Abstract

Background : Stature is required to calculate body mass index and determine the energy needs of patients in nutritional medical therapy. Difficulty was found to obtain stature data in patients who are unable to stand. Therefore, there are some height estimation formula to predict the actual height using knee height. This study aimed to validate the Chumlea formula on outpatient clinic of Dr. Cipto Mangunkusumo General Hospital, Indonesia. Methods : This cross-sectional study used 90 respondents selected by consecutive sampling. Actual height, knee height, age and gender data were collected. Estimated height using the three of Chumlea formula. Pearson correlation was used to see the correlation between actual height and estimated height. Paired t-test were used to determine the difference significance. Results : The average age of the subjects was 45±10 years old with actual height 157±10 cm, and knee height 48.8±3.5 cm. A strong positive correlation was found between actual height with all estimated height on male (p=0.000; r > 0.8) and female subjects (p=0.000; r > 0.6). However, comparison between actual height and estimated height showed no significant differences only in Chumlea L1 formula (p=0.087) and Chumlea L3 formula (p=0.824) on the male subjects. Conclusions : Chumlea L1 and Chumlea L3 formula was a valid fomulation for calculating the estimated body height in adult male patients. There was no valid Chumlea’s formulation to estimate body height in female patients.

Abstract

Introduction: Diabetes mellitus type 2 (T2D) is the most common type of diabetes. There is an ongoing debate as to whether nutrient quality or quantity is mainly responsible for the effects on hyperglycaemic control of patients with T2D. Monounsaturated fatty acid (MUFA) has a metabolic effect in reducing the risk factor for metabolic syndrome to prevent and control hyperglycaemia in patients with T2D and is recommended by professional organizations. Methods: Literature searching was conducted by advanced searching in Pubmed, Cochrane Library, Scopus, and ProQuest database using MeSH Terms combined with Title/Abstract. After removing duplicates, the literature were screened based on the eligibility criteria. Critical appraisal and level of evidence of the selected literature were determined based on Oxford Centre for Evidence-Based Medicine. Results: Two selected literature were relevant to answer our clinical question, both literature are systematic review/meta-analysis studies. The first literature is a study by Fumiaki et al. (2016), while the second is by Frank Qian et al. (2016). All literature provided evidence that a diet high in MUFA could improve glucose-insulin homeostasis among patients with T2D. These effects were marked by lower fasting blood glucose and haemoglobin A1c (HbA1c), and insulin level in the enriched MUFA diet group compared to PUFA. Conclusion: Nutrient quality is mainly responsible for the effects on glycaemic control profile in patients with T2D, with a high MUFA diet consistent with favourable effects to improve fasting glucose and HbA1c.

Abstract

Background: About 41% adults in Kalimantan have inadequate total fluid intake (TFI), which is higher than national inadequacy (28%). Inadequate TFI may cause dehydration. The biggest barrier to comply with TFI is to forget to drink. Smartphone application, Hidrasiku with reminder, is expected to be useful in TFI adequacy which may affect hydration status. Objective: This study aims to determine the effect of the Hidrasiku with reminder on the TFI adequacy and hydration status among adults in Bontang. Methods: The design of this study was quasi-experimental and cluster sampling. Inclusion criteria were healthy men, age 19-64 years and having a smartphone. Participants suffering with diseases that affected fluid balance and on diet and fluid restriction were excluded. Data collection was conducted from April to June 2023 in Bontang. A total of 106 participants were allocated into two groups, intervention and control. Both groups used Hidrasiku for four weeks, with the intervention group activating the reminder while the control group didn’t. Adequacy of TFI and hydration status were assessed before-after intervention. Results: A sample of 87 participants completed the study. The median age of the participants was 32 (19-61) years. At the end of intervention, there was significant difference in the TFI adequacy (90.2% vs 63.0%; p=0.003) and a significant increase in TFI +240 mL/day vs – 62.8 mL/day; p=0.002). Meanwhile no significant difference on hydration status (87.8% vs 73.9%; p=0.103). Conclusions: Hidrasiku with reminder has effect on the TFI adequacy and no effect on hydration status

Abstract

Introduction: Cancer cachexia is a debilitating syndrome characterized by involuntary weight loss, muscle wasting, and functional decline, negatively impacting quality of life and survival. Nutritional interventions, including branched-chain amino acids (BCAAs) and eicosapentaenoic acid (EPA), have shown promise in ameliorating cachexia. This case report illustrates the potential benefits of these nutrients in a patient with nasopharyngeal carcinoma. Case presentation: A 65-year-old female presented with severe fatigue, dysphagia, and significant weight loss (11 kg in 6 months) secondary to nasopharyngeal carcinoma (T1N3Mx, stage IV). She was diagnosed with cancer cachexia and severe malnutrition. Nutritional therapy included enteral nutrition with 1.5-2 g/kgBW/day protein, 11-12 g/day BCAAs, and 1-2 g/day EPA. Conclusion: This case report suggests that a nutritional approach combining high protein, BCAAs, and EPA may improve body composition and functional capacity in patients with cancer cachexia. Further research is warranted to confirm these findings in larger studies.

Abstract

Introduction: Atopic dermatitis (AD) is a chronic skin disease characterized by abnormal skin barrier function and heightened immunologic sensitization. Its incidence has surged recently, with approximately 60% of cases diagnosed within the first year of life. AD can persist or recur, significantly deteriorating the quality of life due to persistent itching and sleep disturbances. Individuals with AD are at increased risk of developing other atopic disorders like asthma and allergic rhinitis. Various dietary approaches, including low carbohydrate, calorie, and fat diets, have been associated with enhanced quality of life by regulating immune responses. Probiotics have been used as adjuvant therapy in allergic conditions, though their effectiveness varies due to the complex nature of allergies. Method: A literature search was conducted using PubMed, Cochrane Library, and Google Scholar. MeSH terms, advanced search strategies, and predefined eligibility criteria were employed to identify relevant studies. Duplicate studies were excluded, and critical assessment tools and levels of evidence were defined per Oxford Center for Evidence-based Medicine standards. Results: One systematic review and meta-analysis of randomized controlled trials (RCTs) met the PICO and eligibility criteria. The meta-analysis demonstrated that probiotics significantly decreased AD severity in adults, enhancing their overall quality of life. Other meta-analyses and systematic reviews supported that probiotics reduce clinical severity and improve life quality in adult AD patients. Conclusion: Probiotic supplementation is a viable option to improve the quality of life for adult AD patients, offering potential therapeutic benefits in managing this condition.

Abstract

Calorie together with protein intake has been proven to be important factors that play a role in critically ill patients. Hypermetabolism in the early acute phase increases energy expenditure and calorie requirement. Insufficient calorie intake will increase protein degradation for gluconeogenesis, which can be assessed by the level of 24-hour urinary urea nitrogen (UUN). Adequate daily calorie intake is expected to improve the UUN level in patients. This study aims to investigates the correlation of calorie intake with UUN level changes in critically ill patients.

Abstract

Introduction: After completion of pulmonary tuberculosis (TB) treatment, some people may continue to experience respiratory issues that can progress into post-TB lung disease (PTLD). Individuals with PTLD exhibit suboptimal nutritional status. The loss of appetite is a significant factor influencing nutritional status. Zinc plays a role in hunger control.  Lack of zinc reduces taste sensitivity and food intake. This study examined the relationship between zinc intake and appetite in patients with PTLD at Persahabatan National Respiratory Referral Hospital, Jakarta. Methods: This cross-sectional study was conducted at Persahabatan National Respiratory Referral Hospital, Jakarta, from November 2024 to March 2025. Eighty-five adult patients with PTLD were included. Zinc intake was assessed by a semi-quantitative food frequency questionnaire (SQ-FFQ) and analyzed by NutriSurvey software. Appetite was measured using the visual analog scale (VAS) for appetite. Results: A total of 85 subjects participated in the study, comprising 78.8% males and 21.2% females. The average daily zinc intake was 4.4 mg/day, and 92.9% of the subjects had zinc intake below the recommended dietary allowance (RDA). The average VAS appetite score was 70 mm. A significant positive correlation was found between zinc intake and appetite, indicating that a decrease in zinc intake might result in a reduced appetite (r=0.266, p=0.014). Conclusion: Zinc intake is positively associated with appetite in patients with PTLD. Therefore, zinc intake monitoring is essential for supporting nutritional recovery and lung function in PTLD.

Abstract

Telaah kritis dilakukan menggunakan Oxford Centre for Evidence-Based Medicine (CEBM) dan kerangka telaah kritis FAITH.
