Abstract

The selection of referral hospitals is not solely determined by the administrative mechanism of the National Health Insurance (JKN) referral system but is also influenced by hospital marketing strategies and patient characteristics. This study aimed to examine the effects of the hospital marketing mix (7Ps) and personal patient factors on referral hospital selection at Segiri Primary Health Center, Samarinda. A quantitative cross-sectional design was employed involving 348 respondents selected through simple random sampling from 1,758 referred patients. Data were collected using a structured questionnaire that had been tested and confirmed to be valid and reliable. Statistical analyses included Chi-square and Fisher’s exact tests for categorical variables, as well as Kruskal-Wallis and Mann-Whitney tests for non-parametric comparisons. The results indicated that most personal characteristics, including age, gender, education, occupation, income, and JKN ownership, were not significantly associated with referral hospital selection (p>0.05), except for domicile (p<0.05). Clinical factors, represented by the destination polyclinic, showed a strong association with hospital choice (p<0.001). Among the marketing mix components, Product (p=0.026), Process (p=0.018), and Physical Evidence (p<0.0001) significantly influenced referral hospital selection, while Price, Place, Promotion, and People showed no significant effects (p>0.05). In conclusion, referral hospital selection was primarily influenced by service needs and the quality of patient experience rather than personal characteristics. Prioritizing improvements in physical facilities and optimizing service processes are essential to enhance the effectiveness of the referral system.

Abstract

Hipertensi masih menjadi masalah penyakit tidak menular yang tinggi di wilayah kerja Puskesmas Lok Bahu. Salah satu kendala pengendalian hipertensi di masyarakat adalah belum optimalnya kemampuan kader dalam mengedukasi, mendemonstrasikan, dan memantau aktivitas fisik yang aman, teratur, serta sesuai kondisi kesehatan penderita hipertensi. Kegiatan pengabdian masyarakat ini bertujuan meningkatkan pengetahuan dan keterampilan kader dalam memberikan edukasi serta demonstrasi aktivitas fisik aman berbasis keselamatan pasien bagi penderita hipertensi. Kegiatan dilaksanakan di Aula Puskesmas Lok Bahu, Samarinda, melalui pelatihan, demonstrasi, praktik penggunaan alat latihan sederhana, monitoring, dan evaluasi. Peserta terdiri atas 19 kader dari lima Posyandu dan 20 penderita hipertensi. Evaluasi pengetahuan dilakukan menggunakan pretest dan posttest, sedangkan keterampilan kader dinilai menggunakan lembar observasi praktik. Dari 20 peserta evaluasi, terdapat 17 peserta dengan data pengetahuan lengkap. Rerata skor meningkat dari 82,35±7,05 menjadi 86,27±6,44, dengan selisih rerata 3,92 poin. Hasil paired t-test menunjukkan peningkatan belum bermakna secara statistik (p=0,096), tetapi secara deskriptif 47,1% peserta mengalami peningkatan skor. Hasil observasi menunjukkan kader mampu menyampaikan edukasi, mendemonstrasikan aktivitas fisik aman, menggunakan alat latihan sederhana, menyampaikan prinsip keselamatan, dan menyusun rencana tindak lanjut. Pelatihan kader tentang aktivitas fisik aman berpotensi memperkuat pengelolaan hipertensi berbasis masyarakat dengan tetap memperhatikan keselamatan pasien.

Abstract

In 2020, the Indonesian Ministry of Health reported that only 35% of 119,665 post-hospital care patients returned to follow-up on their condition and one of the influencing factors is the implementation of discharge planning which has not yet been optimized. The implementation of discharge planning requires high motivation from a nurse to achieve a good result. This study aimed to determine the relationship between nurse motivation and implementation of discharge planning in patients with diabetes. This is a quantitative analytical cross-sectional study. The population in this study was all executive nurses, totaling 71 respondents in the internal medicine and surgical inpatient rooms of Government Hospitals in Kutai Kartanegara. The sampling method used was total sampling. The instruments used were nurse motivation questionnaires and discharge planning. Majority of the respondents were female (67.6%), aged ≥ 25 (70.4%) with a bachelor’s degree in nursing (64.8%) an a lenght of work ≥5 years (78.9%). There was a relationship between nurse motivation and the implementation of discharge planning in DM patients with p-value 0.001 (AOR: 9.6 (95% CI: 3–31)) after adjusting for length of work, education level, and age. This study proves that high nurse motivation affects the implementation of discharge planning in patients with Diabetes Mellitus.

Abstract

ObjectiveThis study examined burnout dimensions and their associations with demographic and professional characteristics among emergency nurses in East Kalimantan, Indonesia, during the post-pandemic transition period.MethodsA quantitative descriptive-analytic cross-sectional study was conducted in December 2022 in the emergency department of a Coronavirus Disease 2019 (COVID-19) referral hospital in East Kalimantan. Seventy-one full-time emergency nurses were recruited through total population sampling.  Burnout was measured using the Maslach Burnout Inventory–Human Services Survey, and demographic and professional data were collected using a structured questionnaire. Data were analyzed using descriptive statistics and Spearman’s rank correlation.ResultsMost participants reported low emotional exhaustion (83.10%), while high depersonalization was found in 45.07% and low personal accomplishment in 94.36%. Age showed weak negative correlations with emotional exhaustion (r = -0.257, p = 0.031), depersonalization (r = -0.324, p = 0.006), and reduced personal accomplishment (r = -0.320, p = 0.006).  Length of employment showed weak negative correlations with depersonalization (r = -0.261, p = 0.028) and reduced personal accomplishment (r = -0.266, p = 0.025).  Gender, marital status, and education level were not significantly correlated with burnout dimensions.ConclusionRoutine multidimensional burnout monitoring and targeted professional support are recommended for emergency nurses.Keywords:Burnout, emergency nurses, post-pandemic

Abstract

Human Immunodeficiency Virus masih menjadi masalah kesehatan masyarakat yang memerlukan perhatian serius, terutama pada kelompok remaja. Kurangnya pengetahuan, sikap yang kurang mendukung, serta motivasi yang rendah dalam pencegahan HIV dapat meningkatkan risiko penularan dan munculnya stigma terhadap orang dengan HIV. Oleh karena itu, diperlukan media edukasi yangmenarik, interaktif, dan sesuai dengan karakteristik remaja, salah satunya melalui Dart Game, untuk meningkatkan efektivitas penyampaian informasi kesehatan. Menganalisis pengaruh media Dart Game terhadap peningkatan pengetahuan, sikap, dan motivasi siswa mengenai pencegahan HIV di SMKN 14 Samarinda. Penelitian ini menggunakan desain pre-eksperimental dengan pendekatan One Group Pretest-Posttest. Sampel berjumlah 32 siswa kelas XII yang dipilih menggunakan teknik cluster sampling. Data dikumpulkan menggunakan kuesioner pengetahuan, sikap, dan motivasi sebelum dan sesudah intervensi, kemudian dianalisis menggunakan uji Wilcoxon. Terjadi peningkatan pada seluruh variabel setelah pemberian intervensi. Proporsi responden dengan pengetahuan kategori baik meningkat dari 69,2% menjadi 87,5%, sikap positif meningkat dari 62,5% menjadi 100%, dan motivasi kategori tinggi meningkat dari 53,1% menjadi 93,8%. Hasil uji Wilcoxon menunjukkan bahwa media Dart Game berpengaruh signifikan terhadap pengetahuan (p=0,000), sikap (p=0,001), dan motivasi (p=0,000) mengenai pencegahan HIV. Media Dart Game efektif meningkatkan pengetahuan, sikap, dan motivasi siswa dalam pencegahan HIV serta berpotensi menjadi alternatif media edukasi kesehatan yang inovatif, menyenangkan, dan mudah diterapkan di lingkungan sekolah untuk mendukung upaya promotif dan preventif.
