Abstract

Congenital coronary artery anomalies are rare. Clinically, coronary artery anomalies can be benign or life-threatening depending on the prognosis. This case report aims to describe a patient who presented with atypical chest pain as an early symptom who underwent CT scan. The CT scan image showed total calcium score 0, anomalous origin of the LAD from the right aortic sinus, and no evidence of plaque or stenosis at LM, LAD, LCX, RCA. This anomaly is included in the classification of anomalous origin. LAD originates from the right aortic sinus, with a pre-pulmonic pathway so that the hemodynamics is still good and is classified as an anomaly without ischemia. Abstract

Latar Belakang : Kanker kepala dan leher mencakup berbagai kelompok tumor biasa yang seringkali agresif dalam perilaku biologis mereka. Selain itu pasien dengan kanker kepala dan leher sering berkembang menjadi tumor primer kedua. Tumor ini terjadi pada tingkat tahunan sebesar 3%-7% dan 50%-75% dari kanker baru seperti terjadi di saluran aerodigestive atas atau paru-paru.4 Massa leher dapat menjadi situasi yang membingungkan dan menantang, terutama pada pelayanan primer. Teknik MRI biasanya digunakan untuk evaluasi pengobatan, Ringkasan Kasus : Seorang laki-laki berusia 66 tahun datang ke poliklinik onkologi dengan keluhan utama leher membesar terus-menerus sejak 6 bulan. Pasien tidak memiliki riwayat keganasan sebelumnya. Pada MRI didapatkan Lesi pertama masih mungkin suatu STT colli dengan perluasan pada submandibular kiri, pterygoideus muscle kiri, sinus maxilaris kiri, infiltrasi optic chiasm kiri, CPA kiri, retroorbital kiri. Lesi kedua masih mungkin swat sphenoid wing meningioma kiri.

Abstract

Latar belakang : Akupunktur merupakan terapi pengobatan yang telah digunakan sejak 2500 tahun lalu dan kini telah digunakan luas di berbagai belahan dunia termasuk Eropa dan Amerika. Perbedaan bahasa yang digunakan dalam penamaan titik akupunkur menyebabkan kesulitan dalam pertukaran informasi mengenai akupunktur. Oleh karena itu pada tahun 1984 WHO menertbitkan “standar acupuncture nomenclature” sebagai sistem penamaan akupunktur standar di seluruh dunia. Namun pada prakteknya masih banyak ditemui publikasi menuliskan titik akupunktur tidak sesuai sistem nomenklatur WHO. Tujuan : mengetahui jumlah publikasi ilmiah yang menuliskan titik akupunktur tidak sesuai sistem nomenklatur WHO, dan bentuk ketidaksesuaian yang umum terjadi. Metode : pencarian literatur menggunakan 1 database yaitu Pubmeb, dengan menggunakan kata kunci “acupuncture”, “acupoint”, dan “randomized controlled trial”. Publikasi ilmiah yang sesuai dengan kriteria inklusi penulisan titik akupunkturnya bila sudah menggunakan kaidah yang sesuai WHO, yang tidak sesuai dicatat dalam tabel dibagian hasil. Hasil : dari 1868 publikasi ilmiah yang ditemukan, terdapat 983 artikel yang ditelaah, dan ditemukan 663 (67.44%) publikasi ilmiah yang penulisan titik akupunktur telah sesuai dengan standar nomenklatur WHO, dan terdapat 320 (32.55%) publikasi yang tidak sesuai dengan standar nomenklatur WHO. Dari 320 publikasi yang tidak sesuai tersebut, terdapat beberapa bentuk ketidaksesuaian yang paling sering ditemukan, antara lain: kode numerik tidak sesuai kode WHO, diantara kode alfabet dan kode numerik terdapat tanda spasi, atau diberi tanda strip (-), dan kombinasi lebih dari satu bentuk penulisan yang tidak sesuai. Kesimpulan : masih cukup banyak peneliti/praktisi akupunktur di dunia yang belum memahami dan/atau menerapkan penulisan titik akupunktur menurut sistem nomenklatur WHO.

Abstract

Background: Leprosy is a chronic infectious disease caused by the bacterium Mycobacterium leprae that attacks the peripheral nerves and can cause permanent deformity or disability. Leprosy can impact not only patient’s health, but also their quality of life. Currently there is no research on the quality of life of people with leprosy, especially in the city of Mataram. Method: This study is a cross-sectional study to determine the prevalence and the quality of life of leprosy patients in 2019-2020. Data on leprosy patients were obtained from all public health center in Mataram. Quality of life was assessed using the WHO questionnaire (WHOQoL-BREF) Results: In 2019-2020, there were 25 leprosy patients in the Mataram City area, equivalent to a prevalence of 0.00000504 or 0.5 cases per 10,000 population. 20 respondents were interviewed, with an average quality of life in the physical health domain of 57.32; psychology 59,79; social relations 59,58; and environment 57.81. Based on the quality-of-life category, 11 respondents (55%) belonged to the low score group in the physical and psychological health and 13 respondents (65%) belonged to the low group in the social and environmental relationship domain. Conclusion: The prevalence of leprosy in Mataram City in 2019-2020 was lower than the national prevalence in 2017. Some patients have a relatively low quality of life, especially in the domains of physical and psychological health and environmental relationships. Management of leprosy patients needs to pay attention to post-treatment management, especially related to leprosy disability.

Abstract

Background: Covid-19 is an infectious disease caused by the novel coronavirus. This disease can attack mental health and cause depression. Depression is a mood disorder that can occur in many people. Depression is caused by many factors. Objective: To determine the relationship between the general characteristics and depression of patients in the Covid-19 isolation room and to know the number of depression in patients in the Covid-19 isolation room at the Sultan Agung Islamic Hospital, Semarang. Methods: This research is an analytic observation with a cross-sectional design. Data collection used medical records. The sampling used is consecutive sampling. The data analysis technique used is the Spearman correlation test. Results: The number of occurrences of depression is more common in the female with 18 samples, with low education example elementary education with 11 samples, and working as IRT with 12 samples. The data obtained from the analysis using the Spearman correlation test does not show a significant difference between each of the general characteristics and depression of patients in the Covid-19 isolation room as indicated by the p-value for gender characteristics of 0.168, the p-value on educational characteristics of 0.349, and the p-value on job characteristics of 0.367. Conclusion: From the results of this study, it can be seen that the general characteristics of patients in the Covid-19 isolation room including gender, education, and occupation are not related to the incidence of depression in patients.

Abstract

Benign Prostatic Hyperplasia (BPH) is a condition of blockage seen in benign prostate enlargement with histologic appearance of prostate adenoma that causes obstruction to vary with or without symptoms. Complications that are usually caused by BPH are cystitis and anemia. High tissue enlargement will cause pressure on the prostatic urethra which causes pressure on the urethral lumen and results in obstruction. Objective: to review a case report of a 76 year old male patient who was diagnosed with BPH with cystitis and anemia. Methods: this article is based on a BPH case report at RSUD dr. Sudirman Mangun Sumarso, Wonogiri. Results: The patient had received therapy with Harnal 1 x 400 mcg, paracetamol 3 x 500 mg and urinary 2 x 400 mg. 0.5 mg PO), then monotherapy can be given Tadalafil 5 mg/day for patients who have low BMI and symptoms of LUTS. Treatment of cystitis the patient is given IV ceftriaxone 1-2 grams / 12-24 hours, Urinary 2 x 400 mg / day, Paracetamol 325-600 mg 4-6 hours if necessary. For the treatment of anemia given folic acid 500-1000 mcg and multivitamins. Conclusion: The patient has received appropriate therapy according to the symptoms experienced.

Abstract

Background: The number of Covid-19 cases continues to increase. Patients with Covid-19 show variousclinical signs and symptoms, a small number of cases can progress to lower respiratory tract infections in the form of pneumonia and rapidly worsen into respiratory failure. Methods: This research is a retrospective descriptive study. Data were obtained from the medical records of pediatric patients who were confirmed positive for Covid-19 and diagnosed with pneumonia in the pediatric isolation room of RSUD Dr. Raden Soedjono Selong from May 2020 to March 2021 Results: There was 55 child confirmed positive for Covid-19, consisting of 32 boys and 23 girls. The number of children who were confirmed positive for Covid-19 with pneumonia was 16 people, of which 2 children had other comorbid diseases. The most common symptom that patients complain about is fever. Most of the patients who were confirmed positive for Covid-19 with pneumonia treated in the isolation room were female, with the highest age range being between the ages of 1 month to 2 years and over 2 years to 5 years. The radiological appearance of the patient is mostly typical of pneumonia.

Abstract

Thyroid-associated ophthalmopathy (TAO) is a thyroid hormone imbalance thatmanifests in the eye. The most common TAO is in Grave’s Disease. TAO could increase theintraocular pressure and could cause a glaucoma optic neuropathy and therefore secondaryglaucoma could happen.

Abstract

COVID-19 infection is marked by SARS-CoV-2 binds with ACE-2 receptors. In this process, the virus will avoid immunity systems, followed by a cytokine storm in some patients. Aside from calcium homeostasis and bone metabolism role, vitamin D plays role in reducing inflammation process and immunoregulation, which is known as the immunomodulation effect. Vitamin D plays a role against pathogens in adaptive and innate immune systems. Vitamin D modulates T-helper (Th) cells response inducing response shift from Th1 to Th2, increasing T-regulatory (Treg) cells development, and balancing T-helper cells response against pathogens and reducing pro-inflammatory cytokines production. Vitamin D may reduce production of pro-inflammatory cytokines such as TNF-?, IL-6, IL-1?, IL-12, and IFN-?, caused by inhibition of nuclear factor kB (NF-kB) activation. Vitamin D also induces ACE-2 vasorelaxant converting Angiotensin II into Angiotensin VII, a vasodilator, to prevent Acute Respiratory Distress Syndrome (ARDS) from happening. Few studies show COVID-19 patients has vitamin D deficiency. A meta-analysis study of 360,972 COVID-19 patients shows 37.7% of them with vitamin D deficiency and 32.2% with vitamin D insufficiency. Vitamin D deficiency correlates with COVID-19 severity and mortality. A study of 42 patients with acute respiratory failure caused by COVID-19 in Bari, Italy shows patients with vitamin D levels < 10 ng/ml have 50% mortality rate after 10-days of care. Vitamin D level examination is recommended for COVID-19 patients. The recommendation for vitamin D supplementation is vitamin D3 4000 IU/day dose for 7 days continued with 800-1000 IU/day maintenance dose for vitamin D deficiency patients.

Abstract

Radionuclide cisternography is often used to prove the presence of cerebrospinal fluid leakage from the nose or ears and to localize the location of the leak. Whether the leak is active or not at the time of examination greatly affects the results of the radionuclide examination. We report a rare study case in a female patient 56 years old, who had the main complaints of nasal discharge, apathy, and weakness. The discharge is mainly felt when the patient is sitting. The patient had a history of transsphenoidal surgery for removal tumor at the epiphyses. Cisternography examination was carried out at the Department of Nuclear Medicine and Molecular Theranostic to confirm a liquor leak in the patient. Imaging was performed at 1.3.6 hours after intrathecal 99mTc-DTPA injection. There is an increase in radioactivity in the nasal area, especially on the 3 hour image. SPECT / CT fusion imaging provides the exact location of the liquor leak at the sphenoid area. The nasal tampon to blood serum ratio is 4: 1. Therapy for the patient is surgery to re-seal the leak.
