Abstract

Penelitian ini bertujuan untuk mengevaluasi efektivitas mouthcare dengan Chlorhexidine dalam mencegah VAP pada pasien yang menggunakan ventilasi mekanik. Penelitian ini menggunakan metode tinjauan literatur sistematis dari berbagai studi yang relevan. Hasil analisis menunjukkan bahwa penggunaan Chlorhexidine secara signifikan dapat mengurangi kejadian VAP. Selain itu, Chlorhexidine terbukti efektif dalam menurunkan jumlah koloni bakteri di rongga mulut, yang merupakan sumber utama patogen penyebab VAP. Oleh karena itu, penerapan mouthcare dengan Chlorhexidine dapat direkomendasikan sebagai bagian dari protokol pencegahan VAP pada pasien yang menerima ventilasi mekanik, sehingga dapat meningkatkan hasil klinis dan mengurangi beban perawatan di rumah sakit. Simpulan penggunaan Chlorhexidine 0,2% lebih efektif dalam mengurangi kolonisasi bakteri di mulut dan faring dibandingkan dengan konsentrasi yang lebih rendah. Kata Kunci: Chlorhexidine, Mouthcare, Pencegahan Infeksi, Ventilator-Associated Pneumonia (VAP), Ventilasi Mekanik.

Abstract

An important aspect in the management of chronic kidney disease is the establishment of adequate vascular access for hemodialysis. Arteriovenous fistula (AV-shunt) is the main and most ideal option for vascular access in the hemodialysis process. This study collected research on the assessment of the effectiveness of hand training programs on arteriovenous shunt (AV-Shunt) maturation in patients with chronic kidney disease (CKD). Searches were conducted on ScienceDirect and Google Scholar. Only experimental studies examining the effects of hand exercises after surgery were reviewed. Data that has been filtered and reviewed by researchers. This study aims to summarize the effects of various hand training programs in the process of AV-Shunt/AVF maturation in CKD patients through a literature review. This study uses the literature review method by analyzing articles from trusted electronic databases such as ScienceDirect and Google Scholar with the keywords maturation AV-Shunt, hand exercises and CKD. The selected articles met the inclusion criteria, namely focusing on hand training interventions analyzed with the PICOT approach. The results of the study show that regular exercise can increase blood diameter and flow, resulting in the patentability of vascular access for the hemodialysis process, which ultimately improves the quality of life. A structured exercise program is an effective intervention in the maturation process of AV-Shunt in CKD patients.

Abstract

Introduction: Herniated Nucleus Pulposus is a tear in the annulus fibrosus of the intervertebral disc, and the nucleus pulposus comes out through the tear in the annulus fibrosus, resulting in compression of the spinal nerve roots. Clinical symptoms of HNP vary according to several factors such as the location (level) of herniation and nerve compression. HNP can cause pain, but the main clinical manifestations are radiating pain and sensitive changes that include the distribution of nerves. Several researchers have recommended methods to relieve pain in HNP through therapy to effective medical treatment

Abstract

The application of chest physiotherapy in children with bronchopneumonia can help overcome the problem of ineffective airway hygiene. Bronchopneumonia is an infection of the lungs that causes phlegm, which can interfere with the breathing process and cause difficulty breathing. This study aims to apply the application of chest physiotherapy in improving airway hygiene in children with bronchopneumonia at Hospital X, South Tangerang City. The method used in this study is a descriptive case study with a qualitative approach. Chest physiotherapy interventions, including postural drainage, chest percussion, and vibration, were carried out for three consecutive days. The results showed a significant increase in the patient's airway hygiene, which was indicated by reduced coughing, increased oxygen saturation, and easier phlegm expectoration. This therapy also helps improve breathing patterns and reduce symptoms of bronchopneumonia. It is estimated that the application of chest physiotherapy can be an effective choice in the treatment of children with bronchopneumonia.

Abstract

Nurses have a major role in providing nursing care to patients, one of which is that nurses can provide nursing actions to reduce pain levels in patients while in the treatment room, one of which is by conducting deep breathing relaxation nursing interventions. The purpose of this study was to determine the effect of deep breathing relaxation nursing interventions on reducing the pain scale in post-cholecystectomy patients at Hospital X. This study was conducted in the inpatient room on December 9-13, 2024. The method used in providing nursing care to post-cholecystectomy patients was carried out for 5 minutes. Respondents in this report were 1 adult patient aged 45 years with female gender and an adult aged 39 years with female gender. The results of the report show that after the assessment was carried out, it was found that post-cholecystectomy patients who were given deep breathing relaxation techniques could reduce pain levels with an initial scale of 5/10 to 3/10, while patients who were not given deep breathing relaxation techniques still felt the same pain with an initial pain scale of 4/10 still on a scale of 4/10, therefore nurses play an important role in providing nursing care to reduce pain levels in post-cholecystectomy patients.

Abstract

Diabetes melitus merupakan penyakit metabolik yang memiliki gejala utama peningkatan kadar gula darah yang sering dikenal dengan istilah hiperglikemia. Tujuan penulisan ini adalah untuk memperoleh gambaran asuhan keperawatan gawat darurat sirkulasi pada pasien Diabetes Melitus. Metode penulisan ini menggunakan desain penelitian deskriptif analitis dengan pendekatan studi kasus. Sampel yang digunakan pada studi kasus ini adalah Ny. A dan Ny. R dengan diagnosis diabetes melitus tipe 2. Hasil: Setelah dilakukan asuhan keperawatan selama 1x4 jam pada Ny. A dan Ny. R didapatkan hasil yang sama yaitu tekanan darah mengalami penurunan yang dapat diartikan tekanan darah membaik, turgor kulit cukup elastis, CRT < 3 detik, skala nyeri Ny. A sudah pada skala 3, ketidakstabilan kadar glukosa darah sebagian teratasi karena hasil Glukosa Ad Random masih tinggi. Nyeri akut sebagian teratasi karena masih terdapat nyeri meskipun sudah menurun. Kesimpulan: Pemberian terapi cairan pada penanganan gawat darurat sirkulasi pada pasien diabetes melitus sudah cukup untuk mengatasi peningkatan osmolaritas yang mengganggu keseimbangan cairan dan elektrolit tubuh pada hiperglikemia.

Abstract

Background: Hypertension is the main and most common cause of acute coronary syndromes such as "heart attacks". Adhering to a hypertension diet is an important thing to do to keep blood pressure stable. Purpose: the purpose of the study was to determine the relationship between the level of knowledge about hypertension and family support with dietary compliance of hypertensive patients at Jakarta Hospital. Method: This research method is quantitative research with a descriptive correlational design and uses a cross-sectional study approach. The sample of 169 patients, using a purposive sampling technique. Collecting data using a questionnaire. Result: From the results of the univariate analysis, the characteristics of the majority of respondents aged > 60 years old (64%), male (54%), good knowledge (47%), poor family support (41%), and dietary compliance (63%). Bivariate analysis using Kendal's tau C test showed a significant relationship between knowledge about hypertension and family support with dietary adherence in hypertensive patients, with each p-value = 0.000. Conclusion: Patients with good knowledge and family support, the patient's adherence to a hypertension diet.

Abstract

ABSTRAK Program Usaha Kesehatan Sekolah (UKS) merupakan upaya promotif dan preventif yang dilaksanakan di lingkungan sekolah untuk meningkatkan derajat kesehatan peserta didik. Salah satu bentuk pelayanan UKS adalah kegiatan screening kesehatan atau penjaringan, yang bertujuan untuk mendeteksi dini permasalahan kesehatan siswa. Kegiatan ini dilaksanakan di lima sekolah dasar wilayah kerja Puskesmas Kecamatan Johar Baru, Jakarta Pusat, pada tanggal 5–7 Maret 2024. Metode yang digunakan adalah deskriptif kuantitatif dengan teknik total sampling, melibatkan 523 siswa kelas 4 hingga 6. Kegiatan dilakukan dalam dua tahap, yaitu pendataan awal dan pemeriksaan langsung yang mencakup kebersihan diri, kesehatan gigi dan mulut, status gizi, penglihatan, pendengaran, serta indikasi anemia. Data dianalisis secara deskriptif dan diuji secara inferensial menggunakan uji Chi-Square (χ²). Hasil menunjukkan bahwa 70% siswa memiliki kuku kotor/panjang, 60% mengalami masalah gigi dan mulut, 15,1% memiliki gangguan penglihatan, 8% terindikasi anemia, dan 14% mengalami status gizi tidak normal. Uji Chi-Square menunjukkan adanya hubungan signifikan antara jenis kelamin dan kejadian karies gigi (χ² = 4,61; df = 1; p = 0,032), namun tidak signifikan pada anemia (χ² = 1,80; df = 1; p = 0,179). Kesimpulannya, masih banyak siswa yang memerlukan perhatian khusus dalam aspek kebersihan dan kesehatan. Diperlukan intervensi edukatif dan pendampingan berkelanjutan dari guru, orang tua, serta kolaborasi lintas sektor dengan puskesmas untuk memperkuat perilaku hidup bersih dan sehat di lingkungan sekolah. Kata Kunci: Screening Dini, UKS, PHBS, Siswa Sekolah Dasar, Kesehatan Anak  ABSTRACT The School Health Unit (UKS) is a promotive and preventive program implemented in schools to improve the health status of students. One of its services is health screening, aimed at the early detection of health problems among students. This activity was carried out in five elementary schools under the jurisdiction of the Johar Baru Community Health Center, Central Jakarta, from March 5 to 7, 2024. This study employed a descriptive quantitative method with a total sampling technique involving 523 students from grades 4 to 6. The screening was conducted in two stages: initial data collection and direct examinations, including assessments of personal hygiene, oral health, nutritional status, vision, hearing, and signs of anemia. Data were analyzed descriptively and further tested using the Chi-Square (χ²) statistical test. Results showed that 70% of students had dirty/long fingernails, 60% experienced oral health problems, 15.1% had vision impairment, 8% were suspected of anemia, and 14% had abnormal nutritional status. Chi-Square analysis indicated a significant association between gender and the incidence of dental caries (χ² = 4.61; df = 1; p = 0.032), but no significant association with anemia (χ² = 1.80; df = 1; p = 0.179). In conclusion, many students require more attention regarding personal hygiene and general health. Sustainable educational interventions and support from teachers, parents, and cross-sector collaboration with health centers are essential to foster clean and healthy living behaviors in schools. Keywords: Early Screening, School Health Unit, Personal Hygiene, Elementary Students, Child Health

Abstract

Introduction: Chronic kidney disease, which involves lasting impairment of kidney function, can cause respiratory issues among various other systemic complications. Breathlessness is a common symptom that patients face, and if it goes unaddressed, it can worsen their overall health condition.. Objectives: The aim of this study is to assess the effectiveness of the semi-Fowler position in boosting oxygen saturation and decreasing the respiratory rate in chronic kidney disease patients receiving treatment in the Emergency Department. Methods: This investigation adopted a descriptive case study method focusing on two individuals who experienced breathing difficulties due to chronic kidney disease. Interventions included the provision of oxygen and repositioning the patients into the semi-Fowler position for four hours of monitoring. The gathered data included oxygen saturation levels and respiratory rates measured before and after the intervention. Results: After being positioned in the semi-Fowler setup, both patients exhibited notable improvements in oxygen saturation and reductions in respiratory rates. Oxygen saturation levels rose from 83–94% to 99–100%, while the respiratory rate declined from 30–32 breaths per minute to 26–28 breaths per minute. Conclusions: Positioning patients in the semi-Fowler posture has been shown to effectively enhance respiratory function in individuals with chronic kidney disease within the emergency setting. This method can serve as a first-line non-invasive nursing intervention to promote patient comfort and help maintain the stability of their condition.

Abstract

Introduction: Hypertension is a non-communicable disease characterized by systolic blood pressure ≥140 mmHg or diastolic blood pressure ≥90 mmHg and may cause serious complications in vital organs. Adherence to antihypertensive medication is a key factor in controlling this condition. Objectives: This study aimed to examine the relationship between demographic factors, knowledge, motivation, and adherence to antihypertensive medication among patients at Kebayoran Baru Community Health Center. Methods: A cross-sectional quantitative study was conducted with 64 respondents. Data were collected using the 8-item Morisky Medication Adherence Scale and analyzed with Kendall’s Tau-C and Chi-Square tests. Results: Most respondents were aged 46–80 years (81%), male (51.6%), had higher education (42.2%), and had lived with hypertension for 1–5 years (75%). Although 73.4% had high knowledge and 85.9% high motivation, medication adherence was low (60.7%). Significant associations were found between adherence and education (p = 0.001), knowledge (p = 0.041), and motivation (p = 0.046). No significant associations were found with age, gender, occupation, or duration of hypertension. Conclusion: Education, knowledge, and motivation were significantly associated with adherence to antihypertensive medication, while other demographic factors were not. Further studies should explore additional variables, such as attitudes, family support, and the role of healthcare providers, as well as develop engaging and accessible educational strategies to improve adherence.
