Abstract

The high rate of deliveries by caesarean section in urban areas is a problem. With BPJS membership, now cesarean delivery can be done, so that the number is increasing. The aim of the study was to determine the dominant maternal factors for cesarean delivery. This type of research is retrospective quantitative. The sample size was 140 respondents and was randomly selected from a population of 238 patients in 2018. Data sourced from patient medical records were analyzed univariately to assess their distribution, bivariate with chi square test, and multivariate with multiple logistic regression analysis. The results of the bivariate analysis found the relationship between cesarean delivery with maternal age (p-value 0.031), the incidence of premature rupture of membranes (p-value 0.032), and obstetric history (p-value 0.008). Apart from that, there was no association between cesarean delivery and parity (p-value 0.274), gestational distance (p-value 0.361), hypertension (p-value 0.586), asthma (p-value 0.366), and diabetes (p-value 1,000). The results of the multivariate analysis found that obstetric history was the dominant variable, giving a 3 (three) times higher chance of having a cesarean delivery. For the maternal age and the incidence of premature rupture of membranes have 2 (two) times higher chance of having a cesarean delivery. Recommendations for the Ministry of Health and its staff to strengthen reproductive health education policies from an early age, namely when women menarch, through mandatory learning at every level of education.

Abstract

The importance of Quality Improvement (QI) in health services is to improve the quality of health services continuously. Quality Improvement (I) can minimize complaints and achieve patient satisfaction. Based on JKN data, the Diponegoro I Primary Clinic is the primary clinic with the highest number of participants in Semarang City, namely 27,522 participants. Based on unstructured interviews with the clinic manager, the high level of complaint reports from patients because of dissatisfaction with the general poly services at the Diponegoro I Primary Clinic. The aim of this research is to analyze the level of patient satisfaction for general poly services at the Diponegoro I Primary Clinic through a balance between expectations and performance in the quality dimension (SERVQUAL). This research is a descriptive quantitative type with a cross-sectional design. The total research sample was 110 general poly patients. The data analysis technique uses the Importance-Performance Analysis (IPA) method, which consists of suitability level analysis and quadrant analysis. The research results show the level of conformity of quality dimensions is 95.78%. While each dimension is tangible (96.88%), reliability (94.17%), responsiveness (96.50%), assurance (96.54%), and empathy (94.81%). There are items should be improved by the clinic, toilet facilities cleanliness, service time by online registration system implementation, service procedure information to patients, doctor’s scheduled board updates, and SOPs implementation to inform medicine side effects to patients. The improvement to increase the level of patient satisfaction.

Abstract

This study explores the management strategies implemented in health services and facilities for the older adult population in West Manggarai, East Nusa Tenggara, Indonesia. The research focuses on the unique healthcare needs of older adult residents and investigates various approaches adopted by local health institutions to enhance the quality of care, promote accessibility, and address specific challenges associated with aging. The sample for this research consists of 7 health care personnel. The method used in this study was qualitative, involving in-depth interviews. Through an examination of management practices, this study provides valuable insights for the effective strategies of healthcare services for older adult in the region that are faced with limited personnel and facilities.

Abstract

Introduction: The right marketing strategy is not only limited to promoting products, but also arranging segmentation, namely dividing product marketing based on target customers and target markets. Hospitals really need a marketing strategy to face all changes and challenges both internal and external to a company.Purpose: To analyze segmentation and determine the target market for the establishment of Bunda Mulya Hospital in Bogor RegencyMethods: Quantitative and qualitative research approaches (mixed methods). The quantitative approach used in this research is analytical observational with a cross sectional design. The qualitative approach in this research uses a grounded theory type of design. Market segmentation analysis was carried out to determine the appropriate target market for the establishment of Bunda Mulya Hospital through geographic, demographic, psychographic and behavioral approaches. The research subjects were the people of Bogor Regency, the Chair of PERSI Banten Province, Deputy Chair II of PERSI West Java Province, the Bogor Regency Health Service, and the Main Director of the Bunda Mulya Pratama Clinic.Results: The appropriate target market for establishing Bunda Mulya Hospital is the community around Bunda Mulya Hospital (travel time < 30 minutes) with an age range of teenagers - early adults (12-45 years), female, with a predominant high school/equivalent education level. by private workers, families of 2-4 people with total family income between 3-5 million. The public hopes to get fast treatment and complete specialist doctors. The community gets most of the information from neighbors/cadres/friends and visits 1-5x/year. The public provides suggestions regarding class I inpatient rates of <250,000, VIP class inpatient rates of <400,000, general practitioner consultation fees between 40,001-50,000, specialist doctor consultation fees between 100,001-125,000, normal delivery package costs between 1.5-2.5 million and The cost of an ultrasound examination is between 150,000-250,000. Most of the people around Bunda Mulya Hospital are willing to undergo examinations at Bunda Mulya HospitalConclusion: Plans for establishing a new hospital should pay attention to the needs of people of productive age (26-45 years), especially women. The public expects to receive fast service and complete specialist doctors. Therefore, this becomes input for Hospital Management to be able to meet community needs. Hospital financing should pay attention to the UMR limit for Bogor Regency where costs are adjusted for middle and lower middle class people

Abstract

Stunting is a condition where a child fails to grow properly during the first 1000 days of life due to long-term malnutrition. This research aim is to identify the factors (Knowledge, Motivation, Training, Attitude) that affect the performance of posyandu cadres in accelerating Stunting reduction within the Sungai Durian Health Center work area, using quantitative research methods with descriptive and verification analysis. The results of the descriptive analysis showed that knowledge had an excellent average score of 4.03, motivation had a good average score of 3.66, training had a good average score of 3.80, and attitudes had a good average score of 3.76. Based on the T count and T table results, knowledge influenced with a score of 6.642 > 2.023, motivation with a score of 4.287 > 2.023, training with a score of 2.598 > 2.023, and attitudes with a score of 3.652 > 2.023. Means there is an influence based on the results of the T test the most influence factor is knowledge on accelerating Stunting reduction. The F test showed that the performance factors of posyandu cadres, which are knowledge, motivation, training, and attitudes, collectively affect the acceleration of Stunting reduction, with an F count score of (752.317 > 2.61). The significance of the F score was 0.000, and the R Square value was 0.893, meaning that 89.3% of the independent variables were bound, while the remaining 10.7% was influenced by other variables that were not included in this research model.

Abstract

Postpartum family planning is a contraceptive service targeting mothers who have just given birth 0-42 days with the aim of preventing unwanted pregnancies. The high maternal mortality rate is caused by complications during pregnancy, so it is very important to use postpartum contraception. However, in Puskesmas Padangsari working area, the utilization of postpartum family planning services was very low,namely 28,47%. Therefore, this research was conducted with the aim of analyzed the utilization of postpartum family planning services in the Puskesmas Padangsari working area.This research was a quantitative research with a cross sectional approach using the accidental sampling technique where from a population of 198 mothers giving birth from January to December 2023, the sample was calculated using the slovin formula and with an error degree of 7%, a minimum sample size of 101 respondents was obtained. The research was carried out form January to March 2024.Bivariate data analysis using logistic regression test. Based on the research results, 44,7% of respondents had poor utilization of postpartum family planning services. Factors that did not influence the use of postpartum family planning services are mistrust, perception,and communication between service providers and patients. Meanwhile, the factors that influence the utilization of postpartum family planning services are cultural competency (p=0,069), perceived need (p=0.001), and customer satisfaction (p=0.005).

Abstract

Padangsari Community Health Center is one of the community health centers in Banyumanik sub-district with a low percentage of SICENTIK's participation. This research aimed to conduct implementation observation research at SDN Pedalangan 1 and SDN Pedalangan 2. This research was a qualitative descriptive research that uses purposive techniques in selecting informants. It was held on November 2023 - April 2024. Data collection using in-depth interviews and literature study. The results of this research show that there are several differences in implementation between SDN Pedalangan 1 and SDN Pedalangan 2 shown by the place of this program was implemented, principal's role, division of labour, form letter, knowledge and experience of implementer, shared commitments, implementer's preparations, and evaluations of the program.  The study's findings demonstrate that the lack of Standard Operating Procedures (SOP), success indicators, and health center controls is the primary cause of the disparities between the two schools. Thus, in addition to creating disparities between the two institutions, it also results in wasteful and poor program execution. Then, during the program's implementation, it was found that the community health center and SDN Pedalangan 2's supervisor had acted in an unprofessional manner, which had led to a lackluster implementation of SICENTIK in schools and community health centers.

Abstract

The swift progress of technology and information in contemporary times opens up avenues for expanding marketing networks, introducing novel products or services, and seamlessly integrating online activities. Online platforms for pharmaceutical sales streamline the process for consumers, offering convenience in finding desired medications at competitive prices and ensuring swift access. This study aims to assess customer contentment concerning the caliber of pharmaceutical procurement services offered through the Halodoc app. Through purposive sampling, data collection was conducted via questionnaires administered to 422 Halodoc users capable of assessing service quality. Utilizing the E-service quality (e-CSI) methodology for analysis, consumer satisfaction with drug purchasing services was measured. Findings revealed high satisfaction levels across various dimensions: reliability at 97.40%, responsiveness at 91.10%, guarantee at 92.06%, empathy at 82.90%, and physical evidence at 91.28%, culminating in an overall Customer Satisfaction Index (CSI) score of 90.98%. Hence, it can be inferred that respondents utilizing the Halodoc application expressed significant satisfaction with the service provided.

Abstract

Activity Based Costing is a cost assignment system based on the activities carried out by the company to produce products. The aim of this research is to find out and obtain information about calculating and comparing prices for inpatient room rates using conventional cost calculation methods with the ABC method. This is the quantitative descriptive research involves conducting interviews with the person in charge of the VIP inpatient room and financial department who relates to the research data. The results of the research show that the calculation of inpatient service rates using the Activity Based Costing method for the VIP class is IDR 160,499. From these results, when compared with the tariff used by Intan Medika Hospital of IDR 266,000, the Activity Based Costing method provides cheaper results with a difference of IDR 105,501. So by reducing rates according to the Activity Based Costing method, Intan Medika Hospital can expand its market because if it uses more appropriate prices and provides good quality, customers will choose to use Intan Medika Lamongan Hospital, so that Intan Medika Lamongan Hospital can increase the occupancy of VIP inpatient room and have a long-term profits.

Abstract

Utilization of telemedicine services decreased after the pandemic conditions improved. Telemedicine utilization is closely related to the internet network. Central Java is the province with the lowest internet penetration rate in Java. The purpose of this study was to determine the factors related to the utilization of telemedicine services after the COVID-19 pandemic in Central Java. This type of research was quantitative research with a cross-sectional study design. This research was conducted in December 2023-March 2024. The sample in this study was 300 people in Central Java. The sampling technique used accidental sampling method. The analysis used was univariate analysis and bivariate analysis using the Chi Square test. This study showed that 50.3% of respondents utilized telemedicine services after the pandemic. Utilized telemedicine services after the pandemic related to education (p-value <0.001), knowledge (p-value <0.001), trust (p-value <0.001), family support (p-value <0.001), accessibility (p-value <0.001), and need (p-value <0.001). All independent variables in this study (education, knowledge, trust, family support, accessibility, needs) are associated with the utilization of telemedicine services after the COVID-19 pandemic.
