Abstract

Nevus Melanositik sejak lahir ditemukan pada satu persen anak di dunia dengan ukuran bervariasi. Salah satu varian nevus melanositik yang sangat jarang, berukuran sangat besar, yakni Nevus Melanositik Kongenital Raksasa (NMKR). Pada tulisan ini dilaporkan kasus NMKR disertai kelainan kongenital ekstremitas. Seorang bayi perempuan lahir di puskesmas dengan partus spontan dirujuk ke IGD, 3-4 jam setelah lahir. Terdapat bercak hitam di area perut, punggung, dan paha. Didapatkan juga perbedaan panjang tungkai, serta hanya ada 3 jari di kaki kanan. Tatalaksana diberi pelembap. Kasus ini jarang; diperlukan konsultasi dan observasi teratur untuk meningkatkan dan memperpanjang harapan hidup.Melanocytic Nevus since birth is found in one percent of children in the world with variable severity. One of the rare variant is very large melanocytic nevus variant called Giant Congenital Melanocytic Nevus (GCMN). This paper reported a case of GCNM accompanied with congenital abnormalities of the extremities. A baby girl who was born spontaneously in a clinic, was referred to the emergency department about 3-4 hours after her birth. The abnormalities were black spots in the abdominal area, back, and thighs. The patient also had different length of legs differences with only 3 toes on right foot. The patient was given moisturizer. GCMN case accompanied with congenital limb abnormalities is rare; regular consultation and observation are required to improve and prolong survival.

Abstract

Salah satu reaksi erupsi obat alergi adalah Pustulosis Eksantematosa Generalisata Akut (PEGA). Dilaporkan kasus PEGA post partum dengan plak eritematosa disertai pustul di hampir seluruh tubuh terutama di daerah lipatan. Kelainan ini muncul beberapa hari setelah melahirkan dan menggunakan ampisilin. Kasus ini memerlukan konsultasi dan observasi teratur.The clinical manifestation of allergic drug eruption may be in the form of Acute Generalized Eczematous Pustulosis (AGEP). This paper reported AGEP case in postpartum patient with erythematous plaque with pustules in almost the whole body, especially in the folds. This eruption comes a few days after delivery and ampicillin use. The patient requires regular consultation and observation.

Abstract

Pemphigus Vulgaris(PV) is an autoimmune disease characterized by vesicles and bullae on the skin and mucosa resulting from an autoantibody reaction to desmosomal adhesion molecules desmoglein (Dsg) 1 and 3, which function as strong adhesions between keratinocytes. Pemphigus Vulgarisis more common in the fifth and sixth decades of age. The lesions are fragile blisters on the mucosa and skin. Diagnosing PV requires anamnesis, physical examination,and investigations such as histopathologic examination and direct immunofluorescence. Therapy generally uses steroids. Adjuvant treatmentis given to reduce the side effects of corticosteroids. We reported a male, 59 years old, was treated with blisters that break easily into blisters on the head, face, chest, back, groin and buttocks accompanied by burning and itching. In the scalp, facial, anterior et posteriortrunk, inguinal,and gluteal regions, multiple erythematous maculeswere found with lenticular-plaqueshape; it waspartially confluentwith thick brown crustswhich werechallengingto remove. The histopathologicexamination found the presence of suprabasal bullae with lymphocyte inflammation cells. The patient was diagnosed with PV and treated with corticosteroids with the sparing agent mycophenolate sodium andshowed clinical improvement. The first-linetreatment for pemphigus Vulgarisis systemic corticosteroids. Adjuvant sparing agent therapy is given to reduce the side effects of corticosteroids. Sodium mycophenolate sparing agent was selected because ofitsminimum side effects. In systemic management, the dose of corticosteroid and sparing agent mycophenolate sodium was gradually decreased. The patient experienced initial remission after treatment

Abstract

The pilosebaceous unit is a sebaceous gland with hair follicles. One of the hormones that affects the pilosebaceous unit is androgen, a steroid hormone that plays an important role in the expression of the phenotype of men and women. Androgen consist of testosterone and dihydrotestosterone which are activated by androgen receptors. Androgen in the pilosebaceous unit affect sebum production and hair growth. Androgen receptors in the sebaceous glands are found on sebocytes and in hair follicles in the dermal papillae of hair follicles

Abstract

Background. Syphilis management is based on the syphilis stage classification. The main treatment for syphilis is currently benzathine penicillin. However, patients allergic to penicillin antibiotics or who are not willing to be injected can be given doxycycline or erythromycin antibiotics. Case presentation. We present a case of a female with latent syphilis. Patient was previously treated with oral antibiotics, such as doxycycline for 30 days and followed by azithromycin for 30 consecutive days. Clinical laboratory examinations show increase of TPHA and VDRL, and the case was considered as unresponsive case. Later, the patients received benzathine penicillin G for three weeks and showed clinical improvement. Conclusion. For patient who refuse to be treated with penicillin injection, the alternative treatments are oral antibiotics. However, there is a reduced number of successful treatments for patients treated with oral antibiotics.
