Abstract

ABSTRAK Surgical Site Infection (SSI) is a major complication of surgery which can increase morbidity,  mortality and cost of hospitality. Incidence of SSI at a healthcare provider/facility reflected a service quality of that institution. WHO reported prevalence of SSI was around 5% to 34%. Elective or emergency laparotomy is one of major surgery in a hospital. Some factors could be influence incidence of SSI were degree of contamination (kind of surgery), ASA (American Society of Anesthesiologists) score, DM (Diabetes Mellitus) co morbidity, temperature of patients, number of leukosit and duration of surgery. We aimed to explore incidence of SSI and contributing factors of emergency laparotomy patients from RSUP MH Palembang. This research was observational analytic with cross-sectional design. We found incidence of SSI 56,67% consist of incisional superficial SSI 70,6%,  incisional SSI 23,5% and organ SSI 5,9%. SSI occurred at third day until seventh day after surgery and mostly at fifth day (52,9%). Pathogen dominant in these cases were Escherichia coli 31,25%. Statistical analytic with confidence interval 95% revealed risks of ASA score (p 0.004, OR 2,4), patient’s temperature (p 0.008, OR 10,2), diabetes mellitus co morbidity (p 0.492, OR 1,87), kind of surgery (p 1.0 OR 1,36), number of leukosit (p 0.29, OR 4,80) and duration of surgery (p 0.123, OR 0,25). Conclusion: Incidence of SSI at  RSUP MH Palembang about two times compare to world average incidence of SSI. ASA score, temperature and diabetes mellitus co morbidity influenced SSI incidence. Abstract

ABSTRACT Background: Learning gross anatomy might be conducted using variety of methods to increase student knowledge such as through tutorials, lectures and laboratory session. These methods require a media to deliver the learning material. Software power point has been used as a media to deliver gross anatomy material in Medical School of Jambi University on preparation session before laboratory session started. However, these preparation session since 2012/2013 academic year were no longer held. In the other hand, some teachers tried to modified Acland video of gross anatomy by dubbing its english language to indonesia language in order to help students comprehend the material better. Objectives: This study aimed to identify score differences of pre-test and post-test among two students group. The first group was an experimental group where students experiencing preparation session using Acland dubbing video. The other group was control group where student experiencing preparation session using power point. Methods: The design of this experimental study is control group pre-test and post-test. 83 medical students of Jambi University who registered the reproductive system modul were totally sampled and than devided into two groups. Short answer question were arranged as an intrumental test to measure students knowledge before (pre-test) and after (post-test) preparation session. Results: Pre-test score between those two groups are not differ significantly (p = 0.397). Post-test score between those two groups are also not differ significantly (p = 0.157). However, the gain score (an avarage spatial score between post-test dan pre-test) among those two groups are differ significantly (Gain: p = 0.005; N.Gain: p = 0.005) where experimental group is higher than the control group. Conclusion : Post-test score of between experimental group where students experiencing preparation session using Acland dubbing video is not differ significantly to control group where students experiencing preparation session using power point. Since, the preparation session is no longer held, institutions may consider the use of Acland dubbing video to facilitate students constructing prior knowledge before the laboratory session started. Abstract

ABSTRACT Background : Radial artery occlusion is one of the transradial vascular complications post cardiac catheterization. To minimize the effect of vascular problem related to local compression by using compression methods after cardiac catheterization would be decrease vascular complication such as radial artery occlusion (RAO). This research was conducted to determine the comparison between the Barbeau test method and the clinical observation method carried out by the researcher in order to examine the patency of radial artery during compression, post procedure cardiac catheterization at the Angiography Coroner Unit and the Cardiac Intensive Care Unit, Hasan Sadikin Bandung General Hospital, Indonesia. Methods : This research was a quantitative research using the comparative study design. The subjects of this research were 20 patients who took cardiac catheterization procedure using radial artery access site at the Angiography Coroner Unit and checked in pairs. The measurement was conducted gradually in three hours using compression band, namely the Stepty-P. The statistical test, specifically the McNemar test, was used to analyze the comparasion between the Barbeau test method and the clinical observation method, meanwhile the single variable data were analyzed by the distribution frequency. Result : According to the research result, it depicts that the distribution frequency of subjects who suffer from radial artery blood flow during compression period based on Barbeau Test at the first fifteen minutes were 7 patients (35%), the number of patients declined negligibly after 3 hours to 5 patients (25%), whereas the result from the clinical observation methods  was 1 patient (5%) at the first fifteen minutes, it then raise marginally after 3 hours to 2 patients (10%).  The statistical result shows that there wsere a significant difference in radial artery evaluation during compression period between two methods, in which at the first fifteen minutes (p=0,035) and the first thirty minutes (p=0,035), respectively, soon after the use of compression band, while overall, the statistical test result shows that there were no significant difference in comparison of two methods within the 45 minutes and 3 hours of compression periods (p>0.05). Conclusion : It is suggested to the nurse in the intensive care unit of cardiology to use the Barbeau Test method in assessing the patency of radial artery during compression period,  post procedure transradial cardiac catheterization.   Keywords : Barbeau test, clinical observation, radial artery, patency, cardiac catheterization.
