Abstract

Laboratory parameters in Covid-19 patients may have a relationship with the clinical severity of the patient on arrival. This study aims to Analyzing the relationship between NLR, ALC, and CT values with the clinical severity of Covid-19 patients on arrival. A total of 101 subjects were collected from medical records of RSI Jemursari Surabaya from April to September 2020. The correlation laboratory parameters (NLR, ALC, and CT-value) with the severity of Covid-19 patients (mild, moderate, severe and critical) were analyzed using One Way Anova and Kruskal Wallis. The relationship of NLR with severity (p-value=0.143), ALC with severity (p-value=0.165), and CT-value with severity (p-value= 0.987). Patients with COVID-19 tend to have a high NLR in all clinical conditions. The mean value of the ALC was within normal limits for all degrees of severity. There is no significant relationship between NLR, ALC, CT-value with the severity of Covid-19 patients.

Abstract

Background: Pancreatic β-cells compensate for the increased insulin demand due to increasing glucose levels during pregnancy or in gestational diabetes mellitus (GDM). The enzyme that regulates the rate of gluconeogenesis production is PEPCK. Circulatory and metabolic disturbances in the brain caused by hyperglycemia could affect brain-derived neurotrophic factor (BDNF) expression as an agent for neurodevelopment. Objective:  The purpose of this work is to ascertain the impact of combining nanoemulsions of Rosmarinus officinalis and Centella asiatica on the expression of BDNF and PEPCK. Methods: This study was a true experimental laboratory using zebrafish larvae. The larvae were exposed to 3% glucose and a combination of nanoemulsion extracts. The study sample of zebrafish larvae at 72 hpf (hour post fertilization) was divided into control, glucose, treatment 1, 2, and 3, with the combination of Rosmarinus officinalis and Centella asiatica nanoemulsions at 2.5 μg/ml, 5 μg/ml, and 10 μg/ml, respectively. PEPCK and BDNF expression were measured by the real-time PCR method. Results: The expression of PEPCK increased significantly in the glucose group (3.05 ± 0.27). The combination of Rosmarinus officinalis and Centella asiatica nanoemulsions significantly decreased PEPCK (p-value 0,001 < 0.05). The BDNF level did not significantly differ (p-value 0.253 >0.05) among groups, but the highest level (1.06 ± 0.60) was gained in the T2 group, which received an embryonic medium (EM), 3% glucose, and the combination of Rosmarinus officinalis and Centella asiatica nanoemulsions at 5μg/ml. Conclusion: The combination of Rosmarinus officinalis and Centella asiatica nanoemulsions significantly reduces PEPCK expression but does not affect BDNF expression.

Abstract

Abstract: Breast milk is the optimal source of nutrition for infants. The provision of exclusive breastfeeding can be influenced by various factors, including the support of the husband. This study aims to explore the experiences of husbands in supporting exclusive breastfeeding from a socio-cultural perspective. Using a qualitative approach, the study involved semi-structured interviews with 10 participants who were husbands of mothers with infants aged 0-6 months. The data collected were analyzed using Atlas.ti 9.0 software with a thematic analysis approach. Three main themes emerged from the analysis: the husband's role in breastfeeding preparation during pregnancy, direct emotional support from the husband, and the husband's readiness to assist in the breastfeeding process. The findings of this study suggest that the active involvement of husbands in supporting mothers, both emotionally, practically, and in preparing for breastfeeding, significantly influences the success of exclusive breastfeeding. The study recommends increasing social awareness to promote the involvement of husbands in supporting the successful implementation of exclusive breastfeeding.Keywords: Breastfeeding, fathers, qualitative study

Abstract

Screening preschool children's growth and development can help detect deviations or delays early, allowing for timely intervention before the child experiences worsening conditions or permanent disorders. Developmental delays in children may include speech and language disorders, autism, and attention deficit disorder. Therefore, it is essential to conduct developmental screenings appropriate to the child's age. This community service program implemented developmental screening using the Developmental Pre-Screening Questionnaire (KPSP) instrument for early childhood at PAUD Anggrek, Surabaya. A total of 26 children participated in this screening activity. The screening results were categorized based on the KPSP assessment: 22 children (85%) were classified as age-appropriate, 3 children (11%) were classified as doubtful, and 1 child (4%) showed developmental deviations. During the evaluation process, each child received a report that was submitted to their PAUD teacher and parents. This report contained the results of the developmental evaluation along with recommendations for age-appropriate stimulation activities.Skrining tumbuh kembang anak prasekolah dapat mendeteksi penyimpangan atau keterlambatan perkembangan anak sehingga dapat dicegah, sebelum anak jatuh dalam kondisi yang lebih buruk bahkan gangguan yang permanen. Kejadian keterlambatan perkembangan anak meliputi gangguan bicara/bahasa, autism, serta gangguan pemusatan perhatian. Dengan demikian, penting untuk dilakukan skrining perkembangan anak sesuai dengan usianya. Pengabdian masyarakat ini menggunakan metode pelaksanaan skrining perkembangan dengan instrumen Kuesioner Pra Skrining Perkembangan (KPSP) kepada anak usia dini di PAUD Anggrek Kota Surabaya. Sebanyak 26 orang anak mengikuti kegiatan skrining ini. Hasil skrining diperoleh penilaian berdasarkan kategori hasil pada instrument KPSP, yakni sesuai umur sebanyak 22 anak (85%), meragukan sebanyak 3 anak (11%), dan penyimpangan sebanyak 1 anak (4%). Dalam proses evaluasi, setiap anak akan menerima rapor yang disampaikan kepada pengajar PAUD dan orang tua. Rapor ini berisi hasil evaluasi perkembangan anak dan saran tentang rekomendasi stimulasi berdasarkan usia anak.

Abstract

Gestational diabetes mellitus (GDM) is hyperglycemia during pregnancy, which can increase oxidative stress and reactive oxygen species (ROS). This oxidative stress can adversely affect fetal developmental processes, including body length, ocular development, blood flow, and heart rate. Curcuma longa extract, containing curcumin, has been shown to possess antidiabetic and antioxidant properties. Nanoemulsions have demonstrated efficacy in enhancing the pharmacokinetics and pharmacodynamics of herbal compounds. This study aimed to evaluate the effect of a nanoemulsion of Curcuma longa extract on body length, ocular apoptosis, blood flow, and heart rate in zebrafish (Danio rerio) embryos exposed to high glucose concentrations. Zebrafish has many benefits as it has 70% orthologous genes with humans, high fecundity, and rapid embryogenesis. Nanoemulsions were prepared using the self-nanoemulsifying drug delivery system (SNEDDS) technique. Embryos were allocated to five groups: control, 3% glucose, and 3% glucose with 1.25, 2.5, or 5 µg/mL nanoemulsion of Curcuma longa extract. The results showed that a 1.25 µg/mL nanoemulsion of Curcuma longa extract reduced phosphoenolpyruvate carboxykinase (PEPCK) expression, increased body length, and mitigated developmental process disorders by decreasing ocular apoptosis and maintaining blood flow and heart rate in high glucose-induced zebrafish embryos. These findings suggest that a nanoemulsion of Curcuma longa extract may have potential therapeutic applications in managing the adverse effects of hyperglycemia on fetal development in GDM.
