Abstract

 The high prevalence of non-communicable diseases (NCDs) in Tarogong kaler Sub-district, Garut Regency has several impacts on the community. The right community behavior to overcome this degenerative disease is to do prevention. This community service aims to implement the SILACAK program (free health check service system) in order to control non-communicable diseases. Through the SILACAK program, the community can access free health check-up services that include checking blood pressure, blood sugar levels, and health education. In addition, the community received socialization related to the demonstration of planting family medicine (TOGA) to prevent NCDs. The examination results show that most residents have a high risk of NCDs, especially high blood pressure. The results of this service are expected to increase public awareness about the importance of prevention and early detection of non-communicable diseases, as well as a decrease in the incidence of these diseases.  Through this effort, it is expected that the quality of life of the community can be significantly improved. Abstract

Introduction: Type 2 diabetes mellitus is the most common type. In patients with type 2 diabetes mellitus the pancreas can still make insulin, but the quality of insulin is poor and cannot function properly so that blood glucose increases. The presence of protein in the urine is caused by leakage of plasma proteins from the glomerulus. Clinically detected proteinuria is abnormal and is usually an early marker of kidney disease. The purpose of this study was to determine the results of urine protein examination in patients with Type 2 Diabetes Mellitus at Cikajang Health Center. The purpose of this study was to determine the results of the description of urine protein levels in patients with type 2 diabetes mellitus at the Cikajang Health Center Method: the research used was descriptive research. The population in this study was 7,338 people. The sample size in this study was 30 respondents. The sampling technique used was purposive sampling technique. The variable in this study was urine protein levels in patients with type 2 DM examined using the dipstick method. Results: The results of this study obtained negative results (-) 19 people (64%), trace (±) 8 people (26%), positive 1 (+) 2 people (7%), positive 2 (++) 1 person (3%). The results of the study can be concluded that in this study there were more negative urine protein results compared to positive urine protein results. Conclusion. It can be concluded that in this study more results were negative urine protein compared to positive urine protein.

Abstract

Background: Anemia in pregnant women is not only a health concern for the mothers themselves but also has an impact on the health of the fetus. The role of health cadres is crucial in addressing anemia in pregnant women.Purpose: To evaluate the influence of community support interventions on the knowledge and self-efficacy of health cadres in managing anemia in pregnant women in the Thousand Islands region, Indonesia.Method: The research design is quasi-experimental with a single intervention group. A total of 30 health cadres on Kelapa Island and Harapan Island received community support interventions for managing anemia in pregnant women. Assessments of knowledge and self-efficacy were conducted before and after the intervention. Data analysis was performed using paired sample t-tests.Results: Indicate that community support interventions significantly improved the knowledge of health cadres about anemia in pregnant women (p < 0.05). Furthermore, the self-efficacy of health cadres also experienced a significant increase after the intervention (p < 0.05).Conclusion: Community support interventions effectively enhance health cadres' knowledge and self-efficacy in managing anemia in pregnant women in the Thousand Islands region. This study has implications for improving the quality of health services and the management of anemia in pregnant women.

Abstract

Stroke is a clinical syndrome that initially arises suddenly, progressively rapidly, in the form of a global focal and neurological deficit that lasts 24 hours or more and or can directly cause death, and is solely caused by circulatory disorders to the non-traumatic brain. The main problem that arises in stroke sufferers is the destruction or death of tissue. One of the efforts to recover and assist the patient in movement is the need for action in nursing, namely exercises by grasping a rubber ball. The purpose of this nursing care plan is to analyze the management case in providing nursing care intervention for Mr.A with stroke with the application of a rubber ball hand-held intervention to increase muscle strength and weakness in the Wilayah Kerja Puskesmas Tarogong. The intervention of Nursing Care in stroke patients with the administration of rubber ball hand-held exercises. After being given nursing care with the intervention of providing rubber ball hand-holding exerc­ises for 3 days, the patient muscle strength was raised. The initial muscle strength before the administration of the intervention was 3, and after being given the exercise of grasping the rubber ball was 4. Nursing care in stroke patients with the provision of rubber ball hand-held exercises can increase muscle strength in patients.

Abstract

Diabetes melitus (DM) adalah penyakit kronis dengan prevalensi yang terus meningkat, terutama pada populasi lanjut usia. Salah satu komplikasi utama DM adalah ulkus diabetikum, yang sulit disembuhkan dan berisiko tinggi terhadap infeksi serta amputasi. Studi kasus ini bertujuan untuk mengevaluasi efektivitas madu sebagai terapi topikal dalam perawatan ulkus kaki diabetikum pada pasien lansia. Seorang pasien laki-laki berusia 67 tahun dengan DM selama 1 tahun dan ulkus kaki diabetikum selama 8 minggu menerima perawatan luka menggunakan aplikasi madu secara topikal. Aplikasi madu dilakukan 1 sampai 3 kali sehari pada luka yang mengalami infeksi.. Observasi dilakukan terhadap perubahan ukuran luka, tanda-tanda infeksi, dan waktu penyembuhan. Hasil menunjukkan bahwa penyembuhan luka berlangsung dalam 8 minggu, dengan kemajuan penyembuhan yang lebih cepat saat luka diaplikasikan madu dibandingkan dengan penggunaan menggunakan antibiotik topikal. Fluktuasi kadar glukosa darah pasien (88–207 mg/dL) tercatat selama masa pengobatan, yang berpotensi memengaruhi waktu penyembuhan luka. Madu sebagai terapi topikal menunjukkan potensi yang menjanjikan dalam mempercepat penyembuhan ulkus diabetikum, dengan manfaat antibakteri yang membantu mencegah infeksi. Meskipun demikian, stabilitas kadar glukosa darah tetap menjadi faktor yang signifikan dalam keberhasilan terapi ini. Oleh karena itu, penggunaan madu sebaiknya dipadukan dengan pengelolaan glikemik yang optimal. Studi lebih lanjut diperlukan untuk mengeksplorasi efektivitas madu pada populasi yang lebih luas dan dalam kondisi yang lebih terkontrol.   Kata Kunci: diabetes melitus, madu, perawatan luka

Abstract

Elevated uric acid levels, also known as hyperuricemia, is a metabolic condition caused by the accumulation of monosodium urate monohydrate crystals in the joints. This condition often leads to painful gout attacks, characterized by excruciating pain that is recurrent and can cause joint damage over time. The presence of uric acid crystals contributes to inflammation and pain by interacting with phospholipid membranes and serum factors. Pain management for hyperuricemia can involve both pharmacological and non-pharmacological treatments, with one such non-pharmacological approach being isometric exercise. This study aims to analyze family nursing care for a patient with elevated uric acid levels above normal and the effect of isometric exercise in reducing joint pain in Cikondang Village, Tanjung Kemuning, within the working area of UPT Puskesmas Tarogong, Garut Regency. A descriptive case study method was used, which included anamnesis, observation, physical examination, and medical record review. The case study showed that chronic pain, the primary nursing problem in this patient with elevated uric acid levels, was effectively managed through isometric exercise. This was evidenced by a reduction in the patient's pain score from moderate pain (4) to mild pain (1). The isometric exercise intervention significantly reduced joint pain in the patient with elevated uric acid levels.

Abstract

The disease known as Gouty Arthritis, or simply Gout, is a condition that affects the joints due to an interference in metabolism, particularly the metabolism of purines, leading to high levels of uric acid in the blood. Elevated levels of uric acid can cause the formation of crystals in the joints and surrounding tissues, resulting in severe pain and potential damage to the joints. In the context of caring for elderly patients with Gouty Arthritis, a study involving warm compress therapy was conducted at the Orchid Griya Elderly in Garut Regency. The study utilized a descriptive case study approach, involving history taking, observation, physical examination, and medical notes. The participants were elderly patients with Gouty Arthritis who received warm compress therapy. The results of the study indicated that the main nursing problem, chronic pain, was effectively addressed through warm compress interventions based on Evidence Based Practice. The analysis revealed a decrease in the intensity of gouty arthritis pain with the use of warm compress therapy, suggesting that this approach can effectively increase blood flow to the affected area and accelerate the healing process.

Abstract

Hypertension or commonly called high blood pressure is an increase in systolic blood pressure above the normal limit, which is more than 140 mmHg and diastolic blood pressure of more than 90 mmHg. Pain is a symptom that arises due to high blood pressure. One of the non-pharmacological therapies that can be used to reduce pain directly and indirectly reduce blood pressure is hydrotherapy. This case study aims to determine the effectiveness of foot soaks using warm water to reduce blood pressure in hypertensive patients. The method used is a descriptive case study by taking anamnesis, observation, physical examination, and medical records. The participant in this study was diagnosed with hypertension sufferers and the blood pressure was measured by a sphygmomanometer. It was found that the client's blood pressure can be overcome by soaking the feet in warm water to lower blood pressure. Provision of hydrotherapy was carried out for 4 days of administration in 1 day 1 time with a duration of 15 minutes effect to lower blood pressure from 170/100 mmHg drops to 140/90 mmHg. Hydrotherapy can be a non-pharmacology intervention from a nurse.

Abstract

Postpartum is the time to start after placenta birth until return of reproductive organs to in normal condition or before pregnancy taking place for 6-8 weeks. Service postpartum given to Mother during period from 6 hours to 42 days, because often give rise to rips in the road birth (episiotomy) which can give rise to happen bleeding and infection in the mother postpartum. The objective was to get an idea of the application of therapy compress cold as intervention in patients postpartum spontaneous with problem inconvenience post-partum. A descriptive case study using anamnesis, observation, physical examination, and medical records. The results showed that the case study of post-partum patients spontaneous with discomfort nursing problems post-partum consequence wound episiotomy obtained subjective data and objective data , author​ get problem nursing main inconvenience post-partum intercourse with perineal trauma during labor and birth , proven with exists wound episiotomy degree II . Discomfort caused by injuries​ episiotomy can overcome with use therapy compress cold, this be marked with exists decline scale painful from 5 to 2 after done intervention in 3 days. Abstract

Hypertension or high blood pressure is a disease of the heart and blood vessels characterized by increased blood pressure. Factors that influence the incidence of hypertension include genetic, racial, regional, socio-cultural and unhealthy lifestyle. This study aims to determine the analysis of nursing care with hypertension through the application of isometric handgrip exercise therapy in the Griya Lansia Garut service unit. The research method used in this study is descriptive qualitative in the form of a case study by conducting nursing care for the elderly which includes assessment, nursing diagnosis, intervention, implementation, and evaluation of nursing carried out on March 3-9, 2025. Based on the results of the assessment, diagnosis, intervention, implementation and evaluation carried out by the researcher, 4 nursing problems were found, namely impaired physical mobility, anxiety, knowledge deficit and risk of falling. Researchers planned mobilization support by implementing Isometric Handgrip Exercise therapy based on Evidence-Based Practice, which aims to lower blood pressure and increase muscle strength. Nursing care was provided for 5 days, including assessment, diagnosis, intervention, implementation, and evaluation. Results showed a decrease in blood pressure from 170/97 mmHg to 154/80 mmHg and an improvement in muscle strength from scale 4 to 5 after the intervention. The study concluded that isometric handgrip exercise therapy is effective in lowering blood pressure and increasing muscle strength in hypertensive elderly patients. Further quantitative research is recommended to examine the long-term effects of this intervention
