Abstract

Background: Chronic urticaria (CU) is one of the most common case found in dermatology and venereology and it decreases the quality of life. Autohemotherapy (AHT) is an innovative therapy that works by desensitization and activation of the local immune system, and it is effective for recurrent or refractory urticaria. AHT technique is simple, inexpensive, and does not require special expertise that can be done at all levels of health services in Indonesia. Purpose: to determine the effectiveness of AHT in the CU treatment. Methods: This was a clinical trial research with involving patients with CU in the Allergo-Immunology Clinic of Department of Dermatology and Venereology Dr. Mohammad Hoesin General Hospital (DV RSMH) and Pertamina Hospital Palembang. The Urticaria Activity Score (UAS7) and Dermatology Life Quality Index (DLQI) assessments were carried out on week 1 and week 10. Result: The research was conducted from 1 July - 30 September 2019 involving 72 eligible  patients with 2 dropouts (2.8%). The mean age was 47 years (15-72 years), with 46 (63.9%) are women. Chronic spontaneous urticaria (CSU) was the most frequent diagnosis observed in 64 people (88.9%). Adverse events included bruises at the injection site (n=1; 1.4%) and at the blood draw location (n=1; 1.4%). On week 1, mean UAS7 was 30.77 ± 2.46, mean DLQI 21.66 ± 3.60. By week 10, UAS7 and DLQI significantly decrease to 4.12 ± 2.89 and 2.51 ± 1.53 (p <0.005). Conclusion: There was a significant decrease in UAS7 and DLQI in chronic urticaria patients after receiving AHT therapy for 10 weeks. This research concluded AHT can serve as an option in chronic urticaria treatment.

Abstract

Background: Lipschutz ulcer, also known as vulvae acutum ulcer, is an acute ulcer in the vulva. The aetiology and pathogenesis of Lipschutz ulcer are unclear, but it is known to be associated with infectious diseases such as Mycoplasma infection, paratyphoid fever, influenza A, and most infections with Epstein-Barr virus (EBV). This ulcer appears in adolescent females aged 14–20 years old, with 70% of cases occurring in virgin women. Purpose: To review the causes of non-Sexually Transmitted Diseases (STD) ulcers, so clinicians can establish an accurate diagnosis and rational therapy; therefore, minimizing the psychological impact on the patient due to possible misdiagnosis of STD-induced ulcer. Case: A painful wound in the genital area of a 20-year-old virgin woman. She was diagnosed with Lipshutz ulcer and vulvovaginal candidiasis based on the anamnesis, physical examination, and laboratory examination obtained from vaginal discharge using potassium hydroxide, Gram staining, and blood test to exclude genital ulcer caused by the sexually transmitted agent. A blood test was taken, including the serological tests for syphilis and genital herpes. Tests for EBV were also performed. The patient was treated only with single-dose fluconazole 150 mg orally and saline compress on the ulcer. Genital ulcer and vaginal discharge improved one week after treatment. Discussion:  Lipshutz ulcer management is symptomatic, usually self-limiting, and disappears spontaneously within 1–2 weeks without recurrences. It can also occur as a single lesion with possible coinfection of other agents, for example, candidiasis, which in this case, requires treatment of comorbidities. Hygiene factors play an essential role in preventing occurrence of the secondary infection and further development of the disease. Conclusion: The diagnosis of Lipshutz ulcer is mainly based on clinical manifestation after excluding several possible ulcers that can affect the genital area, including sexually transmitted infections.

Abstract

Background: The most common opportunistic infection obtained in patients with Human Immunodeficiency Virus (HIV)/Acquired Immune Deficiency Syndrome (AIDS) is Oral Candidiasis (OC). Currently widely reported about increased fluconazole resistance, as a therapy that is often used in patients with HIV/AIDS with OC. For this reason, effective antifungal agents are needed as alternative therapies. Several studies have reported some antifungal activity from rosemary essential oils. Purpose: The aim of this study is to evaluate the antifungal activity between rosemary essential oils and fluconazole against the isolates used by Candida sp. taken from HIV / AIDS patients with OC. Methods: This research was an experimental laboratory study with a post-test only design of 40 Candida sp. isolates from HIV / AIDS patients with OC in the Outpatient Unit and Inpatient Installation of the Infectious Disease Intermediate Care Unit (UPIPI) Dr. Soetomo General Academic Teaching Hospital, Surabaya. Examination of antifungal activity using conventional test methods with disk diffusion. Result: Disk diffusion test result revealed average inhibition zone of rosemary essential oil in Candida albicans 1,75±3,64 mm and Candida non-albicans 1,5±3,08 mm, while the average inhibition zone of fluconazole in Candida albicans 17,9±8,62 mm and Candida non-albicans 4,85±7,9 mm. There was significant difference (p <0.001) between the inhibition zone formed in Candida albicans by fluconazole and rosemary essential oil, but no significant difference of inhibition zone formed in Candida non-albicans. Conclusion: Antifungal activity fluconazole has greater than Rosemary essential oils in Candida albicans.

Abstract

Background: Bacterial vaginosis (BV) is a clinical syndrome caused by the changing of Lactobacillus spp., a producer of hydrogen peroxide, in the normal vagina with a high concentration of anaerobic bacteria, Gardnerella vaginalis and Mycoplasma hominis. This study reports a resistance of BV therapy regimens, high recurrence rates, and side effects of an antibiotic. Therefore, adjunctive therapy for BV management is needed. Research has reported the therapeutic effects of probiotic in BV; however, the results are inconsistent. Purpose: To evaluate the efficacy of standard therapy using metronidazole and Lactobacillus plantarum for BV measured by the cure rate and vaginal flora balance. Methods: A randomized, double-blind, placebo-controlled trial, 30 subjects were randomized to take metronidazole 500 mg twice a day for seven days with weekly evaluation. The Lactobacillus plantarum or placebo group was evaluated every 4 weeks. The cure rate and vaginal flora balance were evaluated using the Amsel criteria and Nugent score before and after the intervention. Result: A total of 30 subjects returned for 28-day follow-up, of which 60% subjects of the antibiotic/probiotic group were cured compared to 40% in the antibiotic/placebo group (p>0.05). Conclusion: There was no difference in the increase of cure rate and vaginal flora balance between the treatment group and significant control group.

Abstract

Background: Oral candidiasis is the most common clinical manifestation in a person infected with HIV which has been linked with the severity of the disease. Candida albicans responsible for most oral candidiasis. The increased resistance to the available antifungal drugs can reduce the efficacy of therapy. Therefore, it will be crucial to discover novel antifungal agents. One of them is tea tree oil (TTO) or Melaleuca alternifolia which has a potent antifungal effect to inhibit the growth of Candida sp.  Purpose: To evaluate the antifungal susceptibility of TTO 5% compared with nystatin against Candida sp. taken from HIV/AIDS patients. Methods: The study assessed the antifungal effect of TTO 5% and nystatin against 33 isolates of Candida sp. that was isolated from the oral cavity of 30 HIV/AIDS patients in Dr. Soetomo General Academic Teaching Hospital Surabaya, Indonesia. Antifungal susceptibility was evaluated by the disc diffusion method. The diameter of the inhibition zone was determined as the result of this study. Result: The most strains found in this study was Candida albicans (84.8%). The mean inhibition zone of nystatin against all strains was 23.24 mm whether it was 17.55 mm for TTO 5%. The inhibition zone of both TTO 5% and nystatin in Candida non-albicans were tended to be higher than in Candida albicans. It revealed that the inhibition zone of TTO 5% significantly lower than nystatin (P = 0.00; P < 0.05).Conclusion: Therefore, the antifungal susceptibility of nystatin was significantly higher than TTO 5% against Candida albicans and non-albicans.

Abstract

Background: Oral candidiasis is one of the most common infections in Human Immunodeficiency Virus/Acquired Immuno Deficiency Syndrome (HIV/AIDS) patients. Some studies have reported an increased number of resistances of Candida sp. against several antifungal therapies. Purpose: To evaluate the pattern of Candida sp. causing oral candidiasis and the pattern of resistance among the Candida sp. against nystatin, ketoconazole, and fluconazole in oral candidiasis patients with HIV/AIDS infection. Methods: This was a descriptive observational study conducted at the Infectious Disease Intermediate Care Unit (UPIPI) Dr. Soetomo General Academic Teaching Hospital. Candida sp. was identified using conventional methods, while the sensibility test was done by using disc diffusion methods. Result: There were 26 subjects with 50 isolates of Candida sp. that had been cultured successfully. Non-albicans Candida sp. was the most common species, which was observed in 28 subjects (56%). The results of the sensibility test on Candida sp. against nystatin, ketoconazole, and fluconazole were 0 (0%), 3(6%), dan 23(46%) respectively. Conclusion: The increased resistance among Candida sp. against antifungal drugs, especially fluconazole, could raise awareness in drug prescription, especially for HIV/AIDS patients.

Abstract

Background: Seborrheic keratosis is a benign epidermal skin tumor caused by sun exposure, virus, and genetic factor. Human papillomavirus (HPV) infection presumed to have an important role in seborrheic keratosis. Protein p53 is a protein produced by tumor suppressor gene. There has been no research that correlates of HPV with p53 increase in seborrheic keratosis. Purpose: To prove the correlation between HPV and increased p53 expression in seborrheic keratosis. Methods: This study is an analytic cross-sectional comparative study. Samples were taken using a consecutive sampling method. There were 22 seborrheic keratosis patients recruited as the research sample. HPV were detected using the polymerase chain reaction (PCR), and p53 expression were detected using the immunohistochemistry examination. Result: The mean age of seborrheic keratosis patients in this study was 54.36±10.09 years, and they were predominantly males (54.5%). HPV were found in 86.4% of the seborrheic keratosis patients. The P53 expression (+) were 77.3%, (++) were 13.6%, and (+++) were 9.1%. All of the seborrheic keratosis with increased  p53 expression had positive HPV results, but with no significant results (p=0.600). Conclusion: There were no correlation between HPV with increased  p53 expression in seborrheic keratosis patients.

Abstract

Background: Mycetoma is a chronic inflammatory disease of the skin that can extend to the fascia, tendons, muscles, and bones. Caused by a bacterium called actinomycetoma and a fungus called eumycetoma are responsible for mycetoma, both of which must be distinguished because they require different medical therapy. Eumycetoma is a localized disease, chronic, and growing slowly. Most of eumycetoma patients had delays in seeking medical care. Purpose: To provide an overview and discuss various aspects of eumycetoma, including its epidemiology, etiology, pathogenesis, classification, clinical picture, diagnosis, differential diagnosis, and therapy. Review: Eumycetoma infection begins as infectious agents enter through the skin with a minor trauma caused by objects contaminated with fungi, forming granular or granulomatous lesions with tract and sinus in there. The infection can spread from the site of inoculation into the muscles and bones. The diagnosis is based on anamnesis (chronic and painless), the triad of tumefaction, draining sinuses and grains, and laboratory investigation. It’s a difficult case, often recur and difficult to be surgical excisison, so that the best first option therapy is the combination of itraconazole and terbinafine itself as an alternative to surgical therapy at least 6 months. Conclusion: Eumycetoma is an infection caused by a fungus with granulomatous lesions with sinus formation and grains in it. The best first option therapy is the combination of itraconazole and terbinafine although it’s still an early case.

Abstract

Background: Oral candidiasis is caused by the mycotic activity of Candida albicans present in the oral cavity, and it is one of the most common opportunistic infections found in patients with Human Immunodeficiency Virus (HIV)/acquired immune deficiency syndrome (AIDS). The growing resistance and side effects to common antifungal drugs have promoted herbal essential oils as antifungal agents in recent years. In this study, essential oils (EO) of Rosmarinus officinalis (Lamiaceae) were examined for in vitro antifungal activ­ity against Candida species. Purpose: To evaluate the antifungal activity of essential oils of Rosmarinus officinalis (Lamiaceae) and nystatin using the microdilution technique by determining the minimum inhibitory concentration (MIC) and the minimum fungicidal concentration (MFC) of Candida spesies. Methods: This was an experimental laboratory study with a post-test-only design conducted in Dr. Soetomo General Academic Teaching Hospital, Surabaya. Forty isolates consisted of twenty isolates of Candida albicans and twenty isolates of Candida non-albicans were collected. The isolates were tested for antifungal activity using the microdilution on 96-well plates. Result: There was a significant difference from the results of the MIC concentration of rosemary essential 100% to 6.25% microdilution method between nystatin and rosemary essential oil (p < 0.05). Conclusion: The antifungal activity of rosemary essential oil was better than nystatin that the lowest MIC value, which was 6.25%, has been obtained the microdilution method. The minimum fungicidal concentration of rosemary essential oil was 25%, while the minimum fungicidal concentration nystatin was higher than 100%.

Abstract

Background: Scabies, an infectious disease caused by Sarcoptes scabiei mites, is still one of the most common skin diseases found in developing countries, including Indonesia. The 5% permethrin cream for scabies treatment has been investigated as having 2–3 time to cure scabies. Tea Tree Oil (TTO) is one of the agents that has been proven to have acaricidal potential as an anti-inflammatory, antibacterial, and antipruritic agent. It has minimal side effects in topical use. Purpose: To evaluate the effectiveness of 5% TTO cream and 10% TTO cream in scabies treatment of pediatric patients. Methods: This was an experimental, analytical study with controlled clinical trial methods, and a double-blind, parallel design comparing 5% TTO cream (treatment 1), 10% TTO cream (treatment 2), with 5% permethrin cream (control) to children with scabies. Result: The 5% TTO cream gave a cure rate of 61.5% and was significantly different from the 5% permethrin cream groups with a p-value = 0.044 in the first week of the study. The 5% permethrin cream gave a cure rate of 15.4%, and the cure rate of 10% TTO cream was 53.8% in the first week of the study. Conclusion: There was a significant difference between the 5% TTO cream and 5% permethrin cream in the first week of the study. The highest and fastest cure rates were found in the 5% TTO cream treatment group.
