Abstract

The aim of this study is to know the incidence of cervical carcinoma stage IB - IIA recurrence after radical hysterectomy and the histopathologic factors which are related to it. This was a retrospective cohort study involving 68 patients with recurrence cervical carcinoma stage IB - IIA who underwent radical hysterectomy, However, there were only 58 cases which fulfilled the study citeria, and therefore were evaluated histopathologically. The histopathologic evaluation consisted of differentiation, inflammatory reaction, lymph-vascular permeation, invasion to the vagina incision margin, and to endometrium, lymph nodes metastasis, and the depth of stromal invasion. The incidence of the cervical carcinoma stage IB-IIA recurrence 2 years after radical hysterectomy was 30.8% (21 out of 68 cases). The recurrence occurred in nine (75%) out of 12 cases with lymph node metastasis, compared to 10 (21.7%) out of 46 cases without lymph node metastasis (P = 0.0004). In addition, eight out of 11 adenosquamous carcinoma cases had recurrence (p = 0.01). There was only one out of the 12 cases with the depth of stromal invasion of < 5 mm that had recurrence compared to 18 out of 45 cases with stromal invasion of > 5 mm (P = 0.02). The clinical factors such as parity, staging, size of the tumor were not statistically significant. The other pathologic factors such as differentiation, inflammatory reaction, and parametrial, vagina incision margin and endometrial invasion were not statistically signifcant. The incidence of the recurrence of cervical carcinoma within 2 years after hysterectomy was related to the pathological risk factors i.e. lymph node metastasis, adenosquamous cell carcinoma and the depth of stromal invasion. Abstract

The aim of this study is to evaluate response of cervical cancer stage IB – IIA after neoadjuvant chemotherapy based on VEGF expression. The data were collected from 51 patients’ cervical cancer stage IB – IIA parafin blocks who received chemotherapy ifosfamide – cisplatin before radical hysterectomy at General Hospital Adam Malik Medan. VEGF expression was evaluated from cervical biopsy tissue, and response therapy was evaluated based on tumor size clinically. 20 out of 51 samples with clinically complete response, and the rest are partial response. 18 out of 20 samples with clinically complete response have negative or weak VEGF expression, and 31 out of 51 samples patients were partialy response with moderate or strong VEGF expression. 23 cases with tumor size > 4 cm and 23 cases stage IIA expressed VEGF moderately or strong. Cervical cancer with tumor size < 4 cm and cervical cenncer stage IB with less expressed of VEGF have good response with chemotherapy adjuvant ifosfamide – cis platin.Keyword: ifosfamide-cisplatin, cervical cancer, VEGF

Abstract

Introduction : Hypertension in pregnancy is one of the causes of maternal morbidity and mortality is high. It cause of 30-40% of perinatal deaths. This state is related to blood viscosity and hematocrit is an important determinant of the blood viscosity. Careful observation of the several indicators, such as hematocrit levels can prevent from unwanted circumstances. Methods : This research is to design pre test - post interventions to determine the relationship between hematocrit levels in preeclampsia before and after childbirth. Pre and post test data is performed using the t test (t-test) and to find the relationship of hematocrit levels with the degree of preeclampsia with Chi-square test.Results : Of the total 55 cases, most cases of severe preeclampsia occurs in the age group 21-35 years were 43 cases (78.2%) cases, have never given birth were 22 cases (40%), history of antenatal care is often enough (4 - 8x during pregnancy) that is found in 30 cases (54.5%), gestational age at term 30 cases (54.5%), and delivery baby with a normal weight in 29 cases (52.7%). Hematocrit levels showed a significant reduction in the time before and after 24 hours of delivery of 34.018 + 0.690 percent to 30.140 ± 0.656 percent (p<0.001). There were no significant differences in hematocrit levels based on the severity of severe preeclampsia both before birth and after 24 hours of labor (p>0.05).Discussion : Found a significant difference in hematocrit levels at the time before and after 24 hours of delivery. Hematocrit levels not influenced by the severity of severe preeclampsia.Keywords : Severe preeclampsia; hematocrit levels

Abstract

Introduction : Ovarian cyst is a benign tumor with liquid content. This is often found in women in their reproductive periods. Knowing the epidemiology of ovarian cysts is important for all aspects of health. Analyzing patients’ charateristic who underwent laparotomy or laparoscopic for ovarian cyst ovarian cysts will give us data about preferences of each method as a useful statistical data in the informed consent or determining therapy choice.Objective : To determine the characteristics of the ovarian cyst patient who underwent laparoscopy and laparotomy. Methods : This study is a retrospective descriptive study with cross sectional design using all patient medical records benign ovarian cysts who have undergone surgery in the Departemen of GynecologyinHaji Adam Malik Hospital from 1 January 200 to 31 December 2013. Parameters assessed were age, parity, tumor size, menstrual status, body mass index, in each surgical group. The data collected is presented in the form of tables and diagrams.Results :I n this study, 163 patients were included.Laparoscopy in benign ovarian cysts entirely (100%) is cystectomy and majority laparotomy method was SOU laparatomy (58.9%). Majority patients who undergo laparoscopy vslaparatomy, aged 21-35 vs. 36-50 years old (52.9% vs 30.82.6%), nulliparity versus multiparity (70.58% vs 30.8%), tumor size 5-10 cm vs>10 cm (88.23% vs 48.63%), both still menstruating (100% vs 56.16%), normoweightvs obese 1 (70.58% vs 30.8%). Conclusions : There are differences between the characteristics of the group who underwent laparoscopy and laparotomy.

Abstract

ABSTRACTBackground: HIV/AIDS is one of the diseases that has been a global problem for a long time. In 2018, there were 37.9 million people worldwide infected with HIV and 1.7 million were children. Knowledge  of the prevention of HIV transmission from mother-to-baby is very important as one of the prevention options. The aim of research knowing the level of knowledge of students of the Faculty of Medicine, Universitas Sumatra Utara  in 2017 against HIV transmission from mother to child.Method: Research method conducted using online survey method with cross sectional design. The samples used are all students based on total sampling. The study used univariate analysis to see descriptive images with primary data sourced from online questionnaires. Result: The level of knowledge of students of the Faculty of Medicine, University of North Sumatra in 2017 in the category of both 128 people (63.7%), enough 67 people (33.3 %), and less 6 people (3%).Conclusion: Students of the Faculty of Medicine, University of North Sumatra in 2017 have good knowledge about HIV transmission from mother to child.

Abstract

Background: Obesity, as defined by the WHO is an abnormal fat consolidation or excessive fat deposit. Those excessive fat deposits have been known to be the risk factors for cardiovascular diseases. One such additive is known to improve the taste of the food is Monosodium Glutamate (MSG). The purpose of the study is to determine the effects of MSG on weight gain. Methods: This is a simple experimental design with a pretest-posttest design. The study was conducted in the Animal House laboratories of the Faculty of Mathematics and Natural Sciences Universitas Sumatera Utara between October and November of 2019. The sample in this experiment is male Wistar rats aged 10-11 weeks that will be fed with standard animal feed mixed with MSG. There are 2 groups of rats (each group 16 rats), one group 1 (non-MSG), and group 2 (MSG). The rats were fed with animal feed (ad labium) for 31 days. The feed given every single day will be weighed and documented. The subcutaneous fat was taken from the abdominal and axillary regions. Result. After 31 days of treatment, re-weighing of both groups of animals was carried out. The initial weight, final weight, total weight gain, total fat extracted and total feed consumed was not different significant (p>0,05). Conclusion. The feeding of MSG for 31 days, did not have significant effects on weight gain. A longer time is needed for evaluation of MSG effect on weight gain

Abstract

HIGHLIGHTS Neutrophil Lymphocyte Ratio (NLR) and Platelet Lymphocyte Ratio (PLR) have potential as independent prognostic markers for ovarian cancer. This research was to evaluate NLR and PLR levels in ovarian cysts with epithelial ovarian cancer patients at RSUP H. Adam Malik Medan, Indonesia.   ABSTRACT Objective: To assess Neutrophil Lymphocyte Ratio (NLR) and Platelet Lymphocyte Ratio (PLR) as independent prognostic markers in epithelial ovarian cancer patients at RSUP H. Adam Malik Medan, Indonesia, considering the disease's common occurrence and typically late-stage diagnosis with a poor prognosis due to inflammatory processes implicated in malignancy mechanisms. Materials and Methods: A comparative case-control study was conducted at the Department of Obstetrics and Gynecology, RSUP H. Adam Malik Medan, from December 2019 to February 2020. The study included 40 patients each diagnosed with ovarian cysts and epithelial ovarian cancer. Bivariate analysis was conducted using independent T-test for normally distributed data and the Mann-Whitney test for non-normally distributed data. Significance was established at p <0.05. Results: The median NLR in the ovarian cancer group was 2.45 (1.06 to 38.44) and in the ovarian cysts group was 2.34 (1.44 to 3.78). Median PLR levels in ovarian cancer were 12813.94 (3178.08 to 19040.0) and in ovarian cysts were 11138.15 (5026.18 to 22839.51). Statistical analysis revealed no significant difference in NLR and PLR levels between ovarian cancer and ovarian cysts (p >0.05). Conclusion: NLR and PLR levels demonstrated no significant difference between ovarian cancer and ovarian cyst patients at RSUP H. Adam Malik Medan, Indonesia. However, NLR and PLR can still serve as valuable markers for identifying malignant processes in patients suspected of malignancy.

Abstract

Nigella sativa seed extract has been shown to have a significant effect on endometrial thickness and vaginal cytology in ovariectomized animal models, suggesting potential benefits for managing menopausal symptoms. However, to the best of the author’s knowledge, no human studies have been done to support these conclusions. The aim of this study was to investigate the effects of N. sativa seed extract on estradiol, follicle-stimulating hormone (FSH), and the vaginal maturity index (VMI) in postmenopausal women. A single-blinded, randomized placebo-controlled experiment was carried out at Haji Adam Malik Hospital, Medan, Indonesia, with 50 eligible postmenopausal women patients randomized into three groups. Group 1 received a placebo, while groups 2 and 3 were given N. sativa seed extract at 910 mg/day and 1,365 mg/day, respectively. All participants were blinded to the treatment they received. The study used Shad Nigella Plus, an Indonesian herbal medicine containing 455 mg of N. sativa seed extract per capsule. Before the treatments, estradiol levels, FSH levels, and VMI were measured at baseline and remeasured after eight weeks of treatment. Two participants in the intervention group withdrew due to nausea, a reported side effect of N. sativa seed extract consumption. Both treatment groups showed significant increases in estradiol levels (p=0.01 and p=0.001) and VMI (p=0.004 and p=0.001) after eight weeks of daily N. sativa seed extract administration compared to the placebo group. However, no significant differences were found between the two doses in estradiol levels and VMI (p=0.12 and p=0.673, respectively). Moreover, FSH levels showed no significant difference throughout both interventions (p=0.53 and p=0.96, respectively). In conclusion, twice-daily N. sativa seed extract at 910 mg/day or 1,365 mg/day for eight weeks significantly increased estradiol levels and VMI in menopausal women but had no significant effect on FSH levels. These findings support the potential role of N. sativa seed extract as a natural treatment for menopausal symptoms.

Abstract

The development of glucocorticoid resistance has complicated the management of acute lymphoblastic leukemia (ALL), leading to increased mortality rates. Ivermectin, a low-cost and well-established anthelmintic, exhibits anticancer potential and may enhance glucocorticoid toxicity in ALL, offering a possible strategy to overcome resistance. The aim of this study was to evaluate the apoptotic effect of combining ivermectin with dexamethasone in ALL. ALL SUP-B15 cells were cultured under standard conditions before treatment with dexamethasone (200 nM) alone or combined with ivermectin (5, 10, and 20 µM), with an untreated group serving as the control.  Cytotoxicity was assessed using the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay by measuring cell viability and inhibition. Apoptosis was evaluated through BAX, BCL-2, and CASP3 gene expression analysis using reverse transcription-polymerase chain reaction (RT-PCR). The findings revealed that the combination of ivermectin and dexamethasone was superior in the repression of ALL cell viability compared to control (p<0.001). The combination of dexamethasone 200 nM + ivermectin 20 μM demonstrated the most significant cell inhibition of 38.16±0.04% (p<0.001) and produced the lowest cell viability of 61.84±0.05% (p<0.001). Moreover, the combination of dexamethasone 200 nM + ivermectin 20 μM demonstrated superior upregulations of BAX (p<0.001) and CASP3 (p<0.001). In conclusion, the addition of ivermectin (5 µM) to dexamethasone regimen (200 nM) increases its cytotoxic and apoptotic activities against SUP-B15 cell line as observed by the CASP3 and BAX upregulation. Studies to confirm the enhanced anticancer activity by this combination by observing the protein levels and animal studies are warranted.
