Abstract

Cardiovascular diseases especially coronary heartdisease (CHD) is the main disease cause death in theworld. The mortality rate of CHD (26.4%) is higherthan the mortality rate of cancer (6%). IndonesianHealth Department in 2002 reported that one offour people were died due to CHD. Hyperlipidemia isa major risk factor for CHD. Hyperlipidemia is lipidmetabolism disorder characterized by an elevationof total cholesterol (TC), triglyceride and low densitylipoprotein (LDL) cholesterol 1.Arcangelisia flava Merr is a plant belongs toMenispesmaceae family, comonly known as yellowroot. A flava is Indonesian medicinal plant that havebeen used traditionally for malaria, diabetes mellitus(DM), urinary stones, jaundice, diarrhea and skinabscess 2,3,4.The water extract of A. flava showedantimicrobial activity5. Methanolic extract of A. flavahas antioxidant and cytotoxic effect against brineshrimps and breast cancer cells (MCF-7)4. The Ethylacetate fraction of A. flava have been proven toreduce blood sugar level of diabetic rats6. The majorchemical compounds of stem of A. flava is alkaloidincluding berberine, palmatine, jathorrizine andcolumbamime7. Flavonoid, saponin, tanin andfuranoditerpene are other compounds of A. flava8.Berberine isolated from Coptis sinensis can decreasetotal cholesterol and triglycerides by increasingexpression of the hepatic low density lipoproteinreceptor (LDLR)9.Our previous study showed that methanol extract ofleaf of A. flava reduced TC and LDL cholesterol.These extract was capable to decrease theatherogenic index value and the number of foamcells10,11. Antihyperlipidemic effect of A flava wascaused by berberine and flavonoid in the extract.The berberine content of stem was higher than leafof A. flava12. In order to know the antihyperlipidemiceffect of stem extract of A. flava, the influence ofmethanolic extract of A. flava stems (EMBAf) ontotal cholesterol, triglyceride, LDL cholesterol andHigh Density Lipoprotein (HDL) cholesterol levels hasbeen determined in hyperlipidemic induced rats.

Abstract

Hypertension is a common clinical problemthat responsible of deaths account for 9.4 millionpeople in worldwide every year.1 The worldwideprevalence of hypertension estimated increase from26.4% in 2000 to 29.2% in 2025.2 An estimated 65million adult Americans, or nearly one fourth of theadult population of the United States, havehypertension. Another quarter of the populationhave prehypertension.3 RISKESDAS results showsthat the prevalence of hypertension in Indonesia was26,5%.4Among hypertensive patients in USA 11%were not on treatment regimen, 25% were not onadequate treatment, 34% were on adequatetreatment.3 In Indonesia, among hypertensivepatients only 9,5% were on treatment.4 In a recentanalysis of National Health and NutritionExamination Survey (NHANES) participants that 35,8million (53,5%) were not reach blood pressuretarget. Among these, an estimated 16.0 million(44.8%) were aware of their hypertension and werebeing treated with medication.5 In a cross-sectionalanalysis of Framingham Heart Study participants,only 48% of treated participants were controlled to<140/90 mm Hg and less than 40% of elderlyparticipants (>75 years of age) were at a goal bloodpressure.6 These data suggested that hypertensionremained largely uncontrolled worlwide.Uncontrolled hypertensive confer an increased ofcardiovascular events. Nearly one fourth of adultswith uncontrolled hypertension have stage 2hypertension (systolic BP ≥160 mmHg or a diastolicBP ≥100 mmHg), putting them at higher risk forheart disease or stroke.5 According to the WorldHealth Report uncontrolled hypertension causesover 7 million premature deaths, takes up 4.5% ofthe total global disease burden, and accounts forsome 64 million disability adjusted life years lost.7Randomized controlled trials haveconvincingly shown that treatment of hypertensionreduces the risk of stroke, coronary heart disease,congestive heart failure, and mortality.8 However itwas not easy to achieve blood pressure target. Itrequired an act of antihypertensive medicationtherapy. The success of a therapy was not onlydetermined by the diagnosis and appropriate drugselection, but also to carry out the patient's therapycompliance, included compliance in takingmedication but in fact, adherent of medication waspoor.9 Moreover, non pharmacology theraphy thattreated hypertension is dietary of sodium. Reducingdietary sodium additives may decrease hypertensionprevalence by 30%, resulting in one million fewerhypertensive patients.10 It remains lacking dataregarding controlled hypertension in Indonesia up tonow. Therefore this research aimed to to investigatemedication adherence and sodium consumption toachieve blood pressure target.

Abstract

The economics in Jember district has two cantilever sectors they are cultivation and agriculture industry. This condition is used to develop the agroindustry like crackers because the central producer of crackers is located at Mangli, Kaliwates sub-district. The purpose of this research is to analyze the variable of social factors, economy, the strategy that having an effect on the income of crackers agroindustry at Mangli, Kaliwates sub-dustrict, Jember district. This research is analytic quantitative analytic using SEM approach. The result of this research shows that interpretation of the test result about SEM-PLS is: a). Each social factor increase one unit, so the income tend to go down about 1,17 unit; b). Each economy factor increase one unit, so the income tend to go up about 0,05 unit; c). The income can be explained by the variable of social factor, economy and the enterprise strategy about 62% and the rest is 38% that influenced by another variable that doesn’t explained in the model.

Abstract

This research is done by taking research object at small scale handmade clothes industry in Rogojampi, amounting to 170 company with its population element that is manager/owner. Sampling technique that used is purposive sampling. Amount of determined responder as research sample is 100 people. Analysis technique that used to interpret and analyze data in this research is technique of Structural Equation Model (SEM) of software package of AMOS. Result of examination proven hypothesis that factor entrepreneurial orientation and knowledge of market become positive effect of marketing capability and marketing performance. It is mean high significance of each other variables. As for model which is raised in this research can be accepted posed at by index according to where value of GFI 0,900, value of chi-square 87,987, probability value 0,084, value of TLI 0,965 and value of CFI 0,973, all have up to standard although value of AGFI 0,853 accepted by marginal.

Abstract

Osteoarthritis is a common progressive joint disease,involving not only the joint lining but also cartilage,ligaments, and bone. Osteoarthritis (OA) of thetemporomandibular joint is a unilateral,degenerative disease of the jaw joint. It ischaracterized by breakdown of the articularcartilage, architectural changes in bone, anddegeneration of the synovial tissues causing painand/or dysfunction in functional movements of thejaw. Temporomandibular joint osteoarthritis (TMJOA) is an important etiology of temporomandibularpain. The temporomandibular joint (TMJ) undergoesdegenerative changes among patients who sufferfrom arthritis, and yet the pathogenesis of TMJosteoarthritis and rheumatoid arthritis is poorlyunderstood. However, until recent years, OA of thetemporomandibular joint was confusing, relativelyunrecognized, and difficult to diagnose and manageand the specific mechanisms of TMJ OA in thedevelopment of temporomandibular pain are stillnot clear. Animal models are useful tools for theinvestigation of association between animalbehavioral and TMJ OA. Several animal models ofTMJ OA have been established using intraarticularinjection of various chemicals. This study aimed toestablish a rat model of TMJ OA using completeFreund’s adjuvant (CFA). Histopathological changewith hematoxylin-eosin staining and radiographicchange of the TMJ was performed. Treatment withCFA induced cartilage damage, synovitis, andproliferation of synovial cells in the fact joints. A ratmodel of TMJ OA was successfully established usingintraarticular injection of CFA. This animal model isconvenient and shows good resemblance to humanOA pathology. We hypothesized that inflammationin the TMJ induces structural abnormalities, andaccordingly characterized the disc and synovium in anovel model with double injections of completeFreund’s adjuvant (CFA), using histopathologicalchange with hematoxylin-eosin staining andradiographic examinition.

Abstract

Asthma is one of the most common chronic diseasesin childhood, with increasing prevalence in the past3 decades (Nievas et al, 2013). The prevalence ofasthma in the world is approximately 7.2% (6%adults and 10% children).There are variations in the prevalence and mortalityof asthma. WHO estimates that there are currently250,000 deaths due to asthma. It is leading cause ofchildhood hospitalization and school absenteeism.Asthma is more prevalent in boys in the first years oflife, but in adolescents it predominates amongfemale subjects. Asthma affects minority and lowincomegroups disproportionately (Herzog et al,2011; Rahajoe, 2013).In Indonesia, Asthma is the top ten causes ofmorbidity and mortality. The study data ofhousehold health survey in 1986 in various provincesin Indonesia showed asthma ranks 5th of 10thcauses of morbidity. In 1992 Household HealthSurvey, asthma, chronic bronchitis and emphysemaas 4th cause of death in Indonesia (5.6%). In 1995,the prevalence of asthma in Indonesia approximately13/1000, compared with chronic bronchitis is11/1000 and pulmonary obstruction is 2/1000(Guidelines for Diagnosis and Management ofAsthma In Indonesia, 2013)Asthma is a heterogeneous disorder in children thatis characterized by recurrent airway obstruction,bronchial hyper-responsiveness, and airwayinflammation. (Herzo et al, 2011).The pathophysiologic basis of asthma is not wellunderstood. It appears to have a complex,multifactorial etiology which results from aninterplay of many hereditary factors and a numberof environmental factors. Bronchial biopsies frompatients with even mild asthma have evidence ofchronic inflammation, and cytokines and othermediators of inflammation are found in bronchialwashings from asthma patients. Some families aremore prone to development of allergies, and there isa well-known association between allergies andasthma. The various factors which can triggerasthma attacks, such as exercise, allergens,infections, sudden changes in air temperature, orexposure to respiratory irritants such as cigarettesmoke and others. (Rahajoe, 2013)Aeroallergen sensitization is a risk factor in thedevelopment of childhood asthma. Aeroallergensensitization occurs in most patients with asthmaand is noted in a high percentage of patients withmild and moderate asthma. The percentage of thosethat are atopic with severe asthma appears less, butstill approximates the percent seen in patients withmild and moderate asthma. The most commonlyimplicated allergens are house dust mite (HDM),cockroach, and furred animals. Aeroallergensensitization can be evaluated using skin testing. (Rajet al, 2013)House dust mites are arachnids that are microscopicin size (~0.33mm long). They are found in dust andproducts with woven material or stuffing such asmattresses, pillows, stuffed animals, and bedding.Their life cycle from egg to adult takes 3 to 4 weeksand they live for 6 to 8 weeks. Females produce 40to 80 eggs during this time.It has been shown that dust mite exposure in earlychildhood is an important determinant in asthmadevelopment. Sporik et al. showed that 16 of 17children with asthma were sensitized to dust mite.urther, the higher the level of dust mite exposure at1 year old, the earlier the first episode of wheezingoccurred. The relative risk of asthma was almost 5-times greater in the subjects who were exposed tohigh levels of dust mite allergen (>10 μg/g) (Baxi etal, 2010)They demonstrated that early exposure to housedust mite was associated with an increased risk ofasthma and late onset wheezing. They followed agroup of 440 children from birth to 7 years andfound that children exposed to high levels of dustmite allergen in their bed at 2 to 3 months old had a3-fold increase in the odds of asthma at age 7 yearsold compared with those exposed to low level dustmite allergen (Baxi et al, 2010)Skin prick testing (SPT) is an easy, cost-effective andconvenient approach to identify sensitization toallergens. SPT detects the presence of allergenspecific IgE bound to mast cells by eliciting mast celldegranulation to the specific allergen being tested(Raj et al, 2013).The purpose of this study was to determine theassociation of aeroallergen sensitization and theseverity of asthma in pediatric patients.

Abstract

Research objective is to produce a model of e-Marketing strategies of MSMEs in Indonesia. The development of the Internet improvethe way of doing business from traditional to electronic, no denying that the economic activity leading to the marketing of electronic. MSMEs are the spearhead of Indonesian entrepreneurs need some improvement, especially in strategic planning, marketing and e-marketing. The research methodology used is: SWOT analysis, marketing survey, e-marketing survey and e-marketing strategy planning. Results of the research is to produce a design strategy e-marketing MSMEs in Indonesia, and the sustainability of this research is to build a portal e-marketing MSMEs with methods Software Development Life Cycle (SDLC) and Object Analysis Application Development (OOAD) and further to facilitate MSMEs in increasing sales and eventually can strengthen the macro economy in Indonesia as a whole.

Abstract

This information system began to be required and used in various sectors. As in the banking sector, when the bloom once the use of Internet Banking as a tool or medium to conduct banking transactions. The information system has become one of the primary needs that is used in part of the banking system at this time. A study related to the management information systems to comply with the Regulation of Bank Indonesia (RBI). Sources of data in this study using internal data obtained using a questionnaire. Results showed that RBI concerning the use of Internet banking services are still not fully understood and the demand by the people who have over 35 years of age because of the possibility they only know to deposit, transfer and withdraw cash through the Bank only. Knowledge of information systems in the use of E-Banking service can not be separated from his individual interest in terms of benefits, ease of use and customer confidence. The knowledge possessed by the customer and interest arising from the individual can enhance a good reputation for the Bank.

Abstract

Leukocyte is the main responsible cell in theimmunity sistem which move freely in thecirculation. Macrophag, neutrophil, dendrytic cell,mast cell, eosinophil, basophil and natural killer (NKcell) are important part of adaptive immunity.Macrophag is monocyte that leaves the circulationand move into the tissue to identify and directly killthe cause of the infection. Its killing mechanism isthrough the phagocytosis which responded to thepathogen bacteria, so the monocyte viabilty isimportant factor of the immune response.1The interest of alternative medicine use in healththerapy has been increased in the past few years.2Many researches has been conducted to discoverthe phytochemical of many herbs around the world,one of them is Jatropha curcas. Its leaves and branchcontain flavonoid, apigenin, vitexin, dan isovitexin. Italso contain dimer of triterpene alcohol (C63H117O9)and two glycoside flavonoid.3 In Indonesia, theJatropha leaves has been used to cure the cough, asantiseptic, antipyretic, bloating relief,antihelminthics, gingivitis, stimulate hair-growth,anti-dandruff, antimalarial, and destruct the kidneystone.4 Flavonoid has been known for its antimicrobaand anti-viral activity. Triterpenoid also hasthe same activity and has been frequently found inherbs. It also helps the skin repair.5 The extract ofJatropha leaves contain the flavonoid as theantiinflammatory agent.In the previous study6, the treatment with Jatrophaleaves extract decreased the peripheral lymphocytesof Escherichia coli (E.coli)-induced male mice.Treatment with 5% concentration of Jatropha leavesextract shows the better effect compared to 10%when given on 1st until 7th day. Maghfirotin 7 statedthe 10% concentration of Jatropha leaves extractalso suppress the elevation of peripheral neutrophilin E.coli-induced male mice.E.coli produce the well-know endotoxin,lypopolisacharide (LPS). Endotoxin has variespotention depend on the microorganisms, but itseffect is quite the same.8 LPS stimulates the immunecell through in vitro or in vivo environment, and hasimportant clinical relevation because it is directlyinvolved in Gram-negative bacteria mediatedinfection.9Based on the introduction, the Jatropha leavesextract mechanism towards the COX-2 expression,the enzyme which responsible in inflammationphase, yet still unknown. This study is to determinethe antiinflammatory activity of Jatropha leavesextract toward the COX-2 expression by using E. Colito induce the inflammation.MATERIALS AND METHODThis in-vitro study use the posttest only with controlgroup design. The independent variable is Jatrophaleaves extract in 5%, 10% and 15 % concentration.The dependent variable is COX-2 expression onmonocyte. The control variabel are monocytes typeand concentration, LPS from E.coli and researchprocedures.COX-2 expression was analyzed byimmunohistochemical method. Monocyte isolationwas incubated in Jatropha leaves extract and then itwas exposed to LPS from E.coli. The cell was washedand then the immunostaining was performed usethe antihuman COX-2 monoclonal antibody (Mab).The monocytes which expressed the COX-2 wasobserved and counted.

Abstract

Arcangelisia flava is a potential candidate to bedeveloped as cancer chemoprevention agent. A.flava was proven to exhibit antioxidant and cytotoxicactivity against MCF-7 breast cancer cell line. Theseability were suggested to be related to its alkaloidcontent: berberine, palmatine, and jatrorrhizine(Keawpradub et al., 2005). Although this plant isstated as a rarely found species (Koran Jakarta,2012), we could find it abudantly in Meru BetiriNational Park, Jember.Our previous studies showed that A. flava leavesincrease the immune system in doxorubicin-treatedrats (Puspitasari et al., 2014b) with no signs oftoxicity based on sub chronic toxicity assay(Puspitasari et al., 2014a). The A. flava leavesethanolic extract (EEAfL) had been proven to havecytotoxic activity on HeLa, MCF-7, and WiDr cancercell lines with the IC50 value of 467 + 70; 136 + 17;and 213 + 79 μg/ml, respectively. The activity wasselective on MCF-7 and WiDr, but not likely on HeLacell line (Puspitasari et al., 2015).OBJECTIVESThis study was conducted to determine whether thecytotoxic activity of EEAfL was occur via apoptotic ornecrotic pathway using flowcytometry annexin VFITCmethod. The concentration used for the assaywere concentration approx. the IC50 and the IC75based on previous study (Puspitasari et al., 2015).
