Abstract

Pelayanan laboratorium klinik merupakan bagian integral dari pelayanan kesehatan yang diperlukan untuk menegakkan diagnosis, dengan menetapkan penyebab penyakit, menunjang sistem kewaspadaan dini, monitoring pengobatan, pemeliharaan kesehatan, dan pencegahan timbulnya penyakit. Penelitian ini bertujuan untuk menganalisis faktor determinan yang berhubungan dengan mutu pemeriksaan laboratorium di Rumah Sakit Agung Jakarta. Jenis penelitian ini adalah penelitian deskriptif korelasi dengan pendekatan cross sectional. Sampel dalam penelitian ini adalah semua orang yang terlibat dengan pengambilan sampel di Rumah Sakit Agung Jakarta sebanyak 20 orang. Hasil penelitian menunjukkan bahwa sumber daya manusia (p=0,814) dan sistem (p=0,117) tidak berhubungan dengan mutu pemeriksaan laboratorium. Sedangkan teknis (p=0,002) dan sarana prasarana (p=0,002) berhubungan dengan mutu pemeriksaan laboratorium. Hasil analisis multivariat menunjukkan bahwa sumber daya manusia, sistem, teknis, sarana prasarana berpengaruh terhadap mutu pemeriksaan laboratorium (p=0,014). Hendaknya pihak rumsah sakit dapat meningkatkan mutu pemeriksaan laboratorium dengan memperhatikan faktor sumber daya manusia, sistem, teknis dan sarana prasarana. Abstract

Observasi pendahuluan terhadap 20 orang perawat dan bidan Rumah Sakit Bunda Sejati Kota Tangerang memperoleh hasil bahwa perawat dan bidan merasa tidak puas dengan kompensasi, pekerjaan, supervisi, pengembangan karir, rekan kerja dan kebijakan organisasi. Tujuan penelitian untuk mengetahui faktor-faktor yang mempengaruhi minat turnover pada perawat dan bidan di RS Bunda Sejati Kota Tangerang. Metode penelitian cross sectional dengan subjek penelitian 64 orang perawat dan bidan. Lokasi penelitian di RS Bunda Sejati Kota Tangerang, waktu penelitian pada bulan Juli-Agustus 2023. Abstract

ABSTRAK Rumah Sakit X sebagai salah satu lembaga pelayanan kesehatan di Kabupaten Bogor terus berupaya memberikan pelayanan terbaik kepada masyarakat, dimana penyelenggaraan rekam medis yang baik adalah salah satu kuncinya. Masalah yang sering timbul dalam penyelenggaraan rekam medis adalah dalam proses pengisian berkas yang tidak lengkap, terbukti dari didapatkanya angka tertinggi berkas rekam medis yang tidak lengkap adalah pada pengisian resume medis sebesar 27,5%. Penelitian ini merupakan studi kualitatif dengan desain studi cross-sectional dengan pendekatan interpretative. Metode kualitatif dilakukan dengan wawancara mendalam (indepth interview) terhadap Direktur mengenai kebijakan rumah sakit tentang rekam medis, Petugas PPA, Petugas Rekam Medis dan Casemanager. Hasil. Seluruh petugas memahami pentingnya pengisian rekam medis dan sebagian memahami SPO yang terdapat dirumah sakit namun sosialisasi belum dijalankan menyeluruh. Sistem reward dan punishment belum diterapkan di rumah sakit tersebut. SPO dan SPM rekam medis berpengaruh kepada kepatuhan petugas dalam pengisian rekam medis, reward dan punishment berpengaruh tehadap sikap dan tanggung jawab dokter dalam kepatuhan pengisian rekam medis dan ukungan dari rekan kerja dapat memotivasi petugas dalam mengisi rekam medis.Kata Kunci : Kepatuhan; Rekam Medis; Dokter; Petugas PPA.

Abstract

Objective: The study aimed to determine the association between information management system, transformational leadership on working effectiveness with work discipline as intervening variables. Method: A cross-sectional study with a survey approach was conducted in this study. The data analysis was conducted using the path analysis. One hundred thirty-one samples were recruited in this study. Result: The results of this research show that the Management Information System has a significant effect on work effectiveness. Transformational leadership has a significant effect on work effectiveness. Management Information Systems have a significant effect on work discipline. Transformational leadership has a significant effect on work discipline. Work discipline has a significant effect on work effectiveness. Work discipline plays a significant role as an intervening agent between the Management Information System and work effectiveness or it could be said that the Management Information System has a significant effect on work effectiveness with work discipline as an intervening variable. Conclusion: Work discipline plays a significant role as an intervening agent between transformational leadership and work effectiveness or it can be said that transformational leadership has a significant effect on work effectiveness with work discipline as an intervening variable. Recommendation: The management level in the hospital need to consider the effective management information system and work discipline to improve working effectiveness. The transformation leadership was effective to increase motivation among employee so that the organization need to consider. Further study may develop the R&D study to develop the program focused on the working effectiveness and working efficiency to improve the level of hospital

Abstract

Introduction: The Hospital Ethics and Law Committee (HELC) holds the primary responsibility of ensuring adherence to ethical and legal regulations within the hospital. However, due to the absence of specific provisions regarding the HELC, its role has often not been fully optimized. This study aims to evaluate the role of the HELC in enhancing the quality of services, ensuring patient safety, and resolving medical disputes at Syarif Hidayatullah Hospital, Jakarta. Methods: This cross-sectional study combines quantitative and qualitative methods, involving employees of Syarif Hidayatullah Hospital as respondents. Data collection was conducted through interviews from January to February 2024. The data were analyzed using univariate and bivariate methods, followed by multivariate variance analysis. Results: A total of 53.1% of respondents assessed the role of the HELC as well-executed. An effectively executed HELC role is correlated with increased patient safety, enhanced service quality, and effective resolution of medical disputes (all p less than 0.001). Multivariate variance analysis revealed that the HELC's role has the most significant impact on the improvement of hospital service quality (adjusted R-squared = 0.885). Conclusion: The role of the HELC is associated with improvement of patient safety, service quality, and the resolution of medical disputes in Syarif Hidayatullah Hospital. Hospital management needs to empower the HELC and enhance coordination with other committees and units within the hospital.

Abstract

Rumah Sakit Islam Jakarta Sukapura dengan 90 % pasien adalah pengguna JKN / BPJS Kesehatan sehingga pendapatan terbesar rumah sakit bersumber dari klaim BPJS Kesehatan. Tidak semua klaim pasien rawat inap disetujui oleh BPJS Kesehatan. Klaim tunda pasien rawat inap periode Januari – Desember 2023 sebanyak 10,17 %. Penelitian ini bertujuaan untuk mengetahui faktor-faktor yang menjadi penyebab klaim tunda pasien rawat inap BPJS Kesehatan di RS Islam Jakarta Sukapura. Penelitian ini merupakan studi kuantitatif dengan design cross sectional. Sample diambil dengan cara sampling pada 69 pegawai RS Islam Jakarta Sukapura terdiri dari tenaga kesehatan, tenaga medis, tenaga penunjang dan direksi. Penelitian ini dilakukan di Rumah Sakit Islam Jakarta Sukapura pada bulan Juli – Agustus 2024.. Data didapatkan melalui kuesioner dan analisis data menggunakan SPSS, dilakukan analisis univariate, analisis bivariate dan analisis multivariate. Kategori penyebab klaim tunda terbesar adalah ketidaktepatan kode sebanyak 49,10% (519 kasus), lalu ketidaklengkapan administrasi / berkas klaim sebanyak 40,87% (432 kasus), dan kategori indikasi medis asebanyak 10,03% (106 kasus). Berdasarkan hasil analisis univariat menunjukkan bahwa penyebab klaim tunda pada pasien rawat inap BPJS Kesehatan di RS Islam Jakarta Sukapura melibatkan faktor-faktor terkait SDM, sarana dan prasarana, teknologi, administrasi kepesertaan, dan administrasi pelayanan. Untuk mengatasi masalah ini, penting bagi manajemen rumah sakit untuk meningkatkan kualitas SDM, memperbaiki sarana dan prasarana, serta mengoptimalkan teknologi dan administrasi layanan. Abstract

This study is motivated by the high number of pending BPJS Health claims at Kartika Husada Setu Hospital. Pending claims can disrupt the hospital’s cash flow and potentially reduce the quality of healthcare services. The purpose of this research is to identify the factors causing pending BPJS Health claims for inpatient services at Kartika Husada Setu Hospital in 2024. This study employs a qualitative research approach with a retrospective design. Data were collected through in-depth interviews, direct observations, and document analysis. The informants included the hospital director, finance manager, service manager, head of the medical records unit, casemix staff, coder, and heads of inpatient wards. The findings indicate that the primary cause of pending claims is the mismatch in medical indications, accounting for 73 files (79.4%), followed by incomplete documentation in 4 files (4.4%), and inaccurate coding in 15 files (16.2%). The mismatch in medical indications often involves unmet emergency and inpatient criteria, as well as a lack of therapeutic evidence. Incomplete documentation is frequently found in medical summaries, while inaccurate coding results from differing interpretations between the hospital and BPJS Health verifiers. The hospital needs to enhance training for staff involved in the BPJS claims process, optimize the use of electronic medical record systems, improve coordination with BPJS Health verifiers, and establish a quality and cost control team. The implementation of these recommendations is expected to reduce the number of pending claims, thereby improving operational efficiency and the quality of healthcare services provided to patients.

Abstract

Background: The development of information technology in the health sector encourages the implementation of Electronic Medical Records (EMR) to improve the efficiency and quality of hospital services. UKI General Hospital has implemented EMR, but still faces technical and operational challenges that hinder system optimization. Objective: This study aims to analyze the implementation of EMR at UKI General Hospital and evaluate its impact on the quality of hospital services. Method: The study used a mixed-method approach with data collection techniques through questionnaires, interviews, and quantitative and qualitative data analysis. The sample consisted of health workers and administrative staff involved in the use of EMR. Results: The results of the study indicate that the implementation of EMR has increased efficiency in patient data management, but there are still obstacles such as limited infrastructure, lack of HR training, and difficulties in adapting to technology. Conclusion: The implementation of EMR contributes to improving the quality of hospital services, but improvements are still needed in technical aspects and HR readiness. Suggestion: Improvements in infrastructure, medical personnel training, and strategic policies are needed to support more effective implementation of EMR. Abstract

Kepuasan pasien merupakan indikator penting dalam menilai mutu pelayanan keperawatan. Kinerja perawat yang efektif menjadi kunci untuk memenuhi kebutuhan pasien dan menciptakan kepuasan. Penelitian ini bertujuan untuk menganalisis hubungan kinerja perawat dengan tingkat kepuasan pasien di unit rawat inap RSU UKI. Penelitian ini menggunakan pendekatan kuantitatif dengan desain cross sectional. Data dikumpulkan melalui kuesioner dari pasien rawat inap di RSU UKI. Analisis bivariat menggunakan uji Chi Square dilakukan untuk mengidentifikasi hubungan antara variabel kinerja perawat dan kepuasan pasien. Berdasarkan analisis bivariat, ditemukan hubungan signifikan antara beberapa aspek kinerja perawat dengan kepuasan pasien. Variabel kualitas pelayanan memiliki p-value sama dengan 0,000 (OR sama dengan 3,111), komunikasi p-value sama dengan 0,001 (OR sama dengan 10,667), kecepatan p-value sama dengan 0,001 (OR sama dengan 2,727), kemampuan p-value sama dengan 0,000 (OR sama dengan 36,000), dan inisiatif p-value sama dengan 0,002 (OR sama dengan 9,208). Semua variabel menunjukkan hubungan yang signifikan dengan tingkat kepuasan pasien (p dan lt; 0,05).Kepuasan pasien di RSU UKI dipengaruhi secara signifikan oleh kualitas pelayanan, komunikasi, kecepatan, kemampuan, dan inisiatif dari kinerja perawat. Untuk meningkatkan kepuasan pasien, RSU UKI disarankan untuk memperkuat pelatihan perawat dalam bidang kompetensi klinis dan komunikasi terapeutik, serta mendorong perilaku inisiatif yang lebih tinggi di kalangan perawat. Abstract

Fulfilling patient rights is one of the important things that is the main key in hospital services. In Indonesia, the fulfillment of these patient rights is legally regulated and the hospital must include these rights in the hospital regulations and explain them to the patient so that they understand their rights and obligations. This is a general consent that must be signed by the patient. However, in its implementation at RSU UKI Jakarta, most of the patients did not understand their rights and obligations. And there are still many nurses who do not understand general consent, and consider general consent to be the same as informed consent. This shows the lack of knowledge and awareness of nurses about the importance of fulfilling patient rights. The purpose of this study was to analyze the effect of knowledge, attitudes towards fulfilling patient rights, and motivation as intervening variables. This research is a quantitative approach with a saturated sample of 78 inpatient nurses at the hospital. This study uses the SEM-PLS method. The results show that knowledge has a positive and significant effect on motivation, attitude has a positive and significant effect on motivation, motivation has a significant positive effect on the behavior of fulfilling patient rights, knowledge has a positive and significant effect on the behavior of fulfilling patient rights, and attitude has a significant effect on behavior fulfillment of patient rights. Motivation as an intervening variable can mediate between attitudes and behavior in fulfilling patient rights, having a positive and significant influence. However, motivation does not mediate between knowledge and behavior in fulfilling patient rights, because it has been shown to have no significant effect.
