Abstract

Preeclampsia is a serious pregnancy complication characterized by high blood pressure and organ damage. This condition poses a health risk for both the mother and the fetus. This article reviews the epidemiology and diagnosis of preeclampsia. Its epidemiology is complex with varying global prevalence. Risk factors include maternal age, history of preeclampsia and family, multiple pregnancies, obesity, diabetes, and kidney disease. Common diagnostic methods include blood pressure measurement, urine analysis, laboratory tests, and Doppler ultrasonography to assess the placenta and fetus. Prevention and early detection are the major priorities to reduce maternal and infant mortality rates. Further research is needed to develop better diagnostic and prevention strategies. Increasing understanding of the epidemiology and diagnostic methods of preeclampsia, more mothers and babies can be protected from the risks of this condition in the future.

Abstract

Gallbladder disease is associated with a malfunctioning gallbladder and highly concentrated bile, leading to conditions such as the formation of gallstones or cholelithiasis. Cholelithiasis can occur due to high concentrations of cholesterol in the gallbladder. Obstruction of the cystic duct, either by stones or functional issues, can cause acute inflammation of the gallbladder. Acute cholecystitis attacks are often associated with gallstones, or cholelithiasis. Common symptoms include right upper quadrant or epigastric abdominal pain, especially after consuming fatty foods, with additional symptoms such as nausea, vomiting, fever, and diarrhea. The development of this disease is believed to involve many factors and likely arises from complex interactions between various genetic and environmental elements. An imbalance in bile composition, characterized by excessive cholesterol secretion, reduced gallbladder motility, and the accumulation of mucin gel, contributes to the formation of gallstones. Gallbladder disease related to cholesterol results from a biochemical imbalance involving lipids and bile salts in the bile stored in the gallbladder. A 27-year-old patient, Mr. Y, has several risk factors that increase the development of the disease, and the following case report discusses the clinical presentation and laboratory examination results related to the onset of this disease. Laparoscopic cholecystectomy was performed on the patient, and appropriate education was provided, illustrating the importance of intra-professional collaboration and positive outcomes for the patient.

Abstract

Gout arthritis is an increase in blood uric acid levels above normal which causes an inflammatory reaction in the joints. Prolonged gouty arthritis can damage the joints, soft tissues and kidneys. Dyslipidemia is a common metabolic condition characterized by a level of cholesterol in the plasma that exceeds normal values. Total cholesterol level is correlated as a strong risk factor for cardiovascular disease. Therefore, application of evidence-based medicine-based family doctor services to patients by identifying risk factors, clinical problems, and patient management based on a patient-centered and family problem-solving framework. so that treatment goals can be achieved, prevent complications that may occur, and improve the patient's quality of life. Primary data were obtained through history taking, physical examination and home visits. Secondary data were obtained from the patient's medical record. A Female patient, 55 years old, came with complaints of pain in both joints of the hands and feet and the back of the neck felt sore and tense. The patient has a history of high uric acid since the age of 36 years. In the knowledge assessment, the initial score before the intervention was 70 (patient), 50 (patient's family) and after the intervention became 100 (patient), 90 (patient's family). In terms of uric acid levels, uric acid levels decreased from 6.8 mg/dL to 6 mg/dL and blood cholesterol from 221 mg/dL to 188 mg/dL. In the aspect of diet there is a change in the type of food that is appropriate. In the aspect of physical activity there is a change in physical activity, sports 30 minutes 4 times a week. Enforcement of diagnosis and management of these patients has been done holistically, patient centered, family approach and community oriented based on several theories and the latest research. In the process of behavior change, the patient has reached the trial stage.

Abstract

Rheumatoid arthritis (RA) is a long-term progressive inflammatory autoimmune disorder that can caused damage to the joints into disability, if it isn’t handled appropriately. There are 1.4 million people in Indonesia that have been found to suffer from rheumatoid arthritis. In the early stages of rheumatoid arthritis, there are symptoms such as fatigue, flu-like syndromes, swollen joints and pain, and morning stiffness; and also increased levels of C-reactive protein (CRP) and increased sedimentation rate of erythorisite (ESR). In contrast, untreated rheumatoid arthritis displays severe systemic manifestations such as pleural effusion, pulmonary nodules and interstitial lung disease, IMF, vasculitis in small and medium arteries, re-evolution, cartilage damage, rheumatic nodules and others. Diagnosis of rheumatoid arthritis is established after assessing symptoms, physical examination, risk factor, family history, assessment along with ultrasound, and lab marker assessment such as increased levels of CRP and ESR in serum and detection of RA-specific autoantibodies. Rheumatoid arthritis is incurable, but it can be controlled with appropriate medication and prognosis monitoring. The prognosis of rheumatoid arthritis is based on clinical evidence and laboratory examination. Early detection and appropriate management of rheumatoid arthritis are key to better prognosis. This article therefore discusses the importance of early diagnosis and treatment of rheumatoid arthritis.

Abstract

Myelopathy is a description of a neurological deficit caused by spinal cord abnormalities. One of the conditions that can cause myelopathy is a spinal cord tumor. Spinal cord abnormalities based on type and location are assessed using the American Spinal Injury Association (ASIA) scale which is divided into grades A, B, C, and D. The prevalence of spinal cord tumors is 15% of all central nervous system tumors. Based on its location, spinal cord tumors are divided into intradural and extradural. Manifestations of spinal cord tumors consist of sensory, motor, and autonomic disturbances according to the location of the compression lesion. The diagnosis is made based on anamnesis, physical examination and supporting examinations. In patients with spinal cord tumors, radiological investigations can be carried out. The principle of management of spinal cord tumors is to remove the tumor to restore maximum neurological function. The choice of further management for spinal cord tumors is consulted with a neurosurgeon. The prognosis of spinal cord tumors depends on the severity of the patient. Case report Mrs. W, 41 years old with diagnosis myelopathy et causa tumor medula spinalis based on anamnesis and physical examination and supporting investigation given therapy NaCl 0,9% infusion 20 drops per minute, vit B12 3x1 tablet, sodium diclofenac 2 x 50 mg tablet, and consulted with neurosurgeon to obtain further management.

Abstract

The kidneys are retroperitoneal organs located at the top on both sides of the III lumbar vertebra and attached directly to the abdominal or ventral wall of the M. psoas major and M. quadratus lumborum. The kidneys function to regulate the amount and concentration of ECF electrolytes and maintain nerve and muscle excitability, activating vitamin D which is important for calcium absorption in the digestive tract and producing erythropoietin which is a hormone to stimulate the bone marrow to form red blood cells. The main function of the kidneys is to filter and excrete metabolic waste products in the urine so that administration of toxic compounds or irritating compounds can cause degenerative changes such as fatty degeneration to necrosis of the kidneys. Disorders of the kidneys can disrupt kidney function. Chronic Kidney Disease (CKD) sufferers have several complications such as overhydration, hyperkalemia, metabolic acidosis, mineral and bone disorders, hypertension, anemia and dyslipidemia. The most common etiologies of CKD in adult patients are diabetes mellitus and hypertension. In children, the most common primary cause of PGTA is congenital abnormalities of the kidneys and urinary tract (such as obstructive uropathy, vesicoureteral reflux, renal dysplasia). Indonesian Nephrology Association (PERNEFRI) in 2006, which found that the prevalence of Chronic Kidney Disease (CKD) was 12.5%. Meanwhile, data according to the 2019 Health Insurance Administration (BPJS) shows that there were 1.93 million cases of kidney failure and 1.79 million cases in 2020. With this data, the author is interested in discussing the kidneys in more depth.

Abstract

Benign prostatic hyperplasia (BPH) is a non-malignant growth of prostate tissue. The incidence of BPH is influenced by various risk factors such as metabolic syndrome, obesity, and genetic predisposition. The prevalence of obesity in Indonesia with a male population over 18 years old was recorded at 26.60%. The research used a systematic review method. The databases used are NCBI and PUBMED with bulletions namely Benign prostate hyperplasia AND obesity, Benign prostate hyperplasia AND cholesterol, or Benign prostate hyperplasia AND body mass index. Obesity can trigger pro-inflammatory conditions, one of which is micro vascular disease which results in inflammation triggering ischemia and oxidative stress. Obesity can involve changes in the expression of genes such as A2M in the stroma and TFFB3 in the prostate gland and induce sIL-1ra levels.Obesity is associated with the risk of BPH through inflammatory response mechanisms from cellular and humoral microvascular inflammation, programmed cell death, gene expression and sIL-1ra levels.
