Abstract

This study aims to determine the root of problem and alternative solutions of inaccuracy return medical record inpatient files. The research was conducted through qualitative descriptive approach in RSUD Blambangan from September until October 2017. Data was collected by  document review, field observation and unstructured interview with Head of Services, Head of Nursing Services, 1 doctor, 2 head of ward, Head of Medical Record Installation, Head of Nutrition Installation and Head of Pharmacy Installation. Root of problem was analyzed with fishbone diagram, focus group discussion (FGD) method and urgency, seriousness, growth (USG) method. Priority determination solution is carried out using filter analysis method with CARL (Capability Accessibility Readiness Leverage). The results show that factor inaccuracy return inpatient medical records file because are incomplete of medical record file, absence of standard operating procedure and unavailability of monitoring and evaluation, and the increasing number of medical record form according to hospital quality standard. The root  problem is not optimally main task and function of professional care giver when filling medical record. Alternative proposed solutions is socialization to professional care giver, revised standard of operational procedure, and monitoring evaluation of the completeness and timeliness of medical record file returns with innovation "ABED TANGI".

Abstract

Inventory is one of the crucial components in hospital operations. The value o inventory may range up until 25% of the total cost of the hospital operation. Inventory optimization may save up to 10% in the minimum. There is a lot of drugs circulation in hospital. There is always a chance for a mistake. A tight control of inventory is necessary for hospital managers to minimize any potential waste. A saving directive for 20% drugs which consume 70% of the total cost will boost the hospital efficiency. A lot of factors influences hospital inventory and how managers strategize for the inventory. Some identified factors are order frequency, order quantity, unit price, and other hidden costs. The purpose of this research is to prove the correlation of each factor and how they influence the total inventory cost. To follow up the result, henceforth a model of simulation by calculating Economic Order Quantity and Reorder Point, the research may help hospital managers in modifying the strategy of managing hospital inventory for the next year period. This study aims to identify the factors which influence total inventory cost in managing group A drugs at private type B hospital, Jakarta. This study using observational analytics, cross-sectional study.

Abstract

The objectives of this study are to find out the knowledge concerning the guidelines fall risk to the patients, to find out the ability of the nurses in the simulation of the guiding steps for the risk for falls, to find out the conformity of the review on the medical records of the patients, and to find out the obstacles in preventing risk for falls. It is a mix method study, which is a quantitative method with cross-sectional approach, and qualitative method with descriptive research design. The population of this study were 37 respondents for the quantitative research and 9 informants for the qualitative research. The results of the questionnaire indicate the presence of knowledge that most have fair criteria that is as much as 46%. The results of the medical record study show most of the full or compliant as much as 89.2%. While the structured interview showed different results. The conclusions of this study were that there was sufficient knowledge but lacked of discipline in completing the medical records, and there were still some obstacles in implementing the guidelines for risk for falls.

Abstract

as a global problem, patient morbidity and mortality in hospital caused by Healthcare Acquired Infections (HAIs) can be prevented by implementation of Hand Hygiene (HH). This research aims to know the influence of Health Belief Model variables to HH compliance. there was a quantitative study by survey approach and cross-sectional design. The population consisted all of nurses in high risk HAIs ward (ICU and surgical ward, 30 respondents). HCW’s perceptions were measured through self-administered questioner. The compliance of HCWs was measured by observation form of HH. compliance rate of HH based on moment was 46,29 %. Compliance of ICU nurses where higer than surgery ward nurses. Only workplace variable that had significant influence HH compliance (p=0,000). Perception variables can not used as predictor to HH compliance. HH compliance should be increase. Workplace factor had significant influence to HH compliance.

Abstract

The aim of this study was to modify and determine the dimensions and themes of the patient safety culture tool by MaPSaF. This study was a qualitative design with study literature approach. The result of this study was MaPSaF composed of 10 dimensions of patient safety culture with 24 aspects which contain the statements in each theme. The dimensions are the commitment to overall continuous improvement, priority given to safety, system errors and individual responsibility, recording incidents and best practice, evaluating incidents and best practice, learning and affecting change, communication about safety issues, personnel management and safety issues, staff education and training, and teamwork. The aspects are committed to improvement, audit, policies, priority of patient safety, risk management system, implementation of patient safety, the cause of the incident, patient safety culture, reporting feeling and system, data analysis, the focus and result of investigation, incidents learning, the people in deciding of change, communication about patient safety between staff, patient or both, share the information, supporting the staff, training needs and purposes, team structure, the flow of information and sharing.

Abstract

This study aims to analyze root problems and the best solution of entry delays in the BPJS claim verification file to IJP (Installation of Financing Guarantee) of the hospital. This research is a qualitative descriptive research using Focus Group Discussion, Fishbone diagram and “5 Why” question methods to find the root problems. After that, the choice of alternative solutions is determined by Nominal Group Technique (NGT) and Capability Accessibility Readiness and Leverage (CARL) methods. This activity involves 2 management staffs, 3 IJP staffs, 2 pharmacists, and 7 nurses from emergency department, outpatient and inpatient units. The result of this research is written feedback by IJP for all outpatient and inpatient unit as the best solution which is expected to overcome the entry delays problem

Abstract

The purpose of this study is to evaluate the implementation of the supervision of nursing in Panembahan Senopati Bantul Hospital. This study is a descriptive qualitative research. All data derived from interviews, observation, and search documents. Sample of respondents is representative of the nursing supervisor scheduled in October 2015. The research variables: the implementation of nursing supervision, seen from the description of the implementation, engineering, area, barriers of implementation, supervision and evaluation program monitor. Of the 67 times the supervision scheduled in October 2015 only 17 were implemented (25.4%). Figures noncompliance supervisor in the execution of supervision is still quite high. The majority of supervisors do with indirect supervision. Supervision has not been carried out throughout the area should be. Barriers to implementation of supervision include external and internal obstacles. Program monitoring and evaluation of supervision is still not implemented. Supervision of nursing has not gone up, judging from the absence rate of non-compliance supervisor and implementation is quite high, and there are still incomprehension supervisor of engineering and supervisory area should be.

Abstract

Special Needs Children (ABK) is one of the vulnerable groups in times of disaster. Some of them have mobility hindrance even to perform a defence and personal escape independently, therefore, information is needed on what procedures or plans to take to rescue ABKs who need help from people around them (e.g. teachers, classmates, school staffs). Objectives: To determine the preparedness of children with special needs in dealing with earthquake disasters in Yogyakarta. This research uses a observational descriptive design. The samples are selected using a purposive sampling technique, while the data are collected by means of observation sheets. The respondents for this research are special needs children with autism at ‘SLB N Pembina’, Yogyakarta. Data analysis is performed using a descriptive qualitative analysis. Disaster awareness trainings give a positive influence in reducing disaster risks and in increasing the disaster preparedness of special needs children with autism. Special needs children with autism show ability and participation after performing 5 times simulation. The preparedness increase of special needs children after training is 39%.

Abstract

RSKIA PKU Muhammadiyah Kotagede Hospital Type C Special, that in order to maintain the quality and safety of hospital patients, especially the handling of High Alert Medication and prepare for the accreditation of the 2012 version, then RSKIA PKU Muh Kotagede working to improve implementation one of that the Goals Patient Safety with training. Occurrence data and research in pharmaceutical care, as well as general data risk high incidence of errors in the pharmaceutical field especially high alert medications make it a priority to take precedence understanding for officers and implemented. This type of research is descriptive research using a type of mixture (mixed methodology), with a quasi-experimental. The study population were hospital health workers, the sampling technique by purposive sampling. Test analysis using descriptive statistics and qualitatively by comparing with the results of observation, interviews and document research. Test Results Kolmogorov Smirnov normality assumption both knowledge and attitude variables to pre-test and post test both normal distribution. Results Paired Samples Test A (Knowledge) note that t is -6.672 with a significance value of 0.000. Hence the significance of 0.000 <0.05, then Ho is rejected, it means that there is a difference between the average variable knowledge of pre-test and post test. Paired Samples Test / T-test B (attitude), it is known that t is -2.932 with a significance value of 0,010. Hence the significance of 0.010 <0.05, then Ho is rejected, it means that there is a difference between the average variable attitude of pre-test and post test. From interviews with respondents felt more knowledgeable and increase in knowledge after the training given. From the aspect of attitude because respondents feel more knowledgeable after being given training, it leads to a positive attitude in handling the implementation of High Alert Medication. There were changes in knowledge to healthcare workers at RSKIA PKU Muhammadiyah Kotagede after training on High Alert Medication. There is a change in attitude to healthcare workers at RSKIA PKU Muhammadiyah Kotagede after training on High Alert Medication.

Abstract

X Hospital has conducted the strategic plan and evaluated it, but there is inexpediency of work program and the budget sets in its implementation. Therefore, the goal of the strategic plan is not achieved and the unit performance is not maximal. Consequently, this study is aimed to analyze the process of the arrangement to the implementation of the Strategic Plan policy and the influencing factors. This research uses qualitative method. The primary data collection is conducted in-depth interview to 14 informants, while that of secondary data are obtained by reviewing the documents. The results show that the process of arranging and evaluating the strategic plan and the work program has not fully involved all the Directors and Managers concerned. Furthermore, the clarity and consistency of the Strategic Plan still lacks of the stage of implementation. The corporate performance-level parameters based on the balanced score card are not also descended to the Key Performance Indicator per directorate. However, the aspect of Human Resource understanding to the Strategic Plan has been fulfilled. Aspects of attitude and commitment both to the Executive Officers, Board of Directors and Managers are also very well.
