Abstract

Obesity is a state of imbalance in body weight and height due to excessive fat tissue in the body. The incidence rate of overweight or obesity is increasing over time and has even become a global pandemic with a prevalence of 2.1 billion in 2013, increasing from 857 million cases in 1980. Obesity can be assessed from the body mass index (BMI) where World Health Organization (WHO) classifies obesity with BMI ≥ 30 kg / m2. Obesity generally occurs in pregnancy, especially pregnancies that occur above 35 years. Obesity is a high obstetric risk because it can increase the risk of maternal and fetal morbidity and mortality. Complications that can occur in pregnant women with obesity in pregnant women are an increased risk of hypertension, gestational diabetes, spontaneous abortion and postpartum hemorrhage. The fetus can increase the risk of stillbirth in the antepartum period, intrapartum complications such as shoulder dystocia, macrosomia and increase the risk of fetal defects such as neural tube defects, spina bifida, congenital heart disease and omphalocele. Macrosomia is a condition in which the baby is born with a larger size or weight> 4000 grams, this is one of the complications of labor which causes increased morbidity and mortality in pregnancy because its complications in childbirth.

Abstract

Ptosis is a condition when the palpebrae drop from their normal position. Ptosis describes the dropping of the upper eyelid  so that it covers half or all of the pupils which can disturb the vision. This condition got worse in downgaze position. Based on its onset, ptosis is divided into congenital and acquired. Neurogenic ptosis is the most common aquired ptosis, mainly due to paresis oculomotor nerve (CN III). In the elderly, it is necessary to suspect the paresis due to ischemia, especially in patients with underlying systemic disease. This case report describes a 48 year old woman with the chief complaint of painless upper eyelids ptosis since 4 months ago. Initially it was only partially closed, and the last 2 months it became completely closed. Visual acquity of the left eye  6/60, eye movements were full, pupil were equal and reactive, Relative Afferent Pupillary Defect (RAPD) were not found, and normal Bell’s phenomenon. Margin Reflex Distance (MRD)-1 score-5 mm, Interpalpebral Fissure (IPF) score 0 mm, and  poor levator function 0 mm. The anterior segment is within normal limits. Laboratory tests, head CT scan, and MRI were within normal limits. Based on the results of the history, ophthalmological examination, and investigations, the patient was diagnosed with ptosis ocular sinistra et causa nerve oculomotor (CN III) paresis incomplete-without pupillary involvement. The patient get non-steroidal anti-inflammatory drugs, observation, and planned for frontalis sling surgery.

Abstract

Leukocytosis can be found in patients with lung cancer at the time of diagnosis or during the course of the disease. Hyperleukocytosis was defined as a leukocyte count above 11.000/mL on pheripheral blood. This case report describes hyperleukocytosis in a paraneoplastic syndrome resulting from lung cancer in a 42-year-old man who has had fever and weakness since the last 3 months. Peripheral blood morphological examination, there is increased leukocyte, 88% segment neutrophils, no blast cell and negative BCR-ABL examination. In people with lung cancer, leukocytosis can be caused by infection, use of corticosteroid,  hematopoietic disorders, bone marrow involvement, or paraneoplastic manifestations. Paraneoplastic leukocytosis is associated with poor prognosis and aggressive disease, so the diagnosis of hyperleukocytosis in paraneoplastic syndromes is important because it determines the patient's prognosis. Paraneoplastic syndrome is a symptom seen in certain malignancies and is not associated with direct cancer invasion and occurs in 7-15% of all patients with cancer. Solid tumors activate a variety of different systems, namely the endocrine, neurological, mucocutaneous, and hematological systems. Granulocyte Macrophage Colony Stimulating Factor (GM-CSF), Granulocyte Colony Stimulating Factor (G-CSF), interleukin (IL) -1, IL-6 and Tumor Necrosis Factor (TNF)-α have been implicated in paraneoplastic leukemoid reactions. Among these, G-CSF is the strongest cytokine that triggers hyperleukocytosis.

Abstract

Indonesia is the second country with the highest incidence of TB cases in the world in 2020. New TB cases in Indonesia is 543,874 cases in 2020 (20 March 2020). Meanwhile, there were 777 new TB cases in Lampung province (June 2020). The increase in TB infection is influenced by several factors, age, gender, nutritional status, level of cleanliness, ventilation, temperature, lighting, occupant density, and education. Gout arthritis is an inflammatory joint disease that is influenced by the intake of high-purine foods or other factors such as anti-tuberculosis drugs. Pyrazinamide can inhibit uric acid excretion in the kidneys and increase the value of blood uric acid. Therefore, it takes the role of a family doctor who can manage patients from various aspects. The aim of research is to apply a holistic and comprehensive family doctor approach in detecting internal and external risk factors and solving problems based on Evidence Based Medicine which is family-approached and patient-centered. This study is a case report. Primary data from autoanamnesis, physical examination, and home visits. Secondary data from patient medical records. The patient has functional grade 2 in pulmonary TB disease and Gout Arthtitis with internal risk factors, the patient's lack of knowledge of the disease and curative treatment behavior; External factors are low income families, home environment is close to dusty and slum areas. The intervention are education for control according to schedule, cough ethics and TB transmission, and low-purine diet. In the evaluation there are changes in the patient's behavior and diet.

Abstract

Trauma is another word injury that can injure. Trauma is another word injury that can injure. Injury to the bones results in fractures (fractures) and dislocations. Fracture is a condition of bone discontinuity. Fractures are divided according to the contact of the outside world, which includes closed and open fractures. The classic symptoms of fracture are a history of trauma, pain and swelling in the fractured bone, eformity (angulation, rotation, discrepancy), tenderness, crepitus, impaired musculoskeletal function due to pain, broken bone continuity and neurovascular disorders. Fracture is one of the causes of trauma to the blood vessels (vascular). Vascular trauma should be suspected in the case of trauma occurring in areas anatomically traversed by large blood vessels. This case report is a man aged 23 years, having difficulty moving his right and left hands, and right lower leg since ± 12 hours of SMRS. Post traffic accident patients ± 10 hours SMRS. From the examination of the localis status of the right brachii region, it was obtained swelling, hematoma ± 5x4 cm, vulnus laceratum ± 2x0.5 cm; there is tenderness, wick pain and increased pain; Pain-limited ROM. The status of the left brachii localist was obtained by swelling, hematoma in the left brachii region ± 6x3 cm, vulnus laceratum ± 2 cm, pulsation A. Radialis not palpable, cold acral from 1/3 distal to the brachial region to the manus, hypoaesthesias and anesthesia present, limited ROM. Localis status of the femoral region until the right cruris is rotated, vulnus laceratum ± 30 cm, pain-limited ROM. In emergency ward, we treated the patient with oxygen administration through no re-breathing mask with 10 ml of oxygen per-minute, infusion with crystaloid solution, wound irrigation,  broad spectrum antibiotic, analgetic and fracture stabilisation with posterior slab. The patient underwent urgent surgery to treat his fracture and neurovascular trauma.

Abstract

Empathy is the ability to participate in feeling or imagining other people's feelings and this is one of the competencies that medical graduates must possess in order to create good doctor-patient relationships. A good doctor-patient relationships can improve their compliance and satisfaction with the services provided, so that it will provide better clinical results. However, it was found that during medical education, the level of student’s empathy could change due to various factors, one of them is level of study. The purpose of this study was to find out the mean empathy score between early and end clinical clerkship student, and to find out whether there were significant differences between the levels of study and the level of empathy of the clerkship students of the Medical Faculty of Lampung University. The research method used was an observational study with a cross sectional analytic approach at Abdoel Moeloek Hospital in February 2019. The research instrument used was The Jefferson's Scale of Physician Empathy - Student Version (JSPE - SV). Before processing the data, a normality test was carried out using the Kolmogorov Smirnov test. Then the data is processed using univariate tests and unpaired t-test as the bivariate tests. The results obtained are a mean empathy score of 100.09 which is included in the medium category. Based on the results of the unpaired t-test, it was found that there were statistically significant differences in the level of empathy among clerkship medical students in Medical Faculty of Lampung University.

Abstract

Clinical Clerkships refers to a program designed for medical students to qualify a competent doctor. This program is carried out in the teaching hospital, network hospitals, community health center (Puskesmas) and companies. The aim of the study was to determine the effectiveness of clinical teaching in the RSUD Dr. H. Abdul Moeloek of Lampung Province. This study was a descriptive study using a purposive sampling. The subject of this research were 29 clinical clerkships students in the first data retrieval, and 21 each in the second and third data retrieval in the Department of Surgery at the RSUD Dr. H. Abdul Moeloek of Lampung Province. The data was collected using The Clinical Teaching Effectiveness Instrument (CTEI) and analyzed using univariate analysis.This study showed the overall effectivity of the clinical teaching in RSUD Dr. H. Abdul Moeloek of Lampung Province. Average category was the most frequent accounted for 91.67% while 8.33% cases was in the Good category. The Clinical Teaching Effectiveness in Dr. RSUD H. Abdul Moeloek of Lampung Province was in Average category.

Abstract

Nutritional problems that are still faced by adolescents in Indonesia are underweight, overweight, and obesity, which are interpretations of malnutrition. Multiple factors play a role in the formation of nutritional status, including psychological factors that can be seen through depression, anxiety, and stress status. This research aimed to find the relationship between depression, anxiety, and stress with nutritional status in students of Medical Faculty Lampung University. A cross sectional research was performed involving 107 subjects, which calculated by unpaired categorical comparative formula, taken from students of Medical Faculty Lampung University by purposive sampling. Nutritional status, which determined by BMI, was measured by using body weight scale and stature meter, while depression, anxiety, and stress were measured by using Depression, Anxiety, Stress Scale 42 (DASS-42). Data were analyzed by using Chi-Square test. There were 45,5% subjects with poor nutritional status, 21,5% subjects with depression, 43,9% subjects with anxiety, and 26,2% subjects with stress. Bivariate analysis showed that there was no relationship between depression and nutritional status (p=0,321), there was no relationship between anxiety and nutritional status (p=0,100), and there was no relationship between depression and nutritional status (p=0,110). There were no relationship between depression, anxiety, and stress with nutritional status in students of Medical Faculty Lampung University.

Abstract

Children are very susceptible to Acute Otitis Media (AOM) because the anatomical shape of the eustachian tube is shorter, more flexible and more horizontal than adult. Two thirds of all children have an episode of acute otitis media in the first 3 years of life. Upper Respiratory Tract Infection (ARI) is one of the most common risk factors for causing acute otitis media (AOM) in children. This is because the inflammatory process that occurs in ARI causes damage to mucocilia, goblet cells, and mucus glands in the epithelium of the nasopharynx and middle ear. Children aged under 5 years will usually experience upper respiratory tract infections of two to seven episodes per year. Children who have frequent episodes of ARI are more likely to experience episodes of AOM. his is also in line with repeated episodes of ARI against the risk of AOM recurrence. Recurrent ARI is an ARI that occurs at least four times a year. Therefore, the diagnosis and management of ARI in children as early as possible must be done properly, so that the incidence of ARI in children does not progress to AOM. The causes of otitis media are multifactorial, the anatomical variation of the eutachian tube, and the invasive ability of the pathogen compared to the human's immune system. The diagnosis of otitis media can be confirmed by fulfilling 3 criteria: sudden appearance of the disease, found signs of effusion (known fluid) in the middle ear, found signs of middle ear inflammation.

Abstract

Coronary Heart Disease (CHD) is a major health problem in developed or developing countries. CHD is classified as a degenerative disease related to lifestyle, and socio-economic community. In 2008 it was estimated that 17.3 million deaths were caused by cardiovascular disease. The prevalence of hypercholesterolemia in Indonesia according to riset kesehatan dasar (riskesdas) in 2013 was obtained by Indonesians with higher than normal cholesterol levels higher in women by 39.6% when used with men by 30%. The full scientific bay leaf plant has the Latin name Eugenia polyantha Wight and has another scientific name, namely syzygium polyantha wight and eugenia lucidula miq. This plant belongs to the myrtaceae tribe. Bay leaf contains essential oils of 0.17%, citral, eugenol, tannin, flavonoids, and metal cavikol. Substances that are significant in lowering cholesterol are flavonoids and tannins. How flavonoids work in reducing cholesterol occurs when the hydroxyl group on cholesterol is considered with the ketone group on flavonoids forming hemiasetal. The carbonyl group in flavonoids will choose the hydroxyl group in cholesterol to determine the hydrogen bond. Tannins work quickly in binding and shrinking proteins. Tannin works as an antioxidant, astringent, and hypocholesterolemic. Tanninakan with mucous protein and intestinal epithelial cells so it needs to prevent fat.
