Abstract

Kangaroo Mother Care (KMC) varies in each hospital. This condition can be viewed from the aspects of types of services, competency of human resource, facilities, and means. Implementation is a crucial step in the process of policy. Readiness and participation of parents will support the success of the KMC implementation. A role of family such as attitude, behaviour, and family participation is as a desire to protect a family member who is sick. This research aimed to identify the implementation process of factors of communication, disposition/attitude, resource, bureaucratic structure, and participation of patients and their families in KMC at Mardi Rahayu Hospital. This was qualitative research using in-depth interview and direct observation. Main informants encompassed 4 mothers who had Low Birth Weight (LBW) Babies. Meanwhile, informants for triangulation purpose encompassed 3 trained midwives, 3 trained nurses, and 3 trained paediatricians, nursing manager, and director of medical services. Furthermore, data were analyzed using thematic analysis in which data as the result of in-depth interview were analyzed in accordance with a theme. The implementation of KMC had not been done optimally. Regarding communication, there was no socialization of the program. Therefore, information of the KMC program was unclear, inconsistent. Regarding attitude/disposition of the program implementer, there was not only lack of support from all implementers in implementing the program but also lack of commitment. Regarding resource, there was lack of trained health workers, lack of facilities, and no sustainable fund for the program. Regarding bureaucratic structure, Standard Operating Procedure was needed to be revised and no clear job-sharing in KMC. Mothers had not participated in making decision, implementing, taking benefits, and evaluating yet. This was due to lack of information. As suggestions, the hospital needs to improve socialization of the program, provide implementation guidelines, and implement KMC followed by monitoring and supervising. In addition, the hospital needs to improve communication, information, education (CIE) for mother/family and provide opportunity for clients to implement KMC at the hospital before going home.

Abstract

Evaluasi SIK bertujuan untuk memastikan SIK berjalan secara efisien, mampu mengumpulkan informasi yang relevan dan berkualitas sebagai dasar pengambilan keputusan oleh pemangku kebijakan. Hasil penilaian SIK sangat dibutuhkan dalam meningkatkan kinerja SIK.SIK Provinsi Jawa Tengah merupakan sub sistem dari SIKNAS sehingga pengembangan SIK Provinsi Jawa Tengah akan berpengaruh terhadap penguatan SIKNAS. Tujuan dari penelitian ini adalah untuk mengevaluasi SIK di Provinsi Jawa Tengah dalam rangka Penguatan SIKNAS. Penelitian ini merupakan penelitian deskriptif evaluatif dengan pendekatan kuantitatif dan kualitatif. Subyek penelitian adalah 35 petugas SIK DKK dan enam petugas SIK Dinas Kesehatan Provinsi Jawa Tengah. Pengumpulan data kuantitatif melalui pengisian kuesioner, pengumpulan data kualitatif melalui FGD dan wawancara. Analisis data penelitian dilakukan dengan menggunakan perangkat Evaluasi SIK Provinsi yang merupakan modifikasi dari Assessment Tool HMN Versi 4.00. Modifikasi Assessment Tool HMN Versi 4.00 dilakukan dengan cara menghapus item pertanyaan yang tidak relevan dengan SIK di provinsi dan menambahkan item pertanyaan berdasarkan PP no 46/2016 tentang SIK. Hasil evaluasi terhadap tujuh komponen SIK, empat diantaranya dinilai “ada tapi tidak adekuat” yaitu komponen pengelolaan (54,7%), sumber daya (54,2%), sumber data (58%) dan manajemen data (41,3%). Sementara tiga komponen lainnya dinilai “adekuat”, yaitu komponen indikator (74,3%), produk informasi (71,3%) dan diseminasi dan penggunaan informasi (74,5%).Disimpulkan bahwa secara umum SIK Provinsi Jawa Tengah berada dalam kategori “adekuat”. Untuk meningkatkan pengelolaan SIK di Provinsi Jawa Tengah, direkomendasikan untuk membuat suatu standar tata kelola SIK sebagai acuan pengelolaan SIK di Provinsi Jawa Tengah. Evaluation of Health Information System (HIS) aimed at ascertaining the efficient implementation of HIS that could result relevant and high-quality information to be used as a basis of making a decision by a decision maker. The result of HIS assessment was needed to improve the HIS performance. HIS of Central Java Province was a sub-system of National Health Information System (NHIS) in which the development of HIS there would strengthen the NHIS. The aim of this study was to evaluate HIS in Central Java Province in order to strengthen NHIS. This was a descriptive-evaluative study using quantitative-qualitative approach. Research subjects were 35 HIS officers at District Health Office (DHO) and six HIS officers at Central Java Province Health Office. Quantitative data were collected by filling a questionnaire whereas qualitative data were collected by conducting FGD and indepth interview. Data analysis was performed using a software of Province HIS evaluation that was modified from Assessment Tool HMN version 4.00. Modification of Assessment Tool HMN version 4.00 was performed by deleting questions that were irrelevant with HIS at province and adding questions based on government regulation number 46/2016 about HIS. The results of evaluation of seven components of HIS, four of these seven components indicated “available but inadequate”, namely components of management (54.7%), resource (54.2%), data source (58%), and data management (41.3%). In contrast, three of these seven components were “adequate”, namely components of indicator (74.3%), information product (71.3%), and dissemination and use of information (74.5%). To sum up, generally HIS of Central Java Province was categorised as “adequate”. To improve HIS management in Central Java Province, a standard of HIS management needs to be made as a reference to manage HIS in Central Java Province.

Abstract

One of the efforts to decrease maternal mortality rate (MMR) was an implementation of a delivery planning and complication prevention program (P4K) in activities of delivery mother’s saving and social fund for delivery conducted by village midwives. The aim of this study was to analyse factors relating to performance of a village midwife as a facilitator in the activities of delivery mother’s saving and social fund for delivery on the P4K in District of Sumenep in 2014.This was an analytic-observational study using a cross-sectional approach. Independent variables consisted of age, marital status, education, knowledge, experience, motivation, and supervision and a dependent variable was performance of village midwives. Number of samples were 90 village midwives working at 30 health centres. Data collection used a structured questionnaire that had been performed tests of validity and reliability. Furthermore, data were analysed using methods of bivariate (Chi-Square test) and multivariate (Logistic Regression test). The results of this research showed that respondents had low performance (30%), young age (31.1%), not married (24.4%), low education (42.2%), low knowledge (35.6%), low experience (77.8%), low motivation (26.7%), and low supervision (24.4%) in managing the activities of delivery mother’s saving and social fund for delivery at their work areas. Variables of knowledge (p=0.019) and supervision (p=0.037) statistically significantly related to the performance of village midwives. These two variables jointly influenced the performance of village midwives with p values= 0.07 and 0.013 respectively. District Health Office, Health Centre, and Indonesian Midwives Association need to conduct training, workshop, or socialisation to village midwives regarding the P4K particularly the activities of delivery mother’s saving and social fund for delivery.

Abstract

 RS Muhammadiyah Darul Istiqomah Kendal merupakan RS tipe D yang ingin meningkatkan mutu pelayanannya setelah bekerjasama dengan BPJS Kesehatan. Peningkatan mutu pelayanan berhubungan erat dengan tingkat kepuasan pasien. Untuk mengukur tingkat kepuasan pasien ada sepuluh indikator  yang dikembangkan Linder-pelz(1982) dalam krowinski antara lain accessibility/convenience;availability of resources; continuity of care; efficacy/outcomes of care; finances; humaneness; information gathering; information giving; pleasantness of surroundings; quality/competence.Desain penelitian ini cross sectional dengan pendekatan kuantitatif, jumlah sampel 120 dengan mengukur tingkat kepuasan antara karakteristik pasien yang meliputi umur; jenis kelamin; pendidikan; pekerjaan; penghasilan; status kepesertaan JKN dengan 10 indikator  yang dikembangkan Linder-pelz(1982).Hasil penelitian ini menunjukkan bahwa sebagian besar responden merupakan pasien peserta BPJS PBI dengan berpenghasilan < 1500000. Rata-rata pasien puas terhadap pelayanan rawat inap. Indikator dengan skor kepuasan tertinggi adalah indikator quality/competence(87,5%) dan dimensi dengan skor kepuasan terendah adalah dimensi availability of Resources (61,66%). Penelitian ini menunjukkan tidak ada hubungan yang bermakna antara jenis kelamin dengan kepuasan (p=0,304), dan antara usia dengan kepuasan (p=0, 346), ada hubungan yang bermakna antara pekerjaan dengan kepuasan,  penghasilan dengan kepuasan, status kepesertaan BPJS dengan kepuasan, antara pendidikan dengan kepuasan (p=0,00).Kesimpulan dari penelitian ini untuk manajemen RSDI yang harus segera diprioritaskan Kesiap siagaan perawat, kelengkapan alat pemeriksaan kesehatan, kemudahan cara pembiayaan pelayanan, tidak adanya biaya tambahan untuk obat tertentu, keseragaman dalam pelayanan keperawatan, penyampaian informasi tindakan, penyampaian informasi keadaan klinis, kebersihan dan kenyamanan lingkungan RS dan kompetensi karyawan..

Abstract

An information system of monitoring health equipment at Balai Kesehatan Indera Masyarakat (BKIM) in Province of Central Java was very simple. The available system had not resulted complete, accurate, and well-timed information in accordance with the needs of management to monitor health equipment in all health service rooms. The aim of this study was to develop an information system of monitoring health equipment to support quality assurance of health services at BKIM in the Province of Central Java. This was a qualitative-quantitative study using Pre-Experimental design with one group pretest-posttest approach. Data were collected using methods of observation and interview. Furthermore, qualitative data were analysed using content analysis whereas quantitative data were presented descriptively using a balanced average value. The result of this study showed that a balanced average value before developing the system was 37.73 and after developing the system was 78.90 with the difference equal to 41.17. There needs commitment and support from the head of BKIM in Central Java Province. Implementation of the developed system needs to be continually monitored. Socialisation and training needs to be conducted for all employees who have main tasks and functions relating to health equipment. In addition, budget for operational costs and system maintenance needs to be allocated.

Abstract

A nurse’s necessity at a health centre viewed from workload analysis is required to identify a nurse’s necessities adjusted with service standards and real condition at a health centre. This requirement is in accordance with the change of an outpatient health centre status to be an inpatient health centre in which infrastructure and human resource need to be improved. This was a qualitative study using case study approach started from data collection, FGD, NGT, work sampling, to indepth interview with stakeholders who had experiences in calculating an employee’s necessities based on ABK and nurse representative. Furthermore, data were analysed using a form to calculate an employee’s necessities obtained from BKN. The results of calculation of a nurse’s necessities showed that number of nurses required for an inpatient unit were nine persons consisted of five skilled nurses and four expert nurses. This number was equal to nine beds that were required (1:1). On the other hand, number of nurses required for an outpatient unit including BP, UKM, and Pustu were 26 persons (16 skilled nurses and 10 expert nurses) or 1 nurse served 1,458 people or 456 families. Rembang 2 Health Centre needs to fulfil a nurse’s necessities by considering innovation of nursing services to reduce burden of human resource. A nursing committee needs to be formed by District Health Office as an institution that is responsible to supervise functional positions. Standards of nursing diagnosis and an information system of nursing care services need to be determined in order to describe an autonomy level of individual, family, group, and community in District of Rembang in terms of self-care. In addition, academics need to conduct further research regarding workload of other functional positions in health in order to achieve goals of national health development.

Abstract

Title :Patient Satisfaction To Doctor’s And Nurse’s Care Associated To The Interest To Be Rehospitalized (A study at RSUD Dolopo, Madiun Regency). AbstractBed Occupancy Rate (BOR) at Dolopo Public Hospital had decreased from 56.16% in 2013 to 35.86% in 2104 and 35.2% in 2015, and the average ALOS (Average Length of Stay) and Turn over Interval (TOI) increased. The preliminary study on patients of Social Security Agency (BPJS Non PBI) showed a dissatisfaction to nurse’s and doctor’s services. The aim of this study was to analyze the associations between patient satisfaction toward doctor’s & nurse’s services to the interest to be rehospitalized.This study was conducted in a cross sectional design. The subjects were 95 patients of  BPJS with Non PBI in inpatient room for class I, II, III, which was choosen by using accidental sampling. Data were gathered through interviews using structured questionnaires. Data analysis was conducted by Chi-square tests and logistic regression.Results showed that the patient satisfaction based on aspects of tangibles, reliability, responsiveness, assurance, and empathy were lower than the expectations. Most of the subjects have positive attitudes, negative subjective norms and perceptions of behavior control and had a poor accesibility to the hospital. However 75.8% of the respondents were interested to be rehospitalized at Dolopo Hospital. There were correlations between patient satisfaction (p=0.009), attitude (p=0.000), subjective norm (p=0.002) with the interest to be rehospitalized. There was no correlation between control perception behavior (p=0.103), and access (p = 0.454) with the interest to be rehospitalized. After controlled for the attitude toward doctors’ and nurses’ services, the patients who were satisfied to the doctor’ and nurses’ services had 3.84 times to be interested to be rehospitalized than the patients who were not satifsfied (OR = 3.84 ; CI 95% : 0.98-15.12, p=0.054).Keywords       : Patient Satisfaction, Rehospitalized, BPJS.

Abstract

Performance is influenced by ability, work motivation, job satisfaction. Some explained in Rivai's theory (2005) ; Bittel, L.R and Nestrom, J.W (1991) ; Gibson (2001) ; Usman (2008), where the common problems faced by organizational leaders. The right nursing process activities have not been carried out adequately by the nurses, which has an impact on the hospital occupancy rate. The high turnover data in 2017 was 18% compared to 2016 as much as 12%. This study uses an analytical survey with a quantitative approach, using a cross sectional design, this population is 89 people, in this study researchers used the entire population to be sampled. Data analysis method Partial Least Square (PLS). The results showed that the variable Ability of Gibson's theory (2001) was able to explain as much as 90,8% (goodness of fit), and the remaining 9,2% could be found by indocators / variables outside the theory under study. Maslow's Work Motivation Variable (1943) is able to explain 80,1% (goodness of fit), and the remaining 19,9% can be found indicators / variables outside the theory under study. Nurse Performance Variables theory of Mondy and Noe (1990) ; Tsui, et, al. (1997) ; L. Mathis and John H. Jackson (2002) ; Simamora (2004) is able to explain 86,6% (goodness of fit), and the remaining 13,2% can be found indicators / variables outside the theory under study. Overall performance is influenced by ability and motivation, but job satisfaction has no effect. It is recommended that the hospital be carried out efforts to increase capacity, namely the ability to interact, conceptual abilities, and technical abilities, while for work motivation, namely physical, sense of security, compassion, self actualization and appreciation.

Abstract

Penelitian ini bertujuan untuk mengetahui kesuksesan sistem informasi kesehatan haji di KKP Semarang melalui pendekatan DeLone and McLean. Jenis penelitian ini adalah deskriptif dengan pendekatan kulitatif. Teknik pengumpulan data melalui wawancara mendalam dan observasi. Wawancara disusun berdasarkan model Update kesuksesan DeLone and McLean terhadap enam dimensi yaitu kualitas sistem, kualitas informasi, kualitas layanan, penggunaan, kepuasan pengguna dan manfaat yang didapat dari menggunakan sistem. Hasil penelitian menunjukan bahwa sistem memiliki fleksibilitas yang baik dengan dapat diakses dari mana saja dan adanya falisitas ekspor dan impor ke excel untuk memudahkan input dan pembuatan laporan. Informasi yang ditampilkan juga sudah lengkap.dan akurat, sehingga membuat pengguna merasa puas selama menggunakannya. Kekurangan sistem ini adalah pada kapasitas penyimpanan yang harus ditambah karena menampung semua sistem yang ada di KKP Semarang. Manfaat yang diperoleh dari sistem adalah pekerjaan menjadi lebih cepat dan mudah dalam pencatatan dan pelaporan. Selain itu hasil informasi dapat digunakan sebagai bahan perencanaan oleh KKP Semarang.

Abstract

Hyperbilirubinemia neonatus adalah suatu kondisi peningkatan kadar bilirubin dalam darah pada bayi baru lahir, baik oleh faktor fisiologik maupun faktor non-fisiologik, yang dapat menyebabkan kematian pada bayi. Lama rawat pasien hyperbilirubinemia neonatus di RS Hermina Pandanaran 4,3 hari (± 0,675) tidak sesuai dengan standar yang ditetapkan yaitu 3 hari. Tujuan penelitian ini adalah mengetahui alasan-alasan tenaga kesehatan melakukan atau tidak melakukan prosedur sesuai dengan clinical pathway.Jenis penelitian ini adalah kualitatif yang disajikan secara explanatory design. Pengumpulan data dilakukan dengan teknik wawancara mendalam dan observasi. Informan utama pada penelitian ini adalah 3 dokter spesialis anak dan 3 perawat pelaksana, dan Informan triangulasi yaitu manajer pelayanan medis, kepala instalasi dan kepala perawat ruang perinatologi. Analisis data dilakukan dengan metode content analysis.Hasil penelitian menunjukkan bahwa dari 10 kasus yang diamati, rata-rata kepatuhan sebesar 92,18% (±1,77). Pemeriksaan uji Coomb dan G6PD tidak dilakukan karena pemeriksaan tersebut tidak bisa dilakukan di RS Hermina Pandanaran dan hanya 70% dari dilakukan pemeriksaan ulang bilirubin total karena alasan biaya. Sosialisasi belum berjalan dengan baik karena baru dilakukan di awal saat masuk bekerja. Belum dilakukan pengecekan sarana dan prasarana secara rutin. Tidak ada tim dan Standar Operasional Prosedur untuk monitoring dan evaluasi pelaksanaan Clinical pathway.Disarankan agar pihak Rumah Sakit  mensosialisasikan kembali Clinical Pathway yang telah disusun, membentuk tim monitoring dan evaluasi pelaksanaan Clinical Pathway dan membuat Standar Operasional Prosedur  monitoring dan evaluasi Clinical pathway.
