Abstract

The new paradigm evolving today in thetreatment of periodontal infection is the use a hostmodulator therapy i.e. therapy that aims to improvethe host immune response to bacterial infection.Good immune response is necessary in increasingthe phagocytic function that in this case is played byneutrophil cells, monocytes and macrophages, whichserved as the first line of defense against bacterialinfections. One of the bacteria causing periodontalinfection is P. gingivalis. It has the biggest role incausing periodontal disease, where in the proteolyticenzyme such as gingipain contributes to thebacterial pathogenicity.1, 2 During the initial invasionP. gingivalis will colonize the gingival sulcus andinvade into the gingival tissues and will induce aninflammatory response and continue to alveolarbone resorption.3The immune response that acts first againstinvading bacteria is neutrophils. The presence ofbacterial adhesion to neutrophils allows neutrophilssecrete microbicidal substance by destroying thebacterial membrane and fagocyting bacteria. Aneutrophil can phagocyte bacteria before becominginactive or cell lysis. Phagocytosis that occurs canaffect the lysis of neutrophils. This can emerge veryfatal impacts because if the neutrophils undergo lysisthen chemical components and enzymes may breakinto the tissue leading to degradation of variousorganic molecules of its adjacent tissue.4 NowdaysIndonesia has developed a lot of research onmedicinal plants, such as Robusta coffee beans.Robusta coffee beans naturally have substances suchas caffeine, phenolic compounds, trigonellin andchlorogenic acid which have antibacterial activity.Besides as antibacterial chlorogenic it also hasantioxidant properties.5 Based on the abovebackground, the researchers want to developRobusta coffee beans as host modulator therapy tomaintain the viability of neutrophils by observingneutrophil cell viability against P. gingivalis appliedby Robusta coffee bean extract which will be usefulfor preventing periodontal tissue infections of theoral cavity.

Abstract

Bone is one part of the body has an important roleto support the body's physiological functions (Porteret al., 2009). Complications of bone diseases andbone disorders caused by traumatic accidents mayresult in a gap (defect) on the bone. The healingprocess of damage or fracture is determined by thelevel of trauma and soft tissue damage (Strobel etal., 2011). Some cases of damage or injury to thebone can not undergo natural recovery (Porter et al.,2009). Therefore, clinical rehabilitation to overcomethe defect on the bone is expected to increase in linewith population growth (Mourino et al., 2010).Treatment rehabilitation of bone cannot beseparated from the risk of infection complications.Complications of bacterial infections can be treatedwith antibiotics. However, in the case of a crack(defect) occurs devascularity of bone tissue so thatthe delivery of antibiotics to the target tissue to beblocked. This resulted in the concentration of theantibiotic to the target so low that it cannotpenetrate the bacteria. The condition can lead tobacterial resistance to antibiotics (Li et al., 2010). Ahigh dose of antibiotics in the long term experiencedproblems because it can cause systemic toxicity andside effects (Mourino et al., 2010). To overcomethese problems, antibiotics can be done locally usinga certain drug delivery systems. The purpose of suchdelivery systems is to provide drug concentration ina specific location and ensure the drug releaseprofile for a certain time period (Dubnika et al.,2012). Drug delivery locally has several advantages, among others, (a) the systemic effects can beavoided, (b) the amount of drugs used less andsecure, and (c) the efficacy and efficiency of drugdelivery locally can be achieved (Harmankaya et al.,2013). Administration of antibiotics locally also tominimize side effects and risk of toxicity comparedto administration of systemic antibiotics. In addition, antibiotics locally also allows conduction in targettissues with high concentration (Mourino et al.,2010). The release of antibiotics on the targetnetwork is expected to last continuously for a certaintime and achieve a greater concentration than theminimum inhibitory concentration (MIC). Drugdelivery systems in a controlled manner (controlledrelease system) can help increase the bioavailabilityof antibiotics in target tissues. The system isdesigned to release the drug at the expectedlocation at a rate appropriate for a certain timeperiod (Mourino et al., 2010). In a previous studyshowed that a good composite is Ciprofloxacin: BHA:Chitosan = 10:30:60. Cross linker withglutaraldehyde (GA) 0.7% and with 10% activeingredient Ciprofloxacin can release Cyprofloxacinfor 30 days (Hendradi et al, 2015). This research willbe seen potency against Staphilococcus aureusATCC25923 Ciprofloxacin for 30 days.

Abstract

Maternal Mortality Rate (MMR) served asa basis or benchmark for the country's welfare.Based on estimation made from the Demographicand Health Survey Indonesia (IDHS) 1990 to 2007,it showed that the maternal mortality ratio (MMR)in Indonesia in 2015 reached 161 / 100,000 livebirths, while the MDG targets Indonesia is 102 /100,000 live births (Health Dept, 2013). The datafrom WHO Maternal Mortality Rate (MMR) inIndonesia in 2013 shows that as many as 8,800 bythe Maternal Mortality Ratio (MMR) of 190 (120-300) per 100,000 live births (WHO, 2013). Thematernal mortality rate in Central Java Province in2012 based on reports from regency / city was116.34 / 100,000 live births, an increase comparedwith MMR in 2011 was 116.01 / 100,000 livebirths. Most maternal deaths are in Brebes with 51deaths. Salatiga is the Regency with the leastnumber of maternal deaths with only 2 cases.According to the Central Java Provincial HealthOffice, there were 32 maternal deaths (CentralJava Health Office, 2012). Additionally, it wasnoted that there were maternal mortality by 711cases and in 2015 there were 115 cases in 2014(Nurdin, 2015).The reason of the high death toll inIndonesia is due to the low quality of life, theaverage low education, health status and poornutrition, anemia, iron deficiency, and theincidence of chronic malnutrition (Amaludin,2015). Pregnant women are in need of adequatenutrition even doubled because nutrition is neededby pregnant women to meet the needs not only forthe fetus but also the mother. However, sometimes pregnant women experience nauseaand excessive vomiting so that nutrition cannot befulfilled. Rose & Neil (2006) states that emesisgravidarium in pregnant women can cause avariety of negative effects, one of which is adecrease in appetite that result in changes in theelectrolyte balance of potassium, calcium, andsodium, causing changes in the body's metabolism. It affects the fetus, it will be lack of nutrients andfluids needed by the body, the baby will be bornwill be in low birth weight and impaired growthprocess. Excessive nausea and vomiting can alsoreduce body fluid, so that the blood becomes thick(hemoconcentration) and blood circulation totissues is late. If that happens, then theconsumption of oxygen and nourishment to thetissues also reduced (Anggarani and Subekti, 2013,Jeffrey et al, 2003). Lack of oxygen and food to thenetwork will cause tissue damage that couldreduce maternal health and fetal development inthe womb. In such cases, it needs serioustreatment (Hidayati, 2009).The purposes of this research are:1. To find out the characteristics of pregnantwomen consisting of age, education level andoccupation.2. To find out the status of social support forpregnant women in the first trimester inPuskesmas Kembaran I Banyumas.3. To find out the incidence of emesis gravidariumin the first trimester pregnant women inPuskesmas Kembaran I Banyumas.4. To find out the correlation between socialsupport and emesis gravidarium in the firsttrimester of pregnant women in PuskesmasKembaran I Banyumas.

Abstract

Poverty is a complex phenomenon, multidimensional and can not be easily viewed from an only absolute numbers. Wide area and causing a big variety of cultural conditions and problems of poverty in the province of East Java become very diverse. The Research was taking a  deliberate location (purposive) in Pasuruan, because it was included in the typology of the coastal region, so the research was expected to determine the condition of poverty in coastal areas. The research used sampling methods FGD at regional government level, and followed by all agencies and FGDs at community level (community consultation), and FGD was held on the stage Rejo village and the village of Tuban district Sekargading Bugul South District. Constraints experienced by the Government of WordPress, was because prevention programs are implemented by local governments still can not be enjoyed by many or the majority of society, poverty reduction programs in Pasuruan still overlap. This was not optimal because of TKPKD role in implementing the synchronization and coordination of programs poverty reduction.

Abstract

Jati Belanda Leaves (G. ulmifolia Lamk.) has beenused traditionally to reduce obesity 1). G. ulmifoliaLamk. leaves extract contained alkaloids, tannins, saponins, flavonoids, terpenoids, glycosides, andsteroids 2). Phytochemical screening resultsconducted by Iswantini et al. showed that theflavonoids and tannins were detected high levels inthis extract 3). Flavonoids contained in the leaves ofG. ulmifolia Lamk. were catechin, kaempferolglycosides, procyanidin B-2, procyanidin B-5,procyanidin C-14).Obesity can occur when the body of excess adiposetissue (fat), particularly white adipose. Excess whiteadipose tissue (WAT) is considered as the maincause of obesity 5). The growth of adipose tissueoccurs due to the formation of new adipocytes andincrease in size of adipocytes, called adipogenesis.Adipogenesis involves the multiplication of cells(proliferation) and cell maturation (differentiation). The treatment which can affect the size and numberof adipocytes and signals expression are involved inthe inhibition or stimulation of specific adipokineshave been used to prove the bioactivity associatedwith antiobesity 6).Natural products that have a specific target in thispathway have the potential to prevent and treatobesity. Some polyphenols, including flavonoidshave anti-obesity effects and also specific effects onadipocytes 7). The presence of the flavonoids i.e.catechin, kaempferol, tilirosida, and prosianidin in G.ulmifolia Lamk leaves. may affect adipogenesis. This research will be conducted extraction andfractionation leaves of G. ulmifolia Lamk. Extracts,non-polar fractions and semi-polar fractions testedtheir effect on the proliferation and differentiationof Wistar rat preadiposit.

Abstract

Infant mortality rate is one indicators used toascertaining of community health degree. Infantmortality closely related with family education level, social economy of family, value system, customs, cleanliness, health environmental, health servicesavailable, childbirth problem, provision ofimmunization and malnutrition. Incident ofmalnutrition in infants caused by bestowalbreastfeeding is wrong and provision ofsupplementary food improper. So provisionbreastfeeding patterns and provision ofsupplementary food proper must be attention1.Breastfeeding is best food for babies in early ages hislife, good physiologically and biological.Breastfeeding containing nutrients, substanceimunogenik to protect a baby from infection2.Breastfeeding rich in carotenoids and selenium, sothat breastfeeding had a role in defense system thebody of a baby to prevent various diseases. Everydrop breastfeeding also contain minerals andenzymes to prevention of disease and antibodiesmore effective than and formula milk3. Manyresearch show the benefit of exclusivebreastfeeding4. World Health Organization (WHO)recommend to give exclusive breastfeeding for thefirst six months to achieve the growth optimal, development and health. After that, babies have toprovided with food a companion nutritious andremain suckling until two years5. Exclusivebreastfeeding is breastfeeding given to a baby eversince born during six months, without adding and orreplace with food or drink other6.Based on world health organization in 2010, 1.5million children below five 0-6 months died of theprovision of supplementary food before the time, and in the provision of breastfeeding around theworld to 15 % of infants given breastfeeding, theprovision of breastfeeding lowest in central Africaand North Africa, Asia and America Latin. Based onHealth research for basic (Riskesdas) 2013 show forgranting breastfeeding in Indonesia has been good ifcompared with the results of Riskesdas 2010 thatonly covers 15.3 %7. Based on health departmentdata at 2010 in east java obtained the scope ofprovision of exclusive breastfeeding for 6 months atprovincy as many as 31,21 %8. Exclusivebreastfeeding program for six months, hasundertaken in Jember regency since the issuance ofIndonesian Kepmenkes no.450/Menkes/IV/2004 inan effort to improve a baby or child toddlersnutritions. But, Jember regency still not reach thegoal of program that has been set is as much as 80%9. In 2010, the scope of provision of exclusivebreastfeeding in Jember regency as much as 67,10%8. Therefore, needed a special act to increase it, sothe targets can be achieved. Many factor thatinfluences mother in decide on and do provisionpatterns breastfeeding, especially physical andpsychological mother not ready, lack of informationand knowledge on benefits breastfeeding, management of lactation10. Mothers that giveexclusive breastfeeding said the real questionrelated knowledge of breastfeeding as many as40,7% and mother who do not give exclusivebreastfeeding said the real question relatedknowledge of breastfeeding as many as 29.6 %. Thisshows there was a significant correlation betweenlevel of knowledge mother about breastfeeding withprovision of exclusive breastfeeding11.
