Abstract

Background: Preeclampsia can lead to maternal and fetal complications due to its ability to cause multiple organ disorders. Interestingly, N-terminal pro-brain-type natriuretic peptide (NT-proBNP) levels were higher in preeclampsia than in non-preeclampsia, representing cardiovascular mal¬function as the potential cause. Moreover, NT-proBNP also has a potential role in predicting complications that will arise in preeclampsia. This systematic review was performed to determine the role of NT-proBNP plasma levels in predicting maternal and fetal complications in preeclampsia.Subjects and Method: This systematic review was conducted based on PRISMA-P by previous observational study from scientific databases, namely Hinari, Cochrane Library, ScienceDirect, Scopus, and grey literature in OCLC’s OAISTER between 2006 and 2021. The search keyword used were ((NTproBNP) OR (NT-proBNP) OR (N-terminal pro-BNP) OR (N-terminal pro-Brain Natriuretic Peptide)) AND ((preeclampsia) OR (pre-eclampsia)) AND (pregnancy complications). Newcastle - Ottawa Quality Assessment Scale (NOS) was used to assess quality of included studies.Results: After study selection, five studies from 156 studies were considered eligible and selected in this systematic review. The results showed that pre-eclampsia complications occurred with NT-proBNP levels above 500 mg/dL, which cardiovascular complications may occurred above 700 mg/dL. NT-proBNP levels were higher in women with maternal complications such as placental abruption, HELLP (Hemolysis, elevated liver enzyme levels, and low platelet levels) syndrome, eclampsia, pulmonary oedema, congestive heart failure, cerebrovascular accident, renal dysfunction, and hypertensive retinopathy. Furthermore, increased NT-proBNP levels were associated with fetal growth restriction, resulting in low birth weight. NT-proBNP was significantly higher in pregnant women due to a combination of pre-existing volume overload and NT-proBNP clearance dysfunction in kidney. Conclusion: NT-proBNP levels were associated with adverse outcomes in preeclampsia. NT-proBNP serum levels could be used to predict maternal-fetal complications in preeclampsia.

Abstract

Diabetic retinopathy is a complication of diabetes that is one of the top five causes of blindness in those over 50. The standard treatment is pan-retinal photocoagulation, which is effective but has established side effects. Anti-vascular endothelial growth factor (anti-VEGF) therapy becomes an alternative to avoid the side effects caused by laser therapy. This systematic review aims to know the effectiveness of anti-VEGF therapy compared to the pan-retinal photocoagulation laser therapy in patients with proliferative diabetic retinopathy. This review was carried out using a systematic review checklist on PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses). The articles reviewed were randomized controlled trial articles that met the inclusion and exclusion criteria. Based on inclusion and exclusion criteria, six articles were selected from a total of 215. Patients who received anti-VEGF medication had better visual acuity (positive values), whereas patients who received laser therapy had poorer visual acuity (negative values). Those results are because the laser directs light towards the retina, damaging photoreceptors and retinal cells as well as reducing visual acuity. On the contrary, anti-VEGF prevents damage to retinal endothelial cells and blood leaks in the vitreous by decreasing VEGF expression and thus resulting in improved visual acuity. Anti-VEGF proved to be a more practical alternative therapy in improving visual acuity than pan-retinal photocoagulation for patients with proliferative diabetic retinopathy.

Abstract

Background: Gastric cancer is the third highest cancer mortality globally. This malignancy can result in emergency complications such as gastric perforation which results in pneumo­peritoneum. Currently, there are not many case reports which describe pneumoperitoneum in gastric cancer. In this report, we describe a gastric adenocarcinoma with pneumoperitoneum following jejunostomy feeding. Case Presentation: A 54 years old male presented with abdominal pain, black-colored feces, and body weight decrease as much as 22 kg within a month. The physical examination revealed anemic conjunctiva, distended abdomen with abdominal pain, and pale skin. On the lab examination, the hemoglobin level was 9.9 mg/dL. The peripheral blood smear showed signs of iron deficiency anemia caused by chronic process. An endoscopic examination and biopsy showed a gastric mass with well-moderately differentiated gastric adenocarcinoma. The three positions abdominal x-ray incidentally found that the patient had pneumoperitoneum. Results: Patient was diagnosed with Adenocarcinoma Gaster based on a gastric mass biopsy with hematoxylin-eosin staining. Then, the patient was treated with jejunostomy feeding. Thoracic and 3-position abdominal X-ray examinations revealed incidental findings of pneumoperitoneum and small bowel obstruction. Conclusion: Jejunostomy feeding in gastric cancer patient may result in pneumoperitoneum as a complication. Gastric adenocarcinoma followed by iron deficiency anemia from chronic process post jejunostomy feeding was found with pneumoperitoneum complication.

Abstract

Acne scars are permanent sequelae of acne vulgaris, classified into hypertrophic, keloid, and atrophic types. Treatment strategies include non-invasive, minimally invasive, and invasive modalities. This systematic review evaluates clinical studies from the past decade on surgical and related interventions for acne scars. Using PRISMA guidelines, searches were conducted on PubMed, Scopus, and Epistemonikos with keywords related to "acne scar" and "surgical therapy." Twenty-four studies met the inclusion criteria out of 396 screened articles. The modalities examined encompass TCA CROSS, radiofrequency (RF) micro-needling, fractional microplasma RF, fractional bipolar RF, fractional CO₂ laser, erbium YAG (2940 nm), 1550 nm erbium, 1540 nm erbium glass, subcision, punch techniques, microdermabrasion, needling, platelet-rich plasma (PRP), hyaluronic acid fillers, hybrid energy devices, trifractional technology, and human adipose-derived stem cell exosomes (ASCE). Results demonstrated variable efficacy, with predominantly positive outcomes across modalities. Specific treatments showed better suitability for certain scar types; however, all carry potential risks of adverse effects, emphasizing the need for cautious application and patient management. Limitations include small sample sizes and heterogeneity among studies, which may affect the generalizability of findings. This review highlights the diversity of surgical and minimally invasive options for acne scar management, supporting tailored treatment approaches based on scar morphology and patient factors.

Abstract

Latar Belakang: Derajat keparahan psoriasis vulgaris ditentukan berdasarkan skor Psoriasis Area and Severity Index (PASI) dan Dermatology Life Quality Index (DLQI). Perjalanan psoriasis dapat dipengaruhi oleh abnormalitas sistem imun, kerentanan genetik, lingkungan dan gaya hidup. Vitamin D juga diduga berperan pada perjalanan berbagai penyakit autoimun termasuk psoriasis. Tujuan: Untuk mengetahui korelasi antara kadar vitamin D serum dengan derajat keparahan psoriasis vulgaris berdasarkan skor  PASI dan DLQI. Metode: Penelitian ini merupakan penelitian analitik eksperimental terhadap 10 subjek dan dilakukan pengukuran kadar vitamin D serum dengan metode Enzyme Linked Immunosorbent Assay (ELISA). Data klinis dan derajat keparahan psoriasis vulgaris dilakukan dengan skoring PASI dan DLQI. Data dianalisis secara deskriptif dan disajikan dalam tabel distribusi frekuensi. Pengolahan data menggunakan uji statistik Spearman untuk menilai korelasi antara kadar vitamin D serum dengan derajat keparahan psoriasis vulgaris berdasarkan skor PASI dan DLQI. Hasil: Analisis data menunjukkan tidak ada korelasi antara kadar vitamin D serum dengan derajat keparahan psoriasis vulgaris berdasarkan PASI (nilai p=0,751 dan koefisien korelasi r=-0,116) dan berdasarkan skor DLQI (nilai p=0,751 dan koefisien korelasi r=-0,116). Kesimpulan: Kadar vitamin D serum tidak berkorelasi dengan derajat keparahan psoriasis vulgaris.

Abstract

A case of a 58-year-old woman with lepromatous leprosy and severe ENL reaction is reported. The patient has symptoms of pustules for 1 year and the nodules were getting worse in the last week, accompanied by fever, joint pain, nausea, and vomiting. Physical examination revealed madarosis, infiltrates of both ears, and multiple painful erythematous nodules on the extremities and trunk. Slit skin smear examination showed bacterial index +3 and morphological index of 15%. Histopathological examination revealed neutrophil infiltration and vasculitis in the upper dermis. The patient was diagnosed with lepromatous leprosy and severe ENL reaction. The patient received multidrug therapy and methylprednisolone (62.5 mg/day), which was gradually reduced. The ENL reaction is caused by the deposition of M. leprae antigens and antibodies in small blood vessels, resulting in vasculitis and the release of enzymes that damage tissues. ENL reactions are commonly found in multibacillary MH and occur before, during, and after treatment. This reaction is mediated by cytokines produced by T-helper-2 (Th2). ENL reactions can cause disability.

Abstract

Colorectal cancer (CRC) had the third highest cancer incidence worldwide. Recent researches are targeting specific gene expression as CRC marker. Leucine rich repeat-containing G-protein coupled receptor 5 (LGR5) is one of the genes that regulate tumor metastasis and growth. The aim of this study is to explain the correlation between LGR5, clinical pathology, and prognosis of CRC. This systematic review was arranged using PRISMA for eligible cohort studies and evaluated using Newcastle-Ottawa scale. From 63 articles, 6 articles were selected as eligible articles for data extraction and evaluation. Based on the studies, LGR5 positivity was positively associated with histopathological characteristics, TNM staging, and vascular invasion. Besides, LGR5 expressions in those studies were high and could be positively detected by this marker. LGR5 regulates Wnt/β-catenin signaling pathway promotion in normal colon stem cell. Methylation of Wnt target gene promoter is potent predictor to CRC recurrence thus the expression of LGR5 can be used as CRC marker. LGR5 plays a role as pro-oncogenic factor in colorectal carcinogenesis via prostaglandin E2 and Epidermal Growth Factor signaling. LGR5 induction also could increase cancer cells chemoresistant ability. Thus, LGR5 could determine prognosis of CRC patient and were related to certain clinicopathological features and tumor progressions.
