Abstract

Nevus Melanositik sejak lahir ditemukan pada satu persen anak di dunia dengan ukuran bervariasi. Salah satu varian nevus melanositik yang sangat jarang, berukuran sangat besar, yakni Nevus Melanositik Kongenital Raksasa (NMKR). Pada tulisan ini dilaporkan kasus NMKR disertai kelainan kongenital ekstremitas. Seorang bayi perempuan lahir di puskesmas dengan partus spontan dirujuk ke IGD, 3-4 jam setelah lahir. Terdapat bercak hitam di area perut, punggung, dan paha. Didapatkan juga perbedaan panjang tungkai, serta hanya ada 3 jari di kaki kanan. Tatalaksana diberi pelembap. Kasus ini jarang; diperlukan konsultasi dan observasi teratur untuk meningkatkan dan memperpanjang harapan hidup. Melanocytic Nevus since birth is found in one percent of children in the world with variable severity. One of the rare variant is very large melanocytic nevus variant called Giant Congenital Melanocytic Nevus (GCMN). This paper reported a case of GCNM accompanied with congenital abnormalities of the extremities. A baby girl who was born spontaneously in a clinic, was referred to the emergency department about 3-4 hours after her birth. The abnormalities were black spots in the abdominal area, back, and thighs. The patient also had different length of legs differences with only 3 toes on right foot. The patient was given moisturizer. GCMN case accompanied with congenital limb abnormalities is rare; regular consultation and observation are required to improve and prolong survival.

Abstract

Salah satu reaksi erupsi obat alergi adalah pustulosis eksantematosa generalisata akut (PEGA). Dilaporkan kasus PEGA post partum dengan plak eritematosa disertai pustul di hampir seluruh tubuh terutama di daerah lipatan. Kelainan ini muncul beberapa hari setelah melahirkan dan menggunakan ampisilin. Kasus ini memerlukan konsultasi dan observasi teratur. The clinical manifestation of allergic drug eruption may be in the form of acute generalized eczematous pustulosis (AGEP). This paper reported AGEP case in postpartum patient with erythematous plaque with pustules in almost the whole body, especially in the folds. This eruption comes a few days after delivery and ampicillin use. The patient requires regular consultation and observation.

Abstract

Nevus Hori (NH) merupakan hipermelanosis dermal didapat yang ditandai bercak coklat keabuan. NH bermanifestasi terutama di daerah fasial seperti zigomatikum, dahi, pelipis, kelopak mata dan pangkal hidung, tetapi dapat juga di ekstra fasial. NH memengaruhi kualitas hidup penderitanya karena dapat menganggu penampilan. Awitan NH bervariasi, dapat terjadi antara usia 12-72 tahun. Genetik memegang peran penting dalam patofisiologi NH. Beberapa faktor pencetus yang telah diidentifikasi terkait NH antara lain, inflamasi, radiasi ultraviolet, kehamilan, kontrasepsi, dan kosmetik. Manifestasi klinis NH sering menyerupai kelainan hiperpigmentasi dermal lain seperti melasma, nevus Ota, dan melanosis Riehl, sehingga masih sering terjadi kesalalahan diagnosis. Penegakan diagnosis dilakukan dengan mempertimbangkan manifestasi klinis pasien. Pemeriksaan penunjang tidak rutin dilakukan pada kasus NH. Bila diperlukan, pemeriksaan baku emas berupa pemeriksaan histopatologi dapat dilakukan. Tata laksana NH memiliki tantangan tersendiri. Tata laksana NH yang utama adalah menghindari tersediri. Tata laksana defenitif untuk kasus MH dilakukan dengan terapi kombinasi, baik terapi farmakologis dan non-farmakologis. Pilihan terapi yang dapat digunakan adalah terapi topikal, dan tindakan invasif seperti dermabrasi, krioterapi dan laser.

Abstract

Empowering health workers at primary health facilities in Marga Sungsang Village: introduction to skin cancer. Skin cancer is a skin malignant tumor derived from keratinocytes and melanocyte cells, known as keratinocyte carcinoma and cutaneous melanoma, the most common is keratinocyte carcinoma consisting of Basal cell carcinoma(BCC) and Squamous cell carcinoma (SCC) and rarely cutaneous melanoma (MM). The incidence has increased sharply around the world, especially in Palembang. The study in Palembang at RSMH (2017-2019) found that there was an increase of skin cancer is five times compared to previous studies. Generally, skin cancer patients referred to tertiary hospitals are in the advanced stage and accompanied by complications, increasing morbidity and mortality, making therapy difficult. so the cost of hospital care is high and becomes a burden to the state. The problem for health workers in primary health facilities is that knowledge about skin cancer is still low and this knowledge is only obtained during education level. The purpose is to improve and develop the knowledge of primary health care workers by Oncologist and skin surgeon and lecturer in Dermatology and Venereology FK Unsri / RSMH Palembang, about skin cancer and its problems, reducing the incidence and finding new cases of skin cancer, preventing skin cancer an advanced stage in primary health facilities. The counseling in the village of Marga Sungsang, Sungsang sub-district, 27 participants consist of 26 women (96,3%) and 1 male (3,7%) participants, with varied in age, the youngest was 18 years and highest was 44 years old. Education level: SMA as 20 participants (74.0%) and S1 / AMK level as 7 participants (25.9%). The results of the pretest and posttest knowledge showed a 5-fold increase posttest compared to the pretest. The knowledge of skin cancer health workers is still low. Counseling is needed for all health workers and community leaders in the Banyuasin district.

Abstract

Leprosy is a chronic granulomatous infection caused by Mycobacterium leprae. Leprosy infects skin, mucosa and peripheral nerves causing loss of skin sensation with or without skin lesions. Clinical feature of leprosy is highly variable, and can resemble a variety of lesions present in other diseases. Differential diagnosis of skin lesions is so broad that it can lead to misdiagnosis. Based on data from the World Health Organization (WHO) in 2018, it was reported that the number of new cases of leprosy in the world was 210,671 cases, with a new case detection rate of 2.77 per 100,000 population. Indonesia is ranked third in the world as a country with the most new cases of leprosy after India and Brazil. Lack of knowledge of health workers and cadres affects the number of leprosy cases found. This community service activity was carried out with the aim of providing counseling regarding early leprosy examination to health workers and cadres in Prabumulih. The activity was coordinated by Dermatology, Venereology, and Aesthetics Department, Faculty of Medicine, Sriwijaya University, in collaboration with AR Bunda Hospital Prabumulih. At the end of the activity the participants had a better understanding of leprosy and the steps for early leprosy examination. This activity is useful for increasing the knowledge of health workers and cadres about leprosy and how to check for leprosy.

Abstract

Scabies is a skin disease caused by infestation and sensitization of Sarcoptes scabiei var. hominis. The prevalence of scabies reaches more than 20% in developing countries and mainly affects children. Scabies is more commonly found in a location with dense population, with high direct or indirect contact, such as in Islamic boarding schools. This counseling and community service activity aims to provide information related to scabies to students and to screen scabies risk factors at Darussalam Prabumulih Islamic Boarding School. The results of the questionnaire screening showed that respondents had a high risk of scabies infestation based on an assessment of the 4 main risk factors. The results of anamnesis and physical examination of 49 students showed that 19 students (38.7%) had scabies infestation and 30 students (61.3%) were free from scabies infestation. This counseling is expected to help reduce the number of scabies infestations in densely populated residential locations, especially in Islamic boarding schools.

Abstract

Background: Hair loss can occur in pediatric and adult populations and may have different patterns. The causes of hair loss in pediatric patients include tinea capitis, alopecia areata, telogen effluvium, traction alopecia, anagen effluvium, androgenetic alopecia, loose anagen syndrome, short anagen syndrome, congenital aplasia cutis, congenital triangular alopecia, atrichia congenita, congenital hypotrichosis, and transient neonatal hair loss.Purpose: to understand etiologies of hair loss in pediatrics, and to determine the appropriate examinations for diagnosing and managing hair loss in children.Review: Hair loss in children can be categorized as congenital or acquired. Congenital hair loss is classified based on distribution, while acquired hair loss is classified as either scarring or non-scarring. Hair loss complications in children can be irreversible. The diagnosis of hair loss in pediatric patients includes anamnesis, physical examination, and supporting examinations. The supporting examinations used for diagnosis include fungal cultures, hair pull tests, hair tug tests, light microscopy, and trichoscopy. Conclusion: There are numerous causes of hair loss in pediatric patients that need to be identified before appropriate management can be implemented. The management of hair loss requires a holistic approach, including psychosocial support. Correct diagnosis and treatment of pediatric hair loss can prevent a decrease in the patient's quality of life.
