Abstract

Ischemic heart disease is still becoming a cause of morbidity and mortality in the world. Regenerative cellular or stem cell therapy is a new and emerging as an alternative option for this disease. There are several stem cell types that used in ischemic heart disease, most used stem cell for this treatment are bone marrow-derived stem cell and mesenchymal stem cell. The mechanism of action for stem cell therapy is depends on transdifferentiation and paracrine effect. It effects the release of cytokines, chemokines and growth factors that inhibits apoptosis and fibrosis, enhance contractility and activate regenerative mechanism. Factors associated with the efficacy of stem cell therapy are delivery route to target myocardium, number of stem cell infusion, injection timing, physical condition of patient and stem cell microenvironment.

Abstract

Chronic limb ischemia (CLI) is a type of peripheral arterial disease (PAD) that is still underdiagnosed and undertreated despite the increasing incidence, thus becoming a global health burden. The prevalence of PAD is quite high, approximately around 200 million globally in the adult population and increased drastically in older age population. It might be difficult to diagnose at an early stage because around 20-50% patients may be asymptomatic. More than 70% of primary physicians also did not notice that the diagnosis was already established during screening of the PAD patients. All these problems could increase the incidence of PAD in the following year. The untreated conditions will develop into a more severe form of PAD known as chronic limb-threatening ischemia (or critical limb ischemia), and patients are at a higher risk of having limb loss, and also increased morbidity and mortality. The primary physicians in the primary health facilities hold an important role in the early diagnosis and management of patients with CLI symptoms or with risk factors of CLI. Due to the limitation of diagnostic testing modality at primary health facilities, the physician can assess the ankle-brachial index (ABI) to determine the presence of CLI. Management of the disease is different for every patient and is customized based on the other comorbidities. Risk factors should be controlled in order to achieve a better outcome. A good management strategy will improve the condition. This review aims to describe how to give an early diagnosis and management for CLI patients in primary health care.

Abstract

Covid19 has changed how we serve patients in all areas of health, and cardiology is no exception. Telemedicine is an option that hospitals can do during this pandemic. Telecardiology is part of telemedicine. By transmitting clinical data and electrocardiograms, telecardiology allows access to cardiology consultation services (telecardiology) without the need to travel to the practice area for both patients and cardiologists. These services can be in the form of teleconsultation, teleelectrocardiography, telerehabilitation, or teleechocardiography. Telecardiology can be applied in prehospital (teleconsultation, teleelectrocardiography,) if there is a referral for a heart attack patient or consult for chronic heart disease through online media; at the hospital in the form of discussions with peripheral hospitals and referral hospitals (teleechocardiography with other small hospitals); and posthospital treatment can be performed for cardiac telerehabilitation in patients with heart failure or teleconsultation of posttreatment patients.

Abstract

Antiphospolypid sí­ndrome (APS) is a systemic autoimmune condition that is related to vascular thrombosis. Patients with APS are at risk for developing pulmonary embolism (PE) events. In this case, we present a 68 years old female with complaints of sudden onset of persistent shortness of breath for two days. No past medical history was identified. Physical and diagnostic examination revealed the presence of PE and antiphospolipid antibodies. Diagnosis of PE and APS were made. Patients then received initial anticoagulant using enoxaparin subcutaneously. Initial anticoagulants in patients with APS and PE should be administered. However, it is important to choose the right anticoagulant. DOAC is contraindicated due to the increased risk of thromboembolic events.

Abstract

Health marketing is interdisciplinary because it uses certain concepts, methods, and techniques for classical and social marketing. The specialty of health marketing is that there are services and markets but no money equivalent.  Healthcare marketing differs mainly by the nature of the demand for health services. The beneficiary may not be the target of a marketing campaign, and the physician decides what, where, when, and how much to provide for a particular service. To understand the impact of marketing strategies on healthcare quality, it is essential to understand today's medical consumers who prefer to seek medical information online. They also have many health services, healthcare providers, and reviews from patients who contact providers.

Abstract

Pendahuluan: Salah satu komplikasi mikrovaskular dari diabetes yaitu nefropati diabetik. Penderita nefropati diabetik yang menjalani hemodialisis membutuhkan waktu yang lama dan membutuhkan kepatuhan dalam mengobati komorbid yang mendasarinya. Hal ini akan memberikan stresor baik fisiologis maupun psikologis penderita yang akan mempengaruhi kualitas hidup. Tujuan penelitian ini menentukan korelasi antara lamanya menjalani terapi hemodialisis pada penderita nefropatik diabetik dengan kualitas hidup di RS Dinda Tangerang. Metode: metode penelitian menggunakan analisis parametrik dengan metode pendekatan non parametrik. sampel penelitian ini menggunakan consecutive sampling yang melibatkan 40 penderita nefropati diabetik. data di analisis menggunakan korelasi spearman. Teknik pengumpulan data menggunakan kuesioner KDQOL SF 1.3. Hasil: berdasarkan hasil penelitian didapatkan 23 penderita 57.5% yang menjalani hemodialisis 1- 3 tahun  dan 6 di antara nya penderita yang memiliki kualitas hidup kurang sebanyak 2 orang, dan yang memiliki kualitas hidup cukup  sebanyak 2 orang serta sebanyak  15 orang lainya memiliki kualitas hidup baik. Simpulan: kesimpulan penelitian ini terdapat korelasi yang bermakna p=0,043 antara lama menjalani hemodialisis dengan kualitas hidup dengan nilai (r): 0.309 yang mana mengarah ke arah positif sehingga ketika penderita telah lama melakukan terapi hemodialisis maka semakin baik kualitas hidup dari penderita.

Abstract

Pendahuluan: Pengertian dari gagal jantung adalah sebuah sindroma klinis dimana jantung mengalami kegagalan dalam menjalankan tugasnya baik. Fungsi dari Beta- blocker adalah menghambat adrenoreseptor beta di jantung, pembuluh darah perifer, bronkus, pankreas, dan hati. Artikel ini menjelaskan mengenai kegunaan Beta- blocker pada pasien gagal jantung dengan fraksi ejeksi yang menurun beserta pengaruhnya terhadap komorbid kardiovaskular dan non kardiovaskular.Pembahasan: Beta-blocker merupakan terapi yang direkomendasikan diberikan pada pasien heart failure dengan ejection fraction yang menurun. Obat golongan beta-blocker membantu mencegah gejala heart failure, meningkatkan remodeling ventrikel kiri, menurunkan resiko rawat inap dan kematian dini. Beta-blocker tidak digunakan pada gangguan nafas fase akut karena dapat memperburuk gagal jantung, bradikardia atau blok jantung dan hipotensi. Pemberian beta-blocker juga direkomendasikan pada pasien aritmia karena berperan dalam kontrol laju ventrikel serta memperlihatkan mortalitas yang lebih rendah dan prognosis yang lebih baik pada pasien atrial fibrilasi yang mendapatkan terapi beta-blocker.Kesimpulan: Beta-blocker direkomendasikan sebagai terapi pada pasien gagal jantung dengan fraksi ejeksi menurun. Tetapi, perlu diperhatikan adanya komorbid kardiovaskular maupun non-kardiovaskular yang dapat mempengaruhi pemberian terapi beta-blockers. Pemberian terapi beta-blocker diharapkan dapat mengurangi mortalitas atau morbiditas pada pasien gagal jantung dengan fraksi ejeksi yang berkurang (HFrEF).
