Abstract

Perusahaan pengelolaan apartemen ini beralamat di Jalan Marsekal Suyadarma No. 1 Tangerang adalah salah satu perusahaan yang bergerak dibidang pengelolaan gedung (Property Management) yang mempunyai enam divisi. Setiap divisi mempunyai potensi bahaya sebagai indikasinya adalah terjadi 18 kasus kecelakaan dalam kurun waktu 2014 sampai 2017. Riset ini bertujuan untuk mengidentifikasi potensi bahaya yang menyebabkan kecelakaan kerja. Dalam riset ini upaya untuk pencegahan terjadinya kecelakaan kerja akan dilakukan menggunakan metode Hazard Operability Study (HAZOPS). Hasil dari penelitian ini adalah terdapat 45 potensi bahaya kecelakaan kerja yang ada di enam divisi yaitu divisi Staff management building 31%, Divisi Engineering 18%, Divisi Parking 9%, Divisi Housekeeping 24%, Security 2%, Vendor/Subcon 16%. Divisi yang mempunyai jumlah potensi bahaya terbanyak yaitu divisi Staff management building 31%. Kemudian untuk tingkat risiko Tinggi 33%, tingkat risiko Sedang 51%, tingkat risiko rendah 16%. Jadi potensi bahaya yang tingkat risiko ekstrim tidak ada. Perbaikan dilakukan dengan mengacu ke hirarki pengendalian potensi bahaya: eliminasi, substitusi, rekayasa engineering, pengendalian administratif dan alat pelindung diri (APD).

Abstract

PT. Polymindo Permata merupakan salah satu perusahaan yang bergerak dalam industri plastik khususnya menggunakan proses extrusion. Penelitian ini bertujuan untuk mengidentifikasi bahaya dan mengukur tingkat risiko bahaya sehingga dapat diketahui faktor apa saja yang menyebabkan tingkat risiko menjadi extreme sehingga dapat menyebabkan terjadinya kecelaka an kerja di area proses produksi PT. Polymindo Permata. Penelitian ini menggunakan metode Hazard Identification, Risk Assesment, and Risk Control (HIRARC) dan Fault Tree Analysis (FTA). Dilihat dari hasil identifikasi dan penilaian risiko yang diperoleh menunjukkan bahwa terdapat 48 potensi bahaya dengan 18 potensi bahaya yang masuk dalam tingkat risiko extreme, 18 potensi bahaya termasuk tingkat risiko high, 7 potensi bahaya termasuk tingkat risiko moderate, dan 5 potensi bahaya masuk dalam tingkat risiko low. Terdapat 2 kegiatan atau aktivitas dengan nilai risiko extreme mencapai 20 yaitu kegiatan pengeboran dan penggerindaan yang terjadi dikarenakan standard keamanan atau Standard Operational Procedure(SOP) yang belum ditetapkan. Sehingga terdapat kondisi atau faktor bahaya saat pengoperasian mesin, area kerja yang tidak aman, serta kurang optimalnya penggunaan alat pelindung diri pada pekerja.

Abstract

Indotehnik Cipta Sembada is a manufacturing company engaged in the design & construction of the Panel Maker electrical system. In an electrical panel, Sheet SPHC (Steel Plate Hotrolled Coiled) is used as the basic material for making box panels. In determining the SPHC Sheet inventory, the company is still based on previous experience, causing frequent stock outs. To help solve the problem of planning raw material requirements, the application of the Min-Max Stock method will assist in meeting the supply of raw materials so that there is no stock out. From this research, it was found that the Safety Stock for 1.5mm SPHC Sheet is 131 sheets and for 2.0mm SPHC Sheet is 151 sheets, the Minimum Stock limit for 1.5mm SPHC Sheet is 106 sheets and for 2.0mm SPHC Sheet is 122 sheets, and the Maximum Stock limit for 1.5mm SPHC Sheets is 213 sheets and for 2.0mm SPHC Sheets is 237 sheets. In addition, in using the Min-Max Stock method the company can save inventory costs, from the calculation results for 1.5mm SPHC Sheet the company can save Rp.2,061,589 and for 2.0mm SheetSPHC Rp.3,240,392. Abstract

Based on Law No. 1 of 1970 every company must prevent accidents (including traffic accidents on the way to and from work), fires occupational diseases. The problem in this study is the number of traffic accidents by employees of the Plumbing Fitting Company in Tangerang. The purpose of this study is to determine the factors that cause traffic accidents and provide solutions for preventing traffic accidents for company employees. Analysis of the factors causing traffic accidents uses the PDCA concept, where this concept has several stages, namely Plan, Do, Check, and Action. The result of this research is employee traffic accidents decreased from 16 accident cases to 3 accident cases or if calculated on average before repair 1.34 accident cases per month and after repair 0.3 accident cases per month decreased by 77.6%.

Abstract

BPJS Health is tasked with administering the health insurance program, or better known as the National Health Insurance (JKN). The problem that occurs is that there are still many community reports indicating that BPJS Health services in Tangerang are still not good. The goal to be achieved by the author is to find out the quality of service that will be provided by the South Tangerang City BPJS Health Service Office to users and provide suggestions for improvement to increase customer satisfaction at the South Tangerang City BPJS Health Service Office. The method used in this paper uses Importance Performance Analysis (IPA) and Potential Gain in Customer Service (PGCV). The results of this study are IPA analysis stating that the majority of services are in quadrant IV, which means that the service is satisfactory but services that are not considered important by customers. While 6 services are considered important by customers but are not satisfactory. Meanwhile, the PGCV analysis stated that long queue waiting times were a top priority for improvement.

Abstract

Kualitas memegang peranan vital dalam keberlangsungan suatu industri manufaktur, bahkan juga kualitas produk menjadi tolak ukur untuk menilai keberhasilan industri manufaktur dalam memproduksi produk yang berkualitas. Permasalahan dalam Penelitian ini adalah jumlah defect proses polishing pada type part s11038z  masih sangat tinggi melebihi standar yang ditetapkan perusahaan yaitu 2.50%. Tujuan dalam penelitian ini guna mengetahui jenis defect dominan  proses polishing pada type part s11038z. Metode  dalam penelitian ini menggunakan fishbone dan Nominal Group Technique dengan pendekatan brainstroming. Hasil penelitian diketahui jenis defect menokori yang paling dominan sebesar 2,40% dan untuk nilai nominal group technique yaitu ≥ 21, faktor penyebab yang melebihi nilai dari NGT dalah  Zeropoint Center buff NC. 46 Puliprom, head 1 ,2 ,3 dan 4 memiliki nilai 35, Tidak ada sill peredam gesekan nozel spray gun memiliki nilai 33, Kesalahan pemakaian torsi program memiliki nilai 28,  Tidak ada panduan setting zeropoint Nc. 46 puliprom memiliki nilai 27 dan Trypoli habis tidak terdeteksi memiliki nilai 24.

Abstract

With many product characteristics, it is very vulnerable in the preparation of products and in the ideal capacity available. This happens in the warehouse. To find the ideal storage, an analysis of the warehouse layout is performed. The purpose of this study is to increase the warehouse's capacity and classify the products that are in the warehouse's storage locations. This Class-Based Storage method is a way of calculating and classifying based on the frequency of product movement. This research was carried out at PT. Cakra Satria Pratama in the warehouse. The results of this study indicate that the placement of the product is not ideal. Therefore, with the results of the proposed layout analysis results in the classification of products, when producing categories A, B and C with category A 53.85% of total products, category B 30.77% of total products and category C 15.38 % of the total products. Keyword: Layout; Class Based Storage Method; Warehouse.

Abstract

The plumbing installation process often harbors potential risks that can lead to workplace accidents. Considering the serious consequences of workplace accidents both in terms of health and economy, there are six types of workplace accidents in the plumbing installation process: foot crushed by a pipe, hand cut by a cutting tool blade, eyes exposed to welding smoke, burnt by hot welding slag, sparked by friction between iron and a cutting blade, and leg scratched by the edge of a pipe. This study aims to analyze workplace accident incidents during the plumbing installation process using the Fault Tree Analysis (FTA) method with the hope of identifying the root causes of the accidents and formulating recommendations to improve work safety. The FTA method allows for the development of a systematic model that illustrates the cause-and-effect relationships of system failures and their components. The primary advantage of using FTA is its ability to provide a comprehensive understanding of complex accident scenarios by breaking them down into simpler basic events. The novelty of this research lies in its application of FTA specifically to the plumbing installation process, which has not been extensively explored in previous studies. The results identified that the cause of eyes exposed to welding fumes led to 11 basic events, while the mocus analysis revealed 7 basic events with 11 failure combinations.
