Abstract

Kata-kata kunci: lama rawat, mortalitas, perawatan intensif, sepsis, sinbiotik ABSTRACT Background: Sepsis is a leading cause of global mortality. Gut dysbiosis exacerbates the inflammatory response in sepsis. Synbiotic supplementation (a combination of probiotics and prebiotics) is proposed as an adjuvant therapy to modulate gut microbiota, but its effectiveness in reducing length of stay and mortality remains debated. Methods: A systematic search was conducted on PubMed, Embase, and Cochrane Library for recent systematic reviews (SRs) and randomized controlled trials (RCTs). Inclusion criteria were critically ill septic patients receiving synbiotics, with outcomes of mortality and length of stay. Results: Two SRs (Gong et al., 2025; Lou et al., 2023) were analyzed. Synbiotics showed a potential reduction in mortality in adult patients (Risk Ratio 0.61; 95% CI 0.34–1.08) and the incidence of ventilator-associated pneumonia (VAP) (RR 0.52; 95% CI 0.31–0.87). However, the effect on ICU length of stay was inconsistent across studies. High heterogeneity was observed in strains, dosage, and duration of administration. Conclusion: Synbiotics may reduce mortality and infectious complications in critically ill septic patients. However, evidence for reducing length of stay remains inconclusive. Further large-scale RCTs with standardized protocols are needed to establish optimal clinical recommendations. Keywords: intensive care, length of stay, mortality, sepsis, synbiotics

Abstract

Abstract

Background: Burn injuries cause significant physical trauma and complex inflammatory responses, accounting for more than 300,000 deaths per year. Probiotics may help modulate inflammation in burn patients by improving biomarkers such as hs-CRP and IgA levels. This case report aims to investigate the effect of probiotics supplementation on reducing inflammation in burn patients. Methods & Materials: A literature search was on three large databases: PubMed, Cochrane Library, and Google Scholar. MeSH terms, advanced search, and eligibility criteria were used for title and abstract screening after removing duplicates. Critical assessment tools and levels of evidence of the final articles are based on the Oxford Center for Evidence-Based Medicine. Results: Two randomized controlled trials (RCTs) met the PICO and eligibility criteria. One RCT found that probiotics supplementation significantly reduced hs-CRP levels (21.38 ± 44.45 vs 36.36 ± 79.03; p < 0.001) and improved wound healing in burn patients. The other RCT found that single and mixed strain probiotic administration significantly increased IgA levels (p < 0.001 and p = 0.025, respectively) in burn patients. Conclusions: Probiotics supplementation demonstrates significant benefits in reducing systemic inflammation and enhancing immune function. Further research is necessary to provide recommendations of probiotic use in burn care.

Abstract

Background: Stunting is a chronic malnutrition problem primarily occurring during the first 1000 days of life. Iron deficiency anemia during pregnancy can threaten fetal growth and may affect the baby's birth weight and length. According to the Timor-Leste Food and Nutrition Survey 2020, stunting in Timor-Leste is 47.1%. Objectives: To determine the relationship between a history of anemia during the third trimester of pregnancy and low birth weight with the incidence of stunting in children aged 12-23 months in Dili city. Methods: This study was a case-control design conducted in five health centers in Dili City, Timor-Leste. The sample consists of 180 children aged 12-23 months, with 90 stunted cases and 90 controls selected consecutively. Data collected encompasses both child-related and maternal information. Results: Anemia during the third trimester of pregnancy OR 95%CI 7.18 (3.38-15.23) and low birth weight OR 95%CI 5.39 (2.09 – 13.90) are significantly associated with the incidence of stunting (p < 0.01). Based on logistic regression analysis, anemia during pregnancy and low birth weight are not significantly associated with an increased risk of stunting. However, there are characteristics of the mother and child that are significantly associated with the risk of stunting. Conclusion: Children with a history of low birth weight are 5.3 times more likely to experience stunting, and children whose mothers had anemia during pregnancy are 7.2 times more likely to experience stunting. The results show that low birth weight and maternal anemia are related to the incidence of stunting in Timor-Leste.

Abstract

Background: Brown rice is thought to have anti-inflammatory effects due to its high dietary fiber and phenolic content; however, evidence in sedentary populations is limited. IL-6 is a key pleiotropic cytokine responsive to postprandial inflammation. Objective: This study compared postprandial IL-6 levels after brown vs. white rice consumption in sedentary workers. Methods: In this open-label, parallel-group trial, 28 normoweight participants (22 women, 6 men) consumed 150 g of rice, 60 g of fried eggs, and 70 g of tofu stew. Nutritional composition was determined from available sources, with fat content of brown rice (2.4 g/100 g) obtained from the manufacturer’s laboratory report and that of white rice (0.5 g/100 g) from published data. Participants were randomized by gender into brown rice (n=13, age 30.15 ± 6.71) and a white rice groups (n=15, age 27 [23-48]). IL-6 levels were measured at baseline (10-hour fast) and 4 hours post-meal. Previous-day intake (24-hour food recall) and physical activity (IPAQ-SF) were recorded. Non-parametric tests were applied. Results: Participants averaged 321.5 ± 181.2 METs. Despite higher fat content in brown rice, intake did not differ significantly between groups. IL-6 changes were not statistically significant (p=0.069). However, the brown rice group showed a numerically higher postprandial IL-6 level, while the control group remained relatively stable. Conclusions: No significant IL-6 differences were observed, however the paradoxical elevation following brown rice consumption is probably due to subtle influences of nutrient composition or participant characteristics. Further studies with stricter dietary control and biomarkers are warranted.

Abstract

Introduction: Head and neck cancer (HNC) patients are at high risk of malnutrition and functional decline. Protein and branched-chain amino acids (BCAA) are key nutrients for muscle protein synthesis and may influence functional capacity. This study examined the association between protein and BCAA intake and functional capacity in malnourished HNC patients receiving chemoradiotherapy. Methods: A cross-sectional study was conducted at a radiotherapy outpatient clinic among malnourished HNC patients undergoing chemoradiotherapy (week 4–5). Energy and protein intake were assessed using three non-consecutive 24-hour food recalls, while BCAA intake was estimated using a semi-quantitative FFQ. Functional capacity was evaluated using the six-minute walk test (6MWT). Associations were analyzed using Spearman’s correlation test. Results: A total of 94 participants were included (mean age 46.9 ± 11.9 years; 61.7% male), with most diagnosed at stage IV disease. Median protein intake was 40.65 g/day (0.79 g/kg BW/day), with 74.5% classified as having inadequate protein intake. Median total BCAA intake was 9.08 g/day. The mean 6MWT distance was 332.9 ± 123.9 m. Significant moderate positive correlations were found between protein intake and 6MWT distance (r = 0.444; p < 0.001) and between BCAA intake and 6MWT distance (r = 0.419; p < 0.001). Conclusion: Protein and BCAA intake showed a moderate association with functional capacity among malnourished HNC patients undergoing chemoradiotherapy. These findings support the importance of optimizing protein and BCAA intake to preserve functional status during active cancer treatment.
