Abstract

Nowadays, the high morbidity and mortality rates caused by diarrhea require prompt and precise initial management. Nutritional conditions are one of the causes of the high morbidity of diarrhea in children. Poor nutritional conditions will have an impact on disruption of growth, mental development and intelligence of children and increase the likelihood of infection in children. In this case family medicine approach needs to be done to identify risk factors and clinical problems in patients, as well as to conduct holistic and comprehensive management with the application of evidence based medicine family doctors through a patient centered approach and family approach. The study conducted was Case Report. Primary data were obtained through alloanamnesis and physical examination by making a home visit, and filling in patient files. Secondary data was taken from the patient's medical record. The assessment is based on an initial holistic diagnosis, process and end of visit. Based on the final holistic diagnostic An. V had acute diarrhea, some dehydration and malnutrition, internal and external risk factors were found. Patient weight is very low (malnutrition), lack of family knowledge of the illness suffered and lack of awareness of cleanliness and balanced nutrition are the main factors of the problem. The family is given intervention by medical treatment and counseling using leafleates. Then an evaluation a conclusion can be drawn that there is an increase in knowledge about disease, hygiene and balanced nutrition

Abstract

Chronic Obstructive Pulmonary Disease is a chronic lung disease that can be prevented and treated, which is characterized by air flow resistance that is not completely reversible. COPD in Southeast Asian countries is estimated at 6.3% with the highest prevalence found in Vietnam (6.7%) and China (6.5%). While in Indonesia, according to data from the Basic Health Research (RISKESDAS) 2018, the national smoking prevalence for people aged ≥ 10 years is 28.8% and for aged 10-18 years is 9,1 %. Pulmonary tuberculosis (TB) is a contagious infectious disease caused by Mycobacterium tuberculosis and most often manifests in the lungs. In Indonesia, TB is a major public health problem. The number of TB patients in Indonesia is the 3rd largest in the world. The study conducted was a case report. Primary data was obtained through history taking and physical examination by making home visits, filling family folders, and filling patient files. The assessment is carried out based on a holistic initial diagnosis, process, and end of the visit quantitatively and qualitatively. Patients were diagnosed with COPD from the history taking and physical examination. As well as supporting examinations. Patients do not have enough knowledge of the disease they are suffering from. Patients have a habit of smoking one pack per day. In the patient's environment there are neighbors who are taking TB packages. The patient's work environment is exposed to smoke from burning trash and dust when sweeping the yard. In the evaluation found an increase in patient compliance to avoid risk factors and family knowledge about the disease and the existence of support in carrying out breathing exercises in patients.

Abstract

Gout or commonly known as gout arthritis is a disease caused by the accumulation of monosodium uric crystals in the body. Gout is estimated to occur in 840 people out of 100,000 people. Hypertension is a state of systolic blood pressure of more than 140 mmHg and diastolic of more than 90 mmHg. Based on blood pressure measurements, the prevalence of hypertension in the population aged 18 years and above in 2013 in Indonesia is 25.8%. The use of evidence-based medicine of family doctor services is done by identifying risk factors, identifying clinical problems, and patient management based on the framework of solving patient problems through a patient-centered and family approach. This is a case report study. Assessment is done based on initial diagnosis, process, and end of study qualitative and quantitatively. The patient was a 63-year-old female, a housewife. Patients were diagnosed with Class I Gout Arthritis and Hypertension, and Class II Obesity. Patients and families have less knowledge about the disease and unhealthy lifestyles. This is a risk factor for disease complications in patients. Gout Arthritis and hypertension with less knowledge about risk factors, adherence to routine drug use, and compliance with periodic control to the doctor can cause acute and advanced complications. Complications can be prevented by the family doctor approach.

Abstract

The Diseases that frequently affect elderly groups includes chronic degenerative diseases such as chronic obstructive pulmonary disease (COPD). Chronic Obstructive Pulmonary Disease (COPD) is a disease characterized by obstruction of air flow in the airways that is not completely reversible. This study aims to identify risk factors, clinical problems, and provide management of patients with the implementation of a holistic family physician services based on evidence based medicine approach to patient and family centered approach. This study is a case report. Primary data were obtained through history taking (directly from the patient and indirectly from family members), physical examination and a home visit to complete the family, psychosocial and environmental data. Secondary data were obtained from medical records of the patient at the health center. Assessment was made based on a holistic diagnosis from the beginning, process, and the end of quantitative and qualitative studies. 64-year-old male patient was diagnosed with COPD, He was worried his diseases can disturbing his activity. Patients do not have enough knowledge of the disease they are suffering from, have a habit of smoking 10-15 cigarettes per day and often exposed to air pollution when going to work and back home. Pharmacological and non-pharmacological interventions were carried out for patient and his families with education related to the patient's disease and exercises for respiratory muscles. In the evaluation found an increase in patient compliance to avoid risk factors and family knowledge about the disease and the existence of support in carrying out breathing exercises in patients. Services with a family medicine approach in pharmacological and nonpharmacological therapy are able to solve health problems and improve the quality of life of patients.

Abstract

Stroke is the second leading cause of death and the third cause of disability in the world. Globally, 87% of strokes related to death and disability occur in low-medium income countries. Recovery from a stroke depends on many factors including the risk, the accuracy and speed of management, the disease that aggravates stroke and treatment and the implementation of early mobilization to prevent one complication of prolonged bed rest. Things to consider in handling post-stroke patients is family support. Family support can support patient recovery and recovery. The study conducted was a case report. Primary data was obtained through history taking and physical examination by making home visits. The assessment was carried out based on a qualitative initial holistic diagnosis, process, and end of visit. Based on the examination, the patient was a 62-year-old woman diagnosed with stroke, pressure sores, hypercholesterolemia, history of hypertension, and nucleus pulposus hernias. Very limited patient knowledge about the disease is evident from patients and family who have never been controlled or treated. Education and intervention are given to patients and families about stroke and its relationship with hypercholesterolemia and a history of hypertension, its complications in the form of pressure ulcers, hernia nucleus pulposus suffered by the patient and its treatment in 3 home visits. In the evaluation found an increase in patient compliance to avoid risk factors and family knowledge about patient care. Unhealthy lifestyles and lack of knowledge of patients about the disease become risk factors for the occurrence of disease in patients. Services with a family medicine approach in pharmacological and non-pharmacological therapy are able to solve health problems and improve the quality of life of patients.

Abstract

Hypertension is one of the non-infectious diseases which becomes an important health problem throughout the world because it keeps increasing. There are 9.4 million deathhs from cardiovascular disease are caused by complications of hypertention. Hypertension itself is a multifactorial disease that arises mainly because of interactions between certain risk factors that can be modified or cannot be modified. Whereas, obesity is caused by an imbalance between the incoming energy and the energy that comes out. The prevalence of obesity throughout the world has tendency to increase in past 30 years.. In addition, Farmingham study stated that 65% of women and 78% of men who suffer from hypertension are directly related to weight gain and obesity. The study was Case Report Study. Primary data is obtained through history taking and physical examination by making home visits, filling out family folders, and filling in patient files. Assessment is carried out based on the initial holistic diagnosis, process, and end of the visit quantitatively and qualitatively. Based on a holistic diagnosis, the patient has a complaint of feeling headache and often followed by a feeling of heaviness behind the neck. The patient also had a history of stroke four years ago. Patient feel worried that the disease is getting worse, has recurrent stroke, and be a burdensome for his family. The patient's expectation is less clinical symptoms , controlled hypertension, no longer obesity, and can do activity and work normally again. Patient was diagnosed with hypertension and obesity in grade II based on the results of history and physical examination. The patient is a 53-year-old man who is obese, often eating foods that contain lots of salt, oily food, and lack of physical activity. Patients are less aware of the disease and the right lifestyle for the illness. After the intervention is done holistically with drugs and education, he obtains a decrease in clinical symptoms and behavioral changes by changing lifestyle and routine taking drugs. Diagnosis and management of this patient has been doing holistically, patients center, family approach based on several theories and current research. In the process of behavior change, patient has reached the trial stage.

Abstract

Occupational skin disease is one of the occupational diseases that occur frequently in the community, and is the second most occupational disease in Europe after musculoskeletal injuries.1 Tinea pedis or also known as athlete's foot is a superficial fungal infection of the skin. feet that often occur in cases of dermatophytosis in general today.2 Tinea pedis infection also attacks various levels of work, especially jobs that require wearing tight and closed shoes, increased moisture due to sweat, mechanical rupture of skin, and exposure to fungi are predisposing factors causes tinea pedis.3 It is estimated that about 70% of the population worldwide has been infected with tinea pedis.Patient Mr. S 65 years old with a complaint of red scaly patches accompanied by itching on both toes since 3 weeks ago. Individual factors that play a role in the form of patients often wearing tight and closed shoes. From the physical examination, it was found that the picture of fungal infection was in the form of reddish spots with scales on top with central healing and active margins. There are no factors outside of work that contribute to the onset of symptoms. From these results, the occupational disease of Tinea pedis was enforced due to the use of tight and closed shoes while working. The treatment given is in the form of medical and non-medicamentous with the main focus to cure and prevent recurrence.

Abstract

Nutrition plays an important role in children health. The prevalence of underweight children in Indonesia by 11.2% to 4.0% was very thin and emaciated 7.2%, while the obese children by 18.8%, consisting of 10.8% obese children and obese amounted to 8.8%. Lampung Province was ranked second among the provinces with a skinny kid. The nutritional status itself can be affected by several things, one of which is the status of demographics. The purpose of this study is known association of sociodemographic factors with nutritional status of children. This type of research is an analytic observational with cross sectional approach. The population is students of SDN 1 South Pringsewu, with a sample size of 225 people. The statistical test used was chi - square. The results of the univariate analysis are mostly had normal nutritional status (68%), age 6-9 years (75.6%), female (51.1%), highly educated mothers (77.3%), father's occupation were self-employed (64%), parents have a high income levels (> Rp. 1.581 million) (69.3%). The results of the bivariate analysis obtained no relationship between maternal education level (p-value < 0.001), the type of father's occupation (p-value < 0.001), and the level of family income (p-value < 0.001) with the nutritional status of children. While there is no correlation between age (p-value = 0.829), and the child's gender (p-value = 0.307) with the nutritional status of children. Expected the school to coordinate with health officials in monitoring the nutritional status of their students. Keywords : Sociodemographic, nutritional status of children

Abstract

yspepsia is one of the health problems that are often encountered in daily practice. Dyspepsia was defined as having one or more symptoms of epigastric pain, burning, fullness after eating, or early satiety. Bloating and nausea are common with dyspepsia but are not specific. Potential lifestyle factors associated with dyspepsia include smoking, alcohol, and consumption of analgesics. Furthermore, dietary habits that include consumption of fast food, salty foods, coffee/tea, and spicy foods are associated with aggravating dyspepsia symptoms and lack of physical activity may contribute to the onset of dyspepsia. In addition, the stress factor is also something that must be overcome because it is one of the trigger factors and also aggravates the condition of dyspepsia patients. This study is a case report. Primary data were obtained through history taking (autoanamnesis and alloanamnesis from family and patients), physical examination, supporting examinations and home visits to assess the physical environment. Assessment based on a holistic diagnosis from the beginning, process, and end of the study qualitatively and quantitatively. Application of family doctor services based on evidence based medicine to patients by identifying risk factors, clinical problems, and patient management based on a patient problem solving framework with a patient centered approach and a family approach. A 56-year-old female patient came to the Kemiling Health Center with complaints of abdominal discomfort such as pain and a lump in the gut, accompanied by nausea and headache that had worsened since 2 days ago. Complaints are felt to come and go but get worse if the patient is late to eat. Similar complaints have been felt to have disappeared since 2017. Management of functional dyspepsia patients in a holistic and comprehensive manner, patient centers, family approved with functional dyspepsia treatment regularly according to EBM is needed in order to eliminate symptoms, trigger factors and change patient behavior so as to prevent complications and recurrence of the disease.

Abstract

In developing countries, the incidence of epilepsy is 61-124/100,000 children per year. The prevalence of epilepsy cases in Indonesia is 8.2 per 1,000 population with an incidence rate of 50 per 100,000 population. In the management of epilepsy patients, cooperation from various parties is needed, one of which is the role of family doctors. Identify internal, external risk factors and clinical problems found in patients, apply a holistic and comprehensive family doctor approach according to the problems found in patients, and carry out evidence-based medicine management that is family-approach, patient-centered and community oriented. The study is a case report. Primary data were obtained through history taking, physical examination and home visits. Secondary data were obtained from the patient's medical record. Assessment was qualitatively and quantitatively carried out based on the initial holistic diagnosis, process, and end of the visit using food recall. A male patient, 17 years old, came to take an epilepsy control referral letter to the hospital. The patient has been diagnosed with epilepsy since 7 years ago. Patients routinely take medication to the hospital every month. At this time the patient said that the seizures began to decrease after taking the drug every day. Enforcement of diagnosis and management of these patients has been done holistically, patient centered, family approach and community oriented based on several theories and the latest research. In the process of behavior change, the patient has reached the trial stage
