Abstract

While digital health literacy (DHL) is crucial for type 2 diabetes mellitus (T2DM) self-management, evidence in primary care settings remains limited. This quantitative cross-sectional study aimed to examine the association between DHL and T2DM management adherence among 115 patients (aged 35–65 years) at Mojolangu Community Health Center. Data were collected using the Electronic Health Literacy Scale (eHEALS) and the Indonesian Diabetes Self-Management Instrument (IDN-DSMI). Spearman's rank correlation analysis revealed a significant positive association between DHL and management adherence (r = 0.622, p < 0.001). In conclusion, higher digital health literacy is associated with better management adherence, suggesting that strengthening DHL may improve patients' self-management capabilities and long-term health outcomes.

Abstract

Abnormalities in platelets can lead to severe consequences renal and systemic blood flow. The aim of this study is to determine the predictive ability of platelet indices (mean platelet volume, platelet-lymphocyte ratio, and platelet count) in relation to changes in blood pressure before and immediately after hemodialysis. This study included 98 patients undergoing hemodialysis at the RSU UMM Malang, aged ≥18 years, hemodialysis duration > 1 month, and signing informed consent. The predictor variable were MPV, PLR, and platelet count and the outcome was the incidence of IDHT. There was a significant difference in the blood pressure of patients undergoing hemodialysis before compared to immediately after hemodialysis, for both systolic and diastolic blood pressure (p < 0.001). To assess the sensitivity and specificity of the accuracy of the platelet count, MPV, and PLR markers in predicting IDHT, ROC curve was created based on the difference in systolic blood pressure before-after hemodialysis. From the receiver operating characteristic (ROC) curve, it was found that the MPV value was the highest above the diagonal line compared to PLR and platelet count with an MPV cut-off value was 9.15. This indicates that MPV is the best predictive marker compared to the other models.

Abstract

Hipertensi pada lanjut usia tidak hanya berkaitan dengan perubahan fisiologis, tetapi juga dengan kemampuan mempertahankan perilaku hidup sehat. Aktivitas fisik yang rendah dapat menjadi salah satu hambatan dalam pengelolaan hipertensi, sementara hambatan kognitif seperti kekhawatiran dan keyakinan yang kurang tepat dapat membuat lansia semakin pasif. Penelitian ini bertujuan menganalisis pengaruh Cognitive Behavioral Therapy (CBT) terhadap tingkat kepatuhan aktivitas fisik dan tekanan darah pada lansia hipertensi di Panti Werdha Pangesti Lawang. Penelitian menggunakan quasi-experimental dengan desain non-equivalent control group. Sebanyak 30 lansia hipertensi dipilih menggunakan purposive sampling dan dibagi menjadi kelompok intervensi dan kontrol, masing-masing 15 responden. Kelompok intervensi memperoleh CBT dalam empat sesi selama dua minggu. Aktivitas fisik diukur menggunakan International Physical Activity Questionnaire (IPAQ), sedangkan tekanan darah diukur dengan tensimeter digital. Analisis menggunakan Wilcoxon Signed Rank Test dan Mann-Whitney U Test sesuai distribusi data. Skor aktivitas fisik kelompok intervensi meningkat dari 511,00 menjadi 897,17 MET-menit/minggu, sedangkan kelompok kontrol menurun dari 360,67 menjadi 310,83 MET-menit/minggu. Perbedaan perubahan aktivitas fisik antarkelompok bermakna (p<0,001). Tekanan darah sistolik turun 13,87 mmHg pada kelompok intervensi dan 5,60 mmHg pada kontrol, dengan perbedaan perubahan antarkelompok p=0,017. Perubahan tekanan darah diastolik tidak berbeda bermakna (p=0,803). CBT berpengaruh terhadap perubahan aktivitas fisik dan tekanan darah sistolik, tetapi belum menunjukkan pengaruh yang bermakna terhadap tekanan darah diastolik.
