Abstract

Introduction: Obesity poses a severe threat to global health, making it a significant epidemiological concern that requires special attention. It is associated with several risk factors for various diseases. Alarmingly, one in three Indonesian adults is obese, with the obesity rate steadily increasing. Studies have shown that excess fat mass is linked to higher mortality and morbidity from cardiovascular disease, which is currently Indonesia's leading cause of death. To assess a person's functional capacity and ability for basic physical activity, the 6-minute walking test (6MWT) is utilized, as it proves to be a powerful predictor of morbidity and mortality rates. Therefore, this study aims to analyze the association between VO2 max using 6MWT in obese patients of Makassar City. Methods: This study was a cross-sectional study. The samples comprised all obese individuals who provided signed approval letters and were registered at Makassar City's 20 Public Health Centers. Consecutive sampling was used to gather the samples. Subsequently, the samples were examined and categorized based on their level of obesity.   The 6MWT results are then used with Nury's Formula to produce an estimated VO2max result. To analyze the association between VO2 max using 6MWT and obesity, statistical analysis was conducted using the Mann-Whitney test, with significant results indicated by (p < 0.05). Results: A total of 163 participants comprised the study's sample, with the majority being female and aged 50 years or older. After filling out the questionnaire, data show that most of the obese participants had a family history of cardiovascular disease (CVD), experienced significant stress levels, and had average BMIs falling into the category of obesity. According to the study, the average distance covered in the 6-minute walking test was 340.18 meters (±82.6 meters). When the study participants were ranked by their estimated VO2 max, the average value was 15.10±5.52 for those with first-degree obesity. Following them were participants with second-degree obesity, with results averaging 15.03±5.39 (p = 0.021). Conclusion: A significant inverse association exists between obesity and VO2 max, which means that the more obese a person is, the lower their VO2 max tends to be.

Abstract

Pemeriksaan hematologi rutin dapat digunakan sebagai penanda inflamasi untuk mengidentifikasi penyakit kardiovaskular, termasuk penyakit jantung koroner (PJK). Penelitian ini bertujuan untuk menganalisis gambaran profil hematologi rutin pasien penyakit jantung koroner yang menjalani angiografi koroner serta hubungannya dengan stenosis lesi arteri coronaria. Penelitian analitik observasional ini dirancang dengan desain cross-sectional. Sampel dipilih dengan teknik purposive sampling dan diperoleh 15 subjek penelitian. Tingkat stenosis ditentukan sesuai panduan PERKI 2017. Data disajikan dalam mean±SD, median (IQR), dan persentase n (%). Data parametrik diuji dengan uji t independen atau one-way ANOVA, data nonparametrik diuji dengan Mann-Whitney test atau Kruskal Wallis test, sedangkan data kategorik diuji dengan Fisher’s exact test. Hasil penelitian menunjukkan kadar nilai WBC, neutrofil, limfosit, basofil, RBC, hemoglobin, MCV, MCH, MCHC, PLT, RDW-CV, dan MPV berada dalam kategori normal. Dengan rerata monocyte/lymphocyte ratio 0,27 ±0,06 yang cenderung meningkat, tidak ditemukan adanya perbedaan nilai MLR antara kedua kelompok berdasarkan signifikasi stenosis arteri coronaria (p>0,05). Oleh karena itu, dapat disimpulkan bahwa tidak terdapat hubungan signifikan antara parameter hematologi dengan stenosis arteri coronary pada pasien PJK.

Abstract

ABSTRAK Bilateral Bidirectional Cavopulmonary Shunt (BCPS) hanya dilakukan pada bilateral Superior Vena Cava (SVC) tanpa vena inominata. Sementara itu pasien dengan bilateral SVC seringkali memiliki kondisi penyerta dan kelainan anatomi seperti heterotaxy dan dextrocardia. Kelompok pasien ini dikategorikan sebagai kandidat dilakukan prosedur Fontan dengan risiko tinggi yang dapat mempengaruhi luaran pasien. Pada laporan kasus ini, seorang anak laki-laki usia 13 tahun dengan bilateral SVC dan riwayat telah dilakukan bilateral BCPS sebelumnya. Kondisi anatomi yang kompleks sangat sulit bila hanya dievaluasi dengan ekokardiografi, maka dilakukan pemeriksaan MSCT Kardiak dan Kateterisasi Jantung Kanan. Hasilnya ditemukan dextrokardia dengan situs inversus, bilateral BCPS, Atresia Pulmonal (AP), Atrial Septal Defect (ASD) dan Ventricular Septal Defect (VSD) dengan functional single ventricle. Kedua anastomosis BCPS paten, tidak ditemukan adanya fistula pulmonal maupun Patent Ductus Arteriosus (PDA) dan Major Aortopulmonary Collateral Arteries (MAPCAs). Prosedur Fontan perlu dipertimbangkan sebagai teknik pembedahan pada kasus ini. Teknik prosedur Fontan saat ini telah mengalami modifikasi dengan tujuan memperbaiki hemodinamik pasien dan mencegah komplikasi akibat dilatasi di tingkat atrium secara progresif dengan cara memperbaiki aliran darah dari sistemik vena ke paru-paru. Teknik modifikasi yang dimaksud adalah mengganti jalur sistem vena ke intra-atrial dengan menghubungkan antara Vena Cava Inferior dengan arteri pulmonal sinistra menggunakan extra cardiac conduit (EC) sebagai jalur anastomosis. Kata kunci: BCPS, bidirectional Cavopulmonary Shunt, Dextrokardi, Heteroraxy, Prosedur Fontan ABSTRACT Bilateral Bidirectional Cavopulmonary Shunt (BCPS) is performed only on bilateral Superior Vena Cava (SVC) without inominate veins. Meanwhile, patients with bilateral SVC often have accompanying conditions and anatomical abnormalities such as heterotaxy and dextrocardy. This group of patients is categorized as a candidate for a high- risk Fontan procedure that can affect the patient's outcome. In this case report, a 13-year-old boy with bilateral SVC and a history of having previously conducted bilateral BCPS. Complex anatomical conditions are very difficult if only evaluated with echocardiography, then carried out MSCT cardiac examination and right heart catheterization. The results were dextrocardy with situs inversus, bilateral BCPS, Atresia Pulmonal (AP), Atrial Septal Defect (ASD) and Ventricular Septal defect (VSD) with functional single ventricle. Both anastomosis BCPS are patent, no pulmonary fistula found nor Patent Ductus Arteriosus (PDA) nor Major Aortopulmonary Collateral Arteries (MAPCAs). The Fontan procedure should be considered as a surgical technique in this case. The Fontan technique has been modified with the aim of improving the patient's hemodynamics and preventing complications from being progressively restricted at the atrium level by improving blood flow from the systemic veins to the lungs. The modified technique is to replace the path of the venous system into the intra-atrial by connecting the vena cava inferior to the pulmonary artery sinistra by using extra - cardiac conduit (EC) as an anastomosis path. Abstract

Latar belakang: Hipertensi pulmoner adalah penyakit fatal dari berbagai etiologi yang diperkirakan memengaruhi lebih dari 100 juta orang di seluruh dunia. Penyakit ini didefiniskan secara hemodinamik sebagai tekanan rata-rata arteri pulmonalis ≥ 25 mmHg saat istirahat. Isi: Hipertensi pulmoner menyebabkan berbagai kondisi dan komplikasi klinis terutama gangguan pada sistem kardiovaskular dan respirasi. Hipertensi polmuner akibat penyakit paru-paru dan/atau hipoksia yang menrupakan klasifkasi tipe 3 adalah salah satu bentuk HP yang sering terjadi. Berbagai penyakit paru dan keadaan hipoksia telah lama dikenal sebagai keadaan yang menginduksi perubahan sistem vaskular pulmoner melalui berbagai mekanisme yang belum sepenuhnya dimengerti. Kesimpulan: Mekanisme patologi hipertensi pulmoner terkait penyakit paru kronis, seperti penyakit paru obstruktif kronik, penyakit paru interstitial, penyakit paru campuran pola restriksi dan obstruksi, gangguan pernapasan saat tidur, gangguan hipoventilasi alveolar, paparan lama terhadap ketinggian dan kelainan perkembangan paru adalah adanya remodeling pembuluh darah disertai disfungsi endotel diakibatkan hipoksia kronik.
