Abstract

Inflammation and oxidative stress are risk factors of cardiovascular diseases among chronic kidney disease patients. There is increasing of HsCRP level in stage V CKD patients wih CAPD. N-Acetyl Cysteine(NAC) is a compounds containing thiol with antioxidant and anti-inflammatory effects. This research obejectives was to examine the effect of oral NAC in the decline of HsCRP level in stage V CKD patients with CAPD. This is an experimental research with Randomized Double Blind Control Trial, involving 30 patients CKD with CAPD Dr Moewardi Hospital and complete inclusion and exclusion criteria, divided 15 patients as a control group (placebo) and 15 patients as a treatment group of oral NAC. Examination of data research variables have been performed before and after 8 weeks treatment. The results were the level of HsCRP in control group before treatment and after treatment (15,76±42,79 vs 16,50±30,63). Level of HsCRP in treatment group before treatment and after treatment (29,53±76,52 vs 4,56±12,85) was decrease statistically significant with p< 0,05. The N-Acetyl Cysteine orally can decreased the level of HsCRP in stage V CKD patients with CAPD.Keywords : N-Acetyl Cysteine, CAPD, HsCRP

Abstract

The increase of energy need during physical exercise of aerobics will increase the need of oxygen, and therefore it can cause free radicals increase. Naturally, endogenous antioxidants can overcome the increase of free radicals. However, if the antioxidants formation which is not comparable with the occurrence of free radicals will cause the occurrence of oxidative stress. So it requiered antioxidants from the outside to take care of it. This research aims to study differences in oxidative stress between groups that consume dates syrup and groups that does not consume date syrup during acute aerobic exercise for untrained men through the lipid peroxide concentration with plasma malondialdehyde (MDA) indicators. This research design is experimental research, non-randomized pre-post test control group design. The subjects divided into two groups namely the group of dates syrup and the group of fresh water. Both group were given treatment of acute aerobic exercise with medium intensity every 2 days for 28 days. Blood samples taken before the aerobic exercise, after the 1st, 7th, and 14th aerobic exercise, and then were checked its plasma MDA levels. Data analysis was done with independent t-test. Analysis of plasma MDA levels t-test showed that there was no difference between the group of dates syrup and the group of fresh water. Plasma MDA levels of the group consuming dates syrup was not much different compared to the group not consuming dates syrup during aerobic exercise with medium intensity for untrained men.Keywords: acute aerobic exercise, untrained men, dates syrup, plasma MDA

Abstract

Eosinophils play a role in the defense against viruses, bacteria, fungi and parasites, as well as be involved in the pathogenesis of allergic and autoimmune diseases. The role of eosinophils in the defense against bacterial run by enzymes found in the granules such as MBP, ECP, EPO, and EDN, and spending EET. Abstract

Tempuyung (Sonchus arvensis) and salam (Syzygium polyanthum) leaves can lower uric acid levels were studied. The purpose of this research was to determine mechanism of combined o f tempuyung and salam extracts to reduce uric acid levels by inhibiting xanthine oxidase in hyperuricemic mice. This research used completely randomized design using mice. Fifteen mice were made hiperuricemic by 250 mg/kgBW potassium oxonate p.o then the mice were divided into 3 groups. Group I induced 10 mg/kgBW allopurinol p.o as positive control, group II induced tempuyung-salam p.o each 100 mg/kgBW as the treatment group, group th III induced 0.5 mL/20 gBW distilled water p.o as negative control. Liver was taken 4 day after 2 hours induced potassium oxonate. Data inhibition xanthine oxidase activity was tested with Mann-Whitney. The result showed that the combination of tempuyung-salam extract each 100 mg/kgBW could inhibit xanthine oxidase activity of 51,16%±2,76%, while allopurinol 90,20%±0,16% (p = 0.000).Keyword: Hyperuricemia, Xanthine Oxidase, Sonchus arvensis, Syzygium polyanthum

Abstract

Juvenile dermatomiyositis (JDM) merupakan suatu penyakit autoimun infl amatif yang jarang dijumpai dan secara khas ditandai oleh adanya lesi-lesi kulit tipikal serta kelemahan otot proksimal yang simetris. Etiologi penyakit tersebut belum diketahui dengan pasti, namun demikian patogenesisnya diketahui bersifat multifaktorial meliputi faktor genetik, paparan sinar ultra violet (UV) serta infeksi oleh berbagai mikroba seperti virus Coxsackie atau Borrelia burgdorferi. Penegakan diagnosis JDM adalah berdasarkan kriteria Bohan-Peter meliputi: 1) kelemahan otot proksimal simetris, 2) peningkatan enzim otot, 3) infl amasi miopati pada hasil biopsi otot 4) gambaran miyopati pada hasil pemeriksaan EMG, dan 5) lesi kulit tipikal. Ditemukannya papul-papul Gottron dan heliotrope rash di kulit, serta adanya kelemahan otot proksimal yang simetris, merupakan manifestasi klinis yang khas untuk JDM. Terapi lini pertama penatalaksanaan JDM adalah kortikosteroid sistemik. Dilaporkan satu kasus JDM pada seorang anak perempuan berusia 6 tahun. Diagnosis ditegakkan berdasarkan adanya lesi kulit tipikal, kelemahan otot proksimal simetris dan peningkatan kadar enzim penanda infl amasi jaringan muskuloskeletal. Pasien berespon baik terhadap pemberian kortikosteroid adekuat serta tabir surya.Kata Kunci: Juvenile dermatomiyositis, systemic corticosteroid

Abstract

Until now there is no standard for measuring the standard willingnesss to pay of patients and patient satisfaction for the quality of dental care. Research purposes to assess the magnitude of willingness to pay for outpatient dental care and want to test the factors that affect willingness to pay for outpatient dental care at the Muhammadiyah Medical Center (MMC UMS). This study was observational analytic study with cross-sectional approach, where all the variables studied were observed at the same time. The study population was patients who received outpatient dental care with simple random sampling. To the adequacy of the sample required a minimum sample size of 120. The data collection by questionnaire content validity technique. The results showed 60.8% of patient education level is college. The average patient?s willingness to pay for dental care Rp.60.833. The average satisfaction score of patients on dental health servicesis 77.35 (including the high satisfaction category). While the average patient income for Rp.2.110.417. Statistical analysis proves? there is a positive correlation? between service quality and willingness to pay for dental care patients (r=0247, p <0.05). In this study also proves? there is a positive correlation? between the patient and the willingness to pay in comedental care (r=0.730, p <0.05). With the regression test showed the ?threev ariables (education, income and quality of service)? jointly able to predict variations in the willingness of patients to pay for 59.7% and statistically ? t regression model (valid) (p <0.001).Keywords: education, income, quality of service, willingnestopay

Abstract

Lumbar spinal stenosis dengan instabilitas, salah satu pilihan pengobatannya dengan laminektomi dekompresi, stabilisasi posterior dan PLIF (Posterior Lumbar Interbody Fusion). Derajat fusi dapat ditentukan dengan pemeriksaan CT-scan post operatif. Sedangkan untuk menilai disabilitas dan skor fungsional pada pasien LSS menggunakan skor Oswestry Disability Index (ODI). Penelitian ini merupakan penelitian analisis observasional pada 18 pasien LSS yang telah dilakukan operasi instrumentasi PLIF. Pasien diminta mengisi kuesioner ODI dan dilakukan evaluasi CT Scan, kemudian dilakukan uji korelasi data yang didapat. Penelitian ini menunjukkan adanya korelasi yang signifikan antara derajat fusi dengan ODI. Koefisien korelasi antara derajat fusi dengan ODI didapatkan 0,904 dengan nilai signifikansi 0,00 (p<0,05). Derajat fusi post operasi dari gambaran CT Scan memiliki korelasi yang signifikan terhadap derajat disabilitas menggunakan skor ODI.Kata Kunci: Lumbar spinal stenosis, Fusion rate, PLIF, ODI Lumbar spinal stenosis with instability one of the treatment options is decompression laminectomy, posterior stabilization and PLIF (Posterior Lumbar Interbody Fusion). The degree of fusion can be determined by CT scan post -operatively. To assess disability and functional scores in LSS patients can use the Oswestry Disability Index (ODI). This study was an observational analysis study in 18 LSS patients who had PLIF instrumentation surgery. Patients were asked to fill in the ODI questionnaire and were evaluated for CT Scan, then the correlation data were obtained. This study shows a significant correlation between the degree of fusion and ODI. The correlation coefficient between the degree of fusion and ODI is 0.904 with a significance value of 0.00 (p <0.05). The degree of postoperative fusion of CT scans has a significant correlation to the degree of disability using the ODI score.Keywords: Lumbar spinal stenosis, Fusion rate, PLIF, ODI

Abstract

Pegagan (Centella asiatica L. Urban) contain saponin asiaticoside, which is a structural protein. The compound plays a role in the wound healing process by the formation of collagen. This research was conducted to determine the effect of wound healing of burns on New Zealand male rabbits using gel extract of Pegagan (Centella asiatica L. Urban) herbs using hydroxypropyl methylcellulose (HPMC) as gelling agent. This research was an experimental with the same subject design, using ? ve male New Zealand rabbits to test the wound healing of burns, and the backs of each rabbit were divided into six treatment i.e : positive control (Bioplasenton), negative control (no treatment), control HPMC base (no extract), and three formula with herbal extracts of Centella asiatica with a concentration of gelling agent HPMC 8%, 9%, and 10% respectively. Gel was applied to the back of rabbits with the use ± 0.3 grams once daily until the wound diameter equal to zero or until healed. The data was analyzed by one way anava with a 95% confidence level. The result showed that of the gelling agent HPMC concentration effect on the increased the viscosity, and decreased the spreadability and the burn wound healing time is a longer. Gel herb pegagan extract (Centella asiatica L. Urban) with a gelling agent HPMC concentration of 8% (17,60±1,14 days) most effectively heal burns compared to the concentration of 9% (19,40 ±1,14 days) and concentration 10% (20,40±1,14 days).Keywords: Centella asiatica L. Abstract

Soil-Transmitted Helminth (STH) menginfeksi lebih dari satu milyar orang di seluruh dunia. Di Indonesia, prevalensi STH di beberapa tempat juga masih tinggi, ada diantaranya dengan prevalensi 40-60 % pada semua umur, dengan jenis cacing dan intensitas yang berbeda-beda. Masyarakat yang tinggal di sekitar Tempat Pembuangan sampah Akhir (TPA) biasanya sanitasinya buruk. Beberapa data menunjukkan bahwa pada penderita dengan infeksi STH sering menunjukkan gejala alergi seperti gatal-gatal pada kulit dan batuk kronis. Penelitian ini dilaksanakan untuk mengetahui hubungan infeksi STH dengan angka eosinofil pada masyarakat di sekitar TPA Kelurahan Mojosongo Kecamatan Jebres Kota Surakarta, Jawa Tengah, Indonesia. Sampel tinja dan darah dikoleksi dari 96 warga yang bersedia berpartisipasi (dengan informed consent). Metode Kato-Katz digunakan untuk mengetahui adanya infeksi STH dan mengetahui intensitas infeksinya. Hitung eosinofil dilakukan pada sediaan apus darah tipis yang dipulas dengan Giemsa untuk mengetahui persentase eosinofil pada penderita. Prevalensi infeksi STH secara keseluruhan sebesar 7,29% (7/96) dengan rata-rata jumlah telur 61 telur per gram feses. Semua menderita infeksi tunggal dan tidak ada yang infeksi campuran. Infeksi hookworm sebesar 6,25% (6/96) dan infeksi Trichuris trichiura sebesar 1,04% (1/96). Tidak ditemukan infeksi Ascaris lumbricoides. Semuanya menderita infeksi ringan. Prevalensi eosinofilia adalah 27,8% (26/96) dengan rata-rata persentase eosinofil 2,63%. Terdapat hubungan yang signifikan antara intensitas infeksi STH dengan angka eosinofil, dengan kekuatan sangat lemah (r=0,190; p=0,032).Kata kunci : infeksi STH, intensitas infeksi STH, angka eosinofil, Kato-Katz, eosinofilia

Abstract

From survey at Dr. Moewardi Hospital in the year 2010, the incidence of stroke patients increase 5-7% every year. Diabetes mellitus has a relationship with ischemic stroke through aterosklerosis. However, the relationship of diabetes mellitus as a risk of ischemic stroke remains uncertain due to diabetes is not a single factor for stroke. Research aim was to determine the relationship between the incidence of diabetes mellitus and ischemic stroke. It was conducted at the neurologi unit in Dr.Moewardi Hospital Surakarta, the design was a cross sectional with sample size of 132 people. The sampling technique used simple random sampling. The result is the number of ischemic stroke patients with diabetes mellitus by 47 people (71.21%). From chi square analysis (x 2) x 2 = 13.516 obtained and p <0.000 with a value of OR = 3.8 and IK = 1.841 to 7.869. Its mean that there was a relationship between diabetes mellitus and incident ischemic stroke and people with diabetes mellitus have a risk 3,8 more times will got stroke ischemic than people without diabetes mellitus.Keywords: diabetes mellitus, ischemic, stroke
