Abstract

Introduction: Aphasia is understood as difficulty in understanding or producing language caused by disorders involving the brain hemispheres. Early assessment of aphasia is very important to prevent the emergence of telegraphic speech styles, improve welfare, independence, social participation, quality of life, reduce length of stay and care costs, but there is little literature on this subject, especially in stroke patients. Purpose: This study is to provide an overview of the assessment of aphasia in stroke patients. Methods: The design of a case study involving 6 participants and data collection was carried out by conducting an assessment using the Language Aphasia Screening Test (LAST) instrument which was monitored for 3 days, including monitoring errors for naming images, monitoring mismatches repeating words and sentences, monitoring spontaneous pronunciation, monitoring image comprehension, monitoring comprehension of verbal instructions. Results: A total of 6 ischemic stroke patients were found wrong in repeating words and sentences. Conclusion: In aphasic stroke patients all language modalities are impaired, ranging from spontaneous speech, repetition, naming, language comprehension, reading and writing

Abstract

Background: Subarachnoid hemorrhage is a neurological syndrome with complex systemic complications. Rupture of an intracranial aneurysm causes acute extravasation of arterial blood under high pressure into the subarachnoid space and often into the brain parenchyma and ventricles. Bleeding triggers a complex series of events, which can ultimately lead to early brain injury, delayed cerebral ischemia, and systemic complications. Cases: There were six cases of subarachnoid hemorrhage. Some patients come clinically with severe headache and loss of consciousness. The patient has been treated quickly and aggressively and even put on a ventilator for complications of respiratory failure, support for shock and management of aneurysm clipping and EVD. Rapid and precise diagnosis in the management of patients with SAH is of paramount importance, within the first few hours after the onset of SAH. The risk for early neurologic damage and high rates of severe long-term complications necessitated aggressive early management. Conclusion: Rapid diagnosis and attentive management of patients with SAH are essential, as early deterioration is possible within the first few hours after the onset of SAH. The risk for early neurologic damage and high rates of severe long-term complications necessitated aggressive early management. Prevention and Management of Complications. The most common complications were pneumonia, aspiration, respiratory failure/distress, sepsis and imbalance electrolyte (hyponatremia). Approximately 50% of deaths after SAH are due to medical complications.

Abstract

Highlights:1. As minocycline plays an important role in stroke microglia activation and iron chelation, it is important to further analyze its effects on stroke treatment.2. This meta-analysis revealed a significant effect of minocycline therapy, as evidenced by improved functional outcomes and inhibited matrix metalloproteinase-9 (MMP-9) activity.   Abstract Stroke is the most common and devastating cerebrovascular disease. Many neuroprotective medications, such as scale and minocycline, have been developed to help the nervous system recover or regenerate after a stroke. However, it remains unclear whether minocycline provides a beneficial effect on stroke. We conducted this systematic review and meta-analysis to synthesize the effects of minocycline in stroke treatment. The systematic review was registered in the International Prospective Register of Systematic Reviews (PROSPERO), with registration number CRD42023485168. The quality of the eligible studies was assessed using the Jadad scale. This systematic review included three ischemic stroke trials, seven intracerebral hemorrhage trials, and one study on acute stroke. There was a significant association between minocycline intervention and stroke severity according to the National Institute of Health Stroke Scale (NIHSS), with a pooled mean difference (MD) of -1.92, a 95% confidence interval (CI) of -3.39 to -0.45, and a value of p=0.01. In the subgroup of ischemic stroke, the modified Rankin Scale (mRS) was significantly lower in the minocycline treatment group compared to the control group (MD=-0.89, 95% CI=-1.54 to -0.25, p=0.007). Additionally, matrix metalloproteinase-9 (MMP-9) levels for the intracerebral hemorrhage subgroup were significantly lower in the minocycline treatment group compared to the control group (MD=-19.93, 95% CI=-36.9 to -2.96, p=0.02). The analysis revealed that minocycline intervention was not significantly associated with hematoma volume, mortality, or stroke recurrence. Our findings indicate that minocycline supplementation is a potential intervention strategy for treating ischemic stroke and intracerebral hemorrhage.

Abstract

Background: Tuberculosis (TB) remains a significant health issue in Indonesia. Central nervous system (CNS) tuberculoma is one of the extrapulmonary TB diseases and accounts for approximately 1% of all cases. The emergence of this disease is primarily associated with a weakened immune system. However, several other factors, such as comorbidities, a history of inadequate TB treatment, and poor nutrition, also play a role in the development of tuberculoma. Case: A 22-year-old male complained of weakness in all four limbs for the past month, accompanied by tingling and numbness from both feet up to the T10-11 dermatome level. The patient has a history of seizures from one year ago, interrupted treatment for military tuberculosis, and malnutrition. An MRI of the head and whole spine with contrast revealed tuberculomas. The patient was treated with medication, including intravenous dexamethasone 5 mg every 8 hours, oral phenytoin 200 mg every 24 hours, and anti-tuberculosis therapy. Discussion: Tuberculoma in the central nervous system is rare, especially multiple tuberculomas co-occurring in the brain and spinal cord. MRI is a sensitive tool for diagnosing tuberculomas, characterized by the presence of a target sign. The combination of corticosteroids, antiepileptic drugs, and an entire course of anti-tuberculosis medications aims to address both the immediate neurological symptoms and the underlying infection. Conclusion: TB can present as lesions in the brain and spinal cord, requiring the ability to correlate clinical manifestations and radiological features to establish a diagnosis and necessitating adequate therapy.

Abstract

Background: Acute ischemic stroke due to large vessel occlusion (LVO) requires rapid identification. Reducing the time to diagnosis and treatment of stroke patients is an important goal to improve clinical outcomes. Non-contrast computed tomography (NCCT) is widely used in clinical practice for suspected stroke patients. Automated analysis using artificial intelligence in NCCT may be a solution to accelerate the early detection of LVO. Objective: To determine the accuracy of artificial intelligence in NCCT to predict LVO. Methods: A systematic literature search was conducted based on the PRISMA flow chart in four databases (PubMed, ProQuest, ScienceDirect, Cochrane Library) until June 2024. Data extraction was performed to evaluate the accuracy of predicting LVO. Quality assessment was performed using QUADAS-2. All data were analyzed using Review Manager 5.4 and MetaDTA 2.0. Results: Five studies involving 4.862 patients were enrolled. The quality of all the studies was high and had a low risk of bias. All studies used different software. Artificial intelligence in NCCT had fairly good accuracy with a sensitivity and specificity of 0.83 (95% CI; 0.78-0.87) and 0.73 (95% CI; 0.52-0.87). NCCT plus clinical status (NIHSS, stroke onset) in two studies slightly improved overall accuracy with a sensitivity and specificity of 0.85 (95% CI; 0.80-0.89) and 0.74 (95% CI; 0.54-0.88). Two studies reported that machine learning took less than two minutes. Conclusion: Artificial intelligence in NCCT was reasonably accurate and took a short time to predict LVO. There are still opportunities for machine learning to improve performance. Further research is still needed.

Abstract

Background: The increasing demand for English certification programs has led educational institutions to refine their digital marketing strategies to reach potential candidates effectively. However, demographic factors, such as gender and occupation, significantly influence how consumers perceive and engage in promotional content. Understanding these differences is crucial for designing targeted and effective marketing strategies in the educational sector.Purpose: This study examines how gender and occupation influence respondents' perceptions of the promotion mix for an English certification program, the preferred channel for discovering certification events, and their motivation for enrolling. The research particularly explores preferences in social media usage, word-of-mouth recommendations, interactivity in official accounts, soft versus hard selling content, and the effectiveness of ads across different platforms.Design/methodology/approach: A quantitative research design was employed with 500 respondents (250 male and 250 female) from different occupational backgrounds: students, freelancers, job seekers, and professionals. Data were analyzed using Multivariate Analysis of Variance (MANOVA) to simultaneously assess the influence of gender and occupation on multiple dependent variables. Normality tests confirmed that the data met the assumptions of MANOVA analysis.Findings/Result: The results reveal the following: Gender significantly influenced respondents’ preference for word-of-mouth recommendations (p2) and interactive official accounts (p3), with females showing higher engagement in both; Occupation significantly affects social media preference (p1), promotional content style (p4), and the preferred channel for event discovery, with students and job seekers favoring Instagram/TikTok, whereas professionals rely more on LinkedIn and email marketing; Interaction effects (gender × occupation) were significant for preference for interactive official accounts, particularly among female students and female job seekers; No significant differences were found in ad placement preferences (p5) across gender and occupation groups. Conclusion: This study highlights that gender and occupation are key determinants of digital marketing effectiveness in English certification programs. Marketing strategies should be tailored to demographic segments, with peer-driven promotions and interactive content appealing more to female audiences, while channel selection and content style should be aligned with occupational preferences.Originality/value (State of the art): This study contributes to the digital marketing and education industry by providing empirical evidence on how demographic factors influence the effectiveness of promotion mix strategies. Unlike previous studies that focus solely on gender or occupation, this study examines their combined effects using MANOVA analysis, offering a more comprehensive understanding of consumer preferences in education marketing. These findings provide practical insights for marketers to develop more personalized and data-driven promotional strategies for online courses and certification programs.
