Abstract

Background:The incidence of sepsis in the Dr. Moewardi Hospital at the end 2007 showed a mortality rate of 50.2 % (115 deaths from 229 patient with sepsis). Associated with the sepsis treatment therapy, the use of low-dose corticosteroids in the early stages of sepsis is still debated. WHO suggests that acupuncture can stimulate corticosteroid. The aim of this study was to know the acupuncture effect of Zusanli point ( ST36 ) to decrease the number of leukocytes in mice (Mus musculus) model of sepsis caused by the exposure of cecal inoculum. Methods:This study was an experimental laboratory research with posttest only control group designs. The samples were 28 male mice, divided by simple random sampling into a control group (KK), Sepsis Group 1 (KS1), Sepsis Group 2 (KS2), and Sepsis Group 3 (KS3). Sepsis groups were made by giving cecal inoculum exposure at a dose of 0.1 mg/mouse/day injected intraperitoneally for 7 days. During the 10 days after exposure, no treatment was given to KS1, while KS2 was treated with corticosteroid therapy, and KS3 was given acupuncture therapy. The low dose corticosteroid used was Methyl prednisolone at a dose of 0.05 mg/mice that was injected intraperitoneally 2 times daily. The acupuncture was given at the Zusanli point (ST36) with a duration of 15 minutes. Blood samples were taken at day 24th. The leukocyte count data were analyzed with One Way ANOVA test (p<0.05) and  the Bonferroni post hoc test  (p>0.005). Results:Sepsis group were successfully made by exposure of cecal inoculum with significance level (p) of 0.000 (p>0.005). Acupuncture of zusanli point (ST36) could reduce leucocyte count in mice (Mus musculus) model of sepsis caused by exposure to cecal inokulum as same as corticosteroid group with significance level (p) 0.000 (p>0.005). Conclusion:Acupuncture of  Zusanli point (ST36) can reduce leukocyte count in mice (Mus musculus) model of sepsis caused by exposure to cecal inoculum. Abstract

Background: Postural Orthostatic Tachycardia Syndrome (POTS) is illness manifestations caused by autonomic dysfunction. Subjects with POTS predominantly are young women between 20-40 year old and most of them are college student. Exercise has proven can improve the function of body systems. College students’s time is mostly for academic activity and not much time for exercise. The main aim of this research was to know the association between exercise activity and  the incidence of POTS in female college students. Method: This research was an analytical observational using case-control design approach. Samples were fifty eight female college students with POTS (16-23 years old) selected using purposive sampling. This study was done from November to December 2014 at Medical School of Sebelas Maret University Surakarta. Data of Exercise activity were obtained by questionnaire. Diagnosis of POTS was conducted by measuring heart rate using pulsemeter on Active Stand Test (AST) position while orthostatic symptoms were gotten by interview. Data were analyzed using Chi-Square test. Result: This study showed that most of the subjects with POTS (27.6%) had lower exercise activity than those without POTS (37.9%). The result of Chi Square analysis was p = 0.007. Conclusion: There was significant association between exercise activity and the incidence of POTS in female college students. Abstract

Background: Iron deficiency anemia is found in clinical practice and community. There is mutasion in a genetic within iron deficency anemia which regulate iron homeostatis.Mutation in the TMPRSS6 resulting in iron refractory iron deficiency anemia  The TMPRSS6 genetic encoding matriptase-2 enzymes which regulate hepcidin. The hepcidin is a principal regulator of iron homeostatis. Indonesia have more than 2.000 medical plants which have many roles and function. Some plants have phytochemisty that potentially in hepcidin antagonis. The phytochemistry is screen using molecular docking method Methods: The research design was a biocomputasion. It had been done at Medical Faculty of Sebelas Maret University on August-Oktober, 2014. The data was collected by using purposive sampling method. All data is docked using Autodock Vina in PyRx software. Docked sample is compared with binding energy of furstulhiamine drug. The data was compared its binding energy and their zone interaction with fursulthamine Results: The result were 10 samples has higher its binding energy than fursulthiamine. The binding energy of fursulthiamine was -6,7 kcal/mol. The highest binding energy was -8,5 kcal/mol and the lowest was -8 kcal/mol. All 10 sample interacted with cys 326. The cys 326 was the residue which can inhibit hepcidin interaction with ferroportin Conclusions: The Molecular Docking can be used to intial screening many phytochemisty. The result was 10 phytochemistry Indonesian medical plants which may inhibit hepcidin interaction. Further research is needed to prove the effect of phytochemistry. Abstract

Background: Increased activity of Adenosine Monophosphate-Activated Protein Kinase (AMPK) can inhibit cell growth and biosynthetic pathwaysin some cancer cells. Recently, several endogenous ligands have been identified to activate AMPK including Adenosine Monophosphate (AMP).Virtual screening is one of the recent technique, which is used for drug discoveries.Therefore the aim of this study was to identify active compounds of Indonesian herbal plants that activate AMPK by using molecular docking. Methods: Samples used in this study wereall of active compounds which were derived from Indonesian herbal plantsand had three-dimensional structures. Molecular docking was performed by using AutodockVina version 1.1.2 to evaluate interaction between AMPK and active compounds. The AMP structure obtained from protein data bank (www.rcsb.org/pdb)was used as a molecule standard of AMPK interaction. Docking score indicated affinity of molecule interaction and was determined in Cal/mol. To visualize docking results that fit to the AMPK molecule, chimera software version 1.9 was used. Results: Curcumin has docking scores as same as docking score of AMP (-7.5 Cal/mol) and has hydrogen bound in γ subunit of AMPK (Thr86, Thr88, and Arg117). Conclusions: Curcumin might be an activator of AMPK. Another molecular docking which has a flexible substrate-enzyme algorithm is required for confirming the docking result. Abstract

Background:Tuberculosis (TB) remains an important public health concern in the world including Indonesia. This study aimed to estimate the relationship between HIV incidence rate, district/ municipality health budget, healthy house rate, and tuberculosis prevalence in Indonesia. Subjects and Method:This was an analytic and observational study with ecologic study design. The data were obtained from 33 provinces, 497 districts/ municipalitiesthat were collected by Basic Health Research (Riset Kesehatan Dasar, Riskesdas)2013.  The dependent variable was TB prevalence. The independent variables were HIV incidence rate, district/ municipality health budget, and healthy house rate. TB diagnosis was made by health personnel using sputum examination, chest X-ray photo, or both. The data were analyzed by multiple linear regression.Results:District/ municipality budget (b=-14.07;95% CI=-24.09to -4.05, p=0.080), and healthy house rate (b=-3.62; 95%CI=-6.28 to-0.96; p=0.009) decreased TB prevalence. HIV incidence  rate (b=0.05,95% CI=0.02 to 0.08, p=0.002) increased TB prevalence. R2 was 56.5 % for this linear regression model, and the overall model was statistically significant (p=0.001).Conclusion:District/ municipality budget, healthy house rate, and HIV incidence rate are related with TB prevalence.Keywords:district/ municipality budget, healthy house rate, HIV incidence rate, TB prevalenceCorrespondence: Balgis. Abstract

Pendahuluan: Tuberkulosis merupakan salah satu penyakit dengan terapi pengobatan yang menggunakan banyak kombinasi obat. Salah satu faktor yang dapat mempengaruhi potensi interaksi obat adalah menggunakan lebih dari satu macam obat (polifarmasi).   Diabetes (DM) merupakan penyakit tidak menular yang saat ini angka kejadiannya meningkat. Saat ini, DM banyak diyakini memiliki keterkaitan dengan TB melalui sistem pertahanan tubuh host. Tidak hanya itu, berbagai interaksi yang terjadi pada obat tidak semuanya bermakna signifikan. Transient Ischemic Attack (TIA) adalah suatu defisit neurologis sementara akibat iskemik fokal pada otak dan tanpa disertai infark akut. Ketika terjadinya Transient Ischemic Attack (TIA), mengenali gejala dengan cepat adalah kunci mencegah peningkatan risiko stroke.  Metode: Penelitian ini menggunakan metode literature review. Literature review ini meringkas literatur yang membahas TB pasien dengan diabetes mellitus, serta hubungannya dengan TIA, Interaksi beberapa macam obat (Polifarmasi), dan Modifikasi Diet. Hasil: Mycobacterium tuberculosis, yang merupakan agen dari TB dipengaruhi oleh beberapa faktor diantaranya pathogenitas, infektifitas dan virulensi, dan host. Beberapa faktor host yang mempengaruhi penularan penyakit tuberkulosis paru adalah; kekebalan tubuh (alami dan buatan), status gizi, pengaruh infeksi HIV/AIDS. Transient Ischemic Attack (TIA) juga dapat mempengaruhi kondisi tubuh pasien dengan DM Kesimpulan: Terdapat keterkaitan antara penyakit Tuberculosis (TB) dan Diabetes Mellitus (DM), serta hubungannya dengan TIA, Interaksi beberapa macam obat (Polifarmasi), dan modifikasi diet. Kata Kunci: Diabetes, Tuberculosis, TIA, Pholypharmacy, Diet modification
