Abstract

The background of this journal explores the urgency of improving pediatric surgery services through infrastructure development, increasing the competence of medical personnel, and implementing better service standards with the end result of increased visits and patient satisfaction. In addition, this study also highlights the challenges faced and effective strategies to overcome obstacles in efforts to optimize pediatric surgery services at Persahabatan Hospital (RSP). This study aims to explain the factors related to the optimization of pediatric surgery services at RSP and formulate the concept of optimizing pediatric surgery services at RSP. This study uses a qualitative research design with descriptive analysis. This study was carried out by observation and filling out questionnaires by the families of pediatric surgery patients and hospital management related to pediatric surgery services at RSP, with a SWOT analysis. The average strength rating is 4.49; The average weakness rating is 3.55; The average opportunity rating is 4.07; The average threat rating is 3.49. The most appropriate strategy for RSUPP is the SO strategy or aggressive strategy (0.475; 0.299). The Strengths Opportunities method is most suitable for optimizing pediatric surgery services at RSP, namely the use of strategic locations, medical personnel, and complete infrastructure. Research on pediatric surgery services has never been done in Indonesia. Unfortunately, this research was only carried out at RSP, and could not be applied nationally.

Abstract

Latar Belakang: Kekurangan tenaga kerja merupakan salah satu masalah global di bidang sumber daya manusia. Salah satu penyebab terjadinya kurangnya tenaga kerja adalah tingginya turnover rate pada sebuah organisasi. Turnover diawali dengan niatan untuk meninggalkan pekerjaan atau yang disebut dengan turnover intention. Banyak faktor yang mempengaruhi turnover intention salah satunya adalah kepuasan kerja. Angka turnover di Rumah Sakit Akademik Universitas Gadjah Mada cukup tinggi setiap tahunnya. Diduga hal ini terjadi dikarenakan belum puasnya pegawai dengan pekerjaannya. Hal ini ditandai dengan angka keterlambatan dan ketidakhadiran yang masih cukup tinggi dan juga permasalahan terkait status kepegawaian. Permasalahan ini perlu diperbaiki karena angka turnover yang tinggi akan membawa dampak yang negatif kepada rumah sakit. Tujuan: Mengidentifikasi faktor terkait kepuasan kerja perawat serta hubungan antara kepuasan kerja dengan turnover intention perawat Rumah Sakit Akademik Universitas Gadjah Mada. Metode: Jenis penelitian ini adalah mixed methods dengan desain sequential explanatory. Penelitian ini dilakukan pada bulan Mei-Juni 2023. Pendekatan kuantitatif dilakukan dengan rancangan analitik cross sectional study dengan melakukan survei kuesioner kepada 182 perawat Rumah Sakit Universitas Gadjah Mada. Instrumen pada pendekatan kuantitatif ini adalah Minnesota Satisfaction Questionnaire (MSQ) untuk mengukur kepuasan kerja dan Turnover Intention Scale-6 (TIS-6) untuk mengukur turnover intention. Sedangkan pendekatan kualitatif dilakukan menggunakan metode wawancara mendalam kepada 4 orang pihak manajemen Rumah Sakit Universitas Gadjah Mada. Hasil: Tingkat kepuasan kerja perawat di RS Akademik UGM adalah 100% sangat puas. Persentase perawat dengan yang memiliki keinginan untuk meninggalkan pekerjaan sebesar 16.5% dan yang tidak memiliki niatan untuk pindah kerja adalah sebesar 83.5%. Hasil uji Spearman’s Rank Correlation pada kepuasan kerja dan turnover intention menunjukkan p value sebesar 0,000 (<0,05) dengan koefisien korelasi yaitu -0.416. Faktor kompensasi pada kepuasan kerja merupakan faktor yang paling mempengaruhi turnover intention. Kesimpulan: Terdapat hubungan tidak searah antara kepuasan kerja dengan turnover intention pada perawat RS Akademik UGM. Dengan diketahuinya bahwa kepuasan kerja perawat RS Akademik UGM berhubungan dengan niatan atau keinginan mereka untuk meninggalkan pekerjaannya, maka penting bagi pihak manajemen untuk selalu menjaga bahkan meningkatkan kepuasan kerja dari perawat RS Akademik UGM guna menurunkan jumlah perawat yang berniat untuk melakukan resign. Abstract

RSUD Bagas Waras Klaten has some potential wastes of process which leads to the need of lean management to improve service quality and efficiency. Although implementation of lean management has been proven in improving quality, patient safety, effectivity, and cost efficiency, it is also potential to fail if some readiness elements are not met. Lean management readiness assessment is required to describe inhibiting as well as supporting elements in implementing lean management.

Abstract

Background: Palliative care is integrated medical care that is active and comprehensive, with a multidisciplinary approach to reduce the pain of terminally ill patients at the end of their lives. Persahabatan Hospital, as a national referral Type A hospital, receives many patients in severe and complex conditions, some of whom require palliative care. Early identification of Palliative Care Screening Tool (PCST) screening can result in more effective interventions and more optimal care in improving quality of life, reducing length of stay, and intensive care unit (ICU) admission rates. This study aims to assess the early implementation outcomes of the PCST in inpatients at Persahabatan Hospital. Method: This research is a sequential explanatory mixed methods design that examines how the implementation evaluation of PCST in palliative care at Persahabatan General Hospital is conducted, utilizing both retrospective quantitative and qualitative approaches. The quantitative approach involves utilizing secondary data from medical records and the PCST at Persahabatan General Hospital in Jakarta from January to December 2023. A random sampling technique was applied to select patients diagnosed with palliative care conditions, including severe liver disease,end-stage kidney disease, chronic obstructive pulmonary disease (COPD), advanced heart failure,and other chronic illnesses. At the same time, the qualitative approach employs the focus group discussion analysis method.Results: Out of 82 patients who underwent PCST screening, 56 patients underwent early screening, and 26 patients underwent late screening. The median length of stay for patients with early screening was 4 days, whereas for those with late screening, it was 18 days (p 0.001). Early screening had a relative risk of 4.34 (1.72–26.2) for successful palliative screening compared to late screening. Based on the analysis of the focus group discussion results, the main issues in implementing early screening were a lack of understanding of the screening process and difficulties in its implementation

Abstract

Latar Belakang: Turnover karyawan merupakan salah satu cara untuk mengekspresikan ketidakpuasan kerja dan mempengaruhi pengelolaan sumber daya manusia sebagai aset penting dalam industri Rumah Sakit. Dampak negatif yang ditimbulkan terkait biaya dan hilangnya karyawan yang memiliki keahlian, di mana faktor kepemimpinan dan kepuasan kerja berperan terhadap turnover intention karyawan. Rumah Sakit Santa Elisabeth yang merupakan Rumah Sakit swasta keagamaan kelas D di Bantul, turut menilai kepuasan dan loyalitas karyawan lewat angka turnover karyawan, akan tetapi hal ini menjadi salah satu indikator yang belum dapat mencapai standar. Tujuan: Mengevaluasi peran gaya kepemimpinan dan kepuasan kerja terhadap turnover intention karyawan. Metode: Penelitian ini merupakan penelitian studi kasus eksploratoris dengan desain penelitian kasus tunggal terpancang. Analisis data menggunakan metode statistik regresi linear berganda antar variabel turnover intention, gaya kepemimpinan, dan kepuasan kerja dan dilanjutkan dengan melakukan wawancara mendalam kepada responden yang memiliki hasil survei anomali. Data hasil wawancara mendalam dilakukan koding, identifikasi kategori, dan tema. Hasil: Hasil uji t pada SPSS didapatkan nilai signifikansi gaya kepemimpinan sebesar 0,362 (> 0,05) dan nilai signifikansi kepuasan kerja sebesar 0,000 (< 0,05). Hasil uji ANOVA didapatkan nilai signifikansi sebesar 0,000 (< 0,05). Berdasarkan wawancara mendalam didapatkan bahwa ketidakpuasan kerja dan faktor keluarga berperan terhadap turnover intention. Kesimpulan: Secara simultan gaya kepemimpinan dan kepuasan kerja berperan terhadap turnover intention karyawan di Rumah Sakit Santa Elisabeth. Gaya kepemimpinan tidak berperan terhadap turnover intention karyawan di Rumah Sakit Santa Elisabeth, namun kepuasan kerja berperan terhadap turnover intention karyawan di Rumah Sakit Santa Elisabeth. Abstract

Background: Physician involvement in leadership roles within healthcare institutions has been declining worldwide, despite evidence linking physician leadership with improved healthcare outcomes. Objective: To examine physicians’ leadership roles, individual characteristics, and factors influencing their leadership effectiveness in government-owned healthcare institutions in Indonesia, and to propose a contextually appropriate leadership development model. Methods: A mixed-method design was used. Phase I was a qualitative study conducted through in-depth interviews and focus group discussions with 49 physicians from 31 institutions, analyzed using thematic framework analysis (NVivo 12, COREQ guidelines). Phase II was a quantitative survey involving 358 physicians from 25 institutions, analyzed using structural equation modeling with partial least squares (SEM-PLS). Results: Qualitative findings revealed structural, personal, and managerial barriers to leadership engagement, including time constraints, administrative burdens, inadequate incentives, and political influences. However, physicians were perceived as strategic leaders due to their technical expertise, tiered experiences, and credibility in clinical management. The quantitative model demonstrated strong reliability and validity. Individual differences (experience, efficacy) significantly influenced motivation to lead (β=0.577), leadership behavior (β=0.496), and leadership outcomes (β=0.176, p<0.001). Motivation (β=0.197) and behavior (β=0.604) also significantly predicted leadership outcomes. Conclusion: Despite low participation, physicians possess leadership characteristics that positively contribute to leadership effectiveness. Physician leadership development should be institutionally driven, integrating managerial competencies with technical expertise, while addressing structural and motivational barriers.

Abstract

Background: Remuneration is an important strategy to improve the competence of hospital services. Remuneration aims to motivate employees which can be achieved if the remuneration system is implemented fairly, but the gap between the ratio of the number of doctors and the average points to rates causes’ physician dissatisfaction toward the remuneration system.  Analyze satisfaction level and measure remuneration satisfaction factors on Pediatricians and Obstetrics and Gynecologists at ABC Hospital Jakarta.  Methods:This study used quantitative methods with multiple linear regression analysis measured by the Equity Scale and Remuneration Satisfaction Scale. Interviews were used to obtain comprehensive analysis results from respondents who were in the lower and upper limits of the questionnaire results. The interview also involved the management of ABC Hospital Jakarta.Results:Total 44 respondents consisting of 30 Pediatricians and 14 Obstetricians and Gynecologists, showed dissatisfaction with the remuneration system (45,46%), neutral (31,53%), and satisfied (23%). There was a relationship between perceptions of external fairness (P= 0,003) and procedural fairness (P= 0,002) with remuneration satisfaction. There was no relationship between perceptions of internal justice (P= 0,929) and perceptions of individual justice (P= 0,130) with remuneration satisfaction. These results were explored using interviews and the causes of doctor dissatisfaction due to the gap in the achievement of the Individual Performance Index, lots of doctor examinations that were not listed in the rates, and remuneration points did not consider the complexity of each job. Meanwhile, the dissatisfaction felt by the management was due to the lack of measurement of quality indicators in the remuneration system.Conclusion: External and procedural justices are factors that affect the satisfaction of remuneration felt by specialist-physician. The remuneration obtained at ABC Hospital Jakarta is lower than other Government Hospitals of the same type. In addition, there are binding rules, doctors' lack of understanding of the remuneration system, and lack of socialization. The management needs to formulate a more detailed remuneration system based on applicable regulations by adjusting the hospital situation.

Abstract

Purpose: CHD is a condition where heart abnormalities develop before birth and can lead to severe complications such as heart failure and premature death, especially among younger adults. Delayed diagnosis and treatment worsen the situation for CHD patients. Early screening can prevent CHD and ensure timely interventions. This study discusses the latest advancements in CHD screening for primary school children and the valuable insights gained from the program. Method: The CHD screening program in Yogyakarta province involved three stages: consolidation, implementation, and incorporation. During the consolidation stage, key decision-makers in the province and local health offices were involved. Primary healthcare workers and primary school teachers were trained to deliver the screening during the implementation stage. Stakeholders, health workers, and teachers were invited to discuss the results, and the Provincial Governor's office was engaged during the incorporation stage. Results: In 2018–2019, 18 CHDs were identified from 6,116 school-age children in 4 districts in Indonesia. The screening program was led by academics in the Universitas Gadjah Mada through 3 stages: consolidation, implementation, and incorporation. Strong collaboration with local stakeholders has contributed to the program's success in engaging 130 schools, 60 community health centers, and 190 health workers. Several opportunities for improvement are: strengthening the capacity and number of health providers at health centers to screen CHDs using 12-lead electrocardiography, addressing patients' reluctance to visit the health facility due to the COVID-19 pandemic, and motivating parents to have their children checked. Conclusion: CHD screening using 12-lead electrocardiography and auscultation should be part of primary school health screenings. Academics can train health workers, and local governments can provide funding and prepare health facilities for follow-up treatment.

Abstract

Purpose: The 2018 healthcare waste management crisis catalyzed a critical review of the concerns expressed by the Health Office (dinkes) and healthcare facilities regarding healthcare waste management in the Special Region of Yogyakarta (DIY). Due to this crisis, the Ministry of Health hired Universitas Gadjah Mada (UGM) to look into potential solutions and promote DIY government policy responses. This paper examines the management strategy during crisis times and possible alternative solutions. Methods: This paper uses focused group discussions reports involving separate groups of (1) environmental health officials from community health centers, (2) hospitals, (3) environment health officials of district health authorities, and (4) cross-sectoral province officials in the Yogyakarta Special Region. It is part of a project 'A case study of strengthening regional-based medical waste management model', fund from the Environmental Health Directorate, Directorate General of Public Health, Ministry of Health (Project KN 01.03/6.1/0198/2019). Result: A simulation of policy options based on health facility managers suggests that a province-based system is the most profitable in the long term for DIY, with several possible options. The national policy roadmap was considered inadequate to respond to DIY's urgent local needs. Furthermore, the series of meetings succeeded in forming an informal forum between health facilities, provincial health offices, and associations of hospital environmental sanitation experts, monitoring medical waste management. Conclusion: The 2018 medical waste management crisis led to the formulation of policy response choices tailored to the capacity of DIY. These choices considered the expenditures and legal sanctions faced by healthcare facilities and the economic value of a region-based waste system for local government authorities. This comprehensive approach highlights the importance of local capacity. It needs to shape effective and sustainable medical waste management policies, underscoring the necessity of region-specific strategies in the face of national health crises.

Abstract

Dalam rangka meningkatkan efektivitas dan optimalisasi proses verifikasi klaim, BPJS Kesehatan membentuk sistem verifikasi klaim digital yang disingkat VEDIKA. Sejak diimplementasikannya sistem Vedika pada tahun 2018, terdapat peningkatan jumlah klaim yang dipending setelah implementasi Vedika di RSUD dr. H. Jusuf SK. Sampai saat ini masih belum ada evaluasi mengenai implementasi Vedika yang telah berjalan lebih dari 5 tahun, baik dari pihak RSUD dr. H. Jusuf SK maupun dari pihak BPJS Kesehatan Cabang Tarakan. Penelitian ini merupakan penelitian kualitatif dengan desain Studi Kasus. Data primer diperoleh dari wawancara mendalam. Data sekunder berupa telaah dokumen pendukung. Dalam pelaksanaan Vedika di RSUD dr. H Jusuf SK Kota Tarakan tahun 2022 terdapat permasalahan klaim yang menyebabkan klaim tidak dibayarkan. Faktor penyebab permasalahan klaim yang paling sering ditemukan adalah faktor verifikator, aplikasi Vedika dan kelengkapan dokumen klaim yang diajukan. Temuan ini dapat menjadi masukan untuk BPJS Kesehatan agar bisa menyeragamkan cara verifikasi yang dilakukan oleh masing-masing verifikator agar hasil verifikasi tidak jauh berbeda. BPJS Kesehatan juga dituntut untuk memperbaiki aplikasi agar permasalahan dalam penerapan Vedika bisa berkurang. Temuan ini juga menjadi masukan bagi RS untuk menyusun dan mengembangkan rekam medis elektronik untuk meminimalisir permasalahan klaim yang disebabkan oleh rekam medis. Dalam penerapan Vedika di RSUD dr. H Jusuf SK Kota Tarakan tahun 2022, ditemukan bahwa masih terdapat klaim-klaim yang ditunda pembayarannya yang disebabkan oleh adanya kasus readmisi, ketidaksesuaian kode diagnosa dan prosedur serta kasus yang masuk dalam penjaminan pihak lain. Selain itu Verifikator BPJS Kesehatan memiliki persepsi yang berbeda-beda terkait proses verifikasi klaim, sehingga hasil verifikasi akan berbeda pada masing-masing verifikator.
