint. The patient has not received measles immunization.

Abstract

Myelopathy is a description of a neurological deficit caused by spinal cord abnormalities. One of the conditions that can cause myelopathy is a spinal cord tumor. Spinal cord abnormalities based on type and location are assessed using the American Spinal Injury Association (ASIA) scale which is divided into grades A, B, C, and D. The prevalence of spinal cord tumors is 15% of all central nervous system tumors. Based on its location, spinal cord tumors are divided into intradural and extradural. Manifestations of spinal cord tumors consist of sensory, motor, and autonomic disturbances according to the location of the compression lesion. The diagnosis is made based on anamnesis, physical examination and supporting examinations. In patients with spinal cord tumors, radiological investigations can be carried out. The principle of management of spinal cord tumors is to remove the tumor to restore maximum neurological function. The choice of further management for spinal cord tumors is consulted with a neurosurgeon. The prognosis of spinal cord tumors depends on the severity of the patient. Case report Mrs. W, 41 years old with diagnosis myelopathy et causa tumor medula spinalis based on anamnesis and physical examination and supporting investigation given therapy NaCl 0,9% infusion 20 drops per minute, vit B12 3x1 tablet, sodium diclofenac 2 x 50 mg tablet, and consulted with neurosurgeon to obtain further management.

Abstract

Definitive airway refers to the creation of a safe and patent airway to ensure adequate gas exchange, usually involving the trachea and special airway devices. Definitive airway is the standard of care in pre-hospital airway management, especially in trauma patients. Definitive airway has an important aspect in airway management, especially in emergency and trauma care, and its implementation is essential to ensure patient safety and stability. Proper training and routine maintenance skills are key to increasing the success rate of airway management procedures. A definitive airway can be performed if a clinical picture is obtained, including guarantees for maintaining a patent airway, failure to maintain adequate oxygenation with an oxygen mask or the appearance of apnea, airway guarantees including respiratory failure (hypoxia or hypercapnia), the presence of a head injury and requiring broadcast assistance, and decreased consciousness with a GCS (Glasgow Coma Scale) score of 8 or less. Patients with serious or persistent airway compromise usually require an airway device. The most common devices used in definitive airway include orotracheal intubation, nasotracheal intubation, and surgical airway (cricothyroidotomy and tracheostomy). The choice of airway device and technique is tailored to the specific clinical scenario and patient condition.

Abstract

Chalazion is a granulomatous inflammation with lipogranuloma or chronic non-infectious inflammation of the eyelid, in the form of a nodule formed due to inflammation and obstruction of the tarsal meibomian gland or zeis. This chalazion often occurs in all age groups, but the chalazion occur more commonly in the productive age. We report the case of a 20-year-old man with complaints of a lump on the upper eyelid, at the left and right eye for 4 months before entering the hospital. The lump is initially small but gets bigger over time, becomes red, feels painful and lumpy. This complaint has been repeated since 10 months ago. Currently the lump no longer feels painful, itchy or hot, but the lump is now permanent. On ophthalmological examination, it was found that the superior eyelid of the right eye had a nodule measuring ±5 mm and the superior eyelid of the left eye had a nodule measuring ±3mm. The diagnosis made was acute exacerbation of chalazion of right eye, then the patient was given non-medicamentosa therapy, medicamentosa, and operative treatment.

Abstract

Lumbar herniated nucleus pulposus (HNP) is a condition where fibroblasts experience degenerative or external changes in the intervertebral disc and nucleus which can cause neurological symptoms due to compression of the dura mater or nerves. The prevalence of disc herniation is around 1 to 3% and increases with age up to age 80 years. It often occurs in men compared to women with a ratio of 2:1. Lumbar invertebral discs are complex structures that frequently experience axial loads. Due to the biomechanics of this structure and its avascular nature, lumbar disc herniation often occurs. A 65 year old woman came to the emergency room at RSAM with complaints of low back pain that radiated to the thigh, calf to the sole of the right foot, as if being pricked and getting worse since 3 weeks before entering the hospital. The patient works as a farmer and when working often carries heavy objects and the patient's working position is often bent. Denied history of falls. The patient did not complain of disturbances in BAK and defecation. From the results of the physical examination, Lasseque's test was positive on the right. A lumbosacral MRI radiological examination was carried out and the results showed multiple protruded discs in the L4-L5 and L5-S1 intervertebralis accompanied by a diffuse posterior herniated nucleus pulposus which was pressing on the anterior saccus thecalis and bilateral spinal roots. Patients are treated with medication and rehabilitation. HNP is the most common cause of lower back pain complaints in the elderly. Immediate treatment can impact the prognosis.

Abstract

Physical activity has an important role in increasing muscle strength. High intensity physical activity can build muscle mass ,endurance and increase muscle strength compared to low intensity physical activity. One way to determine the strength of the hand or upper extremity muscles can be measured by the strength of the hand grip muscles. In this study, researchers wanted to look at the differences in hand grip muscle strength between calisthenics athletes and building construction workers. The research was conducted with a cross sectional approach using total sampling techniques. Data was taken in November 2023 at Saburai Field and Unila Bandar Lampung with 40 calisthenics athletes and 40 building construction workers who met the inclusion and exclusion criteria. Data was obtained by measuring using a Camry dynamometer. Data were analyzed using the Independent T-test. The result showed that there was a significant difference between the hand grip muscle strength of calisthenics athletes and building construction workers with a p value of 0.000. It concluded that there was a significant difference between the hand grip muscle strength of calisthenics athletes and building construction workers.

Abstract

Prematurity Iminens is a threat to pregnancy where signs of labor appear at preterm gestational age (20 weeks – 37 weeks) and the baby's birth weight is less than 2500 grams. In a patient with G5P4A0 28 weeks pregnant with complaints of lower abdominal pain and cramps. Complaints accompanied by blood coming out of the birth canal at 14.00 WIB, BAK and defecation felt painful. Imminent premature parturition is a threat to pregnancy where signs of labor appear at preterm gestational age (20 weeks – 37 weeks) and the baby's birth weight is less than 2500 grams. Imminens premature labor is diagnosed with contractions that repeat at least once every 7-8 minutes, or 2-3 times within 10 minutes, pain in the lower back, spotting bleeding, feeling of pressure on the cervix. Pregnant women who are at risk of preterm labor and/or showing signs of preterm labor need intervention to improve neonatal outcomes. The conclusion has been established through a working diagnosis of G5P4A0 28 weeks pregnant with Partus Prematurus Imminens (PPI) based on clinical considerations, physical examination findings, and supporting examinations. Several types of drugs that can be given as tocolysis are calcium antagonists, for example Nifedipine 10 mg/oral repeated 2-3 times/hour, continued every 8 hours until contractions disappear and can be given again if contractions recur and the maintenance dose is 3 x 10 mg. Other alternative medications are other types of β-mimetics such as salbutamol, terbutaline, rithrodine and soxuprin or magnesic sulfate (MgSO4) and antiprostaglandins (indomethacin), but these are rarely used because of side effects on the mother or fetus. The aim of providing corticosteroid therapy is to mature fetal lung surfactant, reduce the incidence of RDS, prevent intraventricular bleeding, which ultimately can reduce the risk of neonatal death. Corticosteroids need to be given if the gestational age is <35 weeks. The drugs that can be given are dexamethasone (at a dose of 4 x 6 mg i.m with a 12 hour interval) or bexamethasone (at a dose of 2 x 12 mg i.m with a 24 hour administration interval).

Abstract

Hydrocephalus is one of the most common sequelae after subarachnoid hemorrhage, and is a major cause of high morbidity and mortality from this disease. Changes in cerebrospinal fluid (CSF) dynamics, obstruction of arachnoid granulations by blood products, and adhesions in the ventricular system are some of the mechanisms by which hydrocephalus occurs after subarachnoid hemorrhage. Inflammation, apoptosis, autophagy, and oxidative stress are important causes of hydrocephalus. Transforming growth factors, matrix metalloproteinases, and iron ultimately cause fibrosis and blockage. Surgery is the most common and efficient therapy, although there are risks from different surgical methods, including lamina terminalis fenestration, ventricle-peritoneal shunting, and lumbar-peritoneal shunting. Case report of a 46 year old male patient. A 46 year old man with severe cephalgia that comes and goes and interferes with activities throughout the head such as being pricked since 2 weeks of before coming to hospital. Other complaints of vomiting spraying, slightly black color 6x, fainting 2x for <15 minutes. Physical examination revealed stiff neck and Kernig's sign. CT scan showed subarachnoid hemorrhage filling the interhemispheric fissure, bilateral frontal cortical sulci, left temporal, suprasellar cisterna and Hydrocephalus communicans. In addition, hyponatremia was found, (133 mmol/L).

Abstract

Vertigo is a common complaint presenting frequently in primary care and emergency departments, which is a symptom of vestibular dysfunction and is described as a sensation of movement, most commonly rotational movements. Trigeminal neuralgia (TN) is defined as sudden, severe, brief, stabbing, electric shock-like and repetitive pain that is almost always unilateral in the distribution of one or more branches of the trigeminal nerve, which is highly disruptive and impacts basic human function. The analysis of this study is a case report of Mrs. N, 61 years old, came to the emergency room at Abdul Moloek Regional General Hospital with complaints of spinning dizziness as if the environment around him was spinning which occurred suddenly and pain on the left side of his face like a cut that had been getting worse since 1 day. Complaints of pain on the left side of the face spreading to the chin and left side of the neck have been felt for 2 years. On sensory examination of the Trigeminal Nerve, hyperalgesia and allodynia were found according to the left V1,2,3 dermatomes. The results of the patient's physical examination showed good hearing function, horizontal nystagmus (+), and Romberg's sign (+) balance test, heal to toe walking test (+), past pointing test (+). The patient's clinical diagnosis was vertigo and facial pain (neuralgia). Topis diagnosis, peripheral vestibular system and Trigeminal Nerve. Etiological diagnosis, peripheral vertigo and Trigeminal Neuralgia. The patient's management was infusion of Ringer's Lactate 20 drops/minute, Betahistine 2x24 mg, Omeprazole injection 40mg/12 hours, Carbamazepine 3x200 mg, Amitriptyline 1x25 mg, and Paracetamol 2x1000 mg fls. The prognosis for this patient is quo ad vitam dubia ad bonam, quo ad functionam dubia ad night and quo ad sanationam dubia ad bonam.

Abstract

Allergic contact dermatitis (DKA) is caused by antigens (allergens) which give rise to type IV hypersensitivity reactions. In Indonesia, it was shown that 97% of 389 cases were contact dermatitis, 66.3% of them were irritant contact dermatitis and 33.7% were allergic contact dermatitis. Applying the principles of a holistic and comprehensive family doctor approach in detecting risk factors, clinical problems, and patient management based on evidence-based medicine and being family-approached and patient-centered. This study is a case report. Primary data was obtained through history taking and physical examination. Secondary data was obtained from patient medical records at the Community Health Center. Assessment is based on a holistic diagnosis of the beginning, process and end of the study quantitatively and qualitatively. Patient Mr. I, 22 years old, came to the Simpur Community Health Center clinic with complaints of red, scaly patches appearing, accompanied by itching on both legs and since 2 months ago until now. Initially, the patient was exposed to stagnant water at his workplace. Over time, the patient complained of reddish rashes accompanied by itching. The patient wore socks and shoes the next day, complaining that the reddish rashes were becoming more numerous and the itching on both feet was getting worse. The patient worked as a construction worker. The patient was diagnosed with allergic contact dermatitis. There are several factors that influence the patient's condition, namely internal and external risk factors. Holistic management with a family doctor approach is needed for patients with allergic contact dermatitis in identifying problems in various aspects to support the success of therapy.
