Abstract

Hypoglycemic patients who receive 40% dextrose (D40%) concentrated solution in the correction process need to avoid excessive blood glucose spikes. Administration of D40% was found in two different ways, through intravenous infusion (iv infusion) and intravenous bolus (iv bolus) and the effects of both types of administration were unknown. The purpose of this study was to compare the effect of a hypoglycemia treatment protocol using D40% concentrated solution to the post-correction blood sugar response through iv infusion and iv bolus at two different hospitals with two distinct protocols. This comparative analytical cross-sectional study was conducted retrospectively at St. Carolus Hospital (D40% iv infusion group) and Bella Hospital (D40% iv bolus group). Blood glucose responses, in form of coefficient of variation and degree of overcorrection, were compared between groups. The overall median blood glucose response was 69.5 (3–195) mg/dL for iv infusion group (n=60) and 77 (15–249) mg/dL for iv bolus group (n=62) (p=0.259). The coefficient of variation with iv infusion and iv bolus group were 47.18% and 52.75%, respectively. The median of degree of overcorrection in iv infusion group was lower compared with iv bolus group, 10% (0–138%) versus 23% (0–195%), respectively. Both D40% protocols did not have a significant correlation with the degree of overcorrection (Mann-Whitney test; p=0.099). D40% iv infusion and bolus administration had no effect to the post-correction blood sugar response.

Abstract

ABSTRACTChronic constipation is a medical gastrointestinal problem which can degrade patient’s quality of life. So far, the shifting pathophysiologic mechanism to colonic neuropathology has not been supported by diagnostic facility and therapy. Target therapy for chronic constipation related to enteric nervous system is still limited to study. Currently, the discovery of neurotrophin-3 has not shown any significant development needed in daily clinical practice. Until now study related to endoscopic full-thickness biopsy has not been done in human and waiting for its applications in daily practice. Targeted therapy for chronic constipation which is related to enteric nervous system is still limited to study. The discovery of neurotrophin-3, although currently on clinical trial phase II, still not showing any significant contribution in daily clinical practice. New pathophysiologic mechanism shifting the ‘idiopathic’ paradigm or ‘functional’ to colonic neuropathology has not been met with diagnostic modalities and therapy based on this knowledge. Study regarding endoscopic full-thickness biopsy to help revealing this novel mechanism has not been done in human. Henceforth, until now applied studies pertinent to this subject are longing in order for enteric neuron etiology can be firmly established Thus, in the future, translational study is demandingly needed for a firm establishment of diagnostic and therapeutic modalities to its currently evolving pathophysiology.  Keywords: chronic constipation, colon, enteric nervous system, neurotophin-3

Abstract

Introduction: Social isolation during the COVID-19 pandemic brought several effects on one’s daily activity, such as reduced physical activity, increased anxiety and stress towards their environment, and a sleep disorder called Excessive Daytime Sleepiness. EDS is often found in undergraduate students. Many factors could cause EDS, one of which is excessive fat consumption. This study was conducted because of the significant impact that COVID-19 brought on EDS and BMI. In addition, due to the high prevalence of excess BMI, the researcher wanted medical students aged 18 years or above as the target. Methods: This study used observational analysis with a cross sectional method. It used Epworth Sleepiness Scale (ESS) questionnaire distributed online from October to November 2022. The respondents were 108 (46 male, 62 female) pre-clinical students from the School of Medicine and Health Sciences, Atma Jaya Catholic University of Indonesia (FKIK UAJ), who met the inclusion and exclusion criteria. A bivariate analysis test was conducted with a chi-square statistical test with p ≤ 0,05. Results: Seventy respondents (64,9%) had Excess Body Mass Index (BMI), thirty-six respondents (78,3%) were male, and thirty four respondents (54,8%) were female. Sixty-eight respondents (62,9%) experienced Excessive Daytime Sleepiness (EDS) post-COVID-19 pandemic; also, forty two respondents (67,7%) were female, and the rest (56,5%) were male. There is a significant association between excess Body Mass Index (BMI) towards Excessive Daytime Sleepiness (EDS). (p=0,002) Seventy respondents (64,9%) had Excess Body Mass Index (BMI), thirty-six of them (78,3%) were male, and the other thirty-four (54,8%) were female. Abstract

There are no established guidelines for the management of androgenetic alopecia (AGA). A new therapy with promising results is Platelet-Rich Plasma (PRP). PRP has a variety of preparation methods, such as single spin and double spin centrifugation and still there is no consensus on the method of preparation. There is controversy of which preparation method is better. We searched PubMed, EBSCO, Clinical Key, ProQuest, Cochrane Library, Science Direct, Clinical Trial, Open Access, Wolters Kluwer, Google Scholar, GARUDA, and AtmaLib until Oct 2020 for human studies using PRP for AGA treatment. We assessed 3375 papers; 6 studies met the inclusion criteria and evaluated 128 AGA patients, 112 male and 16 female, 18-63 years old. Participants varied from type II-VI based on the Hamilton-Norwood classification system and degree 2 based on the Ludwig classification system. Four studies used the single-spin centrifugation PRP preparation method and two studies used the double-spin PRP preparation method. The studies stated the increases in growth factor levels. All studies showed PRP significantly affected hair growth in AGA patients. PRP has been shown to increase hair growth parameters in form of hair density (P <0.0001). PRP has been shown to increase hair density and hair count. Growth factors in PRP induce cell proliferation and tissue angiogenesis. The results of single-spin centrifugation preparation method provide a greater increase in hair density. But, data reports from the study did not allow comparisons to determine the best PRP preparation method for AGA treatment. Therefore further study is needed.

Abstract

Abstract

BACKGROUND The rising incidence of thyroid cancer in Indonesia has become a public health concern. This study was aimed to evaluate the validity of the thyroid cancer-quality of life-Indonesian version (ThyCa-QoL-ID). METHODS This cross-sectional study involved translating the original ThyCa-QoL questionnaire from English to Indonesian (T1), which was then back-translated into English (T2). The T2 questionnaire was compared with the original ThyCa-QoL questionnaire to refine T1, resulting in T3. The T3 questionnaire underwent cognitive and transcultural validation through individual assessment and focus group discussions with 10 doctors and 10 nurses at Cipto Mangunkusumo Hospital and St. Carolus Hospital, and 5 patients with thyroid cancer from St. Carolus Hospital. This process ensured cultural, language, and literacy adaptation processes for patients across different age groups. RESULTS The ThyCa-QoL questionnaire demonstrated validity, with all questions showing correlation calculation (r) above the r table. Test-retest reliability was measured by calculating the intraclass correlation coefficient for each question on the ThyCa-QoL questionnaire. The reliability test can be seen in the value of Cronbach’s alpha (0.915), exceeding the standard Cronbach’s alpha value standard (0.6). CONCLUSIONS The ThyCa-QoL-ID was confirmed as a valid and reliable tool for assessing the QoL of patients with thyroid cancer in Indonesia.

Abstract

Sleep quality reflects an individual’s satisfaction with the sleep process, which is essential for cognitive, psychological, metabolic, and cardiovascular health. Poor sleep quality is prevalent among medical students and may elevate their risk of metabolic syndrome. This study investigated the association between sleep quality and body mass index (BMI) with metabolic syndrome risk factors in medical students. This was a cross-sectional study involving 98 first- to third-year preclinical medical students. Sleep quality was assessed by using the Pittsburgh sleep quality index (PSQI) questionnaire, BMI was measured from body weight and height, and metabolic syndrome risk factors were evaluated through waist circumference, blood pressure, fasting blood glucose, and high-density lipoprotein (HDL) cholesterol levels. Data was analyzed using Chi square and Fisher’s exact test. Results showed that 54.08% of students had good sleep quality and 42.86% had a normal BMI. However, 59.18% of students exhibited at least one metabolic syndrome risk factor, including central obesity (51.02%), hypertension (20.41%), dyslipidemia (13.27%), and high fasting blood glucose (1.02%). There was no association between sleep quality and metabolic syndrome risk factors. The BMI showed no association with fasting blood glucose and HDL cholesterol level. However, BMI showed a significant association with waist circumference (p<0.001) and blood pressure (p = 0.005). These findings emphasize the importance of addressing the MBI in mitigating metabolic syndrome risks among medical students.

Abstract

IDD (Iodine Deficiency Disorders) is the most common cause of preventable mental retardation in the world. Nowadays, IDD still one of the most important public health problems in Indonesia. Up to now, Indonesia has not yet reached the target of IDD elimination as expected. National IDD Survey at 2003 and National Health Survey at 2007 show the achievement of IDD control program in Indonesia is still below the target of Universal Salt Idozation and Indonesia Health 2010 (RAN KPP GAKI strategy). Cross-sectoral commitment is very important for the elimination of IDD in the future. Clinicians and epidemiologist concern for IDD elimination in Indonesia is still low, so does the publications in the field of IDD from epidemiology and health policy perspective. It is expected that this literature review can give broad description and epidemiological perspective for clinicians and public health experts.

Abstract

Pos pelayanan terpadu (posyandu) lansia belum efektif, informasi minimal, kader belum optimal menunjang kebutuhan lansia. Indonesia is a country with the highest increase numbers of elderly in the world. In the range 35 years since 1990, the increase number of the elder- ly will be 414% that will set Indonesia in the fifth rank of the highest elderly in the world by the year of 2025. In most developing countries on the world, more than two third elderly live in remote and rural area. The aim of this study is to descript and discuss statistic data, health, health services, and programs for elderly in remote and rural Indonesia represented by Tanimbar Islands Maluku Province. The study use qualitative method by observation, participation, and interview with 30 elderly from 2 semiurban and 6 rural area in remote, rural, new developing administration district, and outer is lands of Republic Indonesia, Tanimbar Islands, Maluku Province in April to June 2010. We also interview and discuss elderly problems with elderly section program in health department West-Southeast Maluku region and el- derly program personnel in public health center. From the study, we found major health problems of the elderly in Tanimbar are genus artralgia, chronic gastritis, lower back pain, cataract, hypertension, and diabetes mellitus. Social and culture problems are urbanization which make elderly living alone. Integrated service section for elderly aren’t effective, minimal infor- mation, and the human resources aren’t optimal enough to support elderly needs. In the future, we must strengthen our health services system for elderly; expands the concern and networking with executive and legislative board; do education, service, and research innovations and strategies in the field of elderly health in remote rural Indonesia.
