Abstract

Background: Onychomycosis may cause nail discoloration, thickening, nail bed separation, and other serious complications. For some cases, oral antifungal treatment is not tolerable because of its potential side effects and drug interactions. Laser therapy is considered as an alternative treatment, owing to the features of simple and effective, with only minor potential side effects. This EBCR was made to collect and appraise studies regarding the effectiveness of laser therapy for onychomycosis, and to suggest laser as an alternative treatment. Methods: Literature searching strategy was performed using Pubmed and Cochrane Library database to address the clinical problem. Keywords used were “laser” AND “onychomycosis”. Results: Seventy-nine articles were obtained from the search strategy procedure. After selection based on exclusion and inclusion criteria, and full-text availability, four relevant articles remained. Discussion: The study by Xu et al. was considered as the most valid study while compared to other three studies. This study used intention to treat analysis and had no loss of follow-up patients. Xu et al. compared mycological and clinical clearance rate between patients receiving laser, oral terbinafine, or combination of those two. It showed that laser therapy was less effective when compared to oral antifungal (Number Needed to Harm = 17). Conclusions: Laser has a lower level of effectiveness while compared to oral terbinafine as the current gold standard therapy for onychomycosis. However, laser therapy can still be used as an adjunctive therapy along with oral antifungal to achieve a better cure rate. More studies are needed to prove this hypothesis.

Abstract

Background: Generalized pustular psoriasis (GPP) is characterized clinically by yellowish sterile pustules surrounded by erythema. GPP in children is very rare and may appear following steroid withdrawal in psoriasis vulgaris.Case Illustration: A 14-year-old female presented to the emergency unit with generalized erythematous plaques and multiple pustules with lake of pus after abruptly stopped oral and topical corticosteroid for two weeks. The patient had used topical and oral corticosteroids for almost a year to treat psoriasis. Physical examination revealed moderately ill and tachycardic, with short stature, moon face, amenorrhea, and multiple erythematous plaques with multiple pustules and lake of pus along with red-purple striae all over the body.Discussion: Adverse effects of steroid-induced adrenal insufficiency are time-dependent and generally reversible. Gradual discontinuation of steroids must adhere to physiological dose of steroids, mostly in 3.49 ± 2.92 months. Other treatments available are emollients, salicylic acid, calcipotriol, coal tar, topical steroids, phototherapy, retinoids, and cyclosporine. Topical steroids cannot be the main therapy in severe psoriasis due to the possible side effects of excessive usage. Conclusion: Misuse of topical and systemic steroid in children could lead to Cushing’s syndrome, a multisystem disorder resulting from prolonged exposure to excess glucocorticoids, either both systemic or topical. Children could easily develop systemic reactions to topical steroids due to their higher total body surface area to body weight ratio.

Abstract

Abstract

Toddlers are one of the risk groups for experiencing a lack of nutritional intake, which affects their nutritional status. According to the results of SSG1 2022, it is stated that the prevalence of underweight or malnourished children under five (weight according to age) in Jambi Province is 13.8% and in Muaro Jambi Regency is 16.2%. In Pematang Jering Village itself, there are still health problems among children, including malnutrition, stunting and wasting. Malnutrition is a condition where the body's nutritional needs are not met within a certain period of time so that the body breaks down food reserves under the fat layer and layers of the body's organs. Maternal parenting patterns show that mothers with poor parenting patterns have a greater chance of having children who experience malnutrition problems than mothers with good parenting patterns. Good parenting consists of providing IMD, giving breast milk and MP-ASI as well as practicing hygiene or environmental sanitation. Pematang Jering Village has a Polindes (Village Maternity Center). This Polindes is used once a month for the implementation of posyandu. The results of data measuring height according to age (TB/U) carried out by the Posyandu in Pematang Jering Village, out of a total of 105 children in Pematang Jering Village, found that 25 children had undernutrition problems that did not meet the ideal standards according to age. The methods used were Focus Group Discussion (FGD), Introduction of Alternative Innovations for Toddler MPASI, Education on Tilapia Floss Production, Formation of Cadres and Structures, Monitoring of Shredded Production, Monitoring and Evaluation of the pre-test and post-test results, namely 16.66% to 83, 33% of mothers of toddlers have an understanding regarding providing MP-ASI food and producing shredded Nilla fish properly and correctly.

Abstract

Renewable Energy needs to be implemented in schools to build students' knowledge. This research aims to design a prototype of a Photovoltaic Power Plant (PLTS), which can be used as a medium for experimental high school physics activities. Besides that, the effect of light intensity on the power produced by the PLTS prototype was experimentally tested by observing temperature, humidity, and air pressure. The PLTS prototype is designed to be simple, with component selection (10 wp solar panels, SCC, batteries, lights and panel boxes) that are adapted to the learning media criteria, namely practical, durable, easy to use and able to demonstrate the concepts being taught. The PLTS prototype was tested when charging and discharging the battery. When charging the battery, the solar panel is tilted with a variety of tilt angles of 0º, 15º, 30º, 45º and 60º. When discharging the battery, lamps with varying powers are used, namely 3W, 5W, 7W and 9W. The research results show that the panel tilt angle is used to produce maximum voltage, current, and power when charging the battery, which is 30°. When the battery is discharged, a smaller lamp power can light the lamp for longer compared to a larger lamp power. In conclusion, the PLTS prototype can provide students with experience in conducting experiments and learning the application of physics concepts directly so that it can build students' knowledge and provide meaningful learning.
