Abstract

Pendekatan lean adalah metode pendekatan sistemik yang digunakan untuk menghilangkan waste yang terjadi dalam pelayanan dan meningkatkan efesiensi pelayanan di suatu instalasi. Proses pelayanan kesehatan disuatu rumah sakit terdapat banyak sekali pemborosan terjadi, contohnya mengulangi kegiatan untuk perbaikan diakibatkan adanya kesalahan, meminta pasien untuk menginformasikan data pasien secara berulangkali untuk memastikan ketetapan informasi, dan sebagainya. Tujuan penelitian ini untuk mengidentifikasi waste dan akar penyebab waste yang terjadi di dalam Instalasi Farmasi Rawat Jalan RSUD Soehadi Prijonegoro Sragen. Metode penelitian ini adalah penelitian studi kasus dengan pendekatan kualitatif. Penelitian ini dilakukan di RSUD Soehadi Prijonegoro Sragen pada bulan Juni-juli 2022 dengan tahapan penelitian yaitu observasi dibuat dalam bentuk VSM, penyebaran kuisioner value pasien dan waste, melakukan wawancara dengan metode 5 why untuk mengetahui akar penyebab dan waste kritis, serta usulan perbaikan. Hasil penelitian menunjukan waste defect 21,1%. Transportation 18,1%, overproduction 16,3%,. Akar penyebab dari waste defect adalah kurang fokusnya petugas dan kurangnya sumber daya manusia, waste transportation adalah kekosongan obat dan tata letak yang kurang rapi, waste waiting adalah pasien harus mengantri di kasir dan pengambilan obat. Usulan perbaikan waste defect adalah penambahan SDM, waste transportation adalah mengatur tata letak rak obat yang lebih rapi, waste overproduction adalah tidak membuat racikan sebelum resep masuk.

Abstract

Tea leaves (Camellia sinensis) are one of the plants that contain flavonoids, tannins, alkaloids, and secondary metabolites of catechins that have the potential as antibacterials. Extraction using the Ultrasonic Assisted Extraction (UAE) method is influenced by the duration and concentration of the solvent. The purpose of this study was to determine the effect of different durations and solvent concentrations on the antibacterial activity of Escherichia coli tea extract using the UAE method. Tea leaves were extracted using the UAE method with sonication waves using 50%, 70%, and 96% ethanol solvents with a duration of 15, 30, and 45 minutes, as much as 25 grams in 250 ml. The extract was dissolved in 10% DMSO and an antibacterial activity test was carried out with a concentration of 75% as much as 50 µL per well. The negative control in the bacterial activity test used 10% DMSO, and used ciprofloxacin as a positive control. The antibacterial activity test was determined using the well diffusion method to determine the diameter of the inhibition zone. The test results data were analyzed statistically using SPSS. The results showed that tea leaves have antibacterial activity against Escherichia coli bacteria, the resulting yield related to variations in solvent concentration and extraction duration did not differ significantly, however the results of the inhibition test against Escherichia coli bacteria showed significant differences. Tea leaf extract extracted with UAE with 70% ethanol for 30 minutes had the highest inhibition diameter resulting in antibacterial activity with an average value of 13.475mm ± 0,38.

Abstract

Infectious diseases caused by pathogenic bacteria such as Escherichia coli are increasing, especially due to antibiotic resistance. One potential alternative treatment is the use of natural antibacterial compounds, such as those found in soursop leaves (Annona muricata L.). Soursop leaves contain various bioactive compounds, such as flavonoid polyphenols, acetogenins, and tannins, which have antibacterial activity. This study aims to of soursop leaf-based probiotic kombucha fermentation in inhibiting the growth of Escherichia coli. In this study, probiotic kombucha was produced through a fermentation process using Symbiotic Culture of Bacteria and Yeast (SCOBY) with variations in temperature, time, and soursop leaf extract concentration parameters. Activity testing was carried out using the Minimum Inhibitory Concentration (MIC) and Minimum Bactericidal Concentration (MBC) methods to assess its antibacterial effectiveness. The results showed that soursop leaf kombucha fermentation had significant antibacterial activity against Escherichia coli. The disc diffusion activity test obtained in this study was 9.1 mm (15% concentration); 5.10 mm (10% concentration); and 2.3 mm (5% concentration) with different concentration variations on the 7th day of fermentation. The resulting inhibition zone diameter for E. coli bacteria indicates that kombucha's antibacterial activity is in the moderate category, as it produced the highest inhibition zone of 9.1 mm. This study demonstrates that soursop leaf kombucha has the potential to be developed as a functional probiotic beverage with effective and natural antibacterial effects.
