Abstract

Background: Coumarin is a natural compound found in various plants, including Cinnamomum cassia and Cinnamomum zeylanicum, and has been widely studied for its potential role in metabolic regulation, particularly in diabetes mellitus type 2. While coumarin has shown potential benefits in improving glycemic control, concerns regarding its hepatotoxic and carcinogenic effects remain. Purpose: To review the literature on how coumarin works in insulin resistance in people with type 2 diabetes. Method: A literature review was conducted using the ClinicalKey, Science Direct, and Google Scholar databases. Boolean operators (AND, OR, NOT) were used to refine the search. Inclusion criteria included research articles published between 2015 and 2024 in accredited national or internationally reputable journals, with a JBI critical appraisal tools score above 65%. The PRISMA diagram was used for article selection, resulting in five eligible articles for analysis. Results: The findings indicate that coumarin, predominantly found in cinnamon, has glucose-lowering effects in diabetes mellitus type 2 patients by reducing blood glucose levels, controlling glucose spikes, and lowering HbA1c. Additionally, coumarin derivatives in Opuntia ficus-indica and Smallanthus sonchifolius (yacon) may contribute to metabolic homeostasis through antioxidant activity. However, coumarin also poses hepatotoxic and carcinogenic risks at high doses. Conclusion: While coumarin-containing compounds show promise in improving glycemic control. Further research is needed to determine safe and effective therapeutic doses for clinical application.

Abstract

Hypertension is a significant global health concern, contributing to high mortality rates and increased risks of cardiovascular disease. In Indonesia, hypertension prevalence has reached over 33% of the adult population. Non-pharmacological interventions, such as nature sound therapy and isometric handgrip exercises, have shown promise in managing hypertension by reducing systolic blood pressure. This study introduced Tool Nature Sound and Isometric Handgrip (Toonatric), an innovative tool combining nature sound therapy and isometric handgrip exercises. This research employed a quasi-experimental design with 66 elderly hypertensive patients divided into intervention and control groups. The intervention group used Toonatric for 30 minutes daily over 5 days, while the control group continued standard care. Pre and post-intervention blood pressure measurements were analyzed using Wilcoxon and Mann-Whitney tests due to non-normal data distribution. Results indicated a significant reduction in systolic blood pressure in the intervention group (p = 0.000), with an effect size of Cohen’s d = 0.48, demonstrating a moderate clinical effect. Toonatric has the potential as a complementary tool for managing hypertension in elderly, combining psychological and physical benefits. Further research should focus on long-term effects, broader populations, and usability improvements for clinical and home settings.

Abstract

ABSTRACT Childhood hypertension is a global health issue that can persist into adulthood and increase the risk of cardiovascular diseases. This study aimed to review effective non-pharmacological interventions for lowering blood pressure in children. The research method used a systematic literature review with a PRISMA approach. Articles were searched through PubMed, Science Direct, and Cochrane Library databases using the keywords “hypertension,” “children,” and “intervention.” Six selected articles published between 2021 and 2025 that met the inclusion criteria were analyzed, consisting of randomized control trials and cross-sectional studies with sample sizes ranging from 39 to 5,645 children. The results showed that dietary interventions such as the DASH diet, consumption of nuts and seaweed, and sodium restriction effectively reduced blood pressure. Moderate-to-vigorous physical activity and motor-psychological interventions were also proven to lower blood pressure and improve vascular function. Breastfeeding also helps prevent increased blood pressure, as it contains bioactive compounds and has low sodium levels. In conclusion, non-pharmacological interventions play an important role in the prevention and management of hypertension in children. The implication for nursing practice is the need for family education on healthy diets, increased physical activity, and support for exclusive breastfeeding as promotive and preventive strategies.

Abstract

Type 2 diabetes mellitus is a major public health problem with a continuously increasing prevalence, requiring accurate and continuous blood glucose monitoring. Venous blood glucose measurement using enzymatic methods remains the clinical gold standard; however, it is invasive and less practical for routine monitoring. Recent advances in sensor technology provide opportunities to develop more practical and user-friendly glucose monitoring devices. This preliminary pilot study aimed to develop and validate a needle-free sensor-based device for blood glucose monitoring and to evaluate the agreement of its measurement results with venous blood glucose examination using enzymatic methods as the reference standard. This study employed a research and development approach. The initial stage involved designing and developing a prototype sensor that monitors blood glucose without a needle. Device validation was conducted by experts to assess technical performance, safety, and usability. Subsequently, a pilot measurement of random blood glucose levels was performed among 25 patients with type 2 diabetes mellitus in a primary healthcare setting, and the measurement results were compared with venous blood glucose values obtained using enzymatic methods. The validation results indicated that the device was technically feasible, safe to use, and easy to operate. Pilot testing demonstrated that the sensor-based device produced blood glucose measurements comparable to those from the reference method (Wilcoxon Signed Ranks Test: Z = 0.000, p = 1.000), providing preliminary evidence of statistical similarity. However, further refinement is required, particularly in the interpretation and display of measurement results. These findings represent initial proof-of-concept for the device as an alternative tool for blood glucose monitoring, especially in primary healthcare settings and for patient self-monitoring. Comprehensive studies with larger, more diverse samples and advanced clinical agreement analyses (Bland-Altman plots, and MARD) are needed to confirm clinical accuracy and reliability.
