Abstract

We reported a 64-year-old woman was admitted to Emergency Department of dr.Soetomo hospital with fainting and previous chest discomfort, then we diagnosed this patient with Non ST Elevation Myocard Infarct, diabetes mellitus, hypertension stage II, and long QT syndrome (LQTS) with torsades de pointes episodes. There are two important tests to determine LQTS, electrocardiogram (ECG) and a genetic test. Two kinds of LQTS, congenital LQTS is caused by mutations in genes coding, and acquired LQTS often is associated with drugs or metabolit abnormalities. Myocardial ischemia could change QT interval regionally in the area of ischemia, and these changes are related to extent and severity of coronary atherosclerosis. There are two major treatment options for patients with LQTS, medications and ICD/pacemaker implantation. Beta-blockers are the drugs of choice.

Abstract

Introduction . FTO gen is one of the new recently studied and still requires a lot of studies on the role of this gen on the incidence of metabolic syndrome. FTO rs 9939609 polymorphism in some populations have mentioned as the risk factors for obesity, type 2 diabetes mellitus and metabolic syndrome. Protein FTO role in demethylations of the genes in the hypothalamus that helps maintain energy intake and expenditure. Methods This study used a case- control design with type 2 DM subjects with metabolic syndrome (n = 40) as cases and type 2 diabetes mellitus without metabolic syndrome (n = 40) as a control from DR.Sardjito Hospital Yogyakarta 2009-2010. FTO gen rs9939609 polymorphism was analyzed by PCR-RFLP. Chi square test and odds ratio test are used to examine FTO rs9939609 polymorphism and the relationship with the incidence of type 2 diabetes with metabolic syndrome and it’s components. Results The frequency of TT genotype in subjects with type 2 DM patients with metabolic syndrome 2(5%), AT genotype 35 (87.5%) and AA genotype 3 (7.5%) (n = 40). In subjects with type 2 DM patients without metabolic syndrome TT genotype frequency 6 (15%), AT genotype 32 (80%) and AA genotype 2 (5%) (n = 40).OR are significant for sistolic hypertension (OR=0,039) and for HDL plasma level (OR=0,046) as the components of metabolic syndrome. Conclusion The FTO gen rs9939609 polymorphism is the risk factor for hypertension and high HDL plasma level as the components of metabolic syndrome at DR Sardjito Hospital Yogyakarta.

Abstract

Background: Physical activity can improve physical fitness. By maintaining a good level of physical fitness not only prevent an increase in body fat percentage, but also improve lipid profiles. Elevated value of total cholesterol and low-density lipoprotein cholesterol (LDL-C) with low levels of high-density lipoprotein cholesterol (HDL-C) are an important risk factors for coronary heart disease, in addition to above-normal body mass index, both overweight and obesity. The purpose of this study was to determine the relationship between physical activity and body mass index, lipid profile of first year students of Medical Students Faculty of Medicine, University of Mataram. Method: The research subjects were 93 first year students. Physical activity level was measured using the IPAQ-SF (International Physical Activity-Short Form) questionnaire, body mass index (BMI) by dividing body weight (kg) by height (m2) and examination of lipid profiles which included: triglyceride, total cholesterol , HDL and LDL cholesterol from blood serum using enzymatic colorimetric method. Data were analyzed statistically by Spearman correlation test. Results: The results showed that there was no significant correlation between the level of physical activity with BMI, cholesterol, triglyceride, and LDL value, with significance values (p) respectively: 0.162, 0.14, 0.327, 0.51. There was a significant correlation between level of physical activity and HDL (p=0.018; r=0.242). Conclusion: Body mass index and lipid profile are not only affected by the level of physical activity. The higher the level of physical activity, the better HDL levels

Abstract

Vitamin D has a number of roles in various body systems with receptors scattered in many organs,1,2 showing a link between vitamin D and several chronic diseases such as cancer, diabetes and autoimmune disease.3,4 Several studies have shown the role of vitamin D in type 2 diabetes in controlling blood sugar levels and HbA1c levels. Vitamin D plays a role in increasing insulin sensitivity and secretion and suppressing inflammation in type 2 diabetes. Vitamin D has a protective role in various complications of type 2 diabetes, including diabetic nephropathy, diabetic neuropathy and diabetic retinopathy.5 Monitoring of vitamin D levels and vitamin D supplementation have a therapeutic role to controlling glycemic blood glucose and prevent complications in type 2.6

Abstract

Background. Metastasis (especially in bone) in prostate cancer is often difficult to diagnose and poor prognosis. Hence, it is very important to do an early examination in detecting metastasis. Frequent bone scintigraphy indicators are prostate specific antigen (PSA) level and Gleason score (GS). This study aims to analyze how strong correlation of these factors in affecting bone metastasis in prostate cancer. Subject and method. A retrospective cross-sectional analysis study was performed using medical records of the prostate cancer patients who underwent Tc-99m MDP bone scintigraphy at Hasan Sadikin General Hospital during January 2012 until December 2016. PSA level, GS and Gleason grade (GG) data were analyzed by bivariate and multivariate analysis. Result. From 189 medical records, only 110 medical records were eligible to be analyzed. Bivariate analysis demonstrated statistically significant correlation for PSA level (p <0.001) and GS (p 0.05 and p 0.036) with bone metastasis in prostate cancer. Multivariate analysis showed that PSA remains the strongest factor associated with bone metastasis in prostate cancer (p 0.001). Conclusion. From bivariate analysis, serum PSA level have statistically significant correlation with bone metastasis in newly diagnosed prostate cancer patient. In the GS dan GG classification, the higher-grade of GS or GG the higher bone metastasis in prostate cancer patient. Thus, the above results may be helpful in identifying prostate cancer patients who have a high risk for bone metastasis.

Abstract

Hemophilia is a hereditary disease characterized by heavy bleeding. This disease presents as minor posttraumatic bleeding or spontaneous bleeding. Hemophilia is caused by abnormalities in the coagulation protein factors. Factor VIII (FVIII) causes hemophilia A, and factor IX (FIX) causes hemophilia B. Hemophilia can be diagnosed through gene sequencing by identifying carrier status. At the time of prenatal, hemophilia can be detected when the fetus is 12 weeks old, but has a risk of miscarriage 0.5-1.0%. Hemophilia is treated by two approaches: management of acute bleeding and prophylaxis. Treatment of hemophilia must pay attention to the dose, schedule, and target level. Hemophilia treatment can use recombinant factor VIII by involving virus inactivation and screening technology which is considered safer. Complications can occur due to the presence of inhibitors of the function of the action of coagulation protein factors, hemophilic arthropathy, and pseudotumor that triggers soft tissue bleeding that has the potential to injure internal organs. Prevention efforts are carried out by keeping patients away from activities that trigger physical bleeding wounds, and ensuring that patients have a normal immune system to receive routine vaccinations.

Abstract

Background: Cataract is the main reason of blindness in Indonesia which is 77.7% of blindness is caused by cataracts. It is known that there is an increase in the incidence of cataracts among patients with Diabetes Mellitus, and the possibility is 25 times higher than people without Diabetes Mellitus. Diabetes is a crucial public health problem due to the number of cases and the prevalence of diabetes have continued to increase over the last few decades. Diabetes and cataracts are considered has high impact in term of health and economic burden, especially for developing countries that do not yet have good diabetes management and difficult access to cataract surgery. The aim of this research was to know about the correlation between diabetes mellitus and the incidence of cataract. Methods: This is an analytic study using a cross sectional research design. The sampling technique used is total sampling. This study used secondary data which obtained from patient medical records at the departement of ophthalmology of Unram Hospital from January 2020 to December 2020 with a total number of patients was 34 people. Data was analyzed using the Chi Square test calculation method. Results: The result showed there was no significant relationship between diabetes mellitus and the incidence of cataracts (p > 0.05) Conclusion: There was no significant relationship between diabetes mellitus and the incidence of cataracts with the most cataract patients aged 60-69 years Keywords: Diabetes Mellitus, Senile Cataract, Mataram University Hospital

Abstract

In March 2020, the World Health Organization (WHO) declared COVID-19 as a Pandemic. Vaccines are biological products that can be used to induce the body's immune response which can provide protection against infection and/or disease upon exposure to pathogens. As such, vaccination becomes one of the key roles to prevent the spreading of COVID-19. Based on the results of clinical trials, COVID-19 vaccine shows adequate levels of immunogenicity with various levels of effectiveness and acceptable level of safety. With vaccinations being implemented worldwide, side effects ranging from mild to severe have been reported. Understanding mucocutaneousside effects of COVID-19 Vaccine are important, as they can be seen with the naked eye and have the potential to frighten individuals and populations, leading to public reluctance to being vaccinated.

Abstract

Pendahuluan: Morbus Hansen (kusta/lepra) merupakan penyakit infeksi kronik yang disebabkan oleh Mycobacterium leprae. Bakteri ini menyerang kulit dan nervus perifer. Manifestasi klinis bervariasi dan ditentukan oleh reaksi sistem imunologis pejamu terhadap basil tahan asam M.leprae. Manifestasi klinis yang ditimbulkan dapat berupa lesi hipopigmentasi atau eritem yang mati rasa disertai dengan kerusakan saraf (sensorik, motorik, dan otonom), dan dalam beberapa kasus dapat melibatkan gangguan sistemik. Kasus: seorang laki-laki (32 tahun) merupakan pasien rujukan datang dengan keluhan sesak. Berdasarkan anamnesis, pemeriksaan fisik dan pemeriksaan penunjang didiagnosis sebagai Morbus Hansen tipe multibasiler dengan penyulit Acute on Chronic Kidney Disease dan gagal jantung kongestif. Terdapat lesi makula-patch eritematosa dan hiperpigmentasi berbentuk nodul-plakat yang tersebar simetris pada kedua sisi, dan jumlah lesi lebih dari 5. Pada pasien terdapat hipoanestesi. Pemeriksaan BTA lesi kulit dari sampel cuping telinga dengan hasil 3+ pada cuping telinga kanan dan 4+ pada cuping telinga kiri. Pemberian MDT pada pasien ditunda. Simpulan: Pasien kusta dengan penyulit penyakit ginjal kronis dan gagal jantung kongestif memerlukan penanganan khusus terkait gejala sistemik serta membutuhkan pertimbangan khusus dalam pemberian MDT. Abstract

West Nusa Tenggara province ranks the second as the highest province with blindness due to cataract in people above 50 years old. Although blindness from cataract is preventable, without well-organized and accurate cataract surgical registry, actions for prevention of blindness will not be optimized. Until now, there is no single online cataract surgical registry in Indonesia that results in difficulty in retrieving cataract surgical rate and outcome data. The study aims to develop a website-based cataract surgery registry to collect pre, during and post cataract surgery in West Nusa Tenggara province followed by trials by health workers in Rumah Sakit Universitas Mataram to perfect the application before daily use. They shoud answers questions from questionnaire provided to measure their perceptions of the application. The application named Register Operasi Katarak NTB (ROKANTB) can be operated in windows, chrome or mac operating systems. The questionnaire shows good perception among health workers for application design, easy to operate, practicality, amount of time needed to fill the form, retrieving data and usefulness of the application as cataract surgery registry. ROKANTB application is a practical and useful tool to record and present cataract surgery data online from many healthcare facilities that may be useful to analize and evaluate our local and national cataract surgery program and strategies.
